Prognostic value of flow cytometric DNA content analysis in granulosa cell tumor of the ovary.

The nuclear DNA content and S-phase fraction of 23 ovarian granulosa cell tumors were measured from paraffin-embedded tissue with flow cytometry. Crude survival of the patients with a euploid tumor (17 diploid, one tetraploid) was more favorable than that of the patients with an aneuploid tumor (n = 5, P = 0.02). The percentage of S-phase cells was a good predictor of survival. If more than 6% S-phase cells were present in the DNA histogram, both crude survival (P = 0.0001) and survival corrected for intercurrent deaths (P = 0.0001) were clearly inferior as compared with tumors with less than 6% S-phase cells. The results indicate that DNA flow cytometric study may provide a rapid and valuable method to predict the biological behavior of granulosa cell tumors of the ovary.

Nocturnal oxygen saturation and body movement in asthmatics treated with controlled-release preparations of theophylline or terbutaline.

Nine adult asthmatics with a history of nocturnal symptoms and with morning dips in peak expiratory flow (PEF) were treated for 10-14 days with 24-h controlled-release preparation of theophylline (Th), or a controlled-release preparation of terbutaline (Te), in a double-blind cross-over experiment. During treatment with 450-900 mg Th in the evening morning, plasma drug levels ranged from 53-95 (mean 73) mumol/l. The Te dose was 7.5 mg twice daily. Morning PEF values during Th (mean 338 l.min-1) and Te (316 l.min-1) were not significantly different. There were no significant differences between the treatments in average nocturnal oximetric O2 saturation (91.9% during Th and 91.0% during Te), or the amount of nocturnal body movement, recorded with a static charge sensitive bed (total number of movements 146 during Th and 120 during Te). No difference between the treatments was seen with respect to assessment by the subjects of sleep quality, which was considered fair or good. The findings suggest that in moderately severe asthma, nocturnal oxygenation and sleep quality were similar during the two treatments.

Genetics of congenital nemaline myopathy: a study of 10 families.

In order to investigate the inheritance in congenital nemaline myopathy (CNM), we studied the family histories and pedigrees of 13 patients with CNM from 10 families, and the 20 patients, by physical examination, single fibre electromyography, ultrasonography of muscles, measurement of serum creatine kinase, muscle biopsy, and electrophoresis of muscle proteins. None of the parents was affected. In three families there were two affected children. Of the parents, 15 showed deficiency of type 2B muscle fibres, and all except one father showed some other minor neuromuscular abnormality. These may represent heterozygous manifestations of recessive gene. Most of the ancestors came from sparsely populated rural communities in the west of Finland. We conclude that in the Finnish CNM patients, the mode of inheritance appears to be recessive. Apart from a few instances of dominant inheritance, most cases published also seem compatible with recessive inheritance.

A randomised comparison of ceftazidime and piperacillin, both in combination with flucloxacillin for treatment of febrile episodes in neutropenic children. Finnish Three-Centre Study.

111 febrile episodes in 98 neutropenic children were randomly treated with either ceftazidime (CAZ) 150 mg/kg/day or with piperacillin (PIP) 200 mg/kg/day, both combined with flucloxacillin (FLUC) 50 mg/kg/day. A total of 37/47 (79%) eligible episodes in the CAZ group and 41/53 (77%) in the PIP group were cured without needing to stop the initial therapy. The success without modification of the treatment in the verified septicaemias in the CAZ group was 8/18 (44%) and in the PIP group 5/18 (28%). Of the bacteriologically documented infections, 13/24 (54%) in the CAZ group and 11/24 (46%) in the PIP group were cured without modification of the therapy. Similarly, 17/31 (55%) of all the isolated bacteria in the CAZ group and 14/33 (42%) in the PIP group were eradicated with the initial therapy. Overall there were 13 deaths of which 4 in the CAZ group and 5 in the PIP group occurred during the infection. An increasing granulocyte count exceeding 0.25 x 10(9)/l at the end of the therapy was a good prognostic sign for the outcome of the infection. This study indicates that CAZ is as effective as PIP in the treatment of infections in neutropenic children.

