Bacteriologic evaluation of ultrasonic root canal instrumentation.

The antibacterial effect of ultrasonic instrumentation in the treatment of infected root canals was clinically evaluated. Sodium hypochlorite solution (0.5%) was used as an irrigant, but no antibacterial intracanal dressing was used between the appointments. The ultrasonic technique eliminated the bacteria from the canals more efficiently than hand instrumentation alone. Even though ultrasonication definitely improves the procedure of root canal disinfection, the use of an antibacterial dressing between appointments is necessary to achieve as complete a reduction in bacterial levels as possible.

Platelet counts in myocardial infarction, angina pectoris and peripheral artery disease.

Serial determinations of peripheral venous platelet counts were performed in 43 consecutive patients with acute chest pain. On admission, patients with acute myocardial infarction (AMI) had a significantly lower mean platelet count (p less than 0.05). Thereafter a further reduction was seen with steadily reduced platelet counts by about 20%, both in comparison with healthy controls (p less than 0.001) (n = 113) and patients with peripheral artery disease (PAD) without heart symptoms (p less than 0.005) (n = 54). The platelet number increased one week after admission and even thrombocytosis was observed. The changes in platelet number during AMI seem to parallel the changes in platelet function. Patients with PAD had normal mean platelet counts. Female patients as well as healthy subjects had significantly higher values than men (p less than 0.01).

Automated cytochemical differential leucocyte count in patients hospitalized with acute bacterial infections.

Absolute blood levels of various leucocyte types were measured by an automated cytochemical differential count (Hemalog D/90) in 84 adult patients hospitalised with an acute bacterial infection. The neutrophil granulocyte levels varied widely in all patient categories ranging from a normal value to a high count. Neutrophilic granulocytosis defined by a count exceeding 6.0 X 10(9)/l was detected in 47 patients (56%). An increased level of neutrophils with a high myeloperoxidase activity (Hpx cells) representing leucocytes recruited from the bone marrow was found in 51 patients (61%). Monocytosis was found in 25 (30%) and lymphocytopenia in 23 patients (27%). A high number of large unstained cells indicating an increase in atypical lymphocytes was detected in only 3 persons. Entirely normal cytochemical parameters were recorded in 13 patients (15%). The relationship between various leucocyte types was investigated in 26 patients with erysipelas. A negative association (r = -0.59) was observed between the neutrophil and the monocyte count. There was no correlation between the neutrophil and the lymphocyte count, nor between the levels of monocytes and lymphocytes. A positive correlation (r = 0.76) was established between the neutrophil count and the level of Hpx cells indicating that recruitment of immature cells from the bone marrow reserve pool is a feature of patients with erysipelas. The high precision and reproducibility of the cytochemical differential count indicate that the variability noted in the disease categories studied is a biological one, being the result of the complex interaction of invading microorganisms with the host defence system.

Bone marrow morphology in patients in long-term treatment with azathioprine.

The bone marrow was morphologically analysed in 25 patients receiving long-term azathioprine therapy for chronic renal diseases or after a renal transplantation. There were no significant changes within the granulopoietic or lymphopoietic part of bone marrow even after 10 years of treatment. However, there was a significant "shift to the left" within erythropoiesis as well as greater frequencies of megaloblasts and sideroblasts. A significant increase of "erythroblastic islands" may indicate an increased death rate of defective erythroblasts. The changes within erythropoiesis in some of the patients were so pronounced that the possibility of a preleukaemic condition cannot be excluded.

Diagnostic significance of lysosomal enzymes in different types of leukemias.

The activities of seven different leukocyte hydrolases were studied in 19 patients and ten controls. There was a strong positive correlation between the monocyte count and the activities of the lysosomal enzymes (N-acetyl-beta-glucosaminidase, alpha-fucosidase, beta-galactosidase, and alpha-mannosidase). High alpha-fucosidase and alpha-mannosidase activities were also found in the eosinophilic granulocytes. Using simple commercially available synthetic substrates, it is possible to study the activities of the lysosomal enzymes in different types of leukemias and to recognize the monocytic leukemias even when they present with very immature precursor cells in the peripheral blood.

Prognostic significance of amoeboid movement configuration in lymphoid cells from children with acute lymphoblastic leukaemia.

Bone marrow smears from 72 consecutive children with a recent diagnosis of acute lymphoblastic leukaemia (ALL) have been morphologically analyzed. A high incidence of lymphoid cells with amoeboid movement configuration (AMC) seems to indicate a more favorable prognosis independently of other prognostic factors e.g. age, WBC or sex. A positive correlation between the AMC and the mitotic indices (MI) of the lymphoblasts indicates some connection between the motility and the proliferative activity of the leukaemic cell clone.

L-gamma-glutamyl transpeptidase activity in normal and leukemic leukocytes.

The activity of gamma-glutamyl transpeptidase was investigated in normal and leukemia leukocytes. Enzyme activity was positively correlated with the proportion of mature neutrophils and monocytes. In isolated leukocytes from patients with acute myeloid leukemia low values were obtained compared to controls.

Morphologic studies of the erythropoietic part of bone marrow in myeloid leukaemias.

The erythropoietic part of the bone marrow has been morphologically analyzed in 147 patients with various forms of myeloid leukaemias and in 20 healthy controls. In all the patients the percentage of basophilic erythroblasts was abnormally high and correlated to elevated mitotic indices. Megaloblastic changes were found to be relatively common in the patients. All these findings are compatible with an ineffective erythropoiesis where the normal differentiation towards more mature erythroblasts becomes in creasingly blocked during the course of the disease regardless of the type of myeloid leukaemia.

