Transplacental passage of intravenous immunoglobulin in the last trimester of pregnancy.

Immunoglobulin G was given intravenously (IVIgG) to pregnant women (27 to 36 weeks gestation) with signs of chorioamnionitis who were at risk for preterm delivery. Twenty-four patients received antibiotics alone (control group). Twenty-seven patients received the same antibiotics in combination with IVIgG, either 12 gm in 12 hours (low IVIgG dosage) or 24 gm on each of 5 consecutive days (high IVIgG dosage). Transplacental passage of IVIgG was shown to be a function of gestational age and of dose. Up to the thirty-second week of gestation, IgG infusions had no effect on IgG concentrations in cord sera. After that time, cord serum IgG levels were significantly higher in the high-dose group compared with the low-dose and control groups. All four subclasses of IgG, and two different antibodies present in the IVIgG preparation passed from the mother to the fetus. Thus the infused IgG mimicked the transplacental passage of endogenous IgG.

[Nutrition physiologic studies of swine with ileorectal anastomoses. 1. Surgical method, biochemical and morphologic findings].

For the purpose of studying nutrient digestion and absorption in the small intestine, pigs were provided with ileo-rectal anastomoses. The anaesthetic and operation method is described. X-ray examinations do not show a significant backflow of digesta into the from colon cranially open towards the anastomosis as well as a totally intact sphincter ani. With the help of additional doses of highly digestible, N-free nutrients rich in energy (starch + saccharose), the operated animals achieved a growth development corresponding to that of intact control animals. For the assessment of the physiologic state of the animals, the concentrations in the serum of the total protein, of albumin, urea, glucose, calcium, inorganic phosphorus and creatinine as well as the activities of aspartate amino transferase and alkaline phosphatase were determined. These showed for the respective criteria changes conditioned by age and dependent on feed intake. The fixing of the anastomoses did not result in disturbances of nutritional or organic damages. Histologic studies of the mucosa of stomach, duodenum and jejunum did not show pathologic changes. The weight of stomach, duodenum and jejunum, determined in 2 live weight ranges, is identical for operated and intact animals in relation to the weight of the empty body of the animals. The weight of ileum, caecum and colon of the operated animals is significantly lower, due to inactivity atrophy. The tissue amount available for the intake of a volume unit (weight-volume index) is nearly identical for operated and intact animals in the duodenum and the jejunum. The index functionally increasing for intact animals from the jejunum to the ileum (stenosis due to temporary digesta congestion in the ileum) remained the same with anastomosis animals for 29 or 26 weeks resp. after the operation. As a result of the biochemical and morphological studies it was shown that animals with an ileo-rectal anastomosis are suitable for the determination of nutrient digestibility at the end of the ileum.

[Determination of endogenous nitrogen in feces using the isotope technic].

A ration consisting of wheat gluten and N-free components was supplemented with L-lysine and L-leucine and fed to two groups of growing Wistar rats. Group 1 received 15N Lys and unlabelled Leu, group 2 received unlabelled Lys and 15N Leu in order to study the influence of the utilization of the 15N marker on the labelling quota of faeces and urine as well as various fractions of the body. The good utilization of Lys in group 1 results in a higher 15N excess in faeces and a reduced 15N abundance in urine in comparison to group 2 with a lower utilization of 15N Leu. The results show that the 15N abundance in urine is unsuitable as an indicator of the 15N labelling quota of endogenous metabolic faecal nitrogen.

[15N transamination in the administration of various tracer substances. 1. Whole body studies in rats].

