Changes in muscarinic acetylcholine receptor binding in skin slices of cholinergic urticaria.

Muscarinic acetylcholine receptors were studied in slices of unaffected skin and in lymphocyte membranes from 3 patients with cholinergic urticaria and 5 healthy subjects using receptor autoradiography and the radioligand [3H]quinuclidinyl benzilate binding technique. Autoradiography revealed that muscarinic acetylcholine receptors were located in regions with sweat glands. The distribution of receptors did not differ qualitatively between patients and controls. However, the maximum number of muscarinic acetylcholine receptors was reduced in skin from patients with cholinergic urticaria, by 44% as compared with the controls, whereas the binding affinity was not affected. Muscarinic cholinergic binding in lymphocyte membrane fractions obtained from patients with cholinergic urticaria did not differ statistically from that of healthy control persons.

Adrenergic urticaria and adrenergic pruritus.

We report here on 2 patients with adrenergic urticaria and adrenergic pruritus, respectively. The lesions and features developed during phases of stress and during the attacks were associated with an increase in the plasma concentrations of noradrenalin, adrenalin and prolactin. The dopamine plasma level was elevated only in the case of adrenergic urticaria. The symptoms could be reproduced by intradermal injection of adrenalin and noradrenalin and treated successfully with propanolol, a blocker of beta-adrenergic receptors. Adrenergic urticaria is a rare but distinct entity, which has to be separated from cholinergic urticaria. Adrenergic pruritus seems to be a minor variant of adrenergic urticaria.

[Characterization of lymphocytic infiltrate cells in bullous pemphigoid and pemphigus vulgaris].

By means of monoclonal antibodies (mab) and alkaline phosphatase anti-alkaline-phosphatase (APAAP) technique, the distribution and characteristics of lymphocytic infiltrates were studied in biopsies from 15 patients with bullous pemphigoid (b.P.) and 5 patients with pemphigus vulgaris (P.v.). The biopsies were obtained from freshly developed blisters along with perilesional tissue. The lymphocytic infiltrates in b.P. as well as in P.v. were located in the area of the fresh lesions and consisted almost exclusively of T cells (CD3 positive lymphocytes). In b.P., the discrepancy between a decreased number of CD2 positive lymphocytes (10-20%) and markedly higher number of CD3 staining cells (50-60% of all infiltrate cells) was striking. The characterization of the T cell subpopulations in both dermatoses showed mainly T helper cell infiltrates (CD4), while only up to 10% of T suppressor cells (CD8) were detected. CD45R positive (CD4+/CD45R+: suppression inducing) as well as CD29-positive (CD4+CD29+: cells which provide help for antibody production) T-cell subpopulations were detected in both dermatoses particularly adjacent to blister in up to 10% of the cell infiltrates. Epidermal staining with CD29 mab in b.P. up to the stratum spinosum and in P.v. within intraepidermal blisters was detected. By means of CD19 and CD21 mab B lymphocytes were minimal. Perivascular individual cell staining occurred with CD7 CD16, CD56 and CD57 mab (K/NK cells) in b.P. as well as in P.v. patients. The predominance in T cell infiltrates, particularly T helper cells, suggests the role of cellular immune mechanism in the pathogenesis of these diseases, in b.P. the CD2 antigen (SE receptor) appears to be of particular importance.

[Psychosomatic dermatology].

An appreciative cooperation among dermatology, clinical psychology and psychology and psychiatry is necessary and useful. By selected facts interactions between central and autonomous nervous system on one side and the immune system on the other side were described. As far as the neutrocutaneous interrelations are concerned the cutaneous stimulation appears to be an important factor for the physical and physiologic development. In addition, the skin communicates to the environment and other people. Therefore, it plays a role in the social integration. Psychiatric diseases can affect the skin (e.g. delusions of parasitosis in schizophrenia). On the other hand primary skin diseases such as psoriasis, atopic dermatitis, acune vulgaris, chronic idiopathic urticaria and alopecia areata may induce psychologic features. The characteristics of the personalities of dermatological patients are discussed. The psychodiagnostics (personal interview, questionaires) is followed by psychotherapeutic procedures such as personal consultations, treatment in groups, hypnosis and autogenous training. Our experiences in the psychodiagnostics and treatment are briefly reported. Finally, psychotropic drugs-antipsychotic, anti-depressant, antianxiety, and hypnotic agents - may be useful as an adjunct in the management of dermatologic disorders, if applied under precautious indications.

[N-procollagen(III)peptide and lysosomal beta-galactosidase in progressive scleroderma and silicosis].

The concentration of type III collagen aminopropeptide and the activity of lysosomal beta-galactosidase were determined in the serum of patients with progressive sclerosis (PS), silicosis and PS associated with silicosis. In PS-patients, the serum levels of both parameters were significantly increased with a high correlation to the stage of the disease and to each other. In patients with silicosis normal serum concentrations of both parameters were found, whereas in all patients suffering from silicosis and PS significantly elevated serum levels were detectable, independent of the stage of PS. Both parameters can also be used as markers of therapy (e.g. with D-penicillamine) and as early indicators for the development of PS in patients with silicosis.

[Exacerbation of progressive scleroderma following roentgen therapy].

In a 38-year-old female patient suffering from progressive scleroderma and breast carcinoma the clinical features were exacerbated and became generalized after radiotherapy, and unfortunately regressed only transiently during aggressive polychemotherapy. Links between progressive scleroderma and carcinoma and the immune system are discussed. The risk of post-irradiation worsening of progressive scleroderma is pointed out.

[Effect of pepstatin A on Candida albicans infection in the mouse].

