[A case of intestinal duplication. Clinical study].

Reference to the relevant literature is followed by the presentation of a personal case of ileal duplication, in which pain led to an a mission diagnosis of appendicitis, whereas surgery revealed a second ileum, rising about 6-7 cm from the caecum and running for 35 cm into the sigmoid colon. The aetiopathogenesis of this abnormality is discussed. Pain, as in the present case, is evaluated with respects to its interpretation in preoperative diagnosis.

[Reconversion after Hartmann's procedure. Our experience].

An increasing number of intestinal reconversion after Hartmann have been performed in recent years, especially due to improved surgical techniques and progressively lengthened lifespan. The authors report 33 cases of intestinal recanalization of 100 interventions according to Hartmann from 1984 to 1996 (21 not neoplastic pathologies, 12 neoplasias). The variables considered included: patient age, type of disease requiring intervention according to Hartmann, oncologic characteristic of patients with neoplasia, interval between the two interventions, preoperative examinations performed, morbidity and mortality after reconversion. Furthermore, the fundamental indications for reconversion are described, in particular in patients with neoplasias (CEA, transanal echo, total body Ct, anal manometry). The low frequency of preoperative complications, zero mortality, satisfactory long-term follow-up (only one patient with neoplastic relapse) indicate that colon-rectal reconversion can also be performed in the elderly and patients with neoplasias with favorable prognosis.

[Surgical treatment of common bile duct calculosis: a comparative critical evaluation of long-term results of sphincterotomy and choledochotomy approaches].

From an analysis of operations performed by the Authors between 1974 and 1988, sphincterotomy emerged as the operation of choice given the small percentage of complications compared to the higher percentage of complications that arose with choledochotomy with external biliary derivation, in which use of the Kehr tube is hardly ever directly implicated. However the results of choledochotomy shown also demonstrate a possible improvement through better intraoperative procedures or by complementary endoscopic sphincterotomy where indicated; hence this procedure with external biliary deviation, according to the Authors, merits reevaluation.

[Benign neoplasms of the small intestine].

The authors report four cases of benign tumors of the small bowel (2 leiomyomas and 2 lipomas). After a short review of the literature, they put in evidence the relatively infrequent occurrence and the difficulties involved in early preoperative diagnosis.

[Difficulties in interpreting a conspicuous retroperitoneal hematoma due to rupture of the inferior pancreatico-duodenal vessels at the onset of edematous-type acute pancreatitis].

The authors report on a case of conspicuous retroperitoneal haematoma with haemoperitoneum due to rupture of the lower pancreatico-duodenal vessels appearing at onset of an acute oedematous pancreatitis. After stressing the rarity of this event, the authors go on to discuss a number of aetiopathogenetic, diagnostic and therapeutic aspects.

Pulmonary sclerosing hemangioma in a woman with multiple uterine fibromas, cysts to thyroid and kidney.

Pulmonary sclerosing hemangioma (PSH) is a relatively rare, homogeneous solitary tumor. Its complex histology may suggest a number of malignancies. We report one case diagnosed in a 62-years-old woman with a tendency to benign tumors (uterine fibromas) and cystes (thyroide, kidney). Bronchoscopy was normal. Pulmonary scintiscan was not done. Chest X-ray showed a 2 x 2 cm solitary, dense, homogeneous, delimited nodule of the LLL. The intraoperatory diagnosis of adenocarcinoma led to lobectomy. The final diagnosis of PSH was done on fixed and stained samples. She is in good condition after 13 months of follow-up.

Pulmonary angiodysplasia: a case report.

A case of lung angiodysplasia is reported here in. The diagnosis of this disease before surgery is difficult. The intraoperatory diagnosis of vascular disease was possible since the pulsating mass disappeared after the inflow and outflow blood vessels were selectively clamped. The abnormal placement of a venous vessel made the resection problematical. A careful anatomic and clinical study of the lesion, and the correct instrumental diagnostic approach may lead to a suitable therapeutic stategy, which respects both functional integrity and surgical radicality. The usefulness of a systematical selective clamping of the inflow and outflow blood vessels for an exact delimitation of the lesion emphasised.
