Ferrokinetic evaluation of erythropoiesis in patients with myelodysplastic syndromes.

Erythropoietic activity in patients with myelodysplastic syndrome (MDS) was evaluated by ferrokinetic measurements. Since the conventional plasma iron turnover of MDS patients increased with plasma iron levels after multiple blood transfusions, erythron transferrin uptake was chosen as a parameter of erythroid marrow activity. Although a correlation was shown between plasma iron level and plasma iron turnover (r = 0.50, 0.01 less than p less than 0.02), no correlation existed between the plasma iron level and erythron transferrin uptake (r = 0.25, p greater than 0.1). Erythron transferrin uptake, independent of plasma iron, was significantly higher in MDS patients than in normal subjects (110.6 +/- 67.6 and 67.6 +/- 18.8 mumol/l/dl, respectively; 0.01 less than p less than 0.02). An increased erythropoiesis occurring concomitantly with morphologically normal or increased erythroid cellularity was demonstrated in patients with MDS. The measurement of erythron transferrin uptake might be valuable as an accurate expression of erythroid activity in the hyperferremic state.

Sum peak method applied to the study of human platelet cells.

The sum peak method based on the phenomenon of perturbed angular correlation was applied to study the chemical environments of 111In in intact platelet cells. The sum peak appearing in the gamma-ray spectrum of 111In which emits two gamma-rays in cascade was observed for 111InCl3 solutions and 111In-labeled platelets. An increase in intensity ratios of the sum peak to its single peak was found when 111In incorporated into platelets from its aqueous solution. This fact was discussed in terms of physico-chemical environments of 111In together with an evaluation of sum peak analysis in the spectra.

Influence of vitamin E on platelet survival.

Vitamin E plays an important role in protecting fat tissue from peroxidation. Platelet survival was compared among vitamin E deficient, control and alpha-tocopheryl nicotinate supplemented rats. The mean tocopherol level in the sera of the rats fed vitamin E deficient diet for two months was 21.5% of control, and the lipoperoxide level in the sera of these animals had not significantly (p greater than 0.05) increased. Platelet aggregation was not accelerated significantly (p greater than 0.05) after exposure to a concentration of 10 microM of ADP used as an aggregating agent. The rate of platelet survival was determined by the 51Cr-labeling technique. The survival rates were all normal in vitamin E deficient, control and alpha-tocopheryl nicotinate supplemented rats, with no significant difference (p greater than 0.1) among the groups. These results suggest that depletion of vitamin E per se does not shorten the periods of platelet survival.

Prolongation of platelet survival in idiopathic thrombocytopenic purpura by high-dose intravenous gamma globulin.

The mechanism of the increase of platelet counts after the intravenous injection of high dose gammaglobulin was evaluated by measuring platelet survival before and after the injection. The platelet survival after treatment prolonged more significantly than that before treatment. There was no evidence of platelet release from bone marrow to peripheral blood. These findings might suggest the inhibition of platelet destruction by transient blockade of reticuloendothelial system.

[Rhabdomyosarcoma of the heart involving the left ventricle and left atrium].

A case of rhabdomyosarcoma of the left atrium and left ventricle demonstrated by echocardiography was reported. A 31-year-old man was admitted to our hospital for evaluation of recently developed exertional dyspnea. A holosystolic murmur and a protodiastolic sound were audible at the apex. A chest X-ray showed pulmonary congestion without cardiomegaly. The two-dimensional echocardiogram showed a dense stratified mass of echoes occupying the medial half of the left ventricular cavity, and a part of the abnormal mass of echoes was observed to move toward the left ventricular outflow tract during systole. Another small mass attached to the anterior mitral leaflet was also observed to prolapse partly into the left atrium during systole. The interatrial septum showed a thick and hard band of echo in the short-axis view. Right cardiac catheterization revealed pulmonary hypertension and the levogram of the pulmonary angiography showed left atrial and left ventricular filing defects. The repeated echocardiographic study showed the growth of the abnormal mass. The patient underwent operation, but he died of congestive heart failure thereafter. The necropsy diagnosis was rhabdomyosarcoma of the heart, involving the left atrium and left ventricle.
