Interaction between human neutrophils and corneocytes: corneocyte-induced respiratory burst of neutrophils assessed by chemiluminescence.

In an in vitro study to examine whether corneocytes stimulate neutrophils to cause a respiratory burst, we found that stratum corneum (SC) homogenates obtained from the sole of healthy individuals induced a substantial respiratory burst in human neutrophils when assessed by lucigenin-dependent chemiluminescence (CL) in the presence of the fresh human serum. Electron microscopically, the interaction between corneocytes and neutrophils was shown as distinctive deformation of the neutrophils adhering to the surface of the corneocytes that suggested a specific binding between SC and neutrophils. In contrast, in the heat-inactivated serum-supplemented system, the peak intensity of SC-induced CL was significantly decreased, being only slightly higher than that noted in the SC-free background. To circumvent the time-consuming preparation of the SC homogenates, we measured CL with a sheet of SC obtained by stripping with adhesive cellophane tape. In this case we used plasma because the complement was easily activated by cellophane tape itself when serum was used. To evaluate the influence of the location of horny tissue in the SC on CL in neutrophils, we used SC sheets stripped with cellophane tape from various levels of the SC. However, there was no significant difference in CL responses between SC sheets obtained by 1, 2, 3, 5, 10, or 15 strippings. Our findings suggest that when SC comes in contact with serum, it is opsonized by C3b, and such SC causes a respiratory burst of neutrophils following their specific binding on the surface of corneocytes.

The production of transforming growth factor-beta in acute megakaryoblastic leukemia and its possible implications in myelofibrosis.

Acute myelofibrosis is often associated with acute megakaryoblastic leukemia (AMKBL). Although the exact mechanism for the progression of myelofibrosis in AMKBL is unclear, certain humoral factors from megakaryoblastic cells, the precursors of platelets, may be involved in the enhancement of collagen synthesis by bone marrow fibroblasts. The present study, therefore, is an investigation of the possible pathogenic role of transforming growth factor-beta (TGF-beta), known to be a very potent collagen-stimulating factor found in platelets in the myelofibrosis of AMKBL. The results obtained were as follows: (1) Conditioned media from peripheral megakaryoblasts taken from an AMKBL patient and from established megakaryoblast cell lines (MEG-01) had much greater stimulatory effects on collagen synthesis in bone marrow fibroblasts than conditioned media from other leukemic cell types. (2) Based on an assessment of soft agar colony formation, there was greater TGF-beta activity in media that had been conditioned from megakaryoblasts than in media from other leukemic cell types. (3) When compared with other leukemic-cell types, megakaryoblasts showed substantially greater expression of TGF-beta mRNA that was hybridized at 2.5 kb with a TGF-beta cDNA probe, and TGF-beta polypeptides were detected at 13 Kd with anti-TGF-beta antibodies. (4) The addition of the anti-TGF-beta antibody inhibited the stimulatory effects of the megakaryoblast conditioned medium on collagen synthesis in bone marrow fibroblasts. These results clearly suggest that megakaryoblasts produce and secrete an active form of TGF-beta and stimulate collagen synthesis in bone marrow fibroblasts in a paracrine manner.

Autologous mixed lymphocyte reaction is reduced in patients with psoriasis.

The autologous mixed lymphocyte reaction (auto-MLR) was studied to test the interactions between immunocompetent cells in patients with psoriasis. The auto-MLR in 20 patients with psoriasis was significantly lower than in 16 normal controls. Lower values were found in untreated psoriatic patients than in those in remission following treatment. The values in the latter group were significantly lower than in controls and in six patients with atopic dermatitis in remission. The tendency for an increase in the auto-MLR with a decrease in disease activity was further confirmed in five patients studied before and after treatment. In contrast, the allogeneic lymphocyte reaction (allo-MLR) in psoriatics was similar to that in normal controls.

Analysis of the proinflammatory property of epidermal cyst contents: chemotactic C5a anaphylatoxin generation.

We investigated in vitro the contents of epidermal cysts for complement activation and found that they activated complement mainly through the alternative pathway. Chemotactic C5a anaphylatoxin produced by the cyst contents after contact with serum most likely plays a significant role in the initiation and aggravation of inflammation in ruptured epidermal cysts. Our additional study disclosed that components of three representative follicular resident microorganisms (Pityrosporum ovale, Propionibacterium acnes, and Staphylococcus epidermidis) also produced C5a anaphylatoxin mainly through the alternative pathway; the C5a production was more vigorous than that by a virulent pathogen, Staphylococcus aureus. These results suggest that accidental colonization of the cyst contents by these follicular microbial flora further augments the inflammatory changes in ruptured epidermal cysts.

Effects of psoriatic scale extracts on oxidative metabolic responses in granulocytes assessed by chemiluminescence.

