Alterations in fructosamine and glycated albumin levels during childhood.

Age-related changes in the blood concentrations of glycated proteins during childhood and adolescence were examined by measuring the blood fructosamine (FA), glycated albumin (GA), glucose levels and total serum protein levels in 611 healthy children (325 boys and 286 girls) aged 0-18 years old. These levels were compared with those of 130 healthy adults. GA was measured by a newly developed, highly precise method of high-performance liquid chromatography (inter-assay coefficient of variation, 4.9%). The FA values of all groups of up to 15 years old (grouped in two year age brackets) were significantly lower than those of adults. The mean serum concentrations of GA in infants up to four years old were also significantly lower than in adults. A slight but statistically significant positive correlation was found between age and values of FA (r = 0.36, P less than 0.001), but not of GA (r = 0.04). Levels of total serum proteins showed a positive correlation with those of FA (r = 0.47, P less than 0.001) but a weak correlation with those of GA (r = 0.29). These results suggest that age should be taken into account when the FA levels of children and the GA values of infants are evaluated.

Crystal and molecular structures of carbon-bridged pyrimidine cyclonucleosides. Substrate analogs of ribonuclease A.

The crystal and molecular structures of carbon-bridged 6,5'-cyclo-5'-deoxy-4-thiouridine (6,5'-Cs4U), 6,5'-cyclo-5'-deoxy-2',3'-O-isopropylideneuridine (6,5'-CiU) and 6,6'-cyclo-5',6'-dideoxy-allofuranosyluracil (6,6'-CU) have been determined by X-ray diffraction. The molecular conformations of 6,5'-Cs4U and 6,5'-CiU are very similar; the conformation about the glycosidic bond is anti (low region), the torsion angle O(4')-C(1')-N(1)-C(2) being -150.0 degrees for 6,5'-Cs4U and -145.5 degrees for 6,5'-CiU, and the sugar puckering being both O(4')-exo. On the other hand, 6,6'-CU takes the glycosidic torsion angle of -116.9(4) degrees (middle anti region) and the sugar conformation of C(4')-endo. The cyclization causes little alteration in the geometry of the base moiety. 6,5'-Cs4U and 6,5'-CiU exhibit the similar base-base interactions between adjacent molecules, although their molecular packings are quite different; the 4-thiouracil or uracil moiety interacts with adjacent base moieties through hydrogen bonding and stacking interactions. In 6,6'-CU, cyclonucleosides were connected by hydrogen bondings between the hydroxyl and sugar ring oxygen atoms and between the hydroxyl groups and the base nitrogen and oxygen atoms. As the 2',3'-cyclic phosphates of these carbon-bridged cyclonucleosides are hydrolyzed by ribonuclease A, it is suggested that the conformers found in these cyclonucleosides are recognized by the enzyme.

Studies on antitumor agents. IX. Synthesis of 3'-O-benzyl-2'-deoxy-5-trifluoromethyluridine.

A practical synthesis of 3'-O-benzyl-2'-deoxy-5-trifluoromethyluridine (1), a candidate antitumor agent for clinical testing, was developed from 2'-deoxy-5-iodouridine (3). Benzylation of 2'-deoxy-5-iodo-5'-O-trityluridine (14) with benzyl bromide and sodium hydride in tetrahydrofuran gave the 3'-O-derivative (16). Benzoylation of 16 afforded the N3-benzoyl derivative (17). Coupling of 17 with trifluoromethylcopper, prepared from bromotrifluoromethane and copper powder in the presence of 4-dimethylaminopyridine, gave the 5-trifluoromethyl derivative (19) minimally contaminated with the 5-pentafluoroethyl compound. Deprotection of 19 furnished 1.

Studies on the chemical modification of monensin. II. Measurement of sodium ion permeability of monensylamino acids using sodium-23 nuclear magnetic resonance spectroscopy.

A technique to assay Na+ ions in cells is presented. Intracellular and extracellular Na+ ions in a suspension of guinea pig erythrocytes were conveniently determined by using sodium-23 nuclear magnetic resonance (23Na-NMR), in combination with two anionic shift reagents: Dy(TTHA)3- and Dy(PPPi)2(7-). Monensin (1), monensylalanine (2b), monensylserine (2c), and monensylphenylalanine (2d) induced large increases of intracellular Na+ ion concentration ([Nain]), while monensylglycine (2a), monensyltyrosine (2e), monensylaspartic acid (2f), and monensylglutamic acid (2g) showed slight increases. The values of initial increasing rate (Vi) of 2a-g were much smaller than that of 1. This fact was probably due to the lower lipophilicity of 2a-g than 1, because a good correlation was observed between Vi and Rm50 values of 1 and 2a-g. This lower lipophilicity is a consequence of conformational differences between 1 and 2a-g.

