Biliary decompression in the treatment of acute pancreatitis.

The etiology and treatment of acute pancreatitis are discussed. The two main principles are conservative and surgical treatment. Biliary decompression was chosen as an elective method in the surgical treatment of acute pancreatitis, if signs of bile stasis and tension of the common bile duct could be seen. Peritoneal or peripancreatic dialysis were also applied. Five patients were treated in this way; according to literature and our experience it is a good, short and curative intervention for the treatment of acute pancreatitis.

Focal-segmental glomerulosclerosis associated with herpes simplex viral encephalitis.

A 5-month-old boy presented nephrotic syndrome associated with severe viral encephalitis. From his various body fluids herpes simplex virus (HSV) type 1 and anti-HSV antibodies could be isolated. Renal biopsy showed focal-segmental glomerulosclerosis and cytomegaly within the mesangial region in a single glomerulus. Immunohistology demonstrated mesangial and segmental depositions of IgM and C3. HSV antigen could be detected in two of the glomeruli. Osmiophilic mesangial deposits, segmental sclerosis and foot process fusion of podocytes could be seen by electron microscopy. In the nucleoplasm of a mesangial cell of a single glomerulus typical viral particles were observed. To the best of the author's knowledge this is the first reported case in the literature on human herpes glomerulopathy confirmed by histological examination.

Shunt nephritis with crescent formation.

The case of a 17-year-old girl is reported, who presented shunt nephritis, 8 years after the ventriculo-atrial shunt insertion was made. Renal biopsy showed membranoproliferative glomerulonephritis. The symptoms of uremia mingled with those of chronic septic state. Persistent hypocomplementemia and cryoglobulinemia dominated the picture of the disease. At the beginning of the disease Staphylococcus aureus abunded in the blood culture. In spite of the therapy the general status of the patient declined steadily.

The importance of pulmonary complications as a cause of death in surgical patients.

The authors have retrospectively reviewed 390 autopsy reports from their surgical clinic, to establish the frequency of chronic and acute lung complications in surgical patients. The retrospective analysis revealed that pneumonia was the most frequent acute pulmonary complication among patients who died in the surgical department. In postoperative cases, pneumonia contributed to or was responsible for 65% of all deaths.

Thoracoscopic lung resection by the help of Nd:YAG laser.

The Swedish surgeon Jacobaeus was the first to use a lighted cystoscope for the lysis of pleural adhesions /5/. The same author reported, in 1921, five cases of intrathoracic malignancies diagnosed by thoracoscopy /6/. After the appearance of video-assisted cholecystectomy and abdominal surgery, video-assisted thoracic surgery (VATS) also occupied a major share in thoracic surgery. It was long ago that we introduced a thoracoscope (not the video-assisted type) for thoracic intervention. In 1988 we published 24 cases of thoracic sympathectomies in which we used the thoracoscope /16/.
