Plasma anaphylatoxin concentrations in inflammatory skin diseases.

To study the possible involvement of the complement system in inflammatory skin disorders, we measured the concentrations of C3a and C4a anaphylatoxins in the peripheral blood of 148 patients with various inflammatory skin disorders and 48 healthy control subjects by radioimmunoassay. Significant increases in mean levels of both C3a and C4a anaphylatoxins were found in 59 patients with psoriasis. Remarkable increases in both C3a and C4a anaphylatoxins were also noted in some patients with leukocytoclastic vasculitis, urticarial vasculitis or unspecified toxic erythema. On the other hand, elevation of C4a alone was noted in a case of systemic lupus erythematosus. In contrast, using the mean of the normal control +/- 2 S.D., no significant anaphylatoxin elevation was found in 16 patients with pustulosis palmaris et plantaris, 7 with pityriasis rosea, 3 with erythema multiforme, and 3 with erythema nodosum or autoimmune bullous dermatoses.

Lucigenin-induced chemiluminescence in human neutrophils in the process of adherence and chemotactic migration measured in a modified Boyden chamber system.

To answer the question of whether an oxidative burst in neutrophils occurs in the process of adherence and chemotactic migration, we estimated their lucigenin-induced chemiluminescence (CL) responses in a specially devised Boyden chamber that utilized a vial for CL measurement. Neutrophils placed in the upper compartment immediately after isolation induced lucigenin-dependent CL even in the absence of chemotactic factors in the lower chamber, which was completely scavenged by superoxide dismutase at a concentration of 100 micrograms/ml. This response was suggested to be caused by neutrophils in the process of adherence to a filter. When formyl peptide (FMLP) or zymosan-activated serum (ZAS) was placed in the bottom compartment, they were effective to induce CL in the neutrophils already adhered to the membrane filter. These chemotactic factors induced light emission in neutrophils maximally at the concentration of 10(-7) M for FMLP and 5% for ZAS in the reaction mixture without any more increase in CL being observed above these concentrations. This makes a sharp contrast to the findings observed in the so far used unphysiological experimental system, in which neutrophils are abruptly exposed to chemotactic factors likewise in present study. When FMLP or ZAS was directly added to the cells in a upper compartment, CL increased dose-dependently from 10(-9) to 10(-5) M for FMLP, and from 5 to 50% for ZAS.(ABSTRACT TRUNCATED AT 250 WORDS)

[Combination chemotherapy of UFT with adriamycin in advanced gastrointestinal cancer].

Seventeen patients with advanced gastrointestinal cancer were treated with a combined chemotherapy of UFT with Adriamycin (UFT-A). The UFT-A regimen consisted of UFT, 600 mg/body daily. As for the Adriamycin, 10 mg/body was give intravenously from day 1-4 and was repeated every two weeks. Partial responses were seen in 7 cases (41%) (5 cases of gastric cancer, 1 case of colon cancer, and 1 case of bile-duct cancer) out of 17 evaluable patients (7 cases of gastric cancer, 3 cases of colon cancer, 4 cases of biliary tract cancer, and 3 cases of pancreatic cancer). Two patients had minor responses, and in eight patients their disease had stabilized. As for side effects, nausea and vomiting occurred in seven patients (41%), and anorexia was observed in eight patients (47%). Two patients (12%) showed a leukopenia count of less than 2,000/mm3 and none of these seventeen patients had thrombocytopenia, of less than 5 x 10(4)/mm3. Considering these results, UFT-A therapy appears to be useful in cases of advanced gastrointestinal cancer, especially gastric cancer.

Incontinentia pigmenti: eosinophil chemotactic activity of the crusted scales in the vesiculobullous stage.

To investigate the mechanisms underlying eosinophil infiltration into the epidermis in incontinentia pigmenti (IP), we studied the eosinophil chemotactic activity in extracts of the crusted scales from three patients with IP in the vesiculobullous stage. Eosinophil chemotactic activity was detected in the eluates from a Sephadex G-75 chromatography column between the vitamin B12 and phenol red markers. The chemotactic activity was heat-stable and resistant to enzyme digestion, and recovered after ether extraction at low pH. Leukotriene B4 (LTB4) was demonstrated in the fractions with high eosinophil chemotactic activity. These findings suggest that LTB4 plays an important role in the accumulation of eosinophils within the epidermis in IP, in the vesiculobullous stage. Blood eosinophilia, however, may not be induced by the eosinophil chemotactic factors in the scales.