Evaluation of a one-way airflow system in an animal room based on counts of airborne dust particles and bacteria and measurements of ammonia levels.

Air cleanliness in the working area of an animal room equipped with a conventional turbulent flow air distribution systems was compared with that in a similar room fitted with a one-way-flow air distribution system; in this, the supply air flowed from the working area through the racks of cages and was removed from the exhaust side. Before the introduction of animals, the air in the working and exhaust areas of both rooms was ascertained to be Class 100. With animals in situ, however, whereas in the turbulent airflow room both the workspace and exhaust air reached about Class 10,000 (with particle counts, bacterial counts and ammonia levels being almost the same) in the one-way-flow room, the air in the work space only went up to about Class 1000. With the addition of sliding doors or curtains in front of the rack in the one-way-flow room the work space air was maintained at Class 100 with almost no dust particles over 1 microns in size, airborne bacteria or ammonia being detectable. A comparison of all factors measured showed that whereas in the turbulent flow room the contamination of the work space air was 91% of that of the exhaust air, in the one-way-flow room it was only 47%, with curtains added this was reduced to 7% and with sliding doors to only 2%. In the latter case, contamination levels increased markedly on both sides during and immediately after cage changing, but recovered to the pre-cage changing levels within 30 min in the personnel working area and within 60 min on the exhaust side.

[Urinary alkylphosphate levels as an index of exposure to organophosphorus insecticides in pest control operators].

In pest control operations, organophosphorus compounds (OP) have been sprayed as insecticides, blood cholinesterase (ChE) activities and urinary alkylphosphate levels were measured for both OP-sprayers (n = 102) and non-sprayers (n = 35) in pest control companies, and the relationship between the analytical results and spraying conditions was investigated. Plasma ChE activities of the OP-sprayers and of the non-sprayers, as well as blood cell ChE activities of the OP-sprayers, were significantly lower than those of employees of chemical factories who served as controls (n = 103). Urinary alkylphosphates were not detected in the controls at all but were detected in 83% of the sprayers and in 57% of the non-sprayer employees of pest control companies. A significant negative correlation (r = -0.34, n = 137, p less than 0.01) between the plasma ChE activities and urinary total alkylphosphate levels was observed in employees of pest control companies. In the case of the employees whose urinary total alkylphosphate levels were 0.1 mumol/g creatinine or more, the plasma ChE activities were 25% decreased compared to the control group. The OP-sprayers showed significantly higher urinary total alkylphosphate levels than the non-sprayers. The OP-sprayers were further divided into two groups. One was sprayers engaged in both cockroach- or fly-control and termite-control operations (sprayers I, n = 54) and the other was sprayers engaged in only cockroach- or fly-control operations (sprayer II, n = 48).(ABSTRACT TRUNCATED AT 250 WORDS)

[Relationship between exposure to chlorpyrifos and concentration of urinary alkylphosphates in termite control workers].

Chlorpyrifos (O, O-diethyl-3,5,6-trichloro-2-pyridinyl phosphorothioate; CP) has been used for insecticide to prevent infestation of termites. A study was made on occupational exposure to CP of termite control workers engaged in spraying this organophosphorus insecticide. Workers were engaged in two types of spraying; that is spraying CP under floor and spraying CP around house. All the under-floor sprayers wore respirators during spraying. CP levels in air near the breathing zone, contact amounts of CP with skin, and urinary alkylphosphates levels were higher in the under-floor sprayers than in the around-house sprayers. The personal ambient CP levels of the under-floor sprayers were 0.012 to 0.145 (8 h-TWA, mg/m3) lower than the threshold limit value (0.2 mg/m3) proposed by ACGIH. A maximum diethylphosphate (DEP) excretion was observed in the first urine collected the next day, while the highest diethylthiophosphate (DETP) was excreted in the last urine in the working day. In the excretion amount, DEP was higher than DETP. Amounts of CP collected in respirators used by the under-floor spraymen averaged 0.76 mg/respiratory comparable to their estimated theoretical value of 0.69 mg/m3. The urinary total alkylphosphate (DEP + DETP) level was more closely correlated with the contact amount of CP with skin (r = 0.86) than with the personal ambient CP level (r = 0.05). These results indicate that 1) respiratory absorption of CP was prevented by the use of respirator; 2) the urinary alkylphosphates were mainly derived from the dermal absorption of CP; and 3) biological monitoring using urinary alkylphosphates levels is an useful index for CP exposure.

