Topical psoralen photochemotherapy for atopic dermatitis: evaluation of two therapeutic regimens for inpatients and outpatients.

Forty-seven adolescent and adult patients suffering from long-standing atopic dermatitis (AD) too severe to respond to conventional therapies were treated with topical psoralen photochemotherapy (PUVA) and relatively low doses of ultraviolet (UV) irradiation. For practicality and convenience, two different therapeutic regimens were implemented; short-term hospitalization with almost daily irradiation (inpatient group, n = 23) and weekly irradiation combined with topical corticosteroids which had failed to manage symptoms before initiating the treatment (outpatient group, n = 25). Excellent therapeutic effects were achieved in 72% of the inpatients after 5-38 (mean 18.2) times of irradiation (mean cumulative dose; 44.7 J/cm2). In addition, 56% of outpatients responded excellently to the treatment after 6-22 (mean 13.0) times (mean cumulative dose; 25.8 J/cm2). In fact, 16 of the inpatients and 10 of the outpatients achieved almost complete remission. The duration of remission in these patients was 1-25 months (mean 6.3 months) in the inpatients and 1-6 months (mean 3.0 months) in the outpatients. Peripheral blood eosinophils significantly decreased after the treatments. A follow-up study showed a delayed decrease in serum IgE levels. In contrast, the specific IgE to house dust antigens and the water holding capacity of stratum corneum did not vary after treatment.

HLA-DR antigen expression on peripheral T cell subsets in pityriasis rosea and herpes zoster.

Using 2-color fluorescein-activated cytometric analysis, HLA-DR antigen expression on peripheral blood T cell subsets was studied in patients with herpes zoster (HZ), pityriasis rosea (PR) and psoriasis. In HZ and PR, HLA-DR was found to be significantly expressed on T cell surfaces (CD3+ cells), when compared to that of the normal control (HZ: p less than 0.001, PR: p less than 0.05). Among T cell subsets, such HLA-DR antigen was predominantly expressed on suppressor/cytotoxic cells (CD8+) in HZ (vs. normal control, p less than 0.01). However, in the case of PR, it was predominantly expressed on helper cells (CD4+; vs. control, p less than 0.05). On the other hand, activated T cell antigen (CD25+) was not significantly expressed on T cells (CD3+) in either HZ or PR. In the T cell subsets, HLA-DR antigen expression returned to normal levels during the recovery phases of HZ and PR.

[A case report of progressive symmetric erythrokeratodermia and a review of progressive erythrokeratodermia in Japan].

The patient is a 5-year-old boy. There was no family history of the similar skin eruptions. The erythema with scales appeared on his head, face and neck at 1 month of birth. The erythematous hyperkeratotic plaques spread symmetrically. There was no follicular components. The eruption was chronic and persistent. In histopathology, hyperkeratosis with focal parakeratosis, acanthosis and intact granular layer were observed. In electron microscopy, a large number of mitochondria that frequently appeared to be swollen and lipidlike vacuoles in corneocytes were observed. On account of the features, we diagnosed this case as progressive symmetric erythrokeratodermia (PSE). PSE is a rare disease, and besides, it is difficult to define PSE because of contradictions in existing literatures, the variability of clinical expression and the lack of histologic, hereditary, and biochemical criteria. Nearly 70 cases were diagnosed as PSE and its variants in Japan i.e. 4 cases with PSE, 8 cases with progressive erythrokeratodermia, and 58 cases with Asahi-Ijiri disease and its variants. However, only 2 cases diagnosed as PSE and 4 cases nominated as others satisfied the recently established criteria by Ruiz-Maldonado, et al. In this paper, reported cases of PSE and its variants were reviewed and reassessed.

Purification and characterization of an extracellular proteinase from Hendersonula toruloidea.

An extracellular proteinase from a fast-growing strain of Hendersonula toruloidea was demonstrated when it was cultivated in liquid medium containing bovine serum albumin as the sole nitrogen source. Purification to homogeneity of the proteinase was performed by carboxymethyl cellulose, CM Sephadex G-50, and Sephacryl S-200 column chromatographies. The purified enzyme was a chymotrypsinlike serine proteinase, as indicated by the strong inhibitory activities of diisopropyl fluorophosphate, phenylmethylsulfonyl fluoride, L-1-tosylamido-2-phenylethylchloromethyl ketone, and chymostatin and good kinetic constants for a synthetic substrate, Suc-Ala-Ala-Pro-Phe-MCA. The enzyme had a pI of 8.4, a pH optimum of 9.0, and a molecular weight of 34,000. Skin constituents such as stratum corneum and nail, but not hair, were easily digested by this enzyme. Thus, this extracellular proteinase may play a role in the invasion of thickly keratinized skin and nail by this organism.

Effect of glycyrrhizin on pain and HLA-DR antigen expression on CD8-positive cells in peripheral blood of herpes zoster patients in comparison with other antiviral agents.

