[Transfusion problems in artificial blood circulation].

Results of differentiated administration of blood components to cardiac surgical patients are presented. Frozen washed erythrocytes and erythromass were successively used in the perfusate and postoperatively. A transfusion pattern for patients with rare A2B blood group has been elaborated. The feasibility of prevention of artificial-circulation-induced thrombocytopenia by introduction of large quantities of platelet components right after AC is demonstrated. The necessity of the ACT-test for adequate blood heparinisation and neutralisation during the AC is established. Results of successful autohemotransfusion in cardiac surgery and application of fibrinolytic blood and its components are discussed.

[Role of immunologic reactions in the development, prevention and treatment of postoperative anemia].

On the basis of an analysis of immunological and hematological indices the authors have shown the role of activation of physiological hemolysis, immunization of the organism by erythrocyte antigens, inhibition of erythropoiesis due to intoxication and massive hemotransfusions in the development of early and late anemias after operations for congenital and acquired heart diseases under conditions of the artificial blood circulation. The authors propose recommendations for the prevention of such anemias.

[Blood rheological disorders in atherosclerotic lesion of the abdominal aorta and their correction by the use of aspirin].

The initial hemorheologic values and their dynamics in various periods after reconstructive operations were studied in 70 patients with atherosclerosis of the abdominal aorta. The rheologic blood properties proved to be considerably changed in atherosclerotic affection of the abdominal aorta. The causes of the hemorheologic disorders in this pathological condition are marked changes in regional hemodynamics and the character of the blood flow and significant disorders of protein, lipid and carbohydrate metabolism. Study of the effect of aspirin on the rheologic properties of blood in this contingent of patients showed the expediency of its use both in preoperative management and in the postoperative period. Aspirin reduces significantly the degree of blood platelet aggregation and the seeming viscosity of blood.

[Characteristics of blood coagulation and of the red blood in patients with an arteriosclerotic lesion of the abdominal aorta].

The values of blood coagulation and red blood were studied in 74 patients with various forms of atherosclerotic affection of the abdominal aorta and its branches. Tendency towards hypercoagulation both before operation and in various periods after it was revealed in these patients. The development of thrombotic complications is preceded by a marked increase in the fibrinogen level and in the blood platelet count. High hematocrit with a relatively small erythrocyte count and hemoglobin concentration was revealed, which was linked with the spheric shape of the erythrocytes. It is concluded that a complex of antithrombotic therapy is necessary; it should be begun before the operation and continued particularly actively in the first two postoperative weeks.

[Phagocytic activity of the leukocytes in congenital heart defects before and after an operation under artificial circulation].

The authors confirm a well-known statement concerning a direct correlation between phagocytic activity of neutrophiles and the degree of oxygenation of an organism. According to their findings the percentage of phagocytic neutrophiles and absorbing activity of phagocytes is cases with a "blue type" heart disease is significantly lower than in patients with moderate hypoxemia. The development of an infectious process has been proved to occur in patients with a low immunoreactivity against the background of a further decrease of natural immune protection factors of an organism.