[A questionnaire study on health of taxi drivers--relations to work conditions and daily life].

A questionnaire survey was conducted on 5,523 taxi drivers in Osaka Prefecture to investigate their working conditions and daily life as well as characteristics of their health condition and various effects on health, especially on their cardiovascular system. The major results of this survey were as follows: 1) The rate of subjective symptoms, morbidity rate and rate of poor physical condition over the past year in the taxi drivers were higher than those of the control group. Among the subjective symptoms, the rate of gastrointestinal disorders, fatigue, musculoskeletal system disorders, sensory system disorders, hemorrhoids, etc. was especially high. 2) Work shift, density of work, years of experience in taxi-driving, frequency of fright while driving, pattern of taking meals, way of recuperation on rest days or holidays, obesity, smoking, and intake of coffee and alcohol were found to be factors affecting the health of taxi drivers. 3) Nearly half of the respondents said that they would like to quit or change their job with as much as 62% giving "condition of health" as reason. On the other hand, the rate of heart-related symptoms such as palpitation, and breathlessness did not differ from that of the control group. The possible reason for this deduced from the foregoing results, is that there were some who had changed or quit the job at an early stage for health reasons such as heart trouble and severe physical and mental burden resulting from taxi driving. 4) Many taxi driver are obese and the rate of those with heart-related symptoms was considerably high among those classified as obese. In addition, the results showed that those with longer driving experience tended to be obese.

Molecular cloning and nucleotide sequence analysis of the putative cDNA for the precursor molecule of the chicken LH-beta subunit.

A cDNA expression library was constructed from poly(A)+ RNA of broiler chicken adenohypophyses using lambda gt11 as a vector. After screening with a rabbit antiserum against chicken LH, a cDNA clone (L12) containing a 436 bp insert was obtained. Using a subclone of L12 in pUC19 (pL12) as the hybridization probe, another cDNA clone (LF127) with a 533 bp insert was isolated. The LF127 contained the full-length cDNA encoding the putative chicken LH-beta subunit precursor molecule. Hybridization of the pL12 cDNA insert to adenohypophysial RNA showed that chicken and Japanese quail adenohypophyses contained RNA species of about 0.8 and 1.0 kb respectively. The amount of this RNA species was ten times higher in adult male quails kept under long days at room temperature than in those kept under short days at 7 degrees C. In situ hybridization experiments showed the exclusive distribution of the signal in the LH cells of the adenohypophysis. The similarity of the nucleotide sequence of the apoprotein-coding region of LH-beta cDNA of the chicken to that of mammals is lower than that among mammals. The deduced amino acid sequence of the chicken LH-beta subunit supports the hypothesis that the number of proline residues increases in the LH-beta subunit the closer phylogenetically the vertebrate is to mammals.

[Clinical evaluation of limb-salvage procedures in malignant bone tumors--a study of 54 cases].

Recent advances in therapy such as local and systemic chemotherapy, wide resection, and aggressive thoracotomy improved the survival of the patients with malignant bone tumors. Since 1981, we have performed limb-salvage operations for 54 cases of malignant bone tumors of the extremities. In this study, we report the rate of local recurrence and the prognosis of the patients. We treated 37 osteosarcomas, 8 malignant fibrous histiocytomas of bone, 3 Ewing's sarcomas, 3 parosteal osteosarcomas, 2 chondrosarcomas, and one fibrosarcoma. The age of the patients ranged from 11 to 65 years (mean, 18.5). The male/female ratio was 2:1. The surgical stage was 47 at II B and 7 at III. The follow-up period was 14 to 84 months (mean 36). They received preoperative chemotherapy including intraarterial infusion of Adriamycin, and postoperative chemotherapy for ten months. Local recurrence was detected in 2/54 (3.7%). Lung metastases (Stage II B) developed in 9/47 (19.1%). Five of them (10.7%) died of metastasis, one is alive with metastases, and 41 cases are now disease-free. Six of seven cases (Stage III) died of metastasis, despite aggressive chemotherapy and thoracotomies.

Varied cytochrome oxidase activities of the alveolar type I, type II and type III cells in rat lungs: quantitative cytochemistry.