Successful treatment of psoriasis with topical application of active vitamin D3 analogue, 1 alpha,24-dihydroxycholecalciferol.

We treated 11 psoriatic patients with topical 1 alpha,24-dihydroxycholecalciferol, a new synthetic analogue of active vitamin D3. In 10 of 15 tests the lesions cleared completely within 1-4 weeks, although some relapses occurred shortly after cessation of treatment. There were no side-effects. We suggest that 1 alpha, 24(OH)2D3 merits further investigation as a potentially useful topical therapy for psoriasis.

Immunohistochemical studies on epithelial cells in mixed tumor of the skin.

We performed studies on the lectin-binding pattern in epithelial tumor cell components of 4 cases of mixed tumor of the skin developing on the face in addition to identification of keratin and carcinoembryonic antigen (CEA), compared with those of normal sweat glands. Normal eccrine glands showed specific labelling with Dolichos biflorus agglutinin (DBA) and soybean agglutinin (SBA), whereas none of the studied lectins reacted specifically with normal apocrine glands. In mixed tumors the dark cells, which form the inner layer of the tubuloalveolar and ductal structure, showed the presence of keratin and CEA, as well as of specific sugar structures that bind to DBA and SBA. On the other hand the light cells that form the outer layer of the tubular structures or the solid epithelial cell nests gave only a faint to moderate staining of keratin, and no staining of CEA or lectins. It is probable that the inner dark cells differentiate toward the cells that have the same sugar structures on the cell surface as those of the normal eccrine gland cells, while the outer light cells appear to be immature or in a less differentiated state.

Effect of interleukin 1 on chemotaxis and chemiluminescence of human neutrophils.

We studied the influence of interleukin 1 (IL 1) on chemotaxis and chemiluminescence (CL) of human neutrophils. We found that IL 1 showed chemotactic activity within the concentrations ranging from 1 to 200 U/ml, whereas no luminol-or lucigenin-dependent CL was noted in neutrophils with the stimulation of IL 1 at the concentrations ranged from 1 to 100 U/ml. Moreover, no enhancement of zymosan-induced CL was observed in neutrophils following prior exposure to IL 1 at a concentration of 100 U/ml for 60 min at 37 degrees C. We think that IL 1 may be responsible for the inflammatory reaction only through its chemotactic activity for neutrophils without enhancement of the oxidative metabolism of neutrophils.

Demonstration of leukotriene B4 in the scale extracts of psoriasis and inflammatory pustular dermatoses. Correlation with leukocyte chemotactic activity and C5a anaphylatoxin.

We carried out quantification of the levels of leukotriene B4 (LTB4), a highly potent cell membrane-derived leukocyte chemotactic factor, in scale extracts of psoriasis and related inflammatory dermatoses characterized by sterile subcorneal pustule formation by using radioimmunoassay. A small amount of LTB4 was demonstrable even in extracts from non-inflammatory stratum corneum, but larger amounts were detected in the scale extract from pustular psoriasis and in those from psoriasis vulgaris. There was a significant correlation between the LTB4 and C5a levels in scale extracts, suggesting that complement activation and generation of LTB4 are a closely related event in psoriatic lesions. However, in contrast to highly significant correlation noted between the amount of C5a and chemotactic activity for polymorphonuclear leukocytes (PMN) detectable in scale extracts, LTB4 levels correlated only marginally with the chemotactic activity.

Presence of chemotactic peptides other than C5a anaphylatoxin in scales of psoriasis and sterile pustular dermatoses.

A unique leukocyte chemotactic factor of around 12 kD molecular weight has been suggested to be responsible for the mechanisms involved in transepidermal migration of leukocytes observed in psoriasis and related sterile pustular dermatoses. Although we confirmed with radioimmunoassay the presence of a C5-cleavage product in the chemotactic fractions from psoriatic scale extract eluted by gel filtration HPLC, the neutrophil chemotactic activity demonstrated in the peak fraction was only partially inhibited by rabbit antiserum to human C5a, whereas that noted in the peak fraction, prepared from zymosan activated serum, was totally abrogated by the same treatment. Similarly the chemotactic fraction prepared from the scale extracts of other related pustular dermatoses were only partially inhibited with anti-C5a antiserum. These findings suggest that chemotactic peptides other than C5a also play a role in the transepidermal migration of leukocytes in these dermatoses.

