Rhinitis and asthma in athletes: an ARIA document in collaboration with GA2LEN.

This consensus document is aimed at reviewing evidence that the rhinitis-asthma links have peculiar features in athletes. Beside a review of epidemological data on the high prevalence of rhinitis and asthma in athletes, the effects on intense physical exercise on the immune system and repiratory functions are discussed, with special reference to the role of allergens and pollutants. In extending the Allergic Rhinitis and its Impact on Asthma (ARIA) recommendations to athletes, the issue is addressed of adapting diagnosis and management to criteria set by the International Olympic Committee (IOC) and regulations adopted by the World Anti-Doping Agency (WADA).

Pharmacologic and anti-IgE treatment of allergic rhinitis ARIA update (in collaboration with GA2LEN).

The pharmacologic treatment of allergic rhinitis proposed by ARIA is an evidence-based and step-wise approach based on the classification of the symptoms. The ARIA workshop, held in December 1999, published a report in 2001 and new information has subsequently been published. The initial ARIA document lacked some important information on several issues. This document updates the ARIA sections on the pharmacologic and anti-IgE treatments of allergic rhinitis. Literature published between January 2000 and December 2004 has been included. Only a few studies assessing nasal and non-nasal symptoms are presented as these will be discussed in a separate document.

Continuing Medical Education: an international reality.

We are all aware today of the growing interest in continuing medical education (CME) programmes in many European Countries and it is important to understand why and how CME could become an international reality. It is obvious that patients need a good doctor--the best possible--as far as medical knowledge, attention to the patient's quality of life and cost-control is concerned. All European health care systems have to take into consideration everything that causes patient dissatisfaction, risk management and unjustified expenses. An example is the increase of claims and complaints against doctors and the strong attention of patients to medical procedures. In other words, medicine worldwide is becoming a service industry and has to consider quality and quantity of performances as well as to pay attention to personal responsibility. The object of our work is to evaluate the CME systems present in Europe, to show the work done on CME by the CME Committee of the European Academy of Allergology and Clinical Immunology and to highlight the Consensus Report on CME approved by an international panel of CME experts.

Improved left ventricular relaxation during short-term right ventricular outflow tract compared to apical pacing.

STUDY OBJECTIVES: Pacing-induced asynchrony may deteriorate left ventricular function; however, limited data exists in humans. The aim of our study was to compare left ventricular hemodynamics during short-term atrioventricular sequential pacing from the right ventricular apex and from the outflow tract of the right ventricle. DESIGN: Three 5-min pacing intervals were applied in a random order, at a rate of 15 beats/min above the resting sinus rate. Atrioventricular sequential pacing from the two sites was compared with atrial pacing. During each pacing mode, left ventricular pressure was recorded, and cardiac output was calculated using Doppler echocardiography. SETTING: Cardiac catheterization laboratory. PATIENTS: Twenty patients (18 male, mean age 62 +/- 11 years) without structural heart disease were studied. RESULTS: During atrial pacing, maximum negative first derivative of pressure (dp/dt) was 1,535 +/- 228 mm Hg/s; during pacing from the apex it decreased to 1,221 +/- 294 mm Hg/s (p = 0.0001), but was not significantly different during pacing from the outflow tract (1,431 +/- 435 mm Hg/s, p > 0.05). Isovolumic relaxation time constant (tau) during atrial pacing was 39.7 +/- 11.9 ms; during pacing from the apex, it increased to 47.9 +/- 14.0 (p = 0.001), but was not significantly different during pacing from the outflow tract (42.5 +/- 11.2, p > 0.05). Peak systolic pressure decreased significantly during atrioventricular sequential pacing from either site; however, it did not differ between the two sites. No differences in end-diastolic pressure, maximum positive dp/dt, or cardiac output could be demonstrated. CONCLUSION: In patients with no structural heart disease, short-term right ventricular outflow tract pacing is associated with more favorable diastolic function, compared to right ventricular apical pacing.

Importance of the site of ventricular tachycardia origin on left ventricular hemodynamics in humans.