Glycyrrhizin (GL) is a saponin widely used as an anti-inflammatory agent. Pain intensity and HLA-DR antigen expression on CD8+ cells were assessed during and after treatment with GL. Other agents such as acyclovir, gamma-globulin and interferon beta were also administered for comparison. Pain resolved most rapidly among those treated with acyclovir followed by those treated with GL. Pain resolution correlated with the regression of HLA-DR+ in CD8+ subpopulations in peripheral blood. GL is suggested to be an alternative or additive antiviral agent to herpes zoster.

[Topical psoralen photochemotherapy (PUVA therapy) for severe atopic dermatitis (I): An analysis of correlation between effectiveness of PUVA therapy and clinical parameters].

Twenty-five patients with severe atopic dermatitis (AD) were subjected to PUVA therapy (topical 8-MOP application plus UVA irradiation). Excellent resolution was achieved in 18 cases (72%). Complete remission lasting 6.3 months in average was gained. In addition, the correlation between effectiveness of PUVA therapy and clinical parameters was statistically analyzed. As a result, an effectiveness of PUVA on AD was well correlated with sex (female), lower minimal phototoxic dose, higher severity and higher blood eosinophil counts. On the other hand, such correlation with the effectiveness was little in serum IgE levels, IgE.RAST scores to the house dust antigens and total irradiation doses.

[Relation between movement of acid or alkaline proteinase from Trichophyton rubrum and the growth of fungus, pH in medium].

We have previously reported that T. rubrum produced a new type of extracellular proteinase in liquid medium containing bovine serum albumin. This enzyme had an optimal pH 4.5 for azocoll. In this paper, we studied about the relation between movement of acid or alkaline proteinase and environmental pH. In the beginning of cultivation, acid proteolytic activity increased radically, and 3 or 4 weeks later, it was decreased. In place of acid proteinase, alkaline proteinase which had an optimal pH 8.0 was increased gradually. During incubation of T. rubrum in bovine serum albumin medium which was adjusted to pH 6.0, the pH of medium was gradually increased to about pH 7.0 toward 4 weeks. The growth of T. rubrum increased until pH in medium was fixed to pH 7.0.

[Quantitative evaluation of perspiration with equipment for continuous recording of local perspiration volume (II): Hyperhidrosis and hypohidrosis (or anhidrosis)].

To objectively evaluate perspiration volume, we tried to measure the perspiration volume from 6 cases of hyperhidrosis and 4 cases of hypo-(or an-) hidrosis using recently developed equipment for continuous recording of local perspiration volume. Patients with hyperhidrosis were loaded with various factors to possibly influence the perspiration volume, and the effects were compared. The results revealed: 1) The measurement of local perspiration volume with this equipment provides objective data useful for the diagnosis of hyperhidrosis and hypo-(or an-) hidrosis and for the judgement of its grade; 2) in case of palmar hyperhidrosis, mental stimuli most strongly induced perspiration; and 3) the responses to mental arithmetic or hand grasping and the base-line stable time are reliable parameters for measurement of perspiration volume.

[HLA-DR antigen expression on peripheral T cell subsets in pityriasis rosea Gibert, herpes zoster, and psoriasis].

Using 2 color fluorescein activated cytometric analysis, HLA-DR antigen expression in T cell subsets was studied in pityriasis rosea (PR) and compared to the results from herpes zoster (HZ) of viral origin and psoriasis (Ps). In HZ and PR, HLA-DR was significantly expressed on the T cell surface (Leu-4+ cells). Among the T cell subsets, HLA-DR antigen was predominantly expressed on suppressor/cytotoxic cells (Leu-2a+) in HZ. In contrast, it was predominantly expressed on helper cells (Leu-3a+) in PR. However, activated T cell antigen (Tac) was not significantly expressed on T cells (Leu-4+) in either HZ or in PR. This HLA-DR antigen expression of T cell subsets was depressed to the normal level in the recovery phases of HZ and PR.

[The relation between the growth and extracellular proteolytic activity in trichophyton rubrum, comparing isolates from granuloma trichophyticum with tinea superficialis].

Trichophyton rubrum, a major pathogenic fungus in dermatophytosis, produced a new type of extracellular proteinase, in addition to those previously reported, when cultivated in liquid medium containing bovine serum albumin. This enzyme had an optimal pH 4.5 for azocoll and was found in both isolates from tinea superficialis and granuloma trichophyticum. The proteolytic activity of isolates from granuloma trichophyticum were superior to isolates from tinea superficialis in both acid and alkaline proteinase. But the growth of T. rubrum isolates from tinea superficialis were better than that from granuloma trichophyticum.

[Evaluation of a measurement method for perspiration volume using newly developed equipment for continuous recording of local perspiration volume (Kenz-Perspiro) (I)].

