Immunofluorescence studies on pathologic routine material: application to malignant lymphomas.

In an attempt to apply immunmorphologic techniques to pathologic routine material, malignant lymphomas of the Non-Hodgkin- and the Hodgkin-type were studied by indirect immunofluorescence. To minimize nonspecific background staining formalin fixed paraffin sections were treated with protease prior to incubation with the antisera. The usefullness of this procedure in providing additional diagnostic criteria was tested on specimens of malignant lymphomas (chronic lymphocytic leukemias, immunocytic lymphomas, plasmocytic lymphomas, centrocytic lymphomas, centroblastic-centrocyctic lymphomas, centroblastic lymphomas, immunoblastic lymphomas, lymphoblastic lymphomas of Burkitt, convoluted and unclassified type, Hodgkin's disease). In addition, cases of nonspecific lymphadenitis were studied. The discrimination of single cells particularly of the B-cell line was facilitated using immunofluorescence and different patterns were easily recognized thus aiding the diagnosis based on light microscopic criteria. In addition, preliminary evidence was obtained that lymphoblastic lymphomas categorized as "others" may be further characterized.

[Roentgenmorphologic findings in single homotransplantation of non immunosuppressive treated canine lungs (author's transl)].

According to plain chest roentgenography correlated to needle biopsies the following grades in the course of rejection after single lungtransplantation could be evaluated. 1) Begin of rejection: perihilar alveolar infiltrate. 2) Increase of the infiltrates (positive Airbronchogram). 3) Almost total consolidation of the graft. 4) Complete consolidation of the graft, mediastinal hernia caused by the own lung.

Spontaneous rosette formation by lymphocytes. Differential effect of cytochalasin B on thymocytes and thymus-related peripheral blood lymphocytes in the guinea-pig.

In the present investigation the differences in the capacity of thymocytes (Th) and thymus-related peripheral blood lymphocytes (TBL) of the guinea-pig to form spontaneous rosettes with rabbit erythrocytes (RRBC) were further evaluated. Cytochalasin B (CB) inhibited TBL rosette formation but was largely ineffective with respect to Th. This suggests that membrane modulation and microfilament function are requirements for the formation of TBL but not of Th rosettes. A certain critical RRBC receptor density on the lymphocyte may be necessary for a stable rosette. Our results suggest that different RRBC receptor densities on Th and TBL account for the differences in rosette forming capacity of these two cell types. In native Th, the receptor density may be high enough to warrant stable rosette formation. In TBL, however, active membrane movement and perhaps clustering of receptors may be necessary to achieve the local critical receptor density as prerequisite of a stable rosette.

[Changes of lung function in experimentally induced emphysema before and after unilateral homotransplantation of the canine lung].

Pulmonary function studies after single-lung homotransplantation in dogs with experimentally induced emphysema. Pulmonary emphysema was induced in 8 bastard dogs by intratracheal instillation of papain (dose 2 mg/kg 3 to 5 times in 6 to 10 weeks). Development of emphysema was documented by weekly pulmonary function studies. The emphysematous animals underwent single-lung homotransplantation. Postoperative studies of lung function were performed immediately after transplantation and 1 week postoperatively. Bronchospirometric measurement revealed that ventilatory function and oxygen consumption of the graft was at least equal to the animal's own emphysematous lung. Up to 1 week after transplantation there is no evidence of serious ventilation-perfusion imbalance.

[Pulmonary function tests following single lung homotransplantation in emphysematous dogs (author's transl)].

Experimental emphysema was induced by multiple intratracheal instillation of Papain in a dose of 2 mg/kg body weight in 4 bastard dogs. Development of emphysema was objectivated by pulmonary function tests. Increase of static lung compliance, decrease of lung elastance, and increase of air-way resistance were observed. In addition arterial pO2 decreased while pCO2 remained constant. Following single lung homotransplantation there was an increase of respiratory rate and minute ventilation, the transpulmonary pressure rose in the first postoperative days, static lung compliance was diminished. Four weeks after transplantation typical functional changes of emphysema could be demonstrated concerning the slope and appearance of the compliance loops. Arterial pO2 however remained within the normal range. This functional behaviour does not support the concept of a ventialtion perfusion imbalance after lung transplantation in emphysema patients. Respiratory insufficiency therefore is probably caused by an alveolar type of rejection.

[Peliosis hepatis, complicating treatment with anabolic steroids (author's transl)].

The clinical and pathological findings of peliosis hepatis are reported, complicating a long-term treatment with an anabolic steroid in a 19 year old male patient suffering from chronic aplastic anemia. Following a 15 months course of oxymetholone (Anapolon) a cholestatic jaundice developed without regression despite discontinuing the drug. 4 months later the patient expired as a consequence of his hematologic disease. At autopsy the liver showed a parenchymal type of peliosis hepatis with multiple blood filled cavities and focal liver cell necroses. The pathogenesis of this rare condition is discussed by means of literature demonstrating the hepatotoxic effect of C17-alkylated androgens.

