[Phase III study of carboplatin in ovarian cancer].

A phase III study of carboplatin vs cisplatin in CAP regimen for ovarian cancer was conducted by a cooperative study group consisting of 22 institutions nationwide. The response rate for 23 cases allocated to carboplatin regimen group was 34.8% and 42.1% for 19 cases allocated to cisplatin regimen group. No significant difference was observed between these two regimens in response rates, and no significant differences were noted between the two regimens in each of the clinical toxicities and abnormalities in laboratory findings. Since the carboplatin regimen group did not require hydration, or the use diuretics or antiemetics, these results suggested that carboplatin is more useful in the treatment of ovarian cancer than cisplatin.

HTLV-I carrier mothers with high-titer antibody are at high risk as a source of infection.

High-titer antibody against HTLV-I in carrier mothers is proposed as a secondary parameter for risk of viral transmission to the children via milk. For titration of antibodies, a less expensive modified gelatin particle agglutination assay (approximately 40% of the standard cost) was developed. None of 11 carrier mothers with antibody titers of less than 4000 had carrier children, whereas 11 out of 17 mothers with titers of 256,000 or higher had carrier children. The antibody titer was correlated with antigen-bearing cells detectable in cultures of peripheral blood T-lymphocytes, which was previously described as a marker of risk for transferring HTLV-I to children.

[Papillomavirus infection of the vulva].

The role of Papillomavirus (PV) in the development of external genital neoplasias has recently been documented. Fifty cases of condyloma acuminata, dystrophy, Bowenoid papulosis, carcinoma in situ and invasive carcinoma of the vulva were examined for the presence of PV structural antigens with an immunoperoxidase method on formalin-fixed, paraffin-embedded tissue. Using the avidin-biotin-peroxidase complex (ABC) technique on vulvar tissue obtained from biopsy and vulvectomy specimens, PV structural antigens were identified in association with condyloma acuminata in 5 of 6 cases (83.3%), with hyperplastic dystrophy in 1 of 11 cases (9.1%), with atypia in 9 of 14 cases (64.3%), with Bowenoid papulosis in 2 of 2 cases (100%), with carcinoma in situ in 4 of 7 cases (57.1%) and with invasive carcinoma in three of 3 cases (100%), but in none of the examples of 7 lichen sclerosus cases. A PV-inducing morphologic change in koilocytosis was present in 30 instances of all the vulvar lesions. The results of this study, therefore, provide immunohistochemical evidence demonstrating the close relationship of papillomavirus to vulvar disease ranging from condyloma acuminata and moderate atypia to carcinoma in situ and invasive carcinoma.

[Pathological and clinical observations on precancerous lesions of the vulva].

The vulvar epithelial lesions which have been subjected to histological examinations in Nagasaki University Hospital (1965-1985) and its 5 affiliated hospitals (1975-1985) included 133 cases in vulvar dystrophies, 72 in other benign lesions, 18 in squamous cell carcinoma in situ, 10 in vulvar Paget's disease, and 59 in invasive carcinoma. These lesions were studied pathologically and endocrinologically, and the pathogenesis of vulvar carcinoma and whose clinical problems were discussed. Serum hormone level in lichen sclerosus In a lichen sclerosus group, serum testosterone level was higher, as compared to that of a coetaneous group in the control, and androstenedione remained normal whereas 5 alpha-dihydrotestosterone (DHT) was decreased significantly. This led us to apply a 2% testosterone propionate ointment to the local skin, and which resulted in an apparent increase in DHT; the greater the increase multiple, the more the significance in clinical efficacy; suggesting that application of testosterone ointment may lead to the activation of 5 alpha-reductase and subsequently to an increase in DHT by which clinical symptoms also were alleviated. Papillomavirus antigen A test was carried out, for each lesion to search human papillomavirus (HPV) antigen, using avidin-bioton peroxidase complex (ABC) method with monoclonal antibody against papillomavirus. The results obtained revealed that antigens were negative on all in the case with lichen sclerosus and in a hyperplastic dystrophy case not associated with atypia, but cases judged as being positive included 9 of 14 with atypia and 5 of 6 with carcinoma in situ.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fundamental and clinical studies on imipenem/cilastatin sodium in the field of obstetrics and gynecology].