Cytochemically demonstrated cytochrome oxidase activities in mitochondria of rat pulmonary alveolar epithelial cells were quantitatively compared. As the standardized procedures, lungs were fixed for 10 min at 4 degrees C with 2% pure glutaraldehyde, and incubated for different times at 37 degrees C in a medium containing 1.0 mg/ml 3,3'-diaminobenzidine-4HCl, 1 mg/ml cytochrome c, 0.1 mg/ml catalase, 7% sucrose, and 0.1 M phosphate buffer, pH 7.4. Electron micrographs were then obtained, and the densities of the reaction deposits in the mitochondrial intermembrane-intracristal space were measured in an image analyzer, and were plotted in terms of time in minutes. The initial velocity (maximal rate) of the activity expressed as deposit accumulation rate (% area/60 min) filling the mitochondrial intermembrane-intracristal space of Type I, Type II and Type III cells was 40, 130 and 9.3, respectively. These results indicated that mitochondria are varied in the intensity of cytochrome oxidase activity among 3 types of pulmonary alveolar epithelial cells, and that the image-analyzing measurement of the accumulation rate of reaction product may be useful for quantitative analysis of enzyme activity, in particular, in the cells which are difficult to isolate from complex tissues.

[Rapid and complete hematopoietic reconstitution following peripheral blood stem cell autotransplantation in a child with T cell acute lymphoblastic leukemia].

Rapid and complete hematopoietic reconstitution was achieved in a child with T cell acute lymphoblastic leukemia who was autografted with peripheral blood stem cells (PBSC). A large number of PBSC was collected by two courses of 3-4 hour-lasting lymphopheresis during early remission induced by the second-line chemotherapy and then cryopreserved in liquid nitrogen. A myeloid progenitor cell dose of 203 X 10(4) CFU-GM/kg body weight was reinfused to the patient following marrow-ablative chemotherapy (MCNU 600 mg/m2, cytosine arabinoside 6 g/m2, etoposide 300 mg/m2, cyclophosphamide 160 mg/kg). Neutrophil count reached 0.5 X 10(9)/l by day + 7 and platelet count reached 20 X 10(9)/l by day + 9. Thereafter, white blood cell count continued to increase and reached a maximum of 38 X 10(9)/l on day + 14. Thus, the rapid recovery of hematopoiesis minimised marrow aplasia-related risks. This approach of stem cell rescue operation can be applied to the treatment of children with cancer, who otherwise have no hope to be cured, as an alternative to bone marrow transplantation.

[Measurement of regional hepatic blood flow with 133Xe per-rectal portal scintigraphy].

The hepatic blood flow (HBF) of patients with liver disease was measured by per-rectal portal scintigraphy with the use of 133Xe. To analyze the time-activity curves of the liver and portal vein, washout curves of the liver were plotted, and from those, the HBF and the ratio of the HBF of the right lobe of the liver to that of the left lobe (R/L ratio) were calculated. The HBF were 143 +/- 17 ml/100 g/min in 5 patients with chronic inactive hepatitis, 119 +/- 20 ml/100 g/min in 4 patients with chronic active hepatitis, and 73.2 +/- 23.4 ml/100 g/min in 7 patients with cirrhosis of the liver. All of four patients whose HBF was under 100 ml/100 g/min and whose R/L ratio was under 1.0 had cirrhosis of the liver. Only one of the 9 patients whose HBF was over 100 ml/100 g/min and whose R/L ratio was over 1.0 had cirrhosis of the liver. The results suggested that per-rectal portal scintigraphy is a useful and noninvasive method to measure the hepatic blood flow.

[An eighteen-year evaluation of the Bjork-Shiley valve prosthesis].

We evaluated the long-term (18 years) results of 356 patients undergoing valve replacement with Björk-Shiley valve prosthesis (aortic, 212; mitral 120; double valve, 24) between 1970 and 1988. Actuarial survival rates were 90% (18 years) for AVR, 80% 'years) for MVR and 90% (8 years) for DVR. Actuarial rates of thromboembolism were 99% (18 years) for AVR, 98% (8 years) for MVR and 94% (8 years) for DVR. Actuarial rates of freedom from events (including valve failure, thromboembolism, reoperation and prosthetic valve endocarditis) were 82% (18 years) for AVR, 95% (8 years) and 94% (8 years) for DVR. There were no differences in these results among spherical disc, convexo-concave disc and monostrut valve. In conclusion, this study demonstrated that Björk-Shiley valve showed a low incidence of postoperative events. These results endorse our choice of the Björk-Shiley.
