[Effectiveness of tetrachlorodecaoxide (TCDO) in the treatment of complicated disorders of wound healing. A controlled study: TCDO versus PVP-iodine complex].

In a controlled study on the treatment of wound-healing impairments 29 patients underwent local treatment with TCDO (Oxoferin) while Beta-isodona was used in 31 cases. Regarding the criterion of reducing the wound area, i.e. epithelization, a highly significant superiority of Oxoferin could be demonstrated in the treatment of complicated wounds. The evaluation of the state of granulation, that was documented by a semiquantitative procedure, indicated that Oxoferin tends to be slightly superior. However, the following observations have to be emphasized: Oxoferin induces a development of granulation tissue on exposed tendons, bones, and fasciae and the granular tissue is of much better quality than after treatment with Betaisodona. Concerning its effect on the degree of contamination, Oxoferin proved to be at least as efficacious as the established and effective antiseptic Betaisodona. Both substances are tolerated well and the application is simple, they do not differ here.

In vitro investigations on the antibacterial action and the influence on the phagocytic chemiluminescence of tetrachlorodecaoxide--a new, non-metallic oxygen complex.

The first non-metallic oxygen carrier, tetrachlorodecaoxide (TCDO), showed in vitro antibacterial activity among aerobic and anaerobic bacteria. The lethal dose for Escherichia coli, for example, was 150 micrograms/ml whereas 15 micrograms/ml reduced the bacterial amount after a latent period of two hours, but regrowth started after four hours. The bactericidal effect of TCDO, however, was dose-dependent and species-specific. This suggests that some aerobic bacterial species might not be able to produce sufficient amounts of protecting enzymes like catalase or superoxide dismutase. The computer controlled measurement of chemiluminescence was used as a model for the phagocytic activity. With isolated human granulocytes and opsonized zymosan as antigen no increase in peak counts per minute was observed compared with controls without TCDO. However, with human whole blood, positive effects were seen using TCDO together with zymosan as well as specific and non-specific opsonized Klebsiella pneumoniae K 17. It seems that whole blood possesses additional, but as yet unknown biocatalysers to split TCDO into oxygen and chloride.

Studies on the identification of Campylobacter species using biochemical tests and high-performance liquid chromatography.

A total of 56 strains of Campylobacter jejuni (C. jejuni) isolated from diarrhoeal patients were characterized by biochemical tests. The following reactions were performed: hydrolysis of hippurate, reduction of nitrate and nitrite, activity of deoxyribonuclease, hydrolysis of Tween(R) 40, 60 and 80. Including the hydrolysis of the different Tweens(R), 14 biotypes could be distinguished. 19 out of the 56 strains of C. jejuni and the nomenclatural type strains of C. jejuni, C. coli, C. laridis, C. fetus subsp. fetus, subsp. venerealis, C. faecalis, C. sputorum subsp. sputorum, subsp. mucosalis, subsp. bubulus were examined for the production of volatile (VFA) and non-volatile (NVFA) short-chain fatty acids using high performance liquid chromatography (HPLC) with a column for organic acids (Aminex HPX-87 H). A standard mixture of 21 short-chain fatty acids was taken as reference. By this method 5 biotypes of C. jejuni could be characterized. The most frequent biotype was type 1 (78.9%). All the biotypes produced succinic, acetic and butyric acids. Differences existed in the production of pyruvic, malonic, formic and isobutyric acids. C. jejuni could rapidly and clearly be distinguished from C. coli, C. fetus subspp. and catalase-negative Campylobacter species. No qualitative differences were found between the subspecies of C. fetus, C. sputorum subsp. sputorum and subsp. bubulus, C. sputorum subsp. mucosalis and C. faecalis were characterized by presence of fumaric and malonic acid, respectively.

Influence of cefmenoxime, ceftriaxone, latamoxef, and ceftazidime on the lysis of Klebsiella pneumoniae: a light and electron microscopic study.

The influence of antibiotics on the morphologic alterations of the cell wall of Klebsiella pneumoniae serotype K 17 has been studied by electron microscopy as well as by phase-contrast microscopy. Ceftazidime, cefmenoxime, ceftriaxone, and latamoxef were the antibiotics used. The effect of each concentration between 0.03 and 1,024 mg/liter was investigated on 5 X 10(7) bacteria per milliliter. Two different reactions could be detected: ceftazidime and latamoxef produced at first long forms and suddenly lysis; cefmenoxime and ceftriaxone produced at first long forms, later spherical forms, and then gradually lysis.