The following results were obtained during pharmacokinetic, bacteriological and clinical evaluation of the usefulness of the combination (1:1) of imipenem (MK-0787) and cilastatin sodium (MK-0791), an inhibitor of dehydropeptidase-I, in the treatment of patients with obstetric and gynecologic infections. Plasma concentrations of MK-0787 in antecubital vein and uterine artery were 18.2 micrograms/ml and 16.4 micrograms/ml, respectively, and of MK-0791 were 9.5 micrograms/ml and 10.3 micrograms/ml, respectively, at 1.28 hours after the end of a drip infusion of 0.5 g/0.5 g of MK-0787/MK-0791. Plasma concentrations of both MK-0787 and MK-0791 decreased slowly, and concentrations of the former in antecubital vein and uterine artery were 0.7 micrograms/ml and 1.0 micrograms/ml, respectively, and of the latter were 1.5 micrograms/ml and 1.9 micrograms/ml, respectively at 2.83 hours after the end of a drip infusion. Concentrations of the 2 agents in female genital tissues decreased with increasing time in a manner similar to concentrations in the plasma. Clinical responses in 10 patients were excellent in 2 and good in 8, and the efficacy rate was 100 percent. Organisms were isolated from all 10 patients before the treatment, but were eradicated in 9 patients by the treatment. No side effects were observed, but increase of eosinocytes was observed in 1 patient.

Characterization and comparison of two forms of human chorionic gonadotropin from hydatidiform moles with low and high immunoreactivity.

Two glycoproteins (LM-hCG and HM-hCG) with gonadotropic activity were purified from the chorionic tissue of patients with hydatidiform mole with low and high urinary human chorionic gonadotropin levels and were compared to each other. The immunologic, biologic, and physicochemical properties of the two preparations were very similar. Also, the quantities of LM-hCG and HM-hCG recovered from molar tissue from the two types of patients were similar. However, the molar tissue from patients who excreted high levels of human chorionic gonadotropin immunoreactivity also contained other molecular forms with apparent human chorionic gonadotropin immunoreactivity tissues from patients who excreted low levels did not. These latter molecular forms may account for the differences in the levels of human chorionic gonadotropin immunoreactivity excreted by the two types of patients.

[Clinical studies on ceftriaxone in the field of obstetrics and gynecology].

Clinical studies were made on ceftriaxone (CTRX, Ro 13-9904), a new long-acting cephalosporin antibiotic, with the following results. Following a single intravenous injection of 1 g, the transfer of CTRX to the internal genital organs was found to be good. The transfer of CTRX to exudate of the dead space of pelvis was also good. Elbow vein and uterine artery blood serum levels revealed marked increase immediately after administration, then followed by gradual reduction at very slow rate. CTRX was given to 3 patients of female genital infections. Efficacy was excellent in 1 case and good in 2 cases. As to side effect, 2 cases of diarrhea and 1 case of leukopenia were observed.

[Problems in the treatment of early carcinoma of the uterine cervix].

Limited operation is in side spread use recently in surgical treatment of in situ (Stage 0) and microinvasive (Stage Ia) carcinomas of the uterine cervix. Which purpose is to avoid such postoperative complications as vesical paralysis, therefore the simple total or modified radical hysterectomy is performed. However, the conization of the cervix may be received among the young women with strong desire to preserve their child-bearing potential. It is utmost importance to make a correct histological diagnosis prior to the performance of these procedures. In addition it is of importance to examine the histological rest of cancer, postoperatively. On the radiation therapy for Stage 0 and Stage Ia cancers, it is a general agreement that be treated only by intracavitary irradiation.