Experimental animal data have indicated that the site of ventricular tachycardia origin and, hence, the degree of asynchronous contraction, may influence the hemodynamic tolerance during sustained ventricular tachycardia. However, data in man are scarce. We studied patients with preserved left ventricular function and absence of significant coronary artery disease. Ventricular tachycardia was simulated with rapid pacing (at 120 and 150 beats/min), performed randomly, from the right ventricular apex or the right ventricular outflow tract. Following pacing from one site, it was repeated from the alternate site. Compared to outflow tract pacing, QRS duration was significantly longer during rapid pacing from the apex. Left ventricular pressure was recorded using a micromanometer-tipped catheter. During sinus rhythm, peak systolic pressure was 142 +/- 14 mmHg; at 120 beats/min, it decreased to 109 +/- 12 mmHg during pacing from the apex and to 127 +/- 21 mmHg during pacing from the outflow tract (P = 0.008). This difference diminished at 150 beats/min (101 +/- 16 mmHg vs 112 +/- 16 mmHg, respectively, P = 0.21). During sinus rhythm end-diastolic pressure was 13 +/- 1 mmHg, which did not change significantly during pacing at 120 beats/min. During pacing at 150 beats/min, end-diastolic pressure increased to 21 +/- 3 mmHg during pacing from the apex and to 16 +/- 2 mmHg during pacing from the outflow tract (P = 0.005). Changes in first derivative of pressure and in isovolumic relaxation time constant were comparable during pacing from the two sites. Thus, it seems that tachycardias originating from the right ventricular outflow tract result in more favorable left ventricular hemodynamics, compared to those from the right ventricular apex.

Coronary blood flow changes during atrioventricular sequential pacing with different atrioventricular delays in normal individuals.

This study examined the effects of different atrioventricular (AV) intervals, during AV sequential pacing, on hemodynamics and coronary blood flow in individuals with normal hearts. Left anterior descending artery blood flow velocity was measured, using intracoronary Doppler, in 17 normal individuals. Five pacing tests were applied in random order for 5 min, at 15 beats/min above the sinus rate. Four tests using AV sequential pacing with AV intervals of 175, 150, 100, and 50 ms, and one using atrial pacing were applied. Mean flow velocity was 21 +/- 9 cm/s, 20 +/- 9 cm/s, 17 +/- 7 cm/s, 17 +/- 7 cm/s, and 22 +/- 10 cm/s, respectively (F = 8.87, p = .00001). The hemodynamic effects of these 5 pacing tests were assessed in 8 different normal subjects. Isovolumic relaxation time constant and left ventricular systolic pressure decreased, whereas right atrial pressure increased during AV sequential pacing with short AV intervals. Thus, during short-term AV sequential pacing at rest, coronary blood flow in a normal left anterior descending artery decreases with short AV intervals.

Induced sputum, bronchoalveolar lavage and blood from mild asthmatics: inflammatory cells, lymphocyte subsets and soluble markers compared.

Airway inflammation in asthma can be measured directly by invasive bronchoalveolar lavage (BAL), directly and relatively noninvasively by induced sputum and indirectly from peripheral blood. We compared cellular and fluid phase indices of inflammation in induced sputum, BAL and blood from 11 adults with mild stable asthma. On one day, induced sputum selected from saliva was collected and on the next, blood and BAL. Median results of sputum compared with BAL showed a higher number of nonsquamous cells (53 versus 0.8 x 10(6) cells x mL(-1), p=0.003), more neutrophils (34.3 versus 1.0%, p<0.001), CD4+ and CD19+ T-cells (76.5 versus 54.7%, p=0.01 and 5.2 versus 1.1%, p=0.03, respectively), fewer macrophages (603 versus 95.0%, p=0.002) and markedly higher levels of eosinophil cationic protein (ECP) (264 versus 2.0 microg x L(-1), p<0.001), tryptase (17.6 versus 2.2 UI x L(-1), p<0.001) and fibrinogen (1,400 versus 150 microg x L(-1), p=0.001). Sputum and BAL neutrophils and CD4+ T-cells were strongly correlated. Sputum and BAL differed from blood by having higher proportions of T-cells (94.9 and 98.9% versus 87.7%, p=0.002) and lower proportions of CD19+ T-lymphocytes (p=0.04 and 0.006). Sputum also differed from blood by having higher proportions of CD4+ T-cells (76.5 versus 51.4%, p=0.001), lower proportions of CD8+ cells (24.0 versus 403%, p=0.04) and a higher CD4+/CD8+ ratio (3.3 versus 1.4, p=0.01). We conclude that in mild asthmatics, sputum, bronchoalveolar lavage and blood measure different compartments of inflammation. Induced selected sputum has the advantage over bronchoalveolar lavage of higher density of cell recovery and stronger signal for fluid-phase markers.