Electromyography in congenital nemaline myopathy.

To clarify the discrepancies between earlier reports of electromyography (EMG) in congenital nemaline myopathy (CNM), conventional electromyography was done on 13 patients with CNM, and results were compared with those of 18 earlier EMG examinations of the same patients. Fiber density was measured in 10 patients with a computerized method and neuromuscular jitter in 3 with single-fiber EMG. With age, the EMG abnormality progressed, and "neuropathic" EMG features developed in distal muscles. In 9 of 10 patients fiber density was higher than normal. In two of three patients jitter was abnormal. Motor (13 of 13 patients) and sensory (3 of 3 patients) nerve conduction velocities were normal. Our results seem to explain the conflicting reports of EMG in CNM. We conclude that active degeneration and regeneration of muscle fibers takes place in CNM and suggest that the "neuropathic" motor unit potentials seen in our patients may be secondary to myopathic disease activity.

Changes induced by short-term xylene exposure in human evoked potentials.

Nine healthy male volunteers were exposed to m-xylene for 3 h in the morning and 40 min in the afternoon with a 40-min break in between. The atmospheric m-xylene concentrations were either stable at 8.2 mumol/l (200 ppm) or they fluctuated (5.2-16.4 mumol/l; 135-400 ppm) with peaks of 16.4 mumol/l and duration of 20 min at the beginning of each exposure session. The subjects were either sedentary or exercised at 100 W for 10 min at the beginning of each session during both exposure types. The two control days, with and without exercise, were similar to the exposure days but without exposure. Evoked potentials were recorded in the morning before the exposure and immediately after the morning and afternoon sessions. Visual evoked potentials were studied to a pattern reversal stimulus (pattern VEP) and to a light flash (flash VEP). For pattern VEPs the latencies of P50, N70, P100, N135 and P170 as well as the peak-to-peak amplitude of N70 to P100 were measured. For flash VEPs the latencies of P50, N70, P100, N150 and P200 as well as the peak-to-peak amplitude of P100 to N150 were measured. Short-latency auditory evoked potentials arising in the brainstem (BAEP) were recorded for a click stimulus. The peaks I, II, III, IV and V were identified from the grand averages. The effect of various exposure paradigms was evaluated by comparing the individual changes on an exposure day to those during the control days.(ABSTRACT TRUNCATED AT 250 WORDS)

Fungi carried from farmers' work into farm homes.

Airborne fungal spore concentrations and main fungal genera were compared in rural and urban living environments in Finland during the winter. In addition to conventional viable fungal spore counts (based on the six-stage impactor sampling and cultivation), total spore concentrations were obtained by scanning electron microscope (SEM) investigation of filter samples. The viable spore counts were only 0.2%-25% of the number of total spore aggregates. A high correlation between these two methods was noted, however, at the recommended measuring ranges of the methods. In the farm houses, viable and total spore levels were 10(3) to 10(4) colony forming units/m3 (cfu/m3) and 10(4) to 10(5), spores/m3, respectively. These counts were 10-10(3)-fold higher than the concentrations in an urban apartment. The spore levels of farmers' homes, however, were somewhat lower than those observed in their cow barns. Aspergillus, Cladosporium, and Penicillium spores were present in both urban and rural environments. Actinomycetes and some fungal genera--such as Acremonium, Alternaria, Botrytis, and Chrysosporium--which were detected in cow barns and in farm houses, were not present in urban environment. The results indicated that airborne fungal spores may be carried from cow barns to farmers' homes.

Evaluation of automatic analysis of SCSB, airflow and oxygen saturation signals in patients with sleep related apneas.

