Epithelial crescent in diabetic glomeruli. A case report.

The kidney as a target organ for secondary microvascular complications of diabetes mellitus represents a major problem. The pathology of diabetic glomerulopathy is well known. The coexistence of immunocomplex-mediated glomerulonephritis and diabetes mellitus has rarely been reported. The presence of crescents in glomerular disease of diabetes mellitus has been usually ignored in the literature. The present study describes one patient with epithelial crescentic diabetic glomerulopathy with rapidly progressive renal failure.

Extracapillary tumourous metastatic crescents in glomeruli of the kidney.

This report describes intraglomerular tumor cell metastases, as a rare pathologic observation. Specimens of one hundred autopsy cases (each having extrarenal carcinoma) were examined by light microscopy, and in seven of them metastatic crescents were found in the urinary space of the Bowman's capsule. The capsule itself and the layer of the parietal epithelium seemed to be intact. Glomeruli with metastases in five cases were near or inside the extraglomerular metastatic tumor mass. This phenomenon must be distinguished from metaplastic transformation of the glomerular capsular epithelium.

Surgical management of pulmonary arteriovenous fistulas.

In the post decade seven patients have been operated for pulmonary arteriovenous fistula at the 3rd Department of Surgery of the Semmelweis University Medical School, Budapest. Three of the patients were adults, the others being children and adolescents. Fistules were localized in the left lower lobe in four cases, and in the right upper lobe in three cases. Two of the patients displayed bilateral fistules. The type of the operation was lobectomy in six cases and enucleation in one case. The symptoms, the diagnostic and surgical methods and the results obtained are discussed.

Fetal malformation and serum alpha-fetoprotein concentration of diabetic mothers.

Diabetic pregnant patients belong to a high-risk group from a genetic counselling point of view. Therefore serum AFP estimation and ultrasound examination was carried out in 36 diabetic pregnancies, between the 16-20 gestational weeks. Healthy infants were delivered in 28 cases, fetal malformations or diseases were diagnosed in 3 cases (2 exencephaly and 1 polycystic kidney disease), 5 pregnancies ended up with stillbirth and/or severe fetopathy. According to our findings, the feto-maternal AFP balance is altered in poorly controlled diabetic pregnant patients. Therefore the maternal serum AFP concentration does not always reflect the AFP content of amniotic fluid. It is supposed that the low serum AFP concentration in diabetic patients is due to the disturbed AFP balance, which cannot be observed in well controlled pregnant diabetics. In the pregnancy of diabetic mothers both serum AFP and ultrasound examination should be carried out. With further improvement in the preconceptional treatment of diabetic patients not only the number of malformations caused by hyperglycaemia should diminish, but the problems of false negative and false positive serum AFP values caused by the disturbance in carbohydrate metabolism should also become less frequent.

[Surgical treatment of pectus excavatum by "turn over" of the sternum].

Operations were performed on 20 children for Pectus excavatum in five years. 'Turn over' of the sternum was chosen as surgical approach. About five years have passed from those operations, and results still are better than those obtained from earlier methods based on stabilisation of the sternum.

Thyroid hormones and thyroglobulin autoantibodies in insulin dependent diabetes mellitus.

Serum T4, FT4, T3, and TSH were measured in a group of children with insulin dependent diabetes mellitus and a control group. In the insulin dependent diabetes mellitus group, serum T3 concentration was significantly lower than the control values. Serum T4, FT4 and TSH level did not differ. The difference in serum T3 concentration was significant between diabetic children with good or poor control. Thyroglobulin antibodies were investigated in diabetic children by Serono's "hTg antibodies" kit. Thyroglobulin antibodies were present in 14.5%. TSH concentration did not differ in antibody positive and negative cases, but one child with diabetes had evidence of moderately impaired thyroid reserve.

Renal oncocytoma.

The author reports on a case of renal oncocytoma, showing up as a side finding during autopsy. In the light microscopic picture of the tumour typical tubular alveolar, in some places solid arrangement was observed. The cytoplasm of the tumour cells could be characterized by even, subtle granulation. Malignant transformation was detected. It has been proved by electron microscopic examination that the granular appearance of the cytoplasm originated from mitochondria. In the light of the literary data the author underlines the rarity and the diagnostical importance of the alteration. Oncocytoma should be distinguished from granular, cellular cancer of the kidney and from malignant oncocytoma.

In vivo model for the acute, latent and reactivated phases of cytomegalovirus infection.

Tissue samples from the salivary gland, lung spleen, liver and kidney of Balb/c mice infected with murine cytomegalovirus (MCMV) contained infective MCMV and virus-specific antigens for 6-7 weeks following the infection. After the 8th week no infective virus could be detected in any organ and antigens were found only in the spleen and the mesangial cells of the renal glomerules. As a result of cyclophosphamide treatment applied in the 22nd-24th weeks, the latent viral infection was reactivated in nearly all animals, infective virus production started again in the organs, and lasted for about 3 weeks. During the subsequent latent period the virus was again reactivated by repeated cyclophosphamide treatment. The experimental alternation of the latent and reactivated phases of viral infection renders the model suitable for study of the mechanism and consequences of viral latency and reactivation in vivo.

Quantitative analysis of the sympathetic efferent activity to characterize the integrative function of the sympathetic ganglion.

Using the concept of the Markov dependency, we analyzed the spontaneous activity in the pre- and postganglionic nerve trunks of the feline superior cervical ganglion. The time structure (order and value of time dependency) of the sequence of compound action potentials was determined. The analysis revealed that the time dependency of the postganglionic activity is of higher order and is generally more consistent than that of the preganglionic one. The difference in orders and values between the pre- and postganglionic activities was found to be a measure of the integrative function of the ganglion. A transformation by the ganglion of some additional information, inaccessible in the time structure of the preganglionic activity, seems to underlie this difference.