4 groups of 3 growing Wistar rats each were orally given either 15N methionine, 15N lysine, 15N glycine or 15N ammonia sulphate over 10 days. By means of measuring 15N, the 15N accumulation in the amino acids (AA) of the body protein, statements were to be made on the transamination of the individual 15N substances and thus their suitability as tracer substances for studies of N metabolism. None of the tested 15N AA achieved a proportionate labelling of all AA of the body protein. The AA used as tracer in each case showed the highest 15N labelling of all AA in the body. Of the amino 15N detected in the animal body, ca. 19% were found in Met after a 15N Met application, ca. 88% in Lys after a 15N Lys application and ca. 50% in Gly after a 15N Gly application. After the application of 15N ammonia sulphate ca. 42% of the body amino 15N are apportioned to the essential and ca. 58% to the non-essential AA. Thus this substance produces a more proportional labelling of the essential and non-essential AA of the body protein than 15N Gly. The following quotas of the 15N amounts applied were found in the AA of the animal bodies: tracer substance lysine 52%, glycine 32%, ammonia sulphate 24%, methionine 21%. After summing up the amino acid 15N amounts in the animal body, eliminating in each case the tracer AA and taking into account the molecular weight of the AA, there was a good agreement of the intensity of the accumulation of 15N in the individual AA, irrespective of the applied tracer substance: arginine, glutamic acid, cysteine and aspartic acid highest, threonine, phenylalanine and lysine lowest accumulation.

IgG subclasses and antibodies to group B streptococci, pneumococci, and tetanus toxoid in preterm neonates after intravenous infusion of immunoglobulin to the mothers.

High doses of intravenous immunoglobulin were given to seven pregnant women between the 27th and 36th wk of gestation who were at risk for preterm delivery. Determinations of IgG subclasses and of antibodies against group B streptococcal serotypes, pneumococcal polysaccharides, and tetanus toxoid were done in maternal serum before and after intravenous IgG infusion and after delivery in cord serum. Substantial transplacental passage of the infused material could be observed in five cases where delivery occurred at the 34th wk or later. After the 36th wk of gestation, IgG subclass and antibody concentrations in cord serum were increased up to the levels in the maternal serum.

[Ultrastructure of the terminal vessels in squamous cell carcinoma of the human external ear. A transmission electron microscopy study].

Electron microscopic findings of vascular fine structure of the squamous cell carcinoma of the human auricle are reported. According to morphological criteria, the tissue was divided into three areas: non-infiltrated zone, transitional zone and centre of the tumor. In the non-infiltrated zone the vessels reveal the pattern of normal vessels of the skin. In the transitional zone endothelial cells bearing all signs of newly formed vessels can be detected as well as endothelial cells that show structural characteristics of severe cellular damage. In the tumour centre itself vessels are found with fenestrations and even true endothelial gaps. Interstitially located RBCs imitate the former outline of the vascular lumen. The observations are discussed and compared with the relevant literature.

[Indications for the use of the CO2 laser in gynecology].

In accordance with international results and own experiences twenty-two indications for use of carbon dioxide laser in gynecology are reported. It is referred concisely to intervention, diagnosis, special measures of laser surgery, and specific advantage of laser application. The comprehensive advantage is based on CO2 laser operating technique. The results of CO2 laser use are determined by locality and mode of application. The series of indications is not finished.

[Blood vessels of the respiratory and olfactory mucosa of the rabbit. A light and transmission electron microscopy study].

In our study, light and electron microscopic findings in vessels of the rabbit nasal respiratory and olfactory mucosa are reported. In the respiratory mucosa, most of the subepithelial tissue is occupied by large, sinusoidal veins which usually display oblong profiles in cross sections, while in the olfactory region the venous vessels occur less often and show round profiles. The arterioles are located close to the perichondrium, while the capillaries appear all over the lamina propria in both tissues. Fenestrated and continuous endothelia can be observed in capillaries of both the respiratory and olfactory regions.

Activities in the placenta and fetal membranes of enzymes involved in energy metabolism.

The placenta and the fetal membranes differ in their energy dependent functions and in their blood supply. In a search for quantitative differences in the expression of enzymes involved in energy metabolism in these tissues we measured in the placenta and in amnion and chorion the activities of enzymes involved in carbohydrate metabolism (hexokinase, phosphofructokinase, lactate dehydrogenase, glycogen phosphorylase), a tricarboxylic acid cycle enzyme (succinate dehydrogenase) and an enzyme involved in fatty acid oxidation (hydroxyacyl-CoA-dehydrogenase). The activities of succinate dehydrogenase and hydroxyacyl-CoA-dehydrogenase in the placenta were higher than those in the membranes, whereas the activities of the other enzymes assayed were lower. Lactate dehydrogenase activity was higher in the amnion than in the chorion (p less than 0.01). These results could indicate that the fetal membranes depend mainly on glycolysis for an energy supply.