In-vitro activity of enoxacin, ofloxacin, norfloxacin and nalidixic acid.

The in-vitro activity of enoxacin, ofloxacin, norfloxacin and nalidixic acid was determined against 1499 Gram-negative and 279 Gram-positive isolates from different clinical institutions in the Federal Republic of Germany. 90% of all Enterobacteriaceae were inhibited by 0.5 mg/l. Glucose non-fermenting Gram-negative rods and staphylococci were less sensitive to all 4-quinolones. All Gram-negative strains selected for resistance to nalidixic acid were significantly less sensitive to the new generation of 4-quinolones tested.

Investigation of human bile and serum levels by high pressure liquid chromatography after administration of latamoxef or cefotaxime.

The high pressure liquid chromatography (HPLC) was used for the quantitative determination of R- and S-epimers of latamoxef, cefotaxime and desacetylcefotaxime in serum and bile. Bile levels of latamoxef, 1 and 2 h following administration of the antibiotic, were markedly above serum values. Those of cefotaxime, however, were identical after 1 h, after 2 h bile levels were below serum values. Desacetylcefotaxime concentration in bile after 1 and 2 h was four to five times higher than in serum. The ratio R-:S-epimer of latamoxef in serum was 49:51 after 1 and 2 h, in bile 44:56 after 1 h and 48:52 after 2 h. In the case of latamoxef the comparison of the microbiological and the HPLC assay showed good correlation (r = 0.98). The ratio cefotaxime: desacetylcefotaxime in serum was 72.5:27.5 after 1 h and 61.5:38.5 after 2 h, in bile 41:59 after 1 h and 24:76 after 2 h.

The distribution of Klebsiella pneumoniae serotypes from different sources and their sensitivity to cephalosporins.

153 strains of Klebsiella pneumoniae from 16 microbiological institutes in Germany, Austria and Switzerland were typed biochemically and serologically; their sensitivities to cefoxitin, cefotaxime, ceftazidime, ceftriaxone, lamoxactam, thienamycin and gentamicin were determined. The strains could be classified into seven biotypes and 43 K-antigen types. The distribution of the K-antigen types was random, and spread over the whole spectrum of the 72 possibilities. Regional variations were not observed. 4.6% of the isolates were resistant to cefoxitin and 23% were moderately sensitive. 98% of these Klebsiella strains were sensitive to the new cephalosporins which were also effective against gentamicin-resistant isolates accounting for 12% of all isolates studied. Cefoxitin was only slightly active against such strains. The effect of a standard concentration of each agent on bacterial cultures during their growth phase was measured in the Jouan biophotometer. The beta-lactamase-stable cephalosporins had a permanent bactericidal effect on the enzyme-producing reference strain, although the latent period varied considerably in some cases. Piperacillin and cefoperazone were not bactericidal in this test.

The influence of metronidazole and its two main metabolites on murine in vitro lymphocyte transformation.

In a study of the immunomodulating properties of metronidazole and its two main metabolites, metronidazole and 1-(2-hydroxyethyl-2-hydroxymethyl)-5-nitro-imidazole (hydroxy metabolite) were seen to enhance the phytohaemagglutinin-A-stimulated mitogenic transformation of murine lymphocytes. The stimulation was observed at suboptimal phytohaemagglutinin concentrations. The lymphocyte transformation was suppressed by 1-acetic acid-2-methyl-5-metronidazole (acid metabolite). The enhancement by metronidazole and its hydroxy metabolite was observed at concentrations corresponding to human serum levels following a normal dosage of metronidazole. Considering the normal serum concentrations of the three substances, the stimulating effect of the hydroxy metabolite may be at least as important as that of metronidazole itself.

Polyclonal activation of immunoglobulin secretion without prior DNA synthesis in human B lymphocytes induced by Klebsiella pneumoniae.

