[Electron microscopic and mass spectrometric studies of lymphedema pathogenesis aspects].

A comprehensive examination was made, which included histological, electron microscopic and chromatographic mass spectrometric studies of biological specimens obtained from 18 patients with leg lymphedema. The development of lymphedema was found to accompanied by the specific connective tissue structure being formed along with the presence of low-differentiated and necrotic areas associated with the cytopathology of blood and lymph vessels and impaired microcirculation. The connective tissue areas under study were demonstrated to have a pronounced antigenicity and aggressive properties against other tissues and structures. The morphological changes revealed are directly related to the impaired metabolism of protein, fatty acids, and complex polysaccharides.

[Diagnostic and prognostic significance of some parameters of nonspecific defense humoral mechanism in patients with lymphedema complicated by erysipelas inflammation].

Immunological investigation of 193 patients with uncomplicated lymphedema and lymphedema complicated with erysepalas inflammation registered in the patients with inflammation high levels of antigen-nonspecific circulating immune complexes, beta-lysins, alpha1-antitripsin, serum IgE and IgM in the presence of massive bacterial infection of the limb skin. The above alterations may serve diagnostic and prognostic indicators of the disease chronicity and progression in patients with lymphedema.

[Assessment of the results of surgical treatment of lymphedema based on computerized tomography].

The possibilities of computed tomographic appraisal of the qualitative and quantitative characteristics of the soft tissues in lymphedema of the lower limbs were studied in comparison with the results of pre- and postoperative examination of 21 out of 86 patients with Stage II-IV lymphedema. There were 4 males and 17 females, whose ages ranged from 16 to 41 years. In postoperative periods of 2 weeks to 6.5 years 45 computed tomographic studies were conducted. It was shown that computed tomography is an objective basis for appraising the qualitative and quantitative changes in the soft tissues of the limbs in follow-up patients with lymphedema after creation of lymphovenous anastomoses. The examination confirms objectively functioning of the lymphovenous anastomoses after disappearance of supra- and subfascial edema and diminution and even normalization of the thickness of the subcutaneous fat.

[Lymphedema and hypothyroidism].

The examination covered 1191 lymphedema patients (635 primary and 536 secondary disease cases). Hypothyroidism-like thyroid dysfunction was found in 80% of patients with primary lymphedema. Measurements of thyroid hormones confirmed the above findings. It is hold that lymphedema patients should undergo the thyreoidin test and combined treatment including thyroid hormone therapy.

[Performing lympho-venous anastomoses at several levels of the lower extremity in primary lymphedema].

Operations were conducted on 41 patients, 31 of them had a primary form of lymphedema. Lymphovenous anastomoses (LVA) were formed in the region of the medial collector on the thigh in Scarpa's triangle, in the middle third of the leg, and on the dorsum of the foot. The results were good in the late-term postoperative period in 74.0% of patients. According to earlier studies, the formation of LVA in patients with primary lymphedema of the lower limbs only on one of the standard levels produces good late-term postoperative results in 57.8% of cases.

[Space-time organization of cellular systems in the normal and pathologic states].

The time-space organization of biological systems, cell systems in particular, is one of the most important problems of current theoretical biology having medical implications. The presented view of the general structure of the temporal organization of biosystems and their development in the course of life evolution interprets the temporal organization of a living system as a complex of regular changes in time, the biological rhythms included, its structures and functions which are interactive and show relationships with cyclic alterations of environmental conditions. A special organization is composed of a group of different structures and related functions which are arranged according to a certain pattern in the space occupied by the biological system and interact with each other. The experimental evidence obtained in studies of the proliferative and metabolic systems of the liver, testify to the existence of a general time-space organization in the cell systems. This organization represents an entity formed by the cellular temporal and special organizations and maintained by relationships and interactions between them.

[Late results of lymphovenous anastomoses in lymphedema of the lower extremities].

