[Pharmacogenomical aspects of gastroesophageal reflux disease].

The article covers the modem concept of gastroesophageal reflux disease (GERD) pharmacogenomics. Special attention is payed to polymorphism of CYP2C19 gene and its influence on the metabolism of modern antisecretory agents, such as proton pump inhibitors (PPI). The authors summarize data which demonstrate that the differences in the pharmacodynamics and clinical effects of PPI are caused by CYP2C19 gene polymorphism. The paper shows that this factor should be considered when planning GERD therapy with PPI in order to achieve a complete remission and make the therapy economically rational.

[Use of biologically active supplement containing omega-3 polyunsaturated fatty acids in patients with type 2 diabetes mellitus].

35 patients with diabetes mellitus type 2 complicated with hyperlipidemia, coronary heart disease were treated during 8 months with poseidonol--polyunsaturated fatty acid family omega 3 (PUFA--omega 3). It was shown that hypercoagulation and hypofibrinolysis in these patients occurred. Treatment with poceidonol increased plasminogen activator activity and decreased coagulation and reduced atherogenic index.

[Influence polyunsaturated fatty acids omega-3 on coagulation and fibrinolysis systems in patients with NIDDM].

35 patients with diabetes mellitus type 2 complicated with hyperlipidemia, coronary heart disease, were treated for 8 months with poseidonol which incorporates essential polyunsaturated fatty acids and fat-soluble vitamins. Poseidonol reduced atherogenic index from 4.2 to 3.15. The study of blood coagulation and fibrinolysis points to signs of hypocoagulation and normalization of fibrinolytic activity against previous hypercoagulation and fibrinolysis depression.

[Effects of tycveinol on the lipid transport system and hemostasis and fibrinolysis systems in patients with ischemic heart disease].

33 patients with ischemic heart diseases, hyperlipidemia and blood hypercoagulation took tycveinol which consists of polyunsaturated fatty acids in a dose of 2.0 g twice a day for 2 months. A positive result manifested by a 14% drop in cholesterol, 29.7% in triglycerides, 21% in beta-lipoproteins. Alpha-lipoproteins rose by 26%, atherogenic index reduced from 3.24 to 1.89. Hypercoagulatory tension relieved due to normalization of euglobulin lysis, higher activity of plasminogen tissue activator, slow blood clotting. Side effects on hepatic or renal function, mineral metabolism were not observed.

[Poseidonol in the treatment of hyperlipidemia in patients with type 2 diabetes mellitus].

Thirty five patients with type 2 diabetes mellitus complicated by cardiovascular diseases received 6-month treatment for hyperlipidemia with poseidonol that represents a set of essential polyunsaturated fatty acids. This yielded good results as 11.6%-, 30%- and 24.2% decreases in the levels of cholesterol, triglycerides, and beta-lipoprotein, respectively, and a 9.9%- increase in alpha-lipoprotein. Lipid metabolism became normal in 34.2% of the patients, the remaining had positive changes as normalization of 1 to 3 lipid parameters. The tolerance of the agent was good, no negative effect on hepatic and renal functions was found.

Aminopeptidase PC from the hepatopancreas of the Kamchatka crab Paralithodes camtshatica.

Homogeneous aminopeptidase PC was isolated with yield 67% and purification degree 237 from the hepatopancreas of the Kamchatka crab Paralithodes camtshatica by ion-exchange chromatography on DEAE-Sepharose, hydrophobic chromatography on Phenyl-Sepharose, and gel-filtration on Sephadex G-150. The enzyme is a homodimer with a molecular mass 220 kD (110 x 2). Aminopeptidase PC has pI = 4.1. It hydrolyzes Leu-pNA optimally at pH 6.0 and at the optimum temperature 36-40 degrees C; in the presence of Ca2+ the enzyme is stable at pH 5.5-8.0. Aminopeptidase PC is activated by Ca2+, Mg2+, and Fe2+; it is completely inhibited by EDTA, o-phenanthroline, and bestatin. The enzyme contains four Zn atoms per molecule and is therefore a metalloaminopeptidase. The aminopeptidase PC can effectively cleave N-terminal Arg and Lys residues as well as Leu, Phe, and Met residues. Km and kcat values for hydrolysis of Leu-pNA were 0.075 mM and 0.19 sec-1 and for hydrolysis of Arg-pNA 0.078 mM and 0.48 sec-1, respectively. D-Amino acid residues cannot be cleaved. Thus, aminopeptidase PC of the Kamchatka crab has a mixed substrate specificity which is characteristic of some microbe aminopeptidases. Its N-terminal sequence ESVEIELPEGLSPLV is 46% coincident with that of yeast vacuolar aminopeptidase YSCA.

[Poseidonol in the treatment of patients with hyperlipidemia].

An antilipidemic effect of poseidonol was tested in 123 patients with ischemic heart disease, stable angina of effort (II-III functional class), 40 patients with secondary pyelonephritis and 40 patients with chronic hepatosis-hepatitis associated with chronic alcoholism or diabetes mellitus. The patients had also hyperlipidemia. A positive effect of poseidonol was established: cholesterol content fell by 25.4, 20.6 and 18.7%, respectively; triglycerides level decreased by 63, 42 and 21.4%, respectively; B-lipoproteins fell by 55.5, 36.9 and 13.1%, respectively. High density lipoproteins rose by 40.2, 27 and 69.9%, respectively. No adverse effects either on the liver or kidney functions were observed.

[Isolation of trypsin PC from the Kamchatka crab Paralithodes camtschatica and its properties].

