[Infusion speed as a factor to influence the analgesic level of spinal anesthesia--hypobaric solution of tetracaine].

In spinal anesthesia, as known, the injection speed of local anesthetic drug is a factor which influences the analgesic level. For study it, automatic infusion apparatus was available to control as 5 kinds of infusion speed (0.02-0.51 mL/s), and 78 patients were divided into 6 groups. The results of the painless level was discussed by T-test and distribution analysis (F-test). The mean of painless level in each group was found to be no significant difference as discussed by T-test. But the largest distribution was found in the group of 0.04 mL/s, and high analgesic level as rise up to T6 was occasionally found in groups of 0.04 mL/s & 0.02 mL/s. It means, the infusion speed is not an important factor, but extremely slow infusion is more dangerous than high speed infusion, which will take a risk of high level spinal anesthesia.

[A diffuse, pleural, malignant mesothelioma kept in long remission by chemotherapy combining pirarubicin and cisplatin].

Reported is the case of a 66-year-old male with a diffuse, pleural, malignant mesothelioma that has been successfully treated by combining cisplatin and doxorubicin. The patient received 100 mg of cisplatin and 50 mg of doxorubicin every 5 weeks, after which a regression of the tumor and a pleural effusion decrease was observed. After partial remission was achieved, the dosage was changed to 50 mg of cisplatin and 50 mg of pirarubicin on a five-week cycle. This led to a partial remission of two-years duration and the abatement of a doxorubicin-induced alopecia. The authors thus have concluded that a combined therapy of doxorubicin and cisplatin is effective for treating a malignant mesothelioma, and that pirarubicin is superior to doxorubicin with reference to side effects.

[Comparative study between film mammography and xeromammography; including specimen radiography].

We retrospectively evaluated preoperative film- and xeromammography of 23 cases with breast cancers, and compared with postoperative specimen radiography to assess tumor delineation and microcalcification detectability. In tumor detection and margin delineation, film mammography was superior to xeromammography, and in microcalcification, film mammography was equal to xeromammography. These results had a effect on the diagnosis of breast cancers.

[Analysis of adhesive proteins on the surface membrane of platelet in malignant neoplasm].

Flow cytometry was used to detect fibrinogen (platelet associated fibrinogen: PAFbg) and fibronectin (PAFn) on the surface membrane of platelets in leukemia (9 cases), lung cancer (15 cases) and hepatoma (8 cases) patients (by one color analysis method), and simultaneously to investigate the binding of monoclonal anti-glycoprotein (GP) IIb/IIIa and anti-GP Ib antibodies (by two color analysis). All patient groups showed higher Fbg values than the normal control group, but no differences were found between patient groups. The values of Fbg and Fn showed a correlation, but the pattern of binding did not show a regular tendency in any patient group. There also was no significant correlation between Fbg values and the positive percentage of monoclonal anti-GPIIb/IIIa and anti-GPIb antibodies, and the binding of Fbg did not inhibit that of monoclonal antibody. The results suggest the following. 1. There are no differences in platelet activation in leukemia, lung cancer and hepatoma patients, and the degree of platelet activation is decided by the degree and the kind of stimulation. 2. The increase of both PAFbg and PAFn prove to the existence of activated platelet.

[Chronic ITP with a remarkable response to vitamin C administration after splenectomy].

This report deals with a case of 23-year-old woman with chronic idiopathic thrombocytopenic purpura (ITP) showing a favorable response to vitamin C (VC) administration. She was treated with glucocorticoids and immunosuppressants, whereas inadequately controlled with its tolerable dosage. Next, she underwent splenectomy. However, there were only transient effects on platelet counts, so the therapy with ascorbate was carried out according to the report by Brox et al. The platelet counts increased to 20 x 10(4)/microliters after 4 months. Side effects of VC therapy did not occur.

[A case of cholesteatoma extending into the medial petrous bone].

A case of facial nerve palsy caused by cholesteatoma was reported. High-resolution CT was very excellent in delineating the mass extension and bony destruction. The labyrinthine segment of facial canal was thought to be the route of spread of cholesteatoma to the medial petrous bone as this case of poor pneumatized petrous bone.