The authors examined 152 patients with lymphedema of the lower limbs 2-6 years after operation for lymphovenous anastomoses (LVA). They evaluated the dynamics of edema according to Alberton's formula, the changes in the character of the episodes of erysipelas, and the patient's feeling of heaviness in the involved limb. The function of the anastomosis was studied by the radioisotope method. Positive results were recorded in 99 patients. In the remaining patients the LVA did not function and the symptoms of lymphedema did not change or progressed. The effect of LVA was found to be positive in 51 patients in the group of 69 patients with secondary form of lymphedema and in 48 of 83 patients with the primary form of the disease. The last-mentioned was connected with congenital insufficiency of lymph drainage. The condition of the collecting lymphatics is an important factor of LVA efficacy: the effect of LVA was positive in 68 patients in hyperplasia and in 31 patients in hypoplasia of the lymphatics. The results of LVA are improved by regular nonoperative management for the correction of endocrine-immunological and hemostatic factors as well as by stimulation of lymph drainage in the limb and prevention of inflammatory complications.

[Lymphography in clinical variants of posttraumatic edema].

Early experience with lymphography, used in 28 patients with edema of the legs, descending from the site of injury (13) or ascending to the damaged area (15), is summarized. Purposive identification of the location of lymph reflux to abducting lymphatic vessels of the skin and subcutaneous fat helped to detect anatomic zones of lymph stasis, identify the cause and site of origin of the edema and exclude impaired lymph passage through neighbouring lymphatic vessels and collectors. Local or generalized disorders of lymph flow were shown to be at different distances from the damaged lymphatic vessels. The emergence of different clinical variants of descending and ascending edema is caused by the movement of the lymph reflux area from the original site of edema.

[Lymphovenous anastomoses in the prevention of post-mastectomy edema of the arm].

Analysis of the clinical and experimental data has prompted the authors to try surgical correction of an impaired lymph drainage in the arm after radical mastectomy by effecting lymphovenous anastomoses. These anastomoses are intended to eliminate lymphostasis, the principal reason of a postmastectomy edema, by draining the lymph, outflowing from the arm, to the vein. The optimal sites for the formation of a lymphovenous anastomosis have been found the cubital fossa and the anterointernal surface of the lower third of the shoulder. The formation of such an anastomosis simultaneously with radical mastectomy eliminates operation lymphorrhea of the wound already by days 4-5 postoperation, this facilitating the wound healing. Such an approach to radical mastectomy technique may completely prevent the development and progress of a postmastectomy edema of the arm.

[Topographic distribution of proliferating hepatocytes in the hepatic lobe of intact rats during the course of 24 hours].

The topographic distribution of DNA-synthesizing and divisible hepatocytes was studied in the hepatic lobe of intact rats during the 24-hours. For this purpose the indices of a number of DNA-synthesizing and divisible cells were determined both in a liver as a whole and for each lobe zone (periportal, middle and central one). The obtained results allowed to reveal the presence of the 24-hour rhythm of cell proliferation process in a liver as well as the regular topographic distribution of DNA-synthesizing and divisible hepatocytes during the period of their increased values in the rhythm. The process of activation of proliferation seems to start in the periportal zone, and then to hold the whole zone, with its dominance in the middle zone during the period of maximum values of the cell proliferation indices. One could suppose that this testifies to the equal proliferative potencies of hepatocytes irrespective of their localization as well as to the fact that the cells of all zones take part in the formation of acrophases of rhythms both of the DNA-synthesizing hepatocytes and divisible ones. However, the degree of their participation in this process is unequal and depends on the localization of cells in the hepatic lobe.

[Topographic distribution of dividing hepatocytes in the regenerating liver lobule in the period of maximum mitotic activity].

Topographic distribution of dividing hepatocytes was studied in the liver lobule after hepatectomy performed at 10-11.30 a. m. The studies were conducted 20-32 h after surgery during the first increase in mitotic activity. It has been established that dividing hepatocytes appear simultaneously in all lobule areas, i. e. that all the hepatocytes are capable of responding to regenerating stimuli. However, further behaviour of dividing hepatocytes becomes different. It is concluded that hepatocytes from outer and intermediate lobule areas possess a more pronounced proliferative potential during peak mitotic activity after partial hepatectomy.