The capacity of various cell preparations of Klebsiella pneumoniae K43 (Klebs) to induce [3H]thymidine uptake and immunoglobulin (Ig) secretion by human mononuclear blood cells (MNC) and their lymphocyte subpopulations was investigated. All Klebs preparations were virtually devoid of mitogenic properties, in contrast to control preparations of pokeweed mitogen (PWM) and group A streptococcal cell membranes (A-ScM). Klebs induced differentiation of B cells into Ig-secreting cells. B-cell populations that were sufficiently depleted of T cells to be unresponsive to A-ScM ("highly purified B cells") showed a marked response to Klebs. Similarly, the number of plaque-forming cells (PFC) in Klebs-driven cultures did not change after restitution of T cells, whereas the presence of restituted T cells augmented the B-cell response to PWM and A-ScM. Radical removal of adherent MNC ("monocytes"), however, completely abrogated the PFC response and [3H]thymidine uptake of both MNC activated by Klebs and MNC activated by PWM or A-ScM.

Concentrations of sulphadiazine and trimethoprim in nasal secretion after co-trimazine administration.

Co-trimazine (1 g) was given once daily to 20 healthy volunteers during four days. Serum and nasal secretion concentrations were measured after the fourth dose. Secretion levels exceeded those in serum by a factor of 1.36 (sulphadiazine) and 2.96 (trimethoprim) (as expressed by the ratio of the AUC of secretion and serum concentrations). The average ratios of the concentrations of sulphadiazine to trimethoprim were 11.6-15.3 and 4.8-10.3 in serum and secretion respectively. During the dosage interval of 24 h, sulphadiazine and trimethoprim concentrations exceeded the MIC values of the common respiratory pathogens in serum and secretion.

Klebsiella-induced LIF response in Klebsiella infection and ankylosing spondylitis.

Lymphocyte transformation and leucocyte migration inhibition was studied in Klebsiella-sensitized patients, ankylosing spondylitis patients and healthy controls. The results show that mononuclear cells from ankylosing spondylitis patients respond more vigorously to Klebsiella, one of the 'trigger' bacteria, than do mononuclear cells from healthy persons, either with or without HLA-B27.

[Campylobacteriosis in humans caused by subspecies intestinalis and fetus. Six new diseases (author's transl)].

The subspecies intestinalis and fetus of Campylobacter fetus are opportunistic pathogens in humans. So far, some 200 types of disease due to these pathogens have been reported from all over the world, and six new diseases are briefly described in this paper. The most frequent disease is sepsis, followed by meningitis. While ticarcillin, erythromycin, clindamycin and tetracycline are the antibiotics which show activity against these species, a large degree of resistance is seen against cephalothin, cefazolin, cefuroxime, cefoxitin and cefotaxime. There is a moderate degree of sensitivity to gentamicin. The epidemiology of the disease is not clear, and about twice as many men are affected as women. Subspecies intestinalis was isolated in 95% of all diseases. Antibodies to Campylobacter fetus were detected in 3.9% of the population.

Bacteriological investigations with beta-lactamase inhibitors.

MIC's of beta-lactamase producing strains of P. aeruginosa, E. coli, P. vulgaris, P. rettgeri, E. cloacae, K. pneumoniae, S. marcescens are determined against piperacillin and in the case of S. aureus against amoxycillin and penicillin G. All the strains studied were resistant to these antibiotics. Their MIC's were determined in the combination with different concentrations (2, 5, 10 mg/l) of clavulanic acid and penicillanic acid sulfone respectively. The combination of piperacillin and clavulanic acid acts synergisticly against Klebsiella sp., indolpositive Proteus sp., and E. coli. Penicillanic acid sulfone was less active than clavulanic acid apart from indolpositive Proteus sp. Against S. aureus however both inhibitors showed the same synergistic efficacy. In detail both inhibitors are very active against constitutive penicillinase, they also exhibited a good activity against Type OXA-2, OXA-3 and SHV-1 beta-lactamase; they were less active to Type TEM-1 and OXA-1 enzyme and did not work against Type Ib, TEM-2 and Type IV beta-lactamase. In investigations using the biophotometer it could be demonstrated that the beta-lactamase inhibitor and the penicillin should be given simultaneously in order to obtain an optimal synergistic effect.

Seroepidemiological studies with Campylobacter fetus.

324 sera from unselected male and 581 sera from female patients as well as 268 sera from prostitutes were studied for antibodies against Campylobacter fetus using the complement fixation test. Antigen was Campylobacter fetus subspecies intestinalis. 3.9% of the sera showed low but relevant antibody titers. Statistically significant differences don't exist between the three population investigated. Serological cross reactions could not be observed using Salmonella typhimurium, Shigella flexneri, Escherichia coli O119 and Klebsiella pneumoniae. Antigenic relationship however was observed with the subspecies jejuni and fetus.