[Alpha-fetoprotein producing gastric cancer responding to combined chemotherapy with UFT and adriamycin].

A case of AFP producing gastric carcinoma with liver metastasis that showed marked response to combined chemotherapy with UFT and Adriamycin (ADM) is reported. A 61-year-old man was admitted because of lassitude and abdominal fullness. An upper GI series and computed tomography revealed gastric cancer (Borrmann III) and multiple liver tumors. He had a remarkably high serum AFP level (90,000 ng/ml) and a high CEA level (270 ng/ml). The presence of AFP in the tumor cells of the biopsy specimen was proved immunohistochemically. He was treated with 600 mg of UFT orally every day and ADM (10 mg, iv, on days 1-4, repeated every 14 days), resulting in marked regression (PR, partial response) of both the primary tumor and liver metastasis on the 33 rd day after the start of treatment, with decreasing of serum levels of AFP and CEA. The patient has been asymptomatic without evidence of recurrence for a follow-up period of more than three months with continuing treatment in our outpatient clinic. UFT-ADM therapy appears to be useful for gastric cancer.

Effects of HC 20-511 (ketotifen) on chemiluminescence of human neutrophils.

The effects of ketotifen on the chemiluminescence (CL) and chemotaxis of human neutrophils were studied in vitro. Stimulations of neutrophils by concanavalin A (Con A), calcium ionophore A23187 (CI) and formyl-methionyl-leucyl-phenylalanine (FMLP) were strongly suppressed in contrast with that by zymosan, which was only slightly inhibited in CL assay; the inhibitory effect of ketotifen was dose dependent. The addition of ketotifen even after stimulations of CI' and FMLP also inhibited CL response. The neutrophils which were activated by incubation with FMLP (10(-7)M) at 37 degrees C for 60 min emitted much greater light in CI-induced CL assay than those without the pretreatment with FMLP. Such enhanced CL of activated cells was also markedly suppressed by ketotifen. On the other hand, ketotifen did not show any inhibitory effect on the direct movement of neutrophils by FMLP (10(-7)M) at the concentrations which inhibited CL responses. These unique pharmacological activities of ketotifen are encouraging for its potential clinical usage as an antiinflammatory agent in some disorders associated with neutrophils as well as for its experimental usefulness for the analysis of various functions of neutrophils.

Suppression of zymosan-induced chemiluminescence of whole blood by conjugates composed of drug, guinea pig skin extracts and sera from patients with suspicious drug eruptions.

Conjugates composed of drug, guinea pig skin soluble fraction, and sera from 50 patients with various types of drug eruptions were prepared by the incubation of these mixtures for 60 min at 37 degrees C. And the effects of conjugates on zymosan-induced chemiluminescence (ZI-CL) in whole blood were examined. The conjugates obtained from 32 cases significantly suppressed ZI-CL of whole blood. The mean value of peak intensities of CL in the presence of their conjugates showed 62% of those after the pretreatment of mixtures consisting of drug, soluble fraction and sera from healthy donors. In the systems of non-opsonized and opsonized zymosan-induced CL of leukocyte-erythrocyte sediment, similar inhibitory effects were obtained in both systems in the presence of conjugates, but the time course of CL response was slower in the system with non-opsonized zymosan. In other experiments with the use of immune complexes, which were formed by human albumin and antihuman albumin goat serum in vitro, soluble immune complexes suppressed ZI-Cl of whole blood. These results suggest that immune complexes may be formed during incubation of conjugates and block the receptors on the surfaces of granulocytes, resulting in the suppression of ZI-CL.

Studies on fungal polysaccharide. XVII. A new glucuronan "protuberic acid" produced by a fungus Kobayashia nipponica.

A water-soluble glucuronan "protuberic acid", [alpha] 22-D minus 83.6 degrees, was isolated and purified from Kobayashia Nipponica, and its physicochemical properties were investigated. The purified protuberic acid was homogeneous as shown by zone electrophoresis, gel filtration over Sepharose 4B, and ultracentrifugation. The sedimentation coefficient was 1.8 S and its intrinsic viscosity was 1.1 dl/g. By gel filtration the molecular weight was estimated to be about 170 000. The results of periodate oxidation, methylation analysis, and partial acid hydrolysis indicated that this acidic polysaccharide has a linear structure of mainly 1, 4-linkages and containing an acid-labile linkage. Reduced protuberic acid, [alpha] 22-D minus 44 degrees, is also described.
