[Treatment of obesity with a high-protein, high-fat, "carbohydrate-free" diet].

Sanatorium treatment for 27 days was carried out for 52 patients with obesity of alimentary metabolism type, consisting of moderate loading and high-protein, high-lipid diet with 2430 to 2840 cal daily, with unusually low carbohydrate contents--10 g for the first week and 38 g--in the fourth. The authors established a favourable total and an average daily body weight loss of the patients, irrespectively of the relatively high caloric intake. No sense of hunger was reported from the majority of the patients. The subjective complaints observed are more frequent and more severe, as compared with the other treatment regimens with reducing but balanced dietetic regimens. The majority of the subjective complaints established could be associated with the objectively confirmed compensated metabolic ketoacidosis. The unfavourable changes in the followed-up laboratory indices are: considerable decrease of blood sugar, massive ketonuria, elevation of serum uric acid and deviations in the acid-base parameters. Serum lipid indices fell under the effect of the treatment and body weight reduction. What impresses is the serum cholesterol reduction in spite of the high exogenic import, the diminution of cholesterol-lecithin index and the marked elevation of free fatty acid in serum, manifestation of incresed lipolysis in the lipid depots. (he diet indicated is not balanced and not physiological. It must be applied casually in obesity treatment. Its application is possible only after a strict assessment of each individual patient with obesity with no accompanying diseases, that do not agree with the partial fasting and cannot be included in intensive motor regimen.

[Desopimon in the treatment of obesity].

The authors tested the tolerance and therapeutic effect of desopimon preparation (1-p-chlorophenyl-2-aminopropane hydrochloride) in 44 subjects with obesity--I, II and III degree. They found a manifested appetite-inhibiting effect of the medicament in 41 of the patients and slight in two (the treatment was discontinued in a female, due to urticaria). Side effects, similar to those described with phenformin and phenmethrazin derivatives were not established. Body-weight loss is 12,9 kg per subject for a period of a month and a half, and the analysis of the data revealed that the weight loss was entirely on the account of the fatty tissue reduction. Triglyceride diminution was established in serum whereas free fatty acids were increased. After the confrontation of their own data those of some other researchers, the authors recommend desopimon as a suitable additive means in the treatment of simple nutritive (hyperphagic) obesity.

[Obesity in correlation with nutrition in representative settlements of the industrial-agrarian complex in Burgas].

The authors investigated the obesity incidence in connection with nutrition factors in 13 representative settlements of Industry-Agrarian Complex Burgas. They established that obesity covers 42,4 per cent of the population above the age of 14 and is 1,5 times more often among females as compared with males. The correlation among youths (age 14-25), mature aged subjects (26-50) and advanced aged ones (over 51) is 1:5,5:4,5. The light I and II stages prevail (88,6 per cent of all the cases) and the majority of the "pure" cases of obesity (73,6 per cent) have no complains.

[Treatment of obesity with the anoretic Sanorex].

Pathologically increased apetite is the most tormenting symptom of overweight conditions and therefore, along with dietetic treatment which plays an essential role, it is most expedient to resort to anorectic drugs also. Recently, the Sanorex preparation (Mazindol, Teronak, AN 448 degrees--imidazo-isoindole derivative having a tricyclic structure) is ever more extensively used. The action of the drug is studied in a series of 32 obesity individuals under sanatorial conditions and normocaloric diet, over a period of twenty seven days. The new preparation Sanorex which causes aversion to food, given in small doses, accounts for prompt and reliable control of the apetite--9590 kg reduction, 0,362 kg mean daily weight loss (p less than 0,05), and equal intensity of weight loss in either of the sexes (p less than 0,01). Side effects such as dryness in the mouth, very strong and moderate thurst, insomnia, superficial sleep, headache, elevated excitability, tachycardia, general feebleness are transitory, subside within two weeks, and are tolerated comparatively well by the patients included in the series. Sanorex administration may be recommended provided due consideration is given to the usual limitations implied by the continuous use of new drugs.

[Effect of alcohol and diet on some liver enzymes].

Liver is the main organ associated with alcohol oxidation. The definite liver enzymes role is not elucidated as yet, that participates in alcohol metabolism. The nutrient factors are of a great importance for the development of the alcoholic liver lesions and for the induction of liver enzymes under alcohol effect. Alcohol effect was studied in 80 male albino rats, Wistar strain, as well as some nutritional diets upon microsomal enzymes--cytochrome P-450, aniline-hydroxilase aminopyrine-demetilase., ADH and DALA-C. The changes, developed in the body weight are discussed as well as the liver weight and the microsomal protein under alcohol effect and the respective nutritional regimens. Alcohol, given in a dose of 4g/kg body weight, in the course of 60 days, induces but slightly the microsomal enzymes cytochrome P-450 and aniline-hydroxilase and does not induce aminopyrine-demetilase. ADH activity decrease with the chronic alcohol loading. Mitochondrial enzyme DALA-C is moderately induced by alcohol. Lipid and protein role is decisive in the induction process while the carbohydrate role is less.

[The choleretic effect of Bulgarian dill oil in white rats].

The authors present the results from the experiemental studies of choleretic action of the Bulgarian dill oil, produced by the firm "Bulgarian Rose" from the crop of 1970, on white rats. The authors establish that choleretic action of the dill oil is manifested comparatively slightly in uneven increase of the basic bile components. The highiest increase is found in respect to lipid complex, which at the end of the experiment is increased from 80, 11 mg to 98.2--7,7 mg or with 123, 48% (in 100% of the controls). The cholic acid is increased at the end of the experiment from 147,44 - 3,08 mg to 162,53--2,51 mg or with 110,23%. Inorganic phosphor and the amount of the secreted bile remain almost equal in the control and experimental animals. Comparing the choleretic action of the dill oil with the action of the Bulgarian rose oil, the authors stress its weaker effect and uneven action on the production of some bile components on account of which the dill oil should be used together with other drugs in the complex therapy of patients with inhibited bile formation.