Anthracenyl crown ethers and cryptands as fluorescent probes for solid-phase transitions of phosphatidylcholines: syntheses and phospholipid membrane studies.

Three structurally related crown compounds and cryptands have been synthesized that differ by the number and linkage of coronand units and anthracene moieties. The interaction of the fluorescent dyes with sonicated dimyristoylphosphatidylcholine (DMPC) vesicles is characterized by the relative quantum yields, uptake kinetics, binding curves, lifetimes, fluorescence titrations with water- and lipid-soluble quenching agents, fluorescence anisotropy, and equilibrium cooling curves. The most lipophilic compound II, which displays a similar quantum yield as the parent fluorophore 9,10-dimethylanthracene, shows a nearly equal distribution between solid and fluid lipid and is located at the bilayer surface. The least lipophilic compound IV is excluded from the hydrocarbon phase. The anthracenophane cryptand III preferentially partitions into solid-phase lecithins with the highest affinity for the phases L epsilon and L beta. The binding constant to DMPC amounts to (5.4 +/- 1.3) X 10(2) M-1 at 0 degrees C. From fluorescence quenching titrations it is concluded that the average position of the anthracenoyl dye III discontinuously shifts during the gel to liquid crystalline transition from the glycerol backbone to the choline head group. Electron microscopy and NMR experiments revealed that the anthracenophane induces in the liquid crystalline phase the fusion of small unilamellar DMPC vesicles to unilamellar medium-sized vesicles and macrovesicles, which subsequently fuse at the transition temperature to large multilamellar coacervates. Due to its large change of fluorescence intensity, the anthracenophane cryptand is a very sensitive probe for the detection of the pretransition of symmetrically substituted and of the subtransition of asymmetrically substituted phosphatidylcholines.(ABSTRACT TRUNCATED AT 250 WORDS)

Kinetics of the subtransition of asymmetrically substituted phosphatidylcholines.

The thermodynamics and kinetics of the subtransition L epsilon----P beta of sonicated unilamellar vesicles of 1-myristoyl-2-stearoylphosphatidylcholine (1M-2S-PC) and of 1-stearoyl-2-myristoylphosphatidylcholine (1S-2M-PC) were studied by equilibrium cooling curves and temperature-jump relaxation spectrometry with an anthracenophane cryptand as a mobile fluorescent probe. The unilamellar vesicles exhibit the midpoint temperature TsII of the subtransition about 10 degrees C below the respective main transition. The kinetics of the subtransition in the time range between 10(-4) and 10(3) s is characterized by a cooperative relaxation process in the range of milliseconds and a further noncooperative process in the range of seconds. The slow process is assigned to the rearrangement of lattice defects. The fast process is evaluated in terms of a cyclic reaction scheme that consists of two pathways for the biomolecular association of probe and vesicle coupled with the conformational change of the lipid matrix during the subtransition. The analysis reveals that the fast process comprises the nucleation and growth of cluster. The cooperative lattice transformation of the subtransition follows a first-order rate law. The rate constants at TsII are 70 s-1 for 1S-2M-PC and 170 s-1 for 1M-2S-PC. Since the plots of the relaxation time vs. the degree of transition are in accordance with the predictions of the linear Ising model, it is concluded that clusters are propagated anisotropically in a linear fashion; e.g., fluidlike P beta conformations grow along the ripple.

[Ultrasonic diagnosis of ovarian carcinoma].

Gray-scale ultrasonography was performed prior to surgery in 68 patients with tumors of the ovary. Thirty-four of the 38 benign tumors were considered benign by sonographic criteria. Three of 4 borderline epithelial tumors were considered malignant sonographically, as were 22 of the 26 cancers of the ovary (sensitivity 84%). Nineteen of them were malignant epithelial neoplasms and most were classified as FIGO stages III and IV (76%). In 6 of the 7 patients with stage I and II cancers the latter were considered malignant by sonographic criteria. Twenty-three of the 26 cancers were classified as having a complex structure using the classic ultrasonographic criteria. Histopathological evaluation did not correlate with the ultrasonic findings: sonography could not predict the correct stage and a particular histological type. Nevertheless, ultrasonography is a valuable diagnostic tool as a preoperative procedure in patients with a suspected ovarian neoplasm.
