Malignant lymphoma of the oral cavity: clinicopathologic analysis of 20 cases.

Twenty cases of malignant lymphomas (ML) of the oral cavity were reviewed in the light of recent histologic classification. They occurred in 12 male and 8 female patients, with the age range of 11-80 years (median 51 years of age). Eighty percent of patients were stages IE and IIE. Histologically, 15 cases (75%) were ordinary non-Hodgkin's lymphoma (NHL) with diffuse large cell type being the most common. The remaining 5 cases, undefinable by classification scheme for ordinary NHL, were designated as malignant histiocytosis of the oral cavity (3 cases) and peripheral T-cell lymphoma (2 cases), respectively. There were no cases of follicular lymphoma. The present cases comprised 45% of high grade, 40% of intermediate grade, and 15% of low grade malignancy. A follow-up study showed that the stage of diseage, histologic classification, and frequency of mitosis correlated well with survival.

Inhibitory effect of 2',3'-didehydro-2',3'-dideoxynucleosides on infectivity, cytopathic effects, and replication of human immunodeficiency virus.

It is generally accepted that human immunodeficiency virus (HIV) is the etiologic agent of the acquired immunodeficiency syndrome and related diseases. In this report, we demonstrate the antiviral effect of nucleoside analogs 2',3'-didehydro-2',3'-dideoxythymidine (DHT) and 2',3'-didehydro-2',3'-dideoxycytidine (DHC) by using human T-cell lymphotropic virus type I-carrying MT-4 cells, which are extremely susceptible to HIV infection. These agents efficiently inhibited the cytopathic effects and expression of HIV-specific antigens in MT-4 cells after infection of the virus. Both DHT and DHC also strongly blocked viral replication as determined by our quantitative bioassay system using a plaque-forming assay. These antiviral effects were obtained at concentrations at which the drugs produced little or no toxicity and were comparable to those with 3'-azido-3'-deoxythymidine and 2',3'-dideoxynucleosides. These findings warrant further investigation of the use of DHT and DHC for the treatment of the acquired immunodeficiency syndrome and related diseases.

Effects of hyperprolactinemia on FSH- or LH-induced activities of 17 beta-oxidoreductase, 5 alpha-reductase, and 17-hydroxylase in hypophysectomized rat testes.

Two pituitaries from 7-week-old female rats (Sprague-Dawley strain) were grafted under the capsule of the left kidney of a 49-day old male rat. The pituitary grafted and sham-operated rats were hypophysectomized at 56 days of age. The hypophysectomized rats were given daily injections of NIAMDD-oFSH-13 (20 micrograms/0.5 ml saline), NIAMDD-oLH-23 (9 micrograms/0.5 ml saline) or saline for 4 days starting from day 58. The treated rats and normal male rats were killed at 61 days of age. Testicular homogenates were incubated with [14C]4-androstene-3, 17-dione or [3H] progesterone, and enzyme activities per testes were estimated. Hypophysectomy caused significant decreases in activities of testicular 17 beta-oxidoreductase and 17-hydroxylase. The decreased activity of 17 beta-oxidoreductase was significantly stimulated by FSH or LH treatment, whereas the decreased 17-hydroxylase activity was stimulated only by LH treatment. Although pituitary grafts alone showed little or no effect on these enzyme activities in the hypophysectomized rats, the grafts significantly inhibited FSH-stimulated 17 beta-oxidoreductase activity and the LH-stimulated 17 beta-oxidoreductase and 17-hydroxylase activities but enhanced LH-induced 5 alpha-reductase activity. The present results confirm previous findings that an excess of prolactin directly inhibits LH-stimulated 17-hydroxylase activity but enhances LH-induced 5 alpha-reductase activity in the rat testis. The present results also demonstrate that the same grafts directly inhibit FSH-stimulated 17 beta-oxidoreductase activity but have no effect on FSH-induced 5 alpha-reductase activity.

Prevalence rates and risk factors for allergic symptoms among inhabitants in rural districts.

In order to determine the prevalence of allergic disorders and their association with agricultural factors, a total of 3,717 inhabitants of rural districts in Kumamoto Prefecture were ashed to fill out a questionnaire concerning their allergic status. The results obtained were as follows: One or more items of allergic symptoms were reported by 44% of the subjects. The prevalence rate of each symptom was 8% for respiratory organs, 5% for delayed type of respiratory disorders, 10% for nasal mucosa, 16% for eyes, 16% for urticaria-like derma and 26% for eczema-like derma. Twenty-one percent of the subjects had past histories of allergic diseases such as asthma (3%), nasal allergy (3%), urticaria (6%), contact dermatitis (9%) and so forth. These allergic conditions were seen to be more common among farmers than non-agricultural workers, and also among subjects with allergic constitution than those without such constitution. The highest (62%) prevalence of allergic symptoms was observed in farmers engaged in poultry raising, followed by those engaged in raising flowers (58%), tobacco (58%), cortinellus shiitake (55%), cucumber in plastic greenhouse (53%) and mandarin orange (53%). Among the symptomatic subjects, 12-38% of those with each symptom gave farm work as the direct causative factor for developing the corresponding symptom. Pesticide spraying was the most common agent among those factors.

[Effect of partial transection of the stomach on antral motor function].