The intravenous injection of 10(6) to 10(7) Candida albicans cells revealed to be a reliable model in mice produce infections of the kidney. Higher germ contents could be yielded in the kidney after the application of protease positive Candida-strains as compared to protease negative ones. Additionally, after the infection with protease positive strains a proteolytic activity could be found in the kidney homogenates in vitro on casein plates. The modulation of this Candida infection by pepstatin A, an inhibitor of extracellular yeast proteases in vitro, has also been studied in the same mice model in vivo. The growth rate of Candida albicans has been measured in the left kidney by counting the germ content as described by Haenel. Infections could be reduced after single doses of 120 micrograms pepstatin A contrary to 60 micrograms pepstatin A. The same was with three doses of 180 micrograms at 24 h intervals. This protease inhibitory effect could also be found in kidney homogenates in vitro on casein plates and lasted until 48 h post injection. On the basis of this positive effects on the Candida infection in mice pepstatin A should be considered as an adjuvans in the therapy of severe yeast infections.

[8-MOP serum level profile following administration of Geroxalen to patients with psoriasis and a control group].

UNLABELLED: After 8-MOP application (Geroxalen) the mean serum levels with 11 single values over 8 hours from 21 psoriatics with squamous infiltrative plaques do not differ from serum level curves of 46 healthy volunteers (Psoriatics: maximum plasma level cmax 189 nm 8 MOP/ml serum at the time tmax 1,2h; area under the curve AUC0-8 284 ng.ml-1.h. CONTROL GROUP: cmax 193 ng 8 MOP/ml serum; tmax 0.9h; AUC0-8 290 ng.ml-1.h). A malabsorption of 8-MOP in psoriatic patients could not be found in our study, although this is described for fat among other.

[8-methoxypsoralen serum content of problem patients in PUVA therapy].

Beside the stage, expression and course of psoriasis as well as the compliance of patients low or abnormal 8-MOP levels are the cause of the ineffectiveness of the PUVA-therapy, as we could demonstrate in 7 of 11 problem cases. Thus, we determined only a maximal 8-MOP concentration of 47 ng/ml serum as an extreme value in one patient and on other patient had his maximal serum peak 3-4 hours after 8-MOP-application and only a value of 8-MOP of 18 ng/ml serum at the recommended time of irradiation. In this connection a first pass mechanism for 8-MOP by a limited biotransformation capacity in the liver is significant and one basic reason for the interindividual variation. Therefore in these cases the knowledge of the individual serum profiles of 8-MOP is helpful to adapt the time of irradiation to the individual necessities and/or to increase the 8-MOP doses.

[3d Congress of the German Dermatological Society 1891 in Leipzig].

The course and subject matter of the 3rd congress of the German Dermatological Society held in Leipzig 100 years ago, are described. The main topics were tuberculosis, eczema, theory of psorospermoses and venereology, and these, together with the clinical presentations, reflect the level of research at that time and the charisma of the personalities in dermatology. Some of the subjects discussed are still topical and are still providing the stimulus for future research activity.

[Pharmacokinetic comparison of 3 8-methoxypsoralen preparations and clinical aspects].

In order to register the inter- and intraindividual variability of serum levels of 8-methoxypsoralen (MOP) (Oxsoralen, n = 12; Geroxalen, n = 46; Mopsoralen, n = 30), we investigated a total of 1,276 serum samples obtained at 11 patient appointments over 8 h in 88 patients and compared the individual serum concentrations - time curves with the pharmacokinetic parameters Cmax (maximal serum concentration) on the time tmax, as well as the AUC value (area under the curve) as a parameter of bioavailability. The individual serum concentration value showed a high interindividual variability (Oxsoralen Cmax: 47-473 ng.ml-1; Geroxalen Cmax: 37-416 ng.ml-1 and Mopsoralen Cmax: 44-300 ng.ml-1) and preparation specific differences (Oxsoralen Cmax: 188 +/- 125 ng.ml-1; tmax: 2.0 +/- 0.6 h; AUC: 461 +/- 322 ng.ml-1.h; Geroxalen Cmax: 193 +/- 87 ng.ml-1; tmax: 0.9 +/- 0.4 h; AUC: 290 +/- 198 ng.ml-1.h; Mopsoralen Cmax: 164 +/- 71 ng.ml-1; tmax: 1.7 +/- 0.5 h; AUC: 389 +/- 221 ng.ml-1.h). On repeated administration of Geroxalen (n = 16) and Mopsoralen (n = 12), minimal intraindividual distributions were noted, so the mean serum concentration curves did not show a significant difference. The importance of 8-MOP serum level determinations for better puva therapy, particularly in therapeutic failure in problem cases, is discussed.

Treatment of scabies with permethrin versus lindane and benzyl benzoate.

This open clinical study was designed to evaluate and compare the efficacy and side effects of lindane (1% and 0.3%), benzyl benzoate (20% and 10%) and permethrin (5% and 2.5%) after two, three, and one application at bedtime, in the treatment of scabies in 114 adults and 80 children aged between 0 and 5 years. Treatment failures were registered after lindane in 3 adults and 2 children, whereas benzyl benzoate and permethrin cured all patients as assessed after a 3-week follow-up. The number of irritations and post-scabious eczematous reactions was increased after benzyl benzoate treatment. Permethrin proved to be very reliable and exhibited few side effects when applied once at bedtime. Because of the percutaneous absorption and neurotoxicity of lindane, the application of permethrin can be recommended as a useful alternative in premature infants and small children, patients with seizures and neurological complications, in cases of therapeutic failure with lindane the treatment needs to be repeated, in scabies crustosa, as well as in children, pregnant women and nursing mothers.