Using newly developed equipment for continuous recording of local perspiration volume, we have tried to standardize the measurement of perspiration volume and evaluate it. This equipment enables continuous determination of local perspiration volume more easily and accurately than the ventilation method ordinarily used. Induced perspiration was measured on the inner surface of the right thumbs of 20 healthy male and female volunteers. Four inducing factors, grasping the hand with warm and cold hand mental arithmetic, and deep breathing, were used as stimulations. Perspiration induced by each factor was compared and evaluated. As a result it was concluded that the stimulation of mental arithmetic was the most stable and reliable factor. Another measurement factor, base line stable time from mounting the equipment, was also a considerably reliable measurement basis for evaluating the perspiration volume and mental state.

Epidermolysis bullosa acquisita antigen: relationship between the collagenase-sensitive and -insensitive domains.

To clarify the relationship of the 290 and 145 kDa chains of the epidermolysis bullosa acquisita (EBA) antigen, we subjected urea extracts of skin basement membrane zone (BMZ) proteins and isolated 290 and 145 kDa chains of the EBA antigen cut out of sodium dodecyl sulfate polyacrylamide gels to treatment with clostridial collagenase. When the reaction products were electrophoresed, transblotted, and reacted with EBA patient sera or two monoclonal antibodies to the EBA antigen, the 290 kDa chain was degraded into the 145 kDa band that was resistant to cleavage with collagenase. The 145 kDa domain, isolated after collagenase treatment of the whole BMZ extract, was resistant to degradation by hyaluronidase, chondroitinase ABC, heparinase, and heparitinase but was readily degraded by V-8 protease. These data suggest that the EBA antigen consists of collagen and noncollagen domains of identical size (Mr 145,000), and that the 145 kDa noncollagen domain is generated via degradation of the native 290 kDa species by collagenase.

Effect of calcium and F-actin on the rate of SH2 modification in skeletal myosin.

The rate constant of modification of a specific thiol group, SH2, with N-ethylmaleimide (NEM) has been used to estimate the conformational change in the local area containing SH2 (SH2 region) of skeletal myosin as a structural probe. The rate of Mg2+-ATP-induced SH2 modification of subfragment-1 (S-1) isozymes was regulated by Ca2+ in the pCa range below 6.4 and was not regulated in the pCa range above 6.4. No substantial difference between S-1 containing alkali light chain, A1, (S-1(A1] and S-1 containing alkali light chain, A2, (S-1(A2] was observed in the Ca2+-dependent rate of SH2 modification. Due to the presence of this Ca2+ regulation in myosin (absence in S-1 isozymes) in the pCa range above 6.4, absence of 5,5'-dithiobis-(2-nitrobenzoic acid) (DTNB) light chain in S-1 isozymes, and high affinity of Ca2+ for DTNB light chain, this Ca2+ regulation in the pCa range above 6.4 is possibly related to the Ca2+ binding to DTNB light chain. F-Actin, which is entirely free from tropomyosin and troponin, enhanced the rate of Mg2+-ATP-induced SH2 modification of S-1 isozymes equally and of myosin, and reduced the Ca2+ sensitivity with an increase in F-actin concentration.

Localization of the alpha 3 (V) chain of type V collagen in human skin.

Serum from goats immunized with human type V collagen chains that were cut out of polyacrylamide gels contained an antibody that recognized only type V collagen in an enzyme-linked immunoabsorbent assay and did not label laminin, fibronectin, or types I and IV collagen. Western blot analysis of the antibody showed that its determinant was the alpha 3 (V) chain of type V collagen. Indirect immunofluorescent staining of intact human skin with the antibody produced staining of the dermal blood vessels but not of the dermal-epidermal junction (DEJ). In contrast, both the dermal blood vessels and the DEJ were labeled by the antibody if the skin substrate was first split through the lamina lucida region of the DEJ by incubation in 1 M NaCl solution. Indirect immunoelectron microscopy confirmed the staining pattern found by immunofluorescence and defined the ultrastructural localization of type V collagen in skin. Type V collagen is localized within the DEJ to the lamina lucida region and polar aspects of the basal cell keratinocyte plasma membrane.

Nettleship-Falls X-linked ocular albinism with Axenfeld's anomaly. A case report.

A Japanese male with Nettleship-Falls X-linked ocular albinism (NXOA) and associated with Axenfeld's anomaly is described here. The diagnosis of X-linked ocular albinism was difficult because of moderately pigmented fundus and lack of iris translucency, but macromelanosomes recognized by histological study of skin biopsy were helpful for diagnosis. This is the first report identifying the macromelanosomes in Japanese NXOA. A combination of ocular albinism and Axenfeld's anomaly is very rare. There might be a common defective factor for the development of these two congenital disorders.

Steroid sulphatase deficiency in patients initially diagnosed as ichthyosis vulgaris or recessive X-linked ichthyosis.

Twenty-one patients with ichthyosis were classified as either ichthyosis vulgaris (IV) (five cases) or recessive X-linked ichthyosis (RXLI) (sixteen cases) by using a steroid sulphatase assay of plantar callus and peripheral leukocytes. The patients had presented with various clinical manifestations, which had resulted in some initial misdiagnoses. Cases which initially resemble IV may in fact be RXLI, although we found that if a case is initially diagnosed as RXLI it is unlikely to be a case of IV.
