[The treatment of scabies with permethrin in comparison with lindane and benzyl benzoate].

This open clinical study was designed to evaluate and compare the efficacy and side-effects of lindane (1% and 0.3%), benzyl benzoate (20% and 10%) and permethrin (5% and 2.5%) after twice-, three times- and once application at bedtime, respectively, in the treatment of scabies in 104 adults and 70 children aged between 0 and 5 years. Treatment failures were registered afer lindane in 3 adults and 2 children, while benzyl benzoate and permethrin cured all patients as assessed after 3 weeks follow-up. The number of irritations and postscabious eccematous reactions was increased after benzyl benzoate. Permethrin revealed to be very reliable and exhibited only a few side-effects when applied once at bedtime. Because of the risk of the percutaneous absorption and neurotoxicity of lindane, the application of permethrin can be recommended as an useful alternative in premature infants and small children, patients with seizures and neurological complications, in cases of treatment failures of lindane with the need to repeat the therapy, in scabies crustosa as well as in children, pregnant woman and nursing mothers.

[Expression of leukocyte antigens in human sperm].

The expression of leukocyte antigens on human ejaculated spermatozoa and lymphocytes was compared by means of the immunofluorescence technique and monoclonal antibodies against anti-HLA class I (pan-HLA-A,B,C) and class II (DR, DR/DP). Lymphocytes showed specific fluorescence phenomena after incubation with the antibodies. However, in the case of spermatozoa no immunofluorescent labeling was detected. Modifications of the method of sperm preparation (Ficoll centrifugation, swim-up technique, addition of azide, fetal calf serum or benzamidinchloride) did not lead to different results. We were not able to find HLA-antigen expression on fresh human spermatozoa using immunofluorescence.

[Allergic reactions of the immediate and delayed type following prednisolone medication].

Allergic sensitization due to corticosteroids seldom occur and are compound-specific, as a rule. They induce various clinical features, in particular urticaria, different exanthematous reactions and contact dermatitis as shown by our observations in 10 patients with prednisolone-allergy. In three cases a simultaneous allergy due to methylprednisolone was found. Impairment of the features or alterations in the clinical morphology after the application of prednisolone suggest the diagnosis, which can be confirmed by means of the scratch and epicutaneous testing and in one patient by the oral exposure, additionally. In addition a delayed-type sensitization to propylene glycol could be proved in three cases, and the same was with romulgin and parabens in one case each. Dexamethasone was used and tolerated as alternative and emergency medication in equivalent doses.

[The behavior of membrane receptors of peripheral blood lymphocytes in bullous pemphigoid].

In 35 patients with bullous pemphigoid (BP) various parameters of the cellular immune system were studied. It was shown that the average 29% decrease in the number rosette-forming lymphocytes in sheep red blood cells is converted by the immune complexes and by a second serum factor in the acute stage of BP that has not yet been characterized precisely. These changes were reversible in patients who were placed on immunosuppressants. Obviously, this mechanism is partly due to the binding of immune complexes via Fc receptors on T-lymphocytes and is consequently due to the steral blocking of sheep erythrocyte receptors. No decrease in the number of T-cells was found by means of monoclonal antibodies (CD2, CD3), and the number of suppressor cells was also normal (CD8). On the other hand, the number of T-helper cells increased by an average of 15% (CD4). In addition, in the acute stage, there is a significant increase in the number of activation antigen Tac (CD25) and DR-bearing lymphocytes: 29% and 37% respectively (the normal values are 2% and 11%, respectively). Double staining permitted the phenotype of activated lymphocytes to be detected within the T-cells and the T-helper cell subpopulation, respectively. The number of activated T-helper cells correlates with the disease activity, and from that point it seems to be useful as a marker of activity in follow-up studies.

[Treatment of chronic recurrent erysipelas with streptococcal vaccine].

One hundred patients suffering from relapsing erysipelas were treated with streptococcal vaccine consisting of a phenol- und heat-inactivated mixture of 12 different types of streptococci. Simultaneously the entry point was cured and general disposition and local terrain factors were treated. In the majority of cases further relapses were avoided or their frequency reduced. The results of the treatment with streptococcal vaccine were compared with those of long-term benzathine-penicillin therapy in 47 patients. The vaccine has proved to be equivalent to the penicillin therapy and can be recommended as a real alternative to it for prevention of further infections and late sequelae, especially those of chronic lymphedema. The beneficial therapeutic effect of the vaccine is probably attributable to a non-specific immune stimulation. A partial cellular immune defect involving the protective M-antigen together with simultaneous partial abnormalities of immune reactivity is discussed as a cause of relapsing erysipelas.