Study of erythrocytes, hemoglobin levels, and menstrual cycle characteristics of women exposed to aromatic hydrocarbons.

OBJECTIVES: The impact of occupational hazards on erythrocyte and hemoglobin levels and menstrual cycle characteristics in women exposed to aromatic hydrocarbons was studied. The investigated cohort consisted of 110 women exposed to benzene, toluene, xylene and styrene while working in the transport and storage of petrochemical products (TSPP), the laboratory for the control of aromatic hydrocarbons (LCAH), and rubber and latex (RL) production at the biggest petrochemical plant in Bulgaria, Neftochim. Controls consisted of 45 women from the administrative personnel in the same plant. METHODS: Questionnaires were administrated to all participants to obtain self-reported demographic data, data on characteristics of the menstrual cycle, and psychosomatic symptoms. Erythrocyte counts (RBC) and hemoglobin (HGB) levels were determined by the semiautomatic hematology analyzer Serono System 190, USA. All subjects had signed an informed consent. RESULTS: The mean RBC counts and HGB levels of the women from the LCAH and TSPP departments were statistically significantly lower than those of the control group. We found statistically significant differences in all exposed groups in comparison with controls when the dose-response relationship was investigated. No statistically significant differences were found in menstrual cycle characteristics (duration of the menstrual cycle, days and quantity of bleeding) between the subjects exposed and the controls in all age groups investigated, but the numbers were too small to allow definite conclusions. CONCLUSIONS: The lowering of HGB levels and RBC counts in the investigated women were mainly due to the direct influence of aromatic hydrocarbons in the working environment on hematopoiesis. The studied group was too small to enable us to determine the impact of aromatic hydrocarbons on menstrual cycle characteristics, and the possible combination of these effects.

A comparative assessment of liver function in workers in the petroleum industry.

OBJECTIVES: The group studied included 666 workers from the benzene (n = 30), xylenes (n = 144), ethylene oxide (n = 91), 1,3-butadiene (n = 110) and transport and mechanisation (n = 82) departments of a petrochemical plant in Bulgaria. The leading hazards for the particular departments are as follows: Benzene: benzene (up to ten times the threshold limit value; TLV): xylenes: benzene (up to nine times TLV) and xylenes (up to 1.6 times TLV); ethylene oxide: ethylene oxide (up to 20 times TLV) and ethylene (up to 30 times TLV); divinyl: 1,3-butadiene (five times TLV) and acetonitrile (six times TLV); transport and mechanisation: benzene, xylenes and pentane (0.7-10 times TLV). METHODS: The functional status of the liver was assessed by clinical (medical check-up), echographic and biochemical (ASAT, ALAT, APh, gamma-GT, lipid fractions, reduced glutathione and SH groups) investigations. Biochemical analyses were performed with POINTE 190 (USA). The results obtained were compared with those of a control group (n = 150), consisting of subjects with no occupational exposure to chemical hazards. RESULTS: Comparisons showed that the metabolic disturbances determined were most significant in workers from the ethylene oxide department. Significant decreases in the serum levels of reduced glutathione (up to 48%) and of thiol groups (with 23%), increased enyzme activity (up to 18%) and lipid fractions (up to 24%) were observed. In the benzene and xylenes departments, deviations in the indices studied were equally manifest. In workers exposed to 1,3-butadiene, the changes in lipid indices were prevalent (registered in 25% of the studied persons). The staff in transport and mechanisation showed deviations in enzyme indices at a slight increase of the lipid fractions. The changes determined in serum biochemical characteristics corresponded to the clinical findings--hepatomegaly, confirmed echographically in 10%-20% in the different departments. CONCLUSIONS: The results obtained followed the exposure-response relationship. The deviations registered in workers with a length of service of more than 10 years were the most significant. The occupational genesis of the determined disturbances was confirmed in 11 subjects.

