Serum ferritin level in infants and children with anaemia and malignant disease.

Serum ferritin was determined by immunoradiometry in children aged 6 months to 3 years, immediately before leaving hospital where they had been treated for various acute, non-haematological diseases. A low value was found in 13% of them. Serum ferritin concentration is a sensitive method in differentiating between iron deficiency and infectious anaemia. A significantly higher mean value was found in children affected by malignant disease (acute lymphoid or myeloid leukaemia, various solid tumours).

[Therapy of adult respiratory distress syndrome].

After a brief review of the pertaining literature, 10 patients treated for adult respiratory distress syndrome between 1974 and 1978 are reported. Eight patients died; in 4 cases the cause of death was some other irreversible lesion. Respirator treatment with the Bird-8 apparatus was supplemented with cardiac diuretic and antibiotic therapy. Improvement of the results can be hoped partly from prevention, partly from the early diagnosis of ARDS and its adequate treatment.

[Membrane structures in 13 cases of focal segmental glomerulosclerosis].

13 cases of focal segmental glomerulosclerosis have been studied in needle-biopsy material. In various structural elements of glomeruli electronmicroscopically MS of different shape and size were found. Contrary to the findings of other investigators similar elements in other types of glomerulopathia were not revealed. Author believes that presence of MS in focal segmental glomerulosclerosis is of diagnostic value.

Experimental glomerular lesions induced by chronic immune complex formation. II. Pathomorphological analysis.

Eleven rabbits were given bovine serum albumin i.v., in daily doses of 25 mg. Renal biopsy was performed on the 30th, 60th and 100th day of treatment and the specimens were subjected to light-, electron-microscopic and immunofluorescent studies, so as to follow up the dynamics of the glomerular process. Proteinuria and the serum creatinine and BUN levels were also measured. By the 100th day of treatment mesangioproliferative glomerulopathy had developed in 7, membranoproliferative glomerulopathy in 3 cases and membraneous glomerulopathy in 1 case. On the 30th day of treatment exudative glomerulopathy was demonstrable in the majority of the cases (in 9 animals). It is suggested that the earliest stage of the various glomerulopathies, regardless of their type, is marked by exudative lesions. The heaviest proteinuria was found in membraneous and membranoproliferative glomerulopathies. Changes in the serum creatinine and BUN levels indicative of a deteriorating renal function were noted in the membranoproliferative cases. The results are correlated with clinical observations of human glomerulopathies.

Granulomatous glomerulonephritis, without systemic disorder.

The case of a 12-year-old girl dying of renal insufficiency is described. At autopsy granulomatous glomerulonephritis was found without any systemic disease. The glomerular light-, immunofluorescent- and electron microscopic changes were studied. Four hundred glomeruli were analyzed by light microscopic investigation and graded (I-IV). By immunofluorescent investigation, intraglomerular and in the more severely affected glomeruli, periglomerular immunoglobulin deposition was detected. Electron microscopic investigation revealed mononuclear cells and fibroblasts in the glomerular and periglomerular cellular infiltrate, resulting in fragmentation and rupture of Bowman's capsule and loss of the glomerular structure.

Cell-mediated immunity in subjects immunized with HLA antigens.

Production of cellular immune reaction in subjects immunized against HLA antigens was studied by the capillary migration inhibition test in vitro and by the skin test in vivo. The majority of the immunized subjects (10 out of 13) developed a cell-mediated immunity to the donor antigens. Production of HLA antibodies was demonstrable in a smaller proportion of the cases. The complementary nature of humoral and cellular responsiveness was borne out by the present observations. The result of the skin test performed simultaneously with the migration inhibition test correlated well with the result in vitro.

Long-term results of the surgical treatment for pulmonary malformations and disorders.

Of the 1029 thoracic surgeries carried out since 1950 at the Second and later at the Third Surgical, University Clinic in Budapest on infants, children, and adolescents 215 were cases to correct some developmental anomaly. Pulmonary cysts--both single and multiple--represent the largest number, followed by lung-sequestration, lobar emphysema, and bronchial arterioma. To assess the late results, 106 ex-patients were invited to the clinic for check-up. They had beed operated at least 10 years before; the longest follow-up time was 23 years. Eighty-nine (83.9%) of the 106 patients appeared at the clinic. One pulmonectomized for a polycystic lung (0.9%) died in the meantime; 16 were not available. The 89 patients were submitted to a clinical examination, including radiological examination, spirometry, and ECG. With one exception (Kartagener syndrome), they were found in good condition and free of complaints. They carried on some studies or worked in different jobs. There were no relapses. They developed very satisfactorily no matter whether their operations took place in infancy, childhood or adolescence. The surgical intervention did not affect growth and development. Congenital pulmonary malformations and disorders should be treated by surgery, and the long-term results are better if the operation is performed at the proper time.
