Chemotherapy for carcinomatous peritonitis and pleuritis with MMC-CH, mitomycin C adsorbed on activated carbon particles. Clinical trials.

A new drug dosage form comprising activated carbon particles adsorbing mitomycin C (MMC-CH) is designed to slowly release its components, and has affinity for the tumor surface and lymph nodes and a tendency to stay long in the local portion. The therapeutic index of MMC-CH in experimental carcinomatous peritonitis is 3.1 times as high as that of MMC-solution. In clinical experiments 81 patients with carcinomatous effusions were administered with MMC-CH (2.0-2.4 mg/kg in terms of MMC) in bolus intracavitarily. Fifty-one patients responded well to the MMC-CH therapy. Nineteen patients were alive for more than 6 months. The responders showed marked improvement in subjective symptoms, and 26 patients became dischargeable from hospital. Bone marrow suppression and peritoneal irritation were main adverse effects, but the symptoms were temporary and not so serious in spite of a high dose of MMC.

Synthesis of decadeoxyribonucleotides containing N6-methyladenine, N4-methylcytosine, and 5-methylcytosine: recognition and cleavage by restriction endonucleases (nucleosides and nucleotides part 74).

The naturally-occurring modified bases, N6-methyladenine, N4-methylcytosine, and 5-methylcytosine were chemically introduced in place of the adenine or cytosine in the decadeoxyribonucleotides containing recognition sequences of Bgl II, Sau 3AI, Mbo I and Mfl I. The modified oligomers bind to the enzymes but the rates of cleavage by the enzymes are variable.

Early intravesical instillation of adriamycin with oral administration of 5-fluorouracil after transurethral resection for superficial bladder cancer: preliminary results.

In all, 199 patients were entered in this study by 21 collaborating hospitals. Patients with superficial transitional cell carcinoma of the bladder were randomized postoperatively into four groups. Group A received early (immediately and 2 days after transurethral resection) instillation of adriamycin (30 mg/30 mg); group B received early instillation of adriamycin with oral administration of 5-fluorouracil (200 mg/day); group C received delayed (7 days after transurethral resection) instillation of adriamycin (30 mg/30 ml); and group D received delayed instillation of adriamycin with oral administration of 5-fluorouracil (200 mg/day). All patients subsequently received instillations weekly for 2 more weeks, and then every 2 weeks for a further 14 weeks. After 4 months, they received one instillation per month for 8 months. 5-Fluorouracil was administered p.o. for 1 year. The postoperative follow-up period was 12 months. After 3 and 6 months there were significant differences in the non-recurrence rates between groups B and C. After 12 months the overall non-recurrence rates were 87.9% in group A, 83.5% in group B, 89.2% in group C, and 82.8% in group D, and there were no significant differences among the four groups. The number of patients entered and the follow-up period are not adequate for firm conclusions, and further studies are necessary. The main side effect was bladder irritation, which was observed in 38.8% of patients in the early instillation groups and in 26.3% of those in the delayed instillation groups. No severe systemic side effects were observed in this study.

The distribution of N-isopropyl-p-iodoamphetamine in temporary ischemic brain of the mongolian gerbil.

We studied the distribution of N-isopropyl-p-[131I]-iodoamphetamine (IMP) in the ischemic brain of 44 mongolian gerbils. The right common carotid artery was temporarily occluded by a clip and recirculated 3 h later. After given time intervals, 50 microCi of IMP was injected into 18 gerbils which had symptoms. One min after injection (or 10 min, 1 h, or 6 h after injection in other gerbils), each gerbil was killed and autoradiography was performed. IMP activity in various parts of the brain was calculated as %Dose/g from each autoradiogram. At one min after recirculation, partial no flow phenomenon was observed in the right cerebral hemisphere. From 10 min to 24 h after recirculation, a high uptake region was observed partially in the right cerebral hemisphere and thalamus. The high activity disappeared rapidly 10 min after injection. It seemed that this high uptake indicated luxury perfusion at the location of severe tissue damage. In the left side, the low perfusion recovered to an almost normal value at one to three days after recirculation. These results suggested the possibility that IMP could demonstrate luxury perfusion and diaschisis.

Longitudinal differentiation of metaphase chromosomes of Indian muntjac as studied by restriction enzyme digestion, in situ hybridization with cloned DNA probes and distamycin A plus DAPI fluorescence staining.