Study of the cardiovascular effects of occupational exposure to organic solvents.

OBJECTIVE: The objective of the present study was to evaluate the effects of occupational exposure to over-concentrations of some organic solvents on the prevalence of arterial hypertension (AH) and pathologic changes in electrocardiography (ECG). METHODS: We studied 345 workers (250 men) aged between 20 and 60 years at a petrochemical factory, and 345 age- and sex-matched controls with no occupational contact with chemical and physical factors. According to the main chemical pollutants in the working environment, the exposed subjects were divided into three groups: group 1, exposed to benzene; group 2, exposed to xylene and benzene, and group 3, exposed to phenol. Physical examination of the heart and routine resting electrocardiography with 12 leads were performed. The degree of AH was determined according to the classification of the American Heart Association (JNC- V, 1993). Statistical analysis was carried out with a SPSS software package. RESULTS: The mean values of systolic and diastolic blood pressure and the prevalence of AH were significantly higher than in control groups 1 and 2. The prevalence of AH was 30.51% [OR = 2.44; 95% confidence interval (CI) 1.24-4.85; P = 0.0054] in group 1 and 27.92% (OR = 2.00; 95% CI 1.11-3.61; P = 0.0136) in group 2. The same relationship was found for the prevalence of ECG abnormalities: group 1, 17.7% (OR = 2.34; 95% CI 0.99-5.63; P = 0.0344) and group 2, 18.83% (OR = 2.75; 95% CI 1.28-5.97; P = 0.0044). The supraventricular and ventricular ectopic beats were the most frequent, followed by conduction defects and repolarization disturbances. In group 3, the prevalence of AH (23.29%) and ECG changes (13.7%) were higher than in the controls, but the differences were not significant. A significant correlation between the length of service and systolic and diastolic blood pressure (after controlling for major cardiovascular risk factors) was found in groups 1 and 2. CONCLUSIONS: These results show that those workers occupationally exposed to high concentrations of benzene and xylene have an increased prevalence of AH and pathologic changes in ECG. There was no convincing evidence of increased cardiovascular risk associated with occupational exposure to phenol.

[An evaluation of the toxic exposure to inhalation anesthetics and disinfectants on liver function. II].

The object of the study was evaluation of the functional state of the liver of surgeons and anaesthesiologists, exposed to halothane, paradise gas and disinfectants (formaldehyde). A representative group of 107 individuals was involved, distributed into two professional groups: surgeons (AcAT, AIAT, AP, gamma-GT) was determined. The analyses were performed on "Pointe 190", USA. Increased incidence and degree of deviations in the transaminases were established; a correlation between the results from the biochemical analyses and the toxicometric records. In conclusion the established correlations "dose-effect" and "dose-response" confirmed that the liver is a critical organ under the conditions of combined effect exerted by haolthane and paradise gas.

Occupational exposure to Cr(VI): comparison between chromium levels in lymphocytes, erythrocytes, and urine.

The relationships between chromium (Cr) levels in lymphocytes, erythrocytes, urine, and ambient air were compared among 14 chrome-platers from a metallurgic plant in Bulgaria and two groups of local controls, one from the same heavily polluted industrial town as the chrome-platers (n = 11) and one from a seaside resort town 100 km away (n = 6). Among the chrome-platers, the Cr concentration in peripheral lymphocytes was positively correlated with total Cr and Cr(VI) levels in ambient air and with Cr excretion in urine. As compared to the controls, the chrome-platers had mean Cr levels in lymphocytes twice as high, in erythrocytes ninefold higher, and in urine fourfold to eightfold higher. Although Cr levels in urine and lymphocytes were similar between the two control groups, levels in erythrocytes were 3 times higher among subjects from the industrial area than among those from the seaside town. The study suggests that lymphocyte Cr could be a good indicator of the Cr body burden caused by high exposures to Cr(VI), such as in electroplating operations. In these conditions, erythrocyte Cr may be less useful, possibly owing to increased toxicity due to the high affinity of erythrocytes for Cr. However, when exposure is lower, such as in most environmental situations, erythrocyte Cr should provide a better and more sensitive index than lymphocyte Cr. By contrast, urinary Cr, which provides information on total Cr exposure, including Cr(III) from dietary and environmental sources, does not seem to be of value for studying occupational exposure to Cr(VI).