[Analysis of platelet surface conformation in thrombin-induced aggregation].

We used flow cytometry to investigate the change of platelet membrane glycoproteins (GPIb and GP IIb/IIIa) and the distributions of fibrinogen (Fbg), thrombospondin (TSP) and fibronectin (Fn) on the surface of thrombin-stimulated platelets. The binding of a monoclonal antibody directed at the von Willebrand factor binding site on GPIb decreased in thrombin-stimulated platelets. This antibody caused a reactive delay in thrombin-induced aggregation, but had little influence on aggregability. Slight thrombin-induced aggregation was observed even after blocking the binding of Fbg to GP II b/IIIa. The new expression of GP II b/IIIa was detected on the surface of thrombin-stimulated platelets, whereas there was little increase of Fbg dependent on this GP II b/IIIa. An increase of TSP after thrombin stimulation was observed on the surface of platelets of healthy controls and patients with Glanzmann's thrombasthenia (Type I). The level of on platelet surface was slightly increased by thrombin stimulation. The mechanism involved in thrombin-induced aggregation appears to differ from that in ADP-induced aggregation.

[Analysis of anti-platelet antibody and platelet volume in idiopathic thrombocytopenic purpura].

We investigated platelets and plasma from patients with idiopathic thrombocytopenic purpura (ITP) to elucidate the antigenic determinants at which their autoantibodies are directed, and studied the relationship between anti-platelet antibody and platelet volume. We used flow cytometry to detect platelet-associated IgG (PAIgG), C3 (PAC3), IgM (PAIgM) and platelet volume, and also to determine the binding rate of monoclonal anti-platelet antibodies in patients with ITP. The following results were obtained. 1. Both anti-GPIIb/IIIa autoantibodies (21 of 71 patients) and anti-GPIb autoantibodies (3 of 71 patients) were found in ITP. 2. The decrease in platelet count in patients without anti-GPIIb/IIIa autoantibodies was significant. 3. The increase in platelet volume was found more frequently in patients with a platelet count less than 50,000 and in untreated patients. 4. There was a positive correlation between the platelet volume and PAIgM in patients with a platelet count less than 30,000 and high levels of PAIgM.

[Excretory urography with non-ionic contrast medium; clinical reassessment of the necessity of water deprivation].

The prospective study was performed to reassess the necessity of water deprivation before excretory urography (IVP) with non-ionic contrast medium. 35 adults were deprived of water before IVP and divided into two groups. First group was received 250 ml 5% glucose solution and lopamidol-300 (1 ml/kg, maximum dose; 50 ml). Second group received contrast medium only. Image quality of IVP of the two groups were compared. In the first group the visualization of ureter and bladder were better than the second group. There was no significant difference in the caliceal and pelvic visualization between the two groups. Water deprivation was suggested to be unnecessary in IVP with non-ionic contrast medium.

[Thrombotic thrombocytopenic purpura (TTP) associated with chromosomal aberration and treated with plasma exchange].

We reported a case of 69-year-old female presenting with clinically typical TTP which was treated with Ticlopidine and plasma exchanges four times in total and resulted in successful improvement of her clinical state. The first chromosomal analysis of lymphocytes in the peripheral blood of the patient revealed abnormal patterns of 45, XO/46, XX/47, XXX, the second, 45, XO/46, XX/47, XXX/47.XX, + 21. Measurement of FITC-labelled fibrinogen antibody against the fibrinogen combined with platelet glycoprotein GP IIb/IIIa complex using flow cytometry showed an apparently increased positive rate 76.2% for the platelet of the patient, compared with that of 31.9% for the control. From the comparative study of the platelet agglutination of the platelet rich plasma (PRP) derived from the citrated-added complete blood taken from the patient to whom Ticlopidine was given, added with adenosine-5'-diphosphate (ADP), collagen or acetyl glyceryl ether phosphoryl choline (AGEPC) as the reagents and that of normal control, it was shown that for AGEPC increased. Thus, increased reaction specific to AGEPC in addition to activated platelet was demonstrated in the present study.