The longitudinal differentiation of metaphase chromosomes of the Indian muntjac was studied by digestion with restriction enzymes, in situ hybridization with cloned DNA probes and distamycin A plus DAPI (4'-6-diamidino-2-phenylindole) fluorescence staining. The centromeric regions of chromosomes 3 and 3 + X of a male Indian muntjac cell line were distinct from each other and different from those of other chromosomes. Digestion with a combination of EcoRI and Sau3A revealed a pattern corresponding to that of C-banding. Digestion with AluI, EcoRII or RsaI yielded a band specific to the centromeric region only in chromosomes 3 and 3 + X. Furthermore, HinfI digestion yielded only a band at the centromeric region of chromosome 3, whereas DA-DAPI staining revealed a single band limited to the extreme end of the C-band heterochromatin of the short arm of 3 + X. These results suggest that centromeres of Indian muntjac chromosomes contain at least four different types of repetitive DNA. Such diversity in heterochromatin was also confirmed by in situ hybridization using specific DNA probes isolated and cloned from highly repetitive DNA families. Heterozygosity between chromosome homologs was revealed by restriction enzyme banding. Evidence is presented for the presence of nucleolus organizer regions (NORs) on the long arm of chromosome 1 as well as on the secondary constrictions of 3 and 3 + X.

Synthesis of cysteinylphenol, cysteaminylphenol, and related compounds, and in vivo evaluation of antimelanoma effect.

Phenolic and catecholic compounds were synthesized, by combination with cysteine or cysteamine through thioether bond, and their antimelanoma and melanocytotoxic effects were evaluated. Among nine compounds tested, 4-S-cysteaminylphenol (CAP) resulted in an increase in the life span (% ILS) of melanoma-bearing mice and in the growth inhibition (% GI) of melanoma tissue. 4-S-Cysteinylphenol (CP) and its methyl ester form also showed some increase in % GI. The 2-S-isomers of CP and CAP and diphenolic derivatives of CP did not show any significant antimelanoma effect. In addition, the s.c. injection of 4-S-CAP and 4-S-CP, in particular 4-S-CAP, caused the depigmentation of black hair which was manifested by loss of functioning melanocytes, as seen under light microscopy. The 4-S-CAP appears to provide a basis for development of a new class of antimelanoma and melanocytotoxic agents that are more stable than catecholic compounds, which have been most widely utilized as a source of rational chemotherapy for malignant melanoma.

Patterns in the distribution of intracellular ATP concentration in relation to coordination of amoeboid cell behavior in Physarum polycephalum.

The Physarum plasmodium reacts tactically to external stimuli. The cell behavior of this giant amoeboid cell was studied by analysing intracellular ATP concentration. The two-dimensional (2D) spatial distribution of ATP depended on cell shape: a polar pattern for a unidirectionally migrating plasmodium, a bowl shape for a circular plasmodium, a hump shape for an oval plasmodium, or a wavy pattern for plasmodia stimulated with blue light or confined in a small chamber, etc. Local external stimulation brought about new patterns of ATP distribution. The ATP concentrations around the stimulated frontal region were reduced by about a half stimulation with KCl (repellent) or casamino acids (attractant). In both cases, migration was inhibited. Migration velocity increased almost linearly with increasing concentration of intracellular ATP above the threshold (about 20 micrograms/mg protein). Under anaerobic conditions or at low temperatures, the intracellular ATP oscillated slowly with a periodicity of about 30 min. Pattern formations in the intracellular ATP concentration and amoeboid coordination are discussed in terms of coupled chemical oscillators in a self-organizing system.

Effect of 3'-azido-2',3'-dideoxythymidine (AZT) and neutralizing antibody on human immunodeficiency virus (HIV)-induced cytopathic effects: implication of giant cell formation for the spread of virus in vivo.