Monitoring human lymphocytic DNA-protein cross-links as biomarkers of biologically active doses of chromate.

A simple and sensitive assay for DNA-protein cross-links has been used as a biomarker of chromate exposure and early carcinogenic effects. Pilot studies of DNA-protein cross-links in peripheral blood lymphocytes have been conducted with individuals who had higher exposure to chromate, including welders, and with individuals who had lower levels of exposure such as residents living in a chromium-contaminated area in Jersey City, New Jersey. Studies were also conducted in two Bulgarian cities (Jambol and Burgas) with different levels of air pollution and Cr(VI) exposure and in chrome platers in Bulgaria who had high exposure to chromate. DNA-protein cross-links in U.S. welders and in individuals living in Hudson County, New Jersey around chromium-contaminated areas were significantly higher compared to matched controls. Although blood and urinary levels of chromium were not extensively studied in these populations, we were able to obtain these measurements in the Bulgarian population. Chromium levels in red blood cells of controls living in Burgas were in the order of 1 to 2 ppb chromium, and these individuals had the lowest levels of DNA-protein cross-links. However, the chromium levels in Jambol ranged from about 2 to 7 ppb in red blood cells of city residents to about 22 ppb in chrome platers. DNA-protein cross-links were saturated at about 7 to 8 ppb chromium in the red blood cells, and cross-links correlated well only with chromium levels in red blood cells. Urinary chromium levels did not correlate well with either DNA-protein cross-links or chromium levels in with red blood cells.

[The toxicological characteristics of Gastrofenzin. III. Its chronic oral toxicity].

A risk evaluation has been carried out under conditions of chronic three-month experiment in oral application of Gastrofenzin in doses 1/20, 1/100 and 1/200 LD50 for male and female white rats, respectively 33.2 and 43.8; 6.7 and 8.8; 3.4 and 4.4 mg.kg-1. There have been applied integral, behavioural, laboratory, biochemical (serum, liver, kidney, brain, brain mitochondria), histologic and electron microscopic methods. The highest doses cause functional and biochemical changes in the nerve system, liver and the kidneys without involving the structural elements of the respective tissues. Significant changes have not been observed in the doses 1/100 LD50 and 1/200 LD50. A dose-effect increase has been found out in the level of cytochrome-P-450 in the liver tissue, but the data are insufficient for determining the type of the caused induction. The results of the complex study show that in the three-month application in doses 1/20, 1/100 and 1/200 LD50 Gastrofenzin does not cause development of significant deviation in the organism of the mice from both sexes and its toxicological characteristic is comparatively favourable.

The evaluation of a cell dispersion method of sputum examination.

In recent studies, sputum smear cell counts were found to be reproducible and usefully applied to research in asthma and other airway conditions. However, cell definition on the smears is poor, and the procedure is tedious and has limited utility. The objective of this study is to improve the methods of sputum examination. The subjects used in this study were people with bronchitis or asthma from whom sputum could be obtained. By inverted microscopy, portions of fresh sputum were selected to exclude salivary contamination. These portions were exposed to different volumes of dithiothreitol for varied time intervals. We used the resulting cell suspensions to perform total cell counts and prepare cytospins for differential cell counts and immunohistochemical stains for GM-CSF, EG2, TNF alpha and IL-8. Cytospins were compared with smears for differential cell counts on the same sputum specimens. Excellent cell dispersion and definition in cytospins could be observed. The time required for differential cell counting on cytospins was reduced and cytospin counts were more reproducible than smears. Greater duration of treatment of sputum with dithiothreitol tended to increase total cell counts and significantly decreased EG2 staining but had no effect on differential cell counts or the cytokine cell components. Therefore the proposed method of sputum examination involving cell dispersion and use of cytospins overcomes a number of the disadvantages of the examination of smears.