The peptide fractions containing C5a des arg and anionic neutrophil activating peptide (ANAP) and the low-molecular-weight fractions containing LTB4 in psoriatic scale extracts exert a potent polymorphonuclear leukocyte (PMN) chemotactic activity. To evaluate whether any of these components in the psoriatic scale extracts stimulate a respiratory burst in PMNs, we studied luminol-dependent chemiluminescence (CL) in PMNs under their direct stimulation. After fractionation by molecular sieve chromatography, pooled peptide fractions eluting near the cytochrome c marker from eight samples induced high CL in PMNs, whereas those eluted in low-molecular-weight fractions after vitamin B12 marker were very weak in this response. Moreover, in six samples such low-molecular-weight fractions eluting near the vitamin B12 marker were found to have a suppressive effect on the oxidative metabolism of neutrophils induced by other stimulators. These findings suggest that the fraction of chemotactic peptides in psoriatic scales plays a major role in the activation of PMNs, inducing tissue damage in the psoriatic lesional epidermis. On the other hand, at least some low-molecular-weight components in psoriatic scales seem to exert an inhibitory effect on the respiratory burst induced by other activators.

Stratum corneum activation of complement through the antibody-independent alternative pathway.

We provide evidence that stratum corneum (SC) activates complement through the alternative pathway to generate C5a anaphylatoxin. By immunofluorescence studies it was shown that in addition to circulating IgG autoantibody, there were anti-SC antibodies of IgM and IgA classes in the sera from normal individuals. However, all the titers were significantly lower than the level of C3 deposition between corneocytes. By contrast, no C1q deposition occurred. Immunoelectrophoretically the orthokeratotic SC homogenates were found to induce the conversion of C3 from native C3 to C3b in fresh human serum even when the classic pathway was blocked by Ca2+-chelation. Enzyme immunoassay showed that factor B split product, Bb, was generated by the SC homogenates in the Ca2+-chelated serum. Radioimmunoassay for C5a also demonstrated that the SC homogenates could generate C5a anaphylatoxin in serum to an extent similar to that in non-treated serum when restricted to the alternative pathway activation; neutrophil chemotactic activity was generated in Ca2+-chelated serum at levels comparable to that generated in non-treated fresh serum. We separated the SC samples into a cornified envelope and soluble and keratin fractions. The cornified envelope was more effective in activating complement. This activity resided in heat-stable and non-lipid substances of corneocytes. Our hypothesis is that when the SC comes in contact with serum, it activates complement mainly through the alternative pathway to induce chemotactic C5a anaphylatoxin. Hence, inflammation in normal individuals after a traumatic injury to the skin or rupture of acne comedones, or epidermal cysts and possibly the formation of subcorneal sterile pustules noted in several dermatoses are explainable through this mechanism.

Eczematous drug eruption from carbamazepine: coexistence of contact and photocontact sensitivity.

We report a 46-year-old patient with an eczematous eruption on sun-exposed areas which we believe was caused by carbamazepine intake. Oral provocation with the drug produced papulo-vesicular eruptions that were greatly potentiated by long-wave ultraviolet irradiation. Positive reactions to carbamazepine were elicited by patch and photopatch tests and by lymphocyte stimulation tests 6 months after cessation of the intake of carbamazepine.

Psoriatic lesions do not respond to oral administration of lobenzarit disodium, an inhibitor of interleukin 1 production.

Interleukin 1 (IL-1) has been implicated in the production of characteristic changes in psoriatic lesions such as epidermal hyperproliferation and accumulation of polymorphonuclear leukocytes within the epidermis. Because lobenzarit disodium, an immunomodulator, inhibits the production of IL-1 in peripheral and peritoneal macrophages, we studied the clinical effects of this compound on psoriatic lesions. A 12-week course of oral lobenzarit disodium treatment did not induce any remarkable changes in psoriasis lesions. This does not support the major pathogenic role of IL-1 in the development of psoriasis.

Idiopathic acquired generalized anhidrosis: electron-microscopic and immunohistochemical studies and analysis of lectin-binding pattern of the cell membrane.

Idiopathic acquired generalized anhidrosis with sudden onset is rare and its pathogenesis is not known. We report a patient with this condition, and the results of electron-microscopic and immunohistochemical studies as well as an analysis of the lectin-binding pattern of the cell membrane. Electron-microscopically, the cells of the eccrine sweat gland showed vacuolar changes. Besides their cytoplasmic changes, the results of a deficient lectin-binding pattern of the glandular cells suggest that there are disturbances in the formation of glycoproteins on their membrane.

Lucigenin-induced chemiluminescence in human neutrophils in the process of chemotactic migration measured in a modified Boyden chamber.