We have developed a computerized analysis of respiratory and body movements (static charge sensitive bed [SCSB]), oxygen saturation (pulse oximeter), and airflow (thermistor) for the evaluation of sleep related apneas. The cumulative distribution of oxygen saturation, the number and distribution of desaturation events, and the duration and type of apneas are assessed. Analysis is performed separately during the total recording time and during the time when the patient sleeps on his back. We have compared the automatic analysis with the results obtained on simultaneous daytime polysomnograph naps in 55 subjects (snorers or obstructive sleep apnea syndrome [OSAS] patients). The compressed graphs obtained automatically demonstrated a periodic breathing pattern in all 22 patients who presented sleep-related apneas at polygraphic recording. The cumulative distribution of oxygen saturation was not as steep in the apnea patients as in patients not showing apneas; in 19 of the 22 OSAS patients, the value was outside our normal limits (80 percent of the recording time inside 3.6 percent SaO2 variation band). The apnea index (AI) was 26.4 in manual and 23.3 in automatic analysis. Using the automatic method there were three false negative cases in the analysis of desaturations; in these patients periodic breathing was present in output graphs indicating need for further polygraphic assessment. The duration of apneas in the automatic analysis was shorter than in manual analysis, but the agreement was sufficient for screening purposes (mean error less than 3 s, mean duration of apneas 20.1 s). The automatic method is presently used in clinical routine for screening purposes, for assessment of the severity of the disorder and the type of treatment that a subject may need, in epidemiologic investigation and follow-up of the treatment.

Comparison of smear specimens with biopsy specimens in a nucleic acid hybridization test for human papilloma virus (HPV) infection.

A total of 323 pairs of specimens from women with Papanicolaou class II or III cytology were examined for human papillomavirus (HPV) types 6, 11, 16 and 18 by spot hybridization. Each pair consisted of a representative biopsy specimen and a smear specimen from cervical, vaginal or, more rarely, vulvar lesions. We found a close correlation between HPV findings in biopsies and smears. In 83.9% (271) of the cases, both specimens were either positive for a given HPV type or negative. No discordant HPV-types in the two types of specimens were found. In 15.8% (51) of the cases, one specimen proved positive for a given HPV-type while the other specimen from the same patient was negative. In 8.0% (26) of the cases, the biopsy specimen proved positive and the corresponding smear specimen was negative. On the other hand, in 7.7% (25) of the cases we were able to detect HPV DNA in the smear specimen, whereas the corresponding biopsy specimen was negative. We suggest that a smear specimen would be advantageous for screening large groups of patients for the presence of a HPV infection in the genital tract. By using smear specimens together with biopsy specimens it is possible to maximize the number of HPV infection diagnoses.

Endobrush sampling for endometrial cancer.

The two intra-uterine cytological sampling methods Endobrush and Pistolet were compared for clinical applicability in 66 premenopausal and 47 postmenopausal women. The taking of the specimens succeeded in 94% of the cases with the Endobrush method and in 99% with the Pistolet method. The two intra-uterine sampling methods were both almost painless. The Endobrush and the Pistolet specimens were filtered and stained by the Papanicolaou method. The Endobrush specimens were also used to make smears, which were also stained by the Pap method. According to the separate evaluations of two cytologists the Endobrush smear yielded specimens with a large or moderate number of cells in 59.0 to 71.4%, the Endobrush filter method in 73.6 to 76.5% and the Pistolet filter method in 71.4 to 76.8%. Specimens with good or moderate quality were found in 83.3%, 86.8 to 89.6% and 93.7 to 99.1%, respectively. Unsatisfactory specimens accounted for only 2.8-0.9% of the cases. All four endometrial carcinomas were placed in Pap classes 3 to 5 on the basis of the Endobrush and Pistolet filter specimens. The diagnostic quality of the smears was inferior to that of the filter specimens. The results suggest that the Endobrush filter method yields cytological endometrial samples which are similar in cell number, quality and diagnostic value to those obtained by the Pistolet method. Endobrush method is also simple, quick and painless, and therefore well acceptable to patients and suitable for clinical use.