The effect of 3'-azido-2',3'-dideoxythymidine (AZT) on the human immunodeficiency virus (HIV)-associated giant cell formation was studied in vitro. For this purpose we developed a coculture system using Molt-4 and its virus-producing cell, Molt-4/HTLV-III, which induced syncytia very efficiently. Treatment of the cocultures with 1 and 5 microM of AZT did not inhibit induction of multinucleated giant cells, although only 0.1 microM AZT resulted in almost complete inhibition of HIV replication in Molt-4 cells by cell-free virus infection. This was also evidenced by the assays for viral antigen-positive cells, reverse transcriptase activity, and virus particles released from cell cultures after AZT treatment. When the cocultures were treated with 1% neutralizing antibody (NA) from HIV-infected individuals alone, giant cell formation was inhibited to some extent. However, the concomitant treatment of culture with AZT and NA resulted in much stronger inhibition of giant cell formation. The amount of CD4 antigens on the surface of cells was reduced greatly in the HIV producer cells (Molt-4, H9, and MT-4 cells) as compared to their HIV-free counterparts. These data suggest that (1) both CD4 antigen and viral antigens on the surface of cells play central roles in the induction of multinucleated giant cells and (2) AZT is more effective in inhibition of viral spread in patients with higher NA, probably at an earlier stage of the disease than in patients with lower NA titer.

Relationship between plasma atrial natriuretic polypeptide concentration and hemodynamic measurements in children with congenital heart diseases.

The plasma alpha-atrial natriuretic polypeptide (alpha-ANP) concentration in the peripheral veins of children with congenital heart diseases was measured by radioimmunoassay and compared with cardiac catheterization data. Every patient with heart failure had a higher alpha-ANP concentration (132.1 to 858.7 pg/mL) than the upper limit of the normal range (11.7 to 98.7 pg/mL), whereas more than half of the patients without heart failure had a normal alpha-ANP concentration. Although none of the 13 children with atrial septal defect had heart failure, their mean (+/- SD) plasma alpha-ANP concentration (99.4 +/- 40.7 pg/mL) was significantly higher than that in control children (44.6 +/- 22.3 pg/mL). The plasma alpha-ANP concentration was significantly correlated with the pulmonary blood flow/systemic blood flow (Qp/Qs) ratio in the children with atrial septal defect. In the 34 children with ventricular septal defect the plasma alpha-ANP concentration increased in relation to increasing size of the defect. The plasma alpha-ANP concentration was significantly correlated with the Qp/Qs ratio, pulmonary artery pressure, and left atrial pressure, estimated from mean pulmonary artery wedge pressure, in the children with ventricular septal defect. In the children with tetralogy of Fallot, the mean plasma alpha-ANP concentration was normal, and mean right and left atrial pressures were not increased. The elevated alpha-ANP concentration in the three patients with heart failure decreased after their conditions improved with various treatments. Thus the measurement of alpha-ANP concentration may be valuable in evaluating the presence or absence of heart failure and the response to treatment in children with congenital heart diseases. Distention of the right and left atrial might induce the release of alpha-ANP in patients with atrial and ventricular septal defects, respectively.

Folate analogues as inhibitors of thymidylate synthase.

Recent demonstrations that deazafolate analogues may act as potent inhibitors of thymidylate synthase (TS) provided a firm rationale for the synthesis of N10-propargyl derivatives of 8-deazafolate and 8-deazaaminopterin (4). A complete assignment of the 1H NMR spectra of these compounds was made possible through application of 2D (COSY) techniques at 200 MHz. Data describing the inhibition of TS derived from human leukemia (K562) cells are presented. IC50 values of 2.25 and 1.26 microM were determined for 8-deaza-10-propargylfolate (3) and 8-deaza-10-propargylaminopterin, respectively. Comparison of the data for various folate analogues reveals a striking dependence of TS inhibitory potency upon the number of nitrogens in the folate pyrazine ring.

Free D-amino acids in human plasma in relation to senescence and renal diseases.

Minute but appreciable amounts of D-amino acids were detected in normal human plasma. The content was significantly higher in an elderly population (age 76 +/- 6 years, mean +/- SD, n = 41) than in a younger group (age 42 +/- 4 years, n = 26), i.e. 6.9 +/- 4.8 nmol/ml (mean +/- SD, range 0-18.8 nmol/ml) and 2.5 +/- 1.8 nmol/ml (range 0-6.3 nmol/ml) for the elderly and the younger groups, respectively. Elevation of plasma D-amino acid level was observed in a group of patients with renal disease (3.6-52.6 nmol/ml), in proportion to the serum level of creatinine (n = 50, r = 0.726, P less than 0.001), beta 2-microglobulin (n = 34, r = 0.551, P less than 0.005), and to glomerular filtration rate (n = 39, r = 0.556, P less than 0.001).