The influence of vitamin B6 supplementation on the bone marrow morphology in patients on regular haemodialysis treatment. A double-blind study.

Bone marrow smears from 20 patients with chronic renal failure and on regular haemodialysis treatment (RDT) were morphologically analysed. A double-blind study of treatment with high doses of vitamin B6 showed that the patients receiving pyridoxine got raising frequencies of lymphocytes and monocytes in the bone marrow and there were morphologic signs of a normalization within the granulopoiesis. It is suggested that this is a sign of an enhancement of the immune response. The vitamin supplementation had no significant effects on the pronounced anemia of the patients.

The erythropoiesis in acute lymphoblastic leukaemia. A morphologic study.

The erythropoietic part of the bone marrow has been morphologically analyzed in 18 patients with acute lymphoblastic leukaemia, 44 patients with acute myeloblastic leukaemia and in 20 controls. There were significant differences between the findings in the lymphoid and the myeloid leukaemias and it seems possible that the erythropoietic cells are not involved in the leukaemic process in the lymphoblastic leukaemias in contradiction to the myeloid ones.

Mitotic activity in myeloid leukaemias. A study of 277 cases.

The proportions of myeloblasts, promyelocytes, myelocytes and promonocytes within the granulopoietic precursor pool of the bone marrow and mitotic indices (MI) of the precursor cells were determined in 277 patients with various forms of myeloid leukaemias. The MI of the leukaemic patients were significantly lower than those of 20 normals. Thus the median MI in 186 cases of acute myeloid leukaemias was 0.68%, compared with 1.0% in 85 cases of chronic leukaemias and 1.2% in the normals. Separate MI of the different leukaemic precursor cells were counted in 21 cases of acute myeloid leukaemias and these indices were lower than those of the corresponding normal cells. There were significant differences between the different groups and subtypes of leukaemias and it is suggested that mitotic countings might be of diagnostic and prognostic significance.

Different composition of the erythropoietic tissue in bone marrow, spleen and liver in myelofibrosis.

Aspirates from bone marrow, spleen and liver were analysed in 10 untreated patients with idiopathic myelofibrosis (MF). The proportion of erythroblasts was higher in the spleen and the liver than in the bone marrow. The mitotic indices of the erythropoietic precursor cells were subnormal in the extramedullary sites and significantly lower in the liver compared with the spleen. There was a "shift to the left" within the liver erythropoiesis and a significant megaloblastosis in the spleen. The same tendencies have formerly been found in patients with chronic myeloid leukemia and it is suggested that the discrepancies may be due to differences in the microenvironment of the erythropoietic cells.

Amoeboid movement configuration in tumour cells of bone marrow smears from patients with leukaemia. Incidence and significance.

The incidence of amoeboid movement configuration (AMC), a cell shape suggestive of cell locomotion at the moment of fixation, has been studied in the tumour cells of bone marrow smears from leukaemia patients at the time of diagnosis. The groups of patients with CML (n = 8), ALL (n = 5) and CLL (n = 9) were small, and the incidences of AMC were close to those found in the corresponding cell lines from healthy probands. In 39 patients with AML, the incidence of AMC was higher than in the other cell lines investigated. A positive skew distribution of AMC values and a positive significant correlation between incidence of AMC were found at the time of diagnosis and subsequent survival of the patients with AML, in spite of differences in treatment. It is suggested that this positive correlation may be due to an immune reaction of the patients against their tumour cells.

Mitotic activity of the granulopoietic precursor cells in the peripheral blood in chronic myeloid leukaemia.

The mitotic indices (MI) of granulopoietic precursor cells in peripheral blood and bone marrow were studied in 38 patients with typical chronic myeloid leukaemia (CML) in the chronic phase. The MI in the peripheral blood were very low, in the median 0.07%, compared to those in the bone marrow with a median of 1.07%. The blood MI were significantly increasing with raising WCC and the values of the MI above the median were combined with short survival times. The bone marrow MI were negatively correlated to the blood MI and it is suggested that this is a sign of an increased exchange of cells between bone marrow and blood.

Bone marrow morphology in patients on regular haemodialysis treatment.

Bone marrow smears from 57 patients with chronic renal failure were morphologically analysed. In 55 patients on regular haemodialysis there was a slight 'shift to the left' and a significant eosinophilia within the granulopoiesis concurrently with slight megaloblastic changes of the erythroblasts. It is suggested that these abnormalities are caused by the dialysis procedure and not by the uraemia per se.

Mitotic activity in acute promyelocytic leukaemia and leukaemoid reactions.

Bone marrow smears from 6 untreated patients with acute promyelocytic leukaemia (APL) and from 6 patients with leukaemoid reactions (LR) have been compared with smears from healthy controls. The mitotic indices of the granulopoietic precursor cells were significantly lower in the APL group and significantly higher in the LR group, both compared with the normals. Mitotic countings seem to be of diagnostic value for distinguishing APL from conditions simulating this disease.

Prognostic value of mitotic studies in myeloid leukaemias.

Seven patients with various forms of myeloid leukaemia have been studied by repeated sternal punctures during the course of illness. None of the patients received cytostatics before or during the study. The mitotic indices declined with time and there was a significant relation between the slopes of the mitotic curves and survival time. Repeated mitotic counts seem to serve as a diagnostic and prognostic tool in myeloid leukaemias.