Genetic susceptibility to multiple sclerosis. A co-twin study of a nationwide series.

The problems of differentiation between environmental and genetic influences on the development of multiple sclerosis are well known. Twin studies may provide valuable information on this question. However, most published twin series are selected and no through clinical twin studies based on epidemiologic series have been carried out. In this study, all available same-sex twin pairs with clinically definite multiple sclerosis derived from the Finnish Twin Cohort of 15815 pairs were studied by clinical evaluation, magnetic resonance imaging, and visual and auditory evoked responses. The mean length of follow-up of the pairs after the onset of symptoms of multiple sclerosis was 20 years. Two of the seven monozygotic pairs were concordant; one was definitely so, and in the other, the co-twin of the index case had, in addition to clinical findings, white matter changes suggestive of multiple sclerosis in magnetic resonance imaging and abnormal visual evoked responses. All six dizygotic pairs were discordant. The frequency of the HLA antigen DR2 in probands (69%) was significantly increased, but the distribution among the healthy subjects and patients showed nonsignificant differences. The results indicate a genetic influence on the susceptibility to multiple sclerosis, although still unknown genetic determinants are possible involved.

Large atypical polyps of the vagina during pregnancy with concomitant human papilloma virus infection.

A 25-yr-old primigravid patient with large atypical fibroepithelial polyps of the vagina is presented. The tumors necessitated interruption of the pregnancy by cesarean section at 34 wks. After delivery the tumors were excised, and the patient has been symptom-free for 12 months. There was an intercurrent human papilloma virus (HPV) infection in the vaginal epithelium. The nature of rare atypical vaginal fibroepithelial polyps is discussed on the basis of the literature. These lesions are clinically benign and are curable by simple excision, although histologically they resemble sarcomas.

Long-term recording and automatic analysis of cough using filtered acoustic signals and movements on static charge sensitive bed.

Reliable long-term assessment of cough is necessary in many clinical and scientific settings. A new method for long-term recording and automatic analysis of cough is presented. The method is based on simultaneous recording of two independent signals: high-pass filtered cough sounds and cough-induced fast movements of the body. The acoustic signals are recorded with a dynamic microphone in the acoustic focus of a glass fiber paraboloid mirror. Body movements are recorded with a static charge-sensitive bed located under an ordinary plastic foam mattress. The patient can be studied lying or sitting with no transducers or electrodes attached. A microcomputer is used for sampling of signals, detection of cough, statistical analyses, and on-line printing of results. The method was validated in seven adult patients with a total of 809 spontaneous cough events, using clinical observation as a reference. The sensitivity of the method to detect cough was 99.0 percent, and the positive predictivity was 98.1 percent. The system ignored speaking and snoring. The method provides a convenient means of reliable long-term follow-up of cough in clinical work and research.

Cognitive function in middle-aged snorers and controls: role of excessive daytime somnolence and sleep-related hypoxic events.

Association of snoring and cognitive function was studied in 46 habitually snoring men ages 41-52 years, and 60 occasionally or never-snoring control male subjects of the same age group. Sleep recordings with monitoring of apneas and hypopneas were made with the static-charge sensitive bed method. Blood oxygen saturation was measured with an oximeter and the snoring sounds were recorded with a microphone after clinical and neuropsychological assessment. A questionnaire with items on excessive daytime somnolence (EDS), sleep, and snoring quality was also used. EDS (as measured by items on the questionnaire) associated with tests requiring concentration, memory retention, and verbal and spatial skills in the habitual snorers group. The number of oxygen desaturation episodes exceeding 4% associated with defective delayed Recall of Logical Stories of the Wechsler Memory Scale and with spatial orientation (Clock test) in the habitual snorers' group even after adjusting for age and obesity.