3-Phosphoglycerate-dependent protein phosphorylation.

The importance of protein phosphorylation in biological regulation is widely recognized; thus, a number of extracellular signal- and second messenger-linked protein kinases have been well characterized, and various substrates for these kinases have been identified. We present evidence that the glycolytic intermediate 3-phosphoglycerate can cause a marked stimulation of phosphorylation of specific proteins in mammalian tissues. In the brain, there are at least two types of 3-phosphoglycerate-dependent protein phosphorylation systems, which differ in activator specificity, substrate specificity, and sensitivity to the activator. Both of these phosphorylation systems are, however, insensitive to cAMP, cGMP, Ca2+, calmodulin, and protein kinase C activators and have characteristics different from those of casein kinase II-catalyzed protein phosphorylation. The most prominent endogenous substrates in the bovine brain are rather minor proteins with minimal molecular masses of 72 and 155 kDa, which were not detected in "nonnervous" tissues such as heart, liver, lung, and kidney. In nonnervous tissues, 3-phosphoglycerate affected the state of phosphorylation of other proteins. Evidence provided suggests the occurrence of an as yet undescribed type of protein kinase, which may be designated 3-phosphoglycerate-dependent protein kinase, in mammalian tissues.

Molecular organization of photochemical reaction complex in chromatophore membrane from Rhodospirillum rubrum as detected by immunochemical and proteolytic analyses.

The molecular organization of photochemical reaction (PR) complex in chromatophores from Rhodospirillum rubrum was studied by a combination of proteolytic analysis with proteinase K followed by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and immunochemical analysis with rabbit polyclonal antibodies against its five subunits (H, M, L, alpha, and beta). The preparations used for comparison were reaction center complex (RC) (composed of H, M, and L), PR complex, and chromatophores (closed membranous vesicles of polar lipid bilayer having PR complex buried in the membrane). 1. RC was bound with anti-H, anti-M, and anti-L antibodies, whereas PR complex and chromatophores were bound with anti-H and anti-beta antibodies, but not with the other antibodies. 2. With PR complex, H (Mr 31,000 (31K)) was rapidly degraded into two peptides with Mr of 16K and 14.5K (abbreviated as 16K and 14.5K, respectively), M (27K) into 25.5K, and beta (11K) into 10K. Significantly later, the 25.5K of M was degraded into 24K, L (23K) into 19K, and alpha (12K) into 11K. With chromatophores, H and beta were degraded in a manner similar to that with PR complex, whereas M, L, and alpha were not degraded at all. With RC, H, M, and L were rapidly degraded. 3. With RC, the activity for photooxidation of P870 (photochemical activity) was hardly affected till H, M, and L had been degraded into less than 10K, 24K, and 19K, respectively. With PR complex, the absorbance spectrum due to the bacteriochlorophylls of light-harvesting complex-1 composed of alpha and beta (LH1-Bchl) changed in parallel to the degradation of alpha or 10K (a part of beta). 4. Together with the previous results (Ueda et al. (1985) J. Biochem. 98, 1487-1498), the present findings suggest that: 1) RC is directly surrounded by 12 alpha and further by 12 beta; 2) H and beta are mostly and partially exposed, respectively, on the outer surface of the membranous vesicle; 3) a small part of M is exposed on the inner surface of the membranous vesicle.

Highly controlled carbodiimide reaction for the modification of lysozyme. Modification of Leu129 or Asp119.

In the cross-linking reaction of lysozyme between Leu129 (alpha-COO-) and Lys13 (epsilon-NH3+) using imidazole and 1-ethyl-3-[3-(dimethylamino)propyl]carbodiimide hydrochloride (EDC), a side reaction of the peptide bond inversion from alpha to beta between Asp101 and Gly102 was greatly reduced by addition of beta-(1,4)-linked trimer of N-acetyl-D-glucosamine [(NAG)3]. When methylamine or 2-hydroxyethylamine was further added, the extent of the cross-link formation was decreased and the derivative where the alpha-carboxyl group of Leu129 was modified with the amine was newly obtained. On the other hand, when ammonia was added, the beta-carboxyl group of Asp119 instead of the alpha-carboxyl group was mainly amidated. From these results, the presence of a salt bridge between Asp119 and Arg125 besides that between Lys13 and Leu129, is proposed. Enzymatic activities of the derivatives prepared here indicated that the modification of the alpha-carboxyl group reduced the activity to approximately 90% of that of native lysozyme. Des-Leu129 lysozyme, which lacks Leu129, also showed approximately 90% of the activity of native lysozyme. Therefore, the salt bridge between Lys13 and Leu129 may play some role in maintaining the active conformation of lysozyme.