Trypsin PC from the hepatopancreas of the king crab Paralithodes camtschatica was isolated and purified to apparent homogeneity by ion-exchange chromatography on Aminosilochrom and DEAE-Sephadex and affinity chromatography on arginine-agarose. The yield of the enzyme was 37.7%, and the purification degree was 21. Trypsin PC has a molecular mass of 29 kDa and pI < 2.5. It hydrolysis N-benzoyl-L-arginine p-nitroanilide at the optimum pH of 7.5-8.0 and at the temperature optimum of 55 degrees C (K(m) = 0.05 mM). Trypsin PC retained its activity within the pH range of 5.8-9.0 in the presence of Ca2+. The enzyme was inhibited by the specific inhibitors of serine proteases diisopropyl fluorophoshate and phenylmethylsulfonyl fluoride, by the trypsin inhibitor N-tosyl-L-lysylchloromethylketone, and by the trypsin inhibitors from soybean and potato. Trypsin PC was found to hydrolyze amide bonds formed by carboxylic groups of lysine and arginine in peptide substrates. The N-terminal sequence of this enzyme is IVGGTEVTPG.

[Isolation and properties of serine proteinase PC from the Kamchatka crab, Paralithodes camtschatica--a proteolytic enzyme with broad specificity].

A homogeneous serine proteinase PC has been isolated from the Camchatka crab (Paralithodes camtschatica) hepatopancreas using affinity chromatography on arginine-Sepharose, protamine tryptic peptide-agarose and ion-exchange chromatography on Mono-Q, with a 68% yield. The enzyme is completely inhibited by diisopropylfluorophosphate, a typical inhibitor for serine proteinases. The molecular mass of the proteinase is 29 kDa, pI is 3.0. The proteinase splits Glp-Phe-Ala-pNA optimally at pH 7.5 and 47-55 degrees C; Km is 0.83 mM, kcat is 67 s-1. The enzyme is stable at pH 4-9. Proteinase PC possesses a broad substrate specificity and splits the peptide bonds formed by the carboxyl group of hydrophobic amino acids, arginine and lysine, in peptides and proteins. The enzyme hydrolyzes fibrin and collagen. Its N-terminal sequence, IVGGQEATP, reveals a 90% homology with analogous sequences of collagenolytic proteinases from other crab species.

[Isolation and properties of carboxypeptidase from the Kamchatka crab Paralithodes camtshatica].

Homogeneous carboxypeptidase PC from a hematopancreas of kamchatka crab Paralithodes camtshatica was obtained by means of an affinity chromatography on sorbents containing arginine, protamine hydrolysate, and phenylalanine as ligands with an yield 23% and purification degree 37.4. The isolated enzyme has a molecular mass 34 kDa, as evidenced by an SDS-PAGE; pI 3.1; an optimum pH 6.5, as estimated for hydrolysis of Dnp-Ala-Ala-Arg; pH-stability range 5-8 in the presence of Ca2+; a temperature optimum 55 degrees C; and Km 0.4 mM. The carboxypeptidase is activated by Co2+ and Ca2+ ions and is inhibited by EDTA and o-phenanthroline, and therefore, it is a metallocarboxypeptidase. The enzyme can effectively split off C-terminal residues Phe and Tyr, as well as Arg and Lys. Residues Pro, Glu, and Asp cannot be split off, and they stop the cleaving of a preceding bond. Thus, the carboxypeptidase PC of kamchatka crab has a mixed substrate specificity, which is characteristic of carboxypeptidase from crawfish and of microbial carboxypeptidases T and SG. The new carboxypeptidase has an amino acid composition Asp41Thr24Ser22Glu32Pro15Gly32Ala291/2Cys5Val19Met8Ile14Leu20Ty r18Phe8Lys7His4 Arg8Trp4. The N-terminal sequence of the enzyme demonstrate a 40% homology with the N-terminal sequence of carboxypeptidase from crawfish.

[Correction of disorders of cardiac electric stability in post-infarction cardiosclerosis using a diet enriched with polyunsaturated fatty acids].

In a 14-20 days after feeding of rats with "eikonole" (cod-liver oil, deduced from skeletal muscle and enriched by eicosapentaenic and docosahexaenic fatty acids). Myocardial infarction by Selie has been induced and in a 30 days of the same feeding, contractile function and heart electrical stability in situ have been investigated. Eikonole increased heart fibrillation threshold by 50%, decreased the frequency of spontaneous extrasystole in 3 times and ectopic activation (extrasystole), induced by n. vagus stimulation, in 2.5 times. Eikonole increased the intensity of structures' functioning in the state of relative physiological rest, but did not influence essentially on pressure and the velocity of myocardial contraction and weakening.

[Use of dietary fish oil--an alternative to drug therapy of dyslipidemia].

The clinical efficacy of "Food ichthyenic oil", a new foodstuff, was studied in 129 patients with atherogenic dyslipidemia. The oil was given in a daily dose of 30 ml which contained 8 g polyunsaturated fatty acids of the omega-3 class. All the patients were divided into 3 groups: (1) 44 patients with 5.2-6.5 mmol/l cholesterol; (2) 37 with over 6.5 mmol/l, and (3) 48 with hypercholesterolemia (cholesterol over 5.2 mmol/l and hypertriglyceridemia (triglycerides over 2.3 mmol/l). Following 1-month therapy, all the groups displayed lower low density lipoprotein cholesterol levels and significantly higher high density lipoprotein cholesterol concentrations. After 4-month intake of ichthyenic oil, the levels of total and LDL cholesterol returned to the baseline values, whereas the concentration of HDL cholesterol was significantly higher than the baseline one. Following 12-month therapy, there were 15 and 14% decreases in total and LDL cholesterol, respectively, a 16% increase in HDL cholesterol. The patients from Group 3 exhibited low VLDL cholesterol and triglyceride levels.