To the question of whether an oxidative metabolism of neutrophils occurs in the process of chemotactic migration, we have already demonstrated that formyl-peptide and zymosan-activated serum effectively induced lucigenin-dependent chemiluminescence (CL) in a specially devised Boyden chamber. In the present study we confirmed this and, furthermore, demonstrated that superoxide dismutase partially suppressed the neutrophil chemotaxis to formyl-peptide and concanavalin A, suggesting the participation of superoxide in a chemotactically migrating mechanism. However, monocyte-derived chemotactic factor did not cause any light emission, irrespective of its chemotactic activity. Based on these results, neutrophil chemotactic factors seem to be divided into two types, that is, oxidative metabolism-related and nonrelated. To the former group of chemotactic factors a premature activation of oxidative metabolism in neutrophils may start even at the initial stage of chemotaxis, and these primed cells may respond with a respiratory burst to higher concentrations of the chemoattractant itself or to other chemical mediators present at the site of inflammation.

Absence of IL-1 inhibitor in psoriatic scale extracts.

In the previous report we found that IL-1-like activity in horny tissue extracts from patients with psoriasis vulgaris and related sterile pustular dermatoses was remarkably low as compared to that in orthokeratotic horny tissue extracts prepared from non-inflammatory skin. In this report we studied the inhibitory activity of lesional horny tissue extracts from three psoriatics on recombinant IL-1-induced thymocyte proliferation in order to search for a possible coexistence of substances which may inhibit thymocyte proliferation. However, we failed to demonstrate any remarkable IL-1 inhibitory activity in each fraction after gel filtration high-performance liquid chromatography of the psoriatic scale extracts or in that of extracts from the plantar callus. We conclude that IL-1 stores are decreased in the pathologic horny layers of psoriatic lesions.

[Intraperitoneal (Ip) administration of cisplatin in combination with systemic chemotherapy in patients with malignant ascites of gastrointestinal cancer: clinical evaluation and pharmacokinetics].

Ten patients with carcinomatous peritonitis of gastrointestinal cancer have been treated with administrations of CDDP-Ip (50-150 mg/body), in combination with systemic chemotherapy simultaneously combined with sodium thiosulfate (STS) iv in 4 patients. All patients received transfusions of specific hydration. The pharmacokinetics of CDDP was studied in 4 patients, and the following results were obtained. (1) As for its effect on malignant ascites, according to Koyama's and Saito's criteria, 2 patients showed a complete response, 3 showed a partial response, and 5 showed no change. (2) As for its effect on the primary site, only one patient showed a partial response, and the others no change. (2) As to its side effects, 6 patients manifested nausea and vomiting, and only one showed a mild kidney dysfunction, and no myelosuppression. No significant difference in the side effects between the STS-combined group and the non-STS-combined group was found. (3) Serum CDDP levels were reached maximum 1-2 hours after Ip, and gradually decreased during the beta-phase of iv administration. Significant levels of non-protein-bound CDDP were found in the serum within 4 hours after Ip. These results indicate that the CDDP Ip is safe and useful for carcinomatous peritonitis of gastrointestinal cancer, however the combination of STS might weaken the systemic anti-cancer effect of CDDP by neutralizing the non-protein-bound CDDP in the serum.

Leukocyte chemotactic properties of soluble horny contents in epidermal cysts.

Epidermal cysts are one of the most common tumors of the skin. Although asymptomatic ordinarily, they may sometimes become severely inflamed with massive invasion of polymorphonuclear leukocytes (PMN). We studied in vitro PMN chemotactic properties of the aqueous extract prepared from the horny material of epidermal cysts obtained from three patients. A crude aqueous extract of the horny content of the cysts showed PMN chemotactic activity, which, however, was less than that of a horny layer extract prepared from normal skin. Characterization of PMN chemotactic activity using a Sephadex G-75 column showed a peak in the low molecular weight fractions eluting between the vitamin B12 and phenol red markers, which corresponds with the peak of absorbance at 280 nm. The chemotactic substances withstood boiling and trypsin or protease digestion. Although the chemotactic activity was partially ether-extractable, the presence of leukotriene B4 was not demonstrated by radioimmunoassay. In addition to their own chemotactic activity, the horny extracts of epidermal cysts showed cytotaxigenic properties in the presence of fresh serum.

HLA-DR antigen expression on keratinocytes in highly inflamed parts of psoriatic lesions.

We have investigated the immunoperoxidase staining pattern in the epidermis and dermal infiltrates of highly inflamed portions of psoriatic lesions, selecting for biopsy early pinpoint lesions or margins of active plaque lesions. We found positive intercellular staining for HLA-DR antigens in localized areas of the epidermis in about half of the patients tested. In contrast, OKT6 antigen was found only on the dendritic cells in the epidermis and dermis in all cases. These findings support the hypothesis that an active cellular immune reaction involving the epidermis, possibly associated with the expression of HLA-DR antigens on keratinocytes, occurs in the highly inflamed areas of psoriatic lesions, particularly in early pinpoint lesions or at the edges of spreading plaque lesions.
