The use of CO2 laser in plastic surgery and dermatology.

The applicability of TLS61 CO2 developed in Hungary was investigated in dermatological-plastic surgery. This apparatus can be used with benefit in certain plastic operations, like lipectomy, dermabrasion, removal of keloid-tattoo and skin excisions.

Glomerulocystic kidney. A case report.

This paper presents a rare type of renal cystic disease involving the Bowman's capsule: the glomerulocystic disease associated with multiple malformation. Etiology or pathogenesis of glomerulocystic kidney remained unclear.

The role of catecholamines in lethal immobilization stress.

A 6 h immobilization stress was found to be lethal for rats weighing 125 g in average. Stress of 4 h was sublethal for the majority of these rats, however, simultaneous administration of adrenaline turned the sublethal effect to lethal. Death occurred in the fifth or sixth hour measured from the beginning of the stress, independently of the fact whether the stress was going on or not. Severe morphological changes in the heart muscle, lungs and kidneys were found.

Immunoperoxidase autopsy study of crescentic glomerulonephritis with extracapillary proliferation.

Twenty-five cases of extracapillary crescentic glomerulonephritis were examined immunohistologically. The peroxidase-antiperoxidase (PAP) method was used, the presence of immunoglobulin, complement factors and fibrin was analysed and glomerular positivity was found in all but two cases. Two types of reaction could be discerned: linear and granular, which can be considered as evidence of the pathomechanism. Surprisingly, IgG and IgM positivity was seen most frequently. The author emphasizes the importance of immunoperoxidase examination in the renal histology. This method can be easily applied in paraffin-embedded samples and gives many useful information.

Intraglomerular embolization by tubular cells.

Intraglomerular embolization by tubular epithelial cell is a rare artefact found in needle biopsy specimens. The authors have observed this phenomenon in five out of 300 renal biopsy cases. Tubular epithelial cells, were present in the glomerular capillaries (in two cases) or in the capillaries and within the glomerular urinary space (in three cases).

Use of CO2 laser knife in gastroenterological surgery.

Favourable results obtained by using the RT TLS61 CO2 laser of TUNGS-RAM, of a performance of 60 W are reported. It is emphasized that the device has several advantages in surgical gastroenterology over the traditional cutting instruments. The light of the device coagulates the smaller than 0.5 mm vessels during cutting. On opening the gastrointestinal tract-contrary to electrocautery-the necrotic zone is narrower, which may be of importance from the point of view of suture insufficiency. The device provides more favourable conditions for ablastic operations than do the traditional cutting instruments.

An unusual stress-induced gastric lesion.

An unusual type of gastric lesions, clearly different from erosions, was produced in rats by prolonged immobilization and application of adrenaline at the first and the third hour of stress. The lesions are large, round, highly hyperaemic elevations of the glandular part of the rat stomach. Necrosis of the superficial 2/3 of the mucosa with haemorrhages and leucocytic infiltration as well as marked oedema, focal haemorrhages, necrotic vasculitis and darkly stained phagocytes in the submucosa are the most prominent histological features of it. Some special conditions seem to be necessary for the development of this "superficial gastric necrosis".

[Basal membrane unevenness in glomerular amyloidosis].

Author has studied in 5 needle-biopsy and 25 autopsy samples the wall unevennesses of the glomerular capillary loops developing in kidney amyloidosis. In all cases subepithelial spikes consisting of parallel amyloid fibres were observed. In half of the cases, similar structures occurred also subendothelially. In silver-stained preparations they were readily identified by their increased argyrophilia. In all cases nodular features composed of irregular amyloid fibres were subendothelially present resulting in a considerable narrowing of the lumen. The spikes and nodules causing the unevenness of the basement membrane are of diagnostic value in glomerular amyloidosis.

[Intraglomerular tubular epithelial emboli].

A rare morphological phenomenon is described. In the glomeruli of a kidney needle-biopsy sample, intracapillary cell-clusters were found showing the characteristics of proximal tubular epithelium. The phenomenon is thought to be an artifact of needle-biopsy.

Use of laser knife in thoracic surgery.

CO2 laser knife was applied in 18 thoracic operations. The laser knife can be used for the sterile and ablastic cutting of skin, soft parts, ribs, sternum, bronchi and lung parenchyma. By its use intraoperative bleeding is substantially diminished.

[Initial experiences with the use of the surgical CO2 laser scalpel].

Reported in this paper is the surgical applicability, as established from 35 operations, of the Type TLS 61 laser scalpel of Tungsram Rt, 60 Watt in output. Useful experience has been recorded from surgical approaches to thorax cavity as well as from general, gastro-enterological, vascular, and dermatological surgery. Minimised bleeding and sterile incisions are benefits of laser operations. The above model has proved to be applicable to skin, muscles, lung, stomach, intestines, bones, and other tissues. Wound healing was absolutely okay. After-bleeding or other complications did not occur.

[Endoscopic thoracic sympathectomy].

Technique and advantages of thoracal sympathectomy, using the mediastinoscope, are described in this paper. Clear-cut evidence was produced to lasting suspension of sympathetic action during the follow-up period.