[Bacterial skin flora, host defense and skin infections].

Resident skin flora consists of coagulase-negative staphylococci, micrococci, aerobic and anaerobic coryneform organisms (i.e. propionibacterium acnes) as well as gram-negative rods (i.e. acinetobacter). Their growth is favoured by increased temperature and humidity and modified by body location, age, sex and chronic diseases (i.e. diabetes mellitus). Occupation, hospitalization, soaps and disinfectants as well as medications exert promoting and inhibiting influences, too. In addition, environment gains significant importance due to trauma and implants as well as contact with animals, infected persons and carriers. Whereas bacteria attach by adhesins and succeed other bacteria by factors of pathogeneity such as exotoxins, exoenzymes, bacteriocins and various interference mechanisms, skin exerts its resistance and defense by means of the intact horny layer, proliferation and desquamation, lipids and fatty acids as well as sweat, in addition, by means of IgA and the specific skin immune system with complex interactions between Langerhans-, TH-, TS- and cytotoxic T-cells, interleukins and cytokines. Furthermore the non-specific defense system (complement, NK-cells, granulocytes, mononuclear phagocytic system, inflammation) is involved. Finally, skin infections caused by resident bacterial flora are briefly discussed.

[Treatment of fungus infections of the skin with tioconazole (Mykontral)].

73 patients with fungal infections due to dermatophytes (49), yeasts (16) and Malassezia furfur (8) could be cured clinically in 71% and mycologically in 88% of cases by the application of Mycontral-Lotion or -cream twice daily for 33 days on average. In 28% improvement could be reached, in one patient the therapy failed. Treatment is simple, clean and well acceptable. Side-effects occurred as slight redness, transient burning and pruritus on the eroded skin in the initial phase in 8 patients (11%). Mykontral contains 1% tioconazole as a modern imidazol compound. It has been characterized as an effective topical broad spectrum antimycotic agent. Its introduction and application in the GDR is to be recommended.

[HLA-A, B, C phenotype frequencies in progressive systemic scleroderma].

In 41 patients suffering from progressive systemic sclerosis (PSS) the phenotype frequencies of 28 HLA-A, B, C antigens were determined. Compared to 2947 blood donors the PSS patients exhibited an increase in the HLA-B8 frequency (37% versus 19%). This difference revealed to be not significant after the correction according to the number of antigens tested. The same was with the increase in the frequency of HLA-B5, B13, B27, Cw2 and the decrease in the frequency of HLA-A2, B12, B14, B15, B4 and Cw3. The results were compared with the data reported in the literature. Additionally, there were no differences when compared the diffuse PSS with the acral form and CREST, respectively.

Lymphocytotoxic autoantibodies in progressive systemic sclerosis.

In 53 patients with progressive systemic sclerosis (PSS) the lymphocytotoxic activity of their serum was measured in a microlymphocytotoxicity assay. In 21 of the 53 patients the test reacted distinctly positively in the heterologous system, and in 9 of these 21 also in the autologous system. After preparation of the immunoglobulins from these positive sera, whole cytotoxic activity was detected only in the IgM fraction but not in the IgG fraction. When using prepared T lymphocytes as target cells in the microlymphocytotoxicity test, the cytotoxic activity of the positive PSS sera showed itself to be directed against this lymphocyte population. Further analysis using the Western-blot technique showed that the IgM autoantibody in PSS sera reacted with the cell surface of CD4+ lymphocytes. The cross reactivity with extractable nuclear antigens was rather improbable. These results suggest that lymphocytotoxic autoantibodies may play a role in immunological disturbances in PSS.

Localized nonscarring bullous pemphigoid of the vagina.

Here we report on a 60-year-old female patient suffering from multiple erosive inflammatory lesions of the vagina for 2 years. Subepidermal blister in the histological examination, deposits of IgG and C3 in the direct immunofluorescence studies and the basement membrane zone antibody titers between 80 and 320 have confirmed the diagnosis of bullous pemphigoid. The lesions responded to treatment with prednisolone and azathioprine fairly well. There have been no signs of scarring till now.
