[Malignant lymphomas of the orbit, facial bones and the parapharyngeal space].

The evaluation of 21 patients with non-Hodgkin-Lymphoma and primary extranodal manifestations involving the orbits, the paranasal sinuses, and the parapharyngeal spaces shows no typical features in malignant lymphoma. The analysis of the morphological features in computed tomography allows no classification of the tumor type. The most important differential diagnoses within the region of the facial skeleton are discussed.

[Indications for nuclear magnetic resonance tomography in tumors of the facial skeleton and neck area].

The diagnostic value of magnetic resonance tomography is examined in relation to computed tomography on the basis of 32 patients subjected to magnetic resonance tomography who had pathological processes in the craniofacial, parapharyngeal and neck regions. The high power of contrast resolution, multiplanar imaging, and good circumscription of cervical vessels, offer advantages over computed tomography in respect of space-occupying growths in the median and lower parts of the craniofacial skull. A disadvantage is the poorer detectability of tumour-conditioned bone destruction at the base of the skull and at the facial bones; in such cases, computed tomography remains the method of choice.

[Retroperitoneal gas accumulations: diagnosis using imaging procedures].

In 17 patients, signs of retroperitoneal gas accumulations were assessed on two planes by plain radiographs, and also by means of sonography and computed tomography. In some cases, diagnosis is possible via the plain radiographs if one knows the retroperitoneal compartments and the typical paths of propagation of both gas and inflammatory processes. Provided that conditions of examination are favourable, sonography can help to determine the seat and aetiology of the disease. In any case, however, computed tomography is clearly superior to both methods. Besides enabling the definite identification of gas accumulations, it provides a means for the exact localization in the retroperitoneal area, and in most cases also for arriving at the final diagnosis. This is important for effective treatment planning.

[Possibilities and limitations of computed tomographic staging of bladder carcinoma].

A retrospective analysis of 27 proven cases of bladder cancer was made comparing the CT-results and the histopathological staging made at TUR and/or cystectomy or at autopsy. In about 80% of the cases a differentiation between tumors only infiltrating and those penetrating the bladder wall was possible. Tumors only involving the bladder wall could not be further differentiated by CT(T1S-T3a). In the more advanced stages wall edema and/or fibrous reactions of the paravesical tissue caused difficulties in staging. As a non-invasive procedure CT should be used to stage bladder tumors as it can give additional information about the paravesical soft tissue and thus contributes to the adequate management of the tumor.

[Neurofibromatosis in childhood. Computed tomographic findings in the skull and neck areas].

The value of CT was investigated in sixteen children and adolescents with neurofibromatosis affecting the skull and neck. Its advantage is the simultaneous demonstration of bone and soft tissue changes, which frequently occur together in this condition. Demonstration of the extent of the neurofibromatous soft tissue changes is of crucial importance to the surgeon when planning treatment. Serial observations provide information on progress of the disease and helps in the decision of the optimal time for surgery. The differential diagnosis is discussed.

[Calcification of optic disk drusen on CT].

Four cases of unilateral or bilateral calcification of optic disk drusen have been observed on CT. The clinical indications are considered. The differential diagnosis of intra-orbital calcification is discussed.

[The value of arthroscopy in the diagnosis of knee injuries].

Arthrographic and arthroscopic findings of traumatized knee joints were compared by operation. It was found that endoscopic examination, particularly in case of damage to joint cartilage and cruciated ligaments, is superior to arthrographic examination. The lesions of meniscus could not always be detected radiologically. Hence, whenever there is a suspicious knee joint damage, arthroscopy should be performed liberally.

[Roentgen diagnosis of the veins in the upper half of the body using the digital subtraction technic - digital subtraction phlebography].

Digital subtraction phlebography (DSP) is suitable for x-ray examinations of the shoulder veins and the veins of the upper mediastinum. Acute thromboses, post-thrombotic syndromes, malformations and collateral systems can be demonstrated. The main advantages of DSP over conventional phlebography are the possibility of using contrast medium solutions of low osmolarity enabling pain-free examinations not involving the risk of thromboses, the saving in the amount of contrast medium, and the reduction of examination time.

[Radiologic diagnosis of the orbit and anterior cranial fossa].

Basing on frequent diseases associated with typical changes of the orbita and the anterior cranial fossa - osteomas, fibrous dysplasia, meningiomas of the sphenoid bone, neurofibromatosis, mucoceles and pyoceles, dermoid tumours, malignant tumours and transmitted inflammatory processes - the x-ray diagnostic procedure is explained. Plain x-ray films and computed tomography are the most important methods, whereas examinations employing film tomography and angiography are now restricted to a few selected cases only.

Brain Diseases↗

[Image quality in intravenous digital subtraction angiography of renal vessels].

Visualisation of the renal arteries and image quality of intravenous DSA were evaluated in 62 patients. The left renal artery is visualised less well than overlying more frequently the right renal artery because of gas in the stomach and colon. IV DSA is not adequate for visualisation of the subsegmental arteries, and cannot be used for tumor angiography. It is not possible to prove that central venous injection technique is superior to peripheral venous injection. Patients must be carefully prepared as is necessary for the urogram. Examination is carried out using Buscopan and a compression device.

[Morphology of atypical and metastatic pheochromocytoma in computed tomography].

14 cases of pheochromocytoma were investigated by CT including two of them in full detail. The morphology of pheochromocytoma and the changing CT picture with increasing tumor size are discussed. Diagnostic procedure and usefulness of CT as well as differential diagnosis of adrenal tumors are pointed out. Adrenal and extra-adrenal localization of pheochromocytoma and signs of their malignant degeneration are demonstrated.

[Sonographic detection and the clinical significance of the left retroaortal renal vein].

A retro-aortal left renal vein can be expected in 12.2% of the patients, as anatomical examinations show; in 8,9% of the cases, this presents as a circum -aortal venous ring, whereas in 3.3% it occurs as an isolated V. renalis retroaortalis sin. The sonographic image of this deviation from the standard and its clinical relevance are demonstrated.

[Limits of differentiation of focal splenic lesions by sonography and computed tomography].

Based on the results of sonographic and CT-findings in 192 patients the advantages and disadvantages of ultrasonography and CT in the diagnosis of splenic lesions are presented with particular emphasis on anatomy and topography. The relatively uniform presentation of various focal lesions in ultrasonography and CT is demonstrated and the limits of differentiation are discussed. The importance of angiography and scintigraphy is also mentioned.

[Indications for intra-arterial digital subtraction angiography (DSA) in vascular disease].

Digital subtraction angiography (DSA), using a direct arterial route, diminishes the risk of the examination by reducing the contrast dose by about 75%, making the examination more rapid and making it less likely that catheters will have to be changed. At the same time superimposition is avoided, one of the advantages of selective catheterisation. In view of the low contrast dose, it is possible to carry out several examinations at one time and to use additional projections for intracranial and peripheral disease, thereby improving the diagnostic value of the examination. For certain problems, intra-arterial DSA is already able to replace conventional angiography. The small field size and poor spatial resolution still make conventional angiography necessary as the basic form of investigation in most other circumstances.

[Non-invasive diagnosis of carotid artery-cavernous sinus fistulas using computed tomography].

Typical CT-findings of carotid-cavernous sinus fistulas are asymmetric enlargement and bulging of the lateral contours of the cavernous sinus and dilatation of draining orbital and facial veins. They can be best demonstrated with thin-section scans and rapid intravenous bolus injection of contrast agent. Among the differential diagnoses, a variety of orbital, intrasellar and parasellar lesions must be considered. For definitive radiologic diagnosis cerebral angiography continues to be required.