To elucidate the antral motor function after partial-transection, the author investigated the electromyographic change of the antrum before and after 1/3, 1/2, 2/3 partial-transection from the greater curvature or the lesser curvature at the lower third or the middle of the stomach. The results were summarized as follows: 1. Over 1/2 of partial-transection from the lesser curvature or over 1/3 of partial-transection from the greater curvature at the lower third of the stomach, the remarkable prolongation of discharge intervals, the remarkable acceleration of propagation velocity and an appearance of antiperistaltic discharges were noted. 2. After 2/3 of partial-transection from the lesser curvature or over 1/2 of partial-transection from the greater curvature at the middle of the stomach, the slight prolongation of discharge intervals and the slight acceleration of propagation velocity were noted, however, an appearance of antiperistaltic discharges was recognized on the latter alone. The above results suggested that the partial-transection at the lower third of the stomach rather than at the middle of the stomach or from the greater curvature rather than from the lesser curvature might effect on the antral motor function.

New additional operation for the prevention of the gastric stasis after selective gastric vagotomy with antrectomy.

In order to prevent postoperative gastric stasis, we devised and tested the following improvements in the selective gastric vagotomy with antrectomy. (1) The gastroduodenostomy was made so as to have an acute angle to the longitudinal axis of the remnant stomach, and (2) the downward traction being exerted on the remnant stomach by the transverse colon was eliminated by dissecting the greater omentum, and then (3) the posterior wall of the corpus on the lesser curvature side was fixed to the stump of the hepatogastric ligament, and the posterior wall of the corpus on the greater curvature side was fixed to the retroperitoneum inferior to the pancreas. By these procedures, the corpus was maintained in a position superior to the anastomosis. These operative procedures resulted in preventing the gastric stasis after the start of oral feeding. The two patients on whom this operation was performed have been followed up for 3 to 4 months since the operation, and neither of them has had any complaint of gastric stasis, such as anorexia or a feeling of heaviness of the stomach.

Activated carbon particles as anti-cancer drug carrier into regional lymph nodes.

The effect of a new dosage form for anti-cancer agents was studied on tissue distribution and compared with the aqueous solution form. The new dosage form, developed in order to distribute a greater amount of the agent to regional lymph nodes, comprises polyvinylpyrrolidone and small activated carbon particles adsorbing pepleomycin (PEP-CH) or mitomycin C (MMC-CH) in saline. Pepleomycin at 500 micrograms/kg or mitomycin C at 250 micrograms/kg were injected into the gastric wall of dogs in the new dosage form or the aqueous solution form. The activity level of the agents in tissues was bioassayed within 24 h after injection by the thin agar plate method. Statistically, the new dosage form maintained an activity level in lymph nodes that was significantly higher than the aqueous solution form (ratio of PEP-CH to solution form, 6-32 times in 24 h; MMC-CH, 100-1000 times in 6 h; p less than 0.05-0.01), and a concentration in blood that was significantly lower (PEP-CH, about 1/2 times in 2 h; MMC-CH, 1/10-1/5 times in 1 h; p less than 0.05).

Photoinduced electron transfer reaction from N-formyl-L-kynurenine and L-kynurenine to cytochrome C.

The reduction of cytochrome c was found in the presence of N-formyl-L-kynurenine (NFK) and L-kynurenine (KN) during irradiation, suggesting electron transfer to cytochrome c. The reaction occurred both under aerobic and anaerobic conditions. In the former case, oxygen molecules may act mainly as a quencher of excited NFK and KN, and superoxide anion produced by electron transfer may partially contribute to the reduction. The reaction proceeded via the excited triplet state of NFK and KN. The actual reductive chemical species might be an intermediate from excited state NFK or KN, which is assumed to be ketyl radical type species.

Enhanced anticancer efficacy by use of mitomycin C adsorbed on small activated carbon particles in mice.

A new dosage form of mitomycin C (MMC-CH) was tested for toxicity and therapeutic efficacy against intraperitoneally inoculated cancer cells in mice. MMC-CH is a suspension comprising 7.16 mg/ml of activated carbon particles, 1 mg/ml of mitomycin C (MMC) and 20 mg/ml of polyvinylpyrrolidone in saline. The LD50 value determined by means of the Litchfield-Wilcoxon method after intraperitoneal administration was 2.29 times higher for MMC-CH than for MMC aqueous solution. Mice were inoculated intraperitoneally with 2 X 10(5) P388 leukemia cells and given an intraperitoneal injection of 10 to 1.25 mg/kg of MMC in the form of MMC-CH or MMC aqueous solution 24 hr after the inoculation. The median survival time was prolonged to 270.5%, 223.0% or 168.3% by MMC-CH at the dose equivalent to 10, 5 or 2.5 mg/kg of MMC, respectively, while it was prolonged to 182.7%, 139.6% or 155.4% by MMC aqueous solution at the dose of 5, 2.5 or 1.25 mg/kg of MMC, respectively, as compared with the median survival time in the non-treated group. MMC-CH prolonged the survival time to more than 120% as compared with the same dose of MMC given as MMC aqueous solution, and was less toxic.

[A study on UFT concentration in tissues and serum of patients with malignant gynecologic tumors following oral administration].

UFT was orally given at a dose of 200 mg twice a day to seven female patients with malignant gynecological tumors. Measurements of blood and tissue concentrations of tegafur (TF), fluorouracil (FU) and uracil (U) showed that the concentrations of both FU and U were clearly increased. The ratio of concentration in the tumor to that in peripheral blood was 9.3, while that of the concentration in the internal iliac lymph nodes to that in the internal iliac artery was 11.2. The changes in blood levels of the three compounds were determined one and five days after UFT administration. There was a distinct difference in the blood levels of the drug among TF, FU and U. TF reached its peak concentration two hours after breakfast and four hours after supper; both FU and U reached their peak concentrations two hours after breakfast and one hour after supper. Furthermore, TF showed a tendency to accumulate after continuous administration, but this was not the case with FU or U. These findings suggest that even during continuous administration, the blood level of FU remains low.