[Immunoblastic adenopathy: clinical features, treatment and prognosis (author's transl)].

Clinical features, response to treatment and prognosis of 50 cases of immunoblastic lymph-adenopathy are reviewed. Most of the patients presented with generalized lymphadenopathy, hepatomegaly and/or splenomegaly, and fever. Hyperergic reactions such as pruritus, skin rash or eosinophilia were frequent. Erythrocytic sedimentation rate was increased by differing amounts. In some cases there was a polyclonal increase in immunoglobulins, while in others there was a reduction. Proven hypersensitivity to a wide spectrum of drugs was present in nine cases. Prognosis is uncertain: almost half of the patients died within one to forty-two months, some perhaps as a result of massive chemotherapy and/or radiotherapy. Best management probably is symptomatic treatment alone or with small doses of corticoids or immunosuppressives, supplemented by antibiotics. It is concluded that immunoblastic lymphadenopathy represents a hyperimmune reaction and is not, despite the high death-rate, a true malignant lymphoma.

[Incomplete, combined hereditary immunodeficiency with generalized tuberculosis after BCG-vaccination from bacille Calmette Guérin (author's transl)].

A case of hereditary incomplete, combined immunodeficiency is reported. The patient, a 20-week-old boy, suffered from severe, generalized tuberculosis after BCG-vaccination. A conspicuous discrepancy was found between the normal number of lymphocytes in the peripheral blood and the severe morphological changes in the organs of the lymphoreticular system (deficient development of thymic structures; absence of lymphocytes in the thymus and the thymus-dependent areas of the periphery as well as in the bursa-dependent structures). A partial primary defect of the stem cells and a secondary insufficiency of the functions with incomplete differentiation due to an insufficiency of the primary lymphatic organs are discussed as possible causative factors. The inability to develop epitheloid cells in connection with the tuberculous infection is interpreted in part as a sequence of a T-cell insufficiency.

Adherence of rabbit erythrocytes to guinea-pig brain structures. Evidence for antigenic correlation between guinea-pig thymus and brain?

Similar to thymocytes in the thymic cortex, certain brain structures, such as nerve tracts and nerve cells of the Ammon's horn, show an affinity to rabbit erythrocytes (RRBC) which is exemplified by the adherence of RRBC to frozen brain sections. The experimental conditions of this adherence reaction are identical with that previously found with guinea-pig thymus and lymph node sections. From the results it may be concluded that receptor substances similar in nature on thymocytes and brain are responsible for this adherence phenomenon.

[Electron microscopic investigation following canine pulmonary transplantation with regard to functional effects (author's transl)].

QUESTION: The function of alveolar macrophages in correlation with immunological stimulation, consequently also in pulmonary transplantation, are a subject of discussion. In this study the alveolar manifestations of rejection following pulmonary transplantation, are a subject of discussion. In this study the alveolar manifestations of rejection following pulmonary homotransplantation are investigated by electron microscopy. MATERIALS AND METHODS: In 20 healthy dogs (body weight between 7 and 19 kg) a left side pulmonary homotransplantation was performed using a standardized method. 2 randomized groups of 10 animals each were formed, one undergoing immunosuppressive therapy, the other none. Needle biopsies (Menghini needles) were taken in both groups at the onset of rejection daily until death of the animals (on the 5th to 12th postoperative day). At death of the animals always total rejection of the homograft was observed. The other animals were subjected to a scheduled immunosuppressive therapy (KREUZER et al. 1973, RADASZKIEWICZ et al. 1974). Needle biopsies were taken only at the stage of a clinically or roentgenologically ascertained crisis of rejection. Before implantation of the homograft and 20 mins after occlusion of anastomoses and onset of circulation in the transplant biopsies were taken, too, to exclude early changes or alterations in the donor. The lung specimens 2 mm in size) obtained by biopsy were fixed in phosphate buffered OSO4 and buffered phosphate solution by usual alcohol series. After embedding in epon the specimens were studied by electron microscopy. RESULTS: In the group not undergoing immunosuppressive therapy beginning with the 3rd post-operative day perivascular lymphocytic and plasmacellular infiltrates were seen which increased during the course of rejection. In both groups a swelling and vacuolization of endoplasmatic reticulum, cytoplasmic microvilli with bridge formation accompanied by an increase of alveolar macrophages and by augmentation of lamellar bodies in granular pneumocytes were noted following operation. A correlation between the increase of lamellar bodies and increase of surfactant has been shown previously.