[A case of remission induced in diffuse pleural malignant mesothelioma by the treatment with cisplatin and doxorubicin].

A 66-year-old male patient with diffuse pleural malignant mesothelioma was successfully treated with cisplatin and doxorubicin. He was admitted to our hospital because of massive pleural effusion. The diagnosis was made by pleural needle biopsy. He received 100 mg of cisplatin and 50 mg of doxorubicin every 5 weeks. Regression of the tumor and decrease of pleural effusion were observed after the treatment. A partial remission of 10 months duration was achieved. Cisplatin and doxorubicin were thought to be the most active cytotoxic agents in the treatment of malignant mesothelioma.

Cortical blindness caused by acute general cerebral swelling.

A 7-year-old girl who suffered from acute general cerebral swelling as a result of a traffic accident showed cortical blindness. Computed tomography (CT) scan on admission revealed marked slitlike ventricles and narrowing of the perimesencephalic cisterns, which indicated general cerebral swelling. While hospitalized, the patient developed transtentorial herniation twice on day 3, and CT scans at herniation episodes showed disappearance of the perimesencephalic cisterns. After recovery of consciousness, the patient showed cortical blindness, and during gradual recovery she showed pure alexia without agraphia. The visual evoked potentials at 8 weeks, 16 weeks, and 3 years 4 months after trauma showed normalization of the pattern, but revealed left occipital inactivity.

[A case of lumbosacral lipoma and its interesting histological findings].

A case of 3-month-old girl with lumbosacral lipoma is reported. She had a large soft tissue mass (4 X 5 cm) in the lumbosacral region initially noted at birth. Interpendicular distances below L2 were dilated on X-P. CT image demonstrated a sharply outlined low density area (approximately -80 H. U.) which occupied the latter half of the spinal canal in the level of L2 to S1 level. Defect of vertebral arch was also seen. Lipoma was removed subtotally with laminectomy. CT image demonstrated clear sharp margin of the tumor, neural tissue free zone were not found intraoperatively. Post-operative course was uneventful. Specimen showed the mature adipose tissue which contained rich blood vessels and connective tissue. Connective tissue was composed of collagen fibers and elastic fibers. Small aberrant nerve fibers and smooth muscle fibers were sporadically noted in specimens obtained from nearby transitional area of its lipoma and spinal cord. Although there were a few reports about the morphology of lipoma, the existence of nerve cell, neuroglia, embryonic bone, cartilage, smooth muscle fiber, striated muscle fiber, respiratory-like cell and others were reported in the previous reports. Our histological findings also suggest that the lipoma possibly arise from pluripotential caudal cell mass which survived by disturbance of the 3rd stage of neural tube formation (retrogressive differentiation).

Experimental infection of chickens with Cryptosporidium.

To study the clinical and pathological manifestations of Cryptosporidium infections, 24 h-old chickens were inoculated via four routes: orally, nasally, cloacally and by contamination, using 10(5) oocysts of Cryptosporidium in each case. The chickens began to shed oocysts of Cryptosporidium on days 7 to 8 after inoculation. There was no difference in the clinical manifestations or histological damage with respect to the inoculation routes. Various developmental stages of Cryptosporidium were also demonstrated microscopically in the smears or sections of bursa of Fabricii, larynx, trachea and cecal tonsils of inoculated chickens. Although the intestinal mucosa and respiratory tract mucosa were scarcely infected with the parasites, the bursa Fabricii was heavily infected in association with hypertrophy and hyperplasia of the lining epithelial cells.

Effect of high-frequency ventilation on extravascular lung water volume in dogs.

We studied the effect of high-frequency ventilation (HFV) (3 Hz for 2 h) on extravascular lung water in normal dogs and dogs with pulmonary edema. We used an endobronchial dividing tube to ventilate one lung with HFV and to keep the contralateral lung of the same dog static with an identical airway pressure, allowing each animal to serve as its own control. Results showed no change in the extravascular lung water with HFV compared with the static lungs in normal, mild pulmonary edema, and severe pulmonary edema groups. We conclude that HFV has no appreciable effect on lung fluid balance; the lymphatic system retains its ability to increase flow even with experimentally produced pulmonary edema.
