CT appearance of experimental thalamic radiofrequency lesion in dog.

The location and extent of thalamic lesions following thalamotomy can be identified as a large low density area in the acute stage to a small spot in the chronic stage. Stereotactic experimental radiofrequency lesions were placed in the thalamus of 35 mongrel dogs. CT images and whole brain specimens were obtained on days 0, 3, 7, 10, 14, 21 and 60. The appropriate time to map the lesions in the thalamus was thought to be about day 14, because of the thalamic size ratio and the correction of the ventricular deformity. The histological area of the lesion at day 14 corresponded better to the contrast-enhanced area than to that of the plain CT. These results suggest that the lesion demonstrated by contrast-enhanced CT scans at day 14 gives the best anatomical mapping of the actual histological defect.

Semisynthetic beta-lactam antibiotics. III. Synthesis and antibacterial activity of 7 beta-[2-(2-aminothiazol-4-yl)-2-(substituted carbamoylmethoxyimino)acetamido]cephalosporins.

Syntheses of cephalosporins modified with a 7 beta-[2-(2-aminothiazol-4-yl)-2-(substituted carbamoylmethoxyimino)acetamido] group at the C-7 position and with various hetero aromatics at the C-3 position are described. The effects of substituents on the carbamoyl group in the 7-side chain were investigated in order to improve antibacterial activity. Some of these compounds exhibited high antibacterial activity against Gram-positive and Gram-negative bacteria, including Pseudomonas aeruginosa, as well as good resistance to beta-lactamase.

[Observer variability in the histopathologic grading of prostatic carcinoma].

To evaluate the feasibility of histopathological grading systems for prostatic carcinoma, interobserver variability was studied. Fifty patients with prostatic carcinoma were independently observed by five experienced pathologists according to each grading system (Gleason, Mayo Clinic, Mostofi, Gaeta, M.D. Anderson Hospital and Japanese Uro-Pathological Committee). Consensus reading was made in each case, and subsequently a mean discrepancy score was calculated in each grading system. Among the six systems, the lowest discrepancy score was obtained by the Japanese Uro-Pathological Committee's. It seems that this system is the most suitable for the histopathological grading of prostatic carcinoma.

Evaluation Studies as Topic↗

Reliability of cytological grading of prostatic carcinoma compared with histological grading.

Thirty prostatic carcinomas diagnosed simultaneously by fine needle aspiration cytology and punch biopsy were graded into three groups according to their cytological and histological appearances. Among them 23 carcinomas were graded equally by both methods. Cytological examination appears to be suitable for grading prostatic carcinoma, thus being a possible indicator of the patient's prognosis and response to hormonal therapy. In addition to the results obtained here, fine needle aspiration cytology has many advantages such as simplicity, quickness and safety. With increase of attention of urologists and pathologists to fine needle aspiration cytology, this evaluation will become comparable to the histological examination not only as a routine diagnostic tool.

Zoonotic subcutaneous filaria of undetermined classification.

A necrotic adult male filaria was found in sections of a dense, fibrous, subcutaneous nodule excised from below the breast of a 61-year-old woman, resident of Mie Prefecture in Honshu, Japan. Owing to its necrotic condition, and to a lack of essential knowledge of the comparative micro-anatomy of filarial worms, a differentiation between Dirofilaria and Dipetalonema species, or member of a related group, was not possible.

[Atypical hyperplasia and latent carcinoma of the prostate as an early stage of carcinoma].

The prostate gland exhibits various borderline lesions and latent prostatic carcinoma is important as a preceding lesion of clinically manifest carcinoma. In this report, we describe a study of these lesions which was carried out using preparations from five different races. Atypical hyperplasia (AH) is the most important of these borderline lesions. Histologically, latent carcinoma exhibits a wide range, from poorly to well differentiated types, and is widely accepted to be the preceding state of clinically manifest carcinoma. Both lesions were common in all five races with a frequency several hundred times greater than the mortality rate for prostatic carcinoma. A significant correlation was found between the prevalences of AH and latent carcinoma, both of which had a tendency to show multifocal growth. Although there were many latent carcinoma cases exceeding 130 mm3 in size and poorly or moderately differentiated, rare cases would advance to clinically manifest carcinoma. Since the formation of tiny carcinomas and progression to clinically manifest carcinoma are two different phenomena, it is considered important to study the factors affecting the latter phenomenon.

[Adult identical twins with moyamoya disease].

Adult identical twins with Moyamoya disease are reported. Laboratory examinations such as blood types, AB, Rh(+), HLA (human leucocytic antigen) analysis [AW26(9), A26(10), BW62(15), BW61(40), CW3], and finger prints, and hairs confirmed that two patients were identical twins. Both had normal serum immunoglobulin levels and no anti-DNA antibodies. Patient No. 1, a 39-year-old female, was admitted to our service with chief complaints of nausea, vomiting and left hemiparesis with sudden onset on Oct 31, 1981. The patient had an episode of tetraplegia that occurred suddenly at 4 years of her age and lasted about a week. Motor weakness had completely disappeared but moderate mental retardation remained. She had been employed just prior to this attack. CT and angiographic examinations revealed ventricular hemorrhage with Moyamoya disease, which had miliary aneurysm in the branch of the right posterior choroidal artery. The operation of encephalomyosynangiosis was performed on her left hemisphere. The postoperative course was uneventful. Patient No. 2, the twin sister of patient No. 1, was in good health and mental state until 19 years of her age, when she suddenly lost consciousness and developed hemiparesis. After this attack, her motor function had recovered well. However, mental ability gradually deteriorated and she was admitted to a psychiatric ward. CT examination and angiographic studies were performed on Oct 12, 1982, and she was diagnosed as having Moyamoya disease.(ABSTRACT TRUNCATED AT 250 WORDS)

Massive hemoptysis from a pulmonary sequestration controlled by embolization of aberrant pulmonary arteries: case report.

A case of massive hemoptysis due to pulmonary sequestration is presented. Initially, the diagnosis of sequestration was unsuspected and bronchial artery embolization was done for management of hemoptysis from the left lower lobe, but 4 days later massive hemoptysis recurred. Repeat arteriography, including a thoracoabdominal aortogram, revealed two large abdominal arteries penetrating the left diaphragm and feeding the lower lung. Embolization of the aberrant artery from the celiac trunk and the left inferior phrenic artery resulted in complete hemostasis until elective surgical ligation of the two arteries was performed 2 months later. The massive hemoptysis from a pulmonary sequestration could only be controlled after embolization of the transdiaphragmatic aberrant pulmonary arteries.

Characterization of a novel activation-specific antiplatelet monoclonal antibody.

We developed a murine monoclonal antibody (KmT-50) which binds to a protein expressed on the surface of human platelets after activation. KmT-50 is an immunoglobulin G1 monoclonal antibody that recognizes a 50-kD antigen localized in intracellular organelles and the open canalicular system of unstimulated platelets. After platelets were stimulated, the antigen was secreted via the surface-connected canalicular system and exposed on the platelet membrane. KmT-50 bound not only to human platelets, but also to rabbit and monkey platelets. This new antibody may be useful for experimental studies on thrombosis and hemostasis.