Lectin histochemistry of malignant fibrohistiocytic tumors.

The staining pattern by Ricinus communis agglutinin (RCA), a lectin used as a good marker for histiocytes, in 24 cases with malignant fibrous histiocytoma (MFH) was studied and compared with that of 12 cases of fibrosarcoma (FS). In 20 (83%) of 24 cases of MFH, varying degrees of RCA binding were observed, whereas only four (33%) of 12 cases of FS were positive. RCA-positive FS included three cases with infantile FS and one adult case with post-radiation FS. Eight adult patients with FS were entirely negative. This positivity rate of RCA binding in MFH was much higher than those of antisera against lysozyme, alpha-1-antitrypsin, and alpha-1-antichymotrypsin previously reported. Seven MFH patients with focal aggregation of RCA-positive benign-appearing (reactive) histiocytes died earlier than the other patients with only scattered RCA-positive histiocytes; 5-year survival rates were 32% and 69%, respectively (p less than 0.05). These findings suggest that RCA reactivity can be used as a potential diagnostic and prognostic marker for MFH.

Monophasic and biphasic synovial sarcoma. An immunohistochemical study.

Seven cases of synovial sarcoma (SS), two with biphasic and five with monophasic histology, were studied immunohistochemically using monoclonal antibodies to intermediate filaments, keratin, and vimentin. Slender spindle cells and plump cells were constant components of all the tumors. Epithelioid cells were present only in biphasic SS. Epithelioid cells and plump cells were positively stained by both keratin and vimentin. Slender spindle cells were stained positive for vimentin and negative for keratin. Staining manner of each cell type was similar irrespective of monophasic or biphasic pattern. Present immunohistochemical studies suggested that monophasic or biphasic patterns in SS should be regarded as a different expression of the same disease. In addition, immunohistochemistry proved to be a useful tool to detect plump cells which were difficult to find on routine staining.

Incidental detection of renal carcinoma during radiological imaging.

The manner of presentation and tumour stage in 16 consecutive patients with renal carcinoma who were treated surgically between 1976 and 1980, and in 37 patients treated similarly between 1981 and 1985, were reviewed. Two patients (13%) were discovered incidentally between 1976 and 1980 compared with 13 (35%) between 1981 and 1985. The renal tumours were discovered incidentally at the time of intravenous urography (IVU), ultrasound, computed tomography (CT) and angiography. There were 13 stage I and two stage II lesions, as assessed pathologically after nephrectomy. The tumours were less than 6 cm in diameter. Thirteen of 15 patients are alive between 6 and 90 months after surgery. No patient died of renal carcinoma. Our data have shown an increase in the proportion of incidentally diagnosed cases. The routine use of intravenous urography, ultrasound and computed tomography has led to earlier diagnosis, lower stage and possibly longer survival in patients with incidentally detected renal carcinomas compared with patients where the diagnosis was suspected.

Synapsin I is associated with cholinergic nerve terminals in the electric organs of Torpedo, Electrophorus, and Malapterurus and copurifies with Torpedo synaptic vesicles.

Using an affinity-purified monospecific polyclonal antibody against bovine brain synapsin I, the distribution of antigenically related proteins was investigated in the electric organs of the three strongly electric fish Torpedo marmorata, Electrophorus electricus, Malapterurus electricus and in the rat diaphragm. On application of indirect fluorescein isothiocyanate-immunofluorescence and using alpha-bungarotoxin for identification of synaptic sites, intense and very selective staining of nerve terminals was found in all of these tissues. Immunotransfer blots of tissue homogenates revealed specific bands whose molecular weights are similar to those of synapsin Ia and synapsin Ib. Moreover, synapsin I-like proteins are still attached to the synaptic vesicles that were isolated in isotonic glycine solution from Torpedo electric organ by density gradient centrifugation and chromatography on Sephacryl-1000. Our results suggest that synapsin I-like proteins are also associated with cholinergic synaptic vesicles of electric organs and that the electric organ may be an ideal source for studying further the functional and molecular properties of synapsin.
