[A method of peritoneostomy for treating diffuse purulent peritonitis].

For improvement of exudate outflow and prevention of the development of abscesses, a modified method of peritoneostomy has been suggested and successfully used in treatment of 59 patients. After elimination of a source of peritonitis and sanitation of the abdominal cavity, the greater omentum is covered with a hydrocellulose film of corresponding sizes, rolled up as a rouleau, and placed in the epigastrium. The intestinal loops are covered with a perforated polyethylene film sutured to the ends of a laparotomy wound.

[Morphofunctional state of the heart and activity of the kallikrein-kinin system in patients with chronic heart failure during treatment with captopril].

Coronary patients with heart failure (HF) stage I-III received captopril. The drug was found to attenuate symptoms of HF, to reduce the size of the left heart, to improve contractility. For stage HF I, kallikrein urinary excretion was similar to that of normal subjects, while in stage II-III patients it fell significantly. Captopril induced a drop in relevant secretion for stage I and a rise for stage II-III subjects. The drug administration did not influence the relations between the initial serum Na levels and kallikrein urinary excretion, nor it changed serum levels of Na and K. Alterations in morphofunctional characteristics of the heart and changes in diurnal urinary excretion of kallikrein demonstrated close relationships.

[Outcome of surgical treatment of habitual shoulder dislocation].

There has been carried out an analysis of treatment of 134 patients with habitual shoulder-slips according to 4 known methods and I proposed by the authors. There has been detected a number of typical defects, made by doctors in the process of treatment during acute period. The essence of the proposed method consists in strengthening by lavsan bandage "North-CITO" of the anterior part of the shoulder joint that allows to begin early restorative treatment.

A mathematical model for the interaction between cytotoxic T lymphocytes and tumour cells. Analysis of the growth, stabilization, and regression of a B-cell lymphoma in mice chimeric with respect to the major histocompatibility complex.

A mathematical model of the response of cytotoxic T lymphocytes (CTL) to the nonexponential growth of an immunogenic tumour is presented. The model describes satisfactorily the kinetics of growth and regression of the B lymphoma BCL1 in the spleen of mice chimeric with respect to the major histocompatibility complex (H-2b----H-2d). Numerical estimations of the parameters describing processes that cannot be measured in vivo have been derived. It is predicted that the development of the tumour and its clinical manifestation have a recurrent profile with a cycle of three to four months. According to the model the limited growth of the BCL1 tumour and its transition into the dormant state are associated with a high rate of accumulation of CTL in the tumour, rapid lysis of lymphoma cells, and the inability of tumour cells to suppress the affector functions of cytotoxic lymphocytes. The model is also capable of representing the phenomenon of tumours 'sneaking through' and the immunostimulation of tumour growth.

[Bilateral exclusion of the duodenum in reconstructive surgery of the stomach].

The article analyses the results of modified gastric resection after Roux with bilateral exclusion of the duodenum in 13 patients with disease of an operated stomach. The operations were performed for peptic ulcer of the gastroenteroanastomosis in 5 patients, for biliary reflux gastritis in 3, and for reflux gastritis and coexistent Stages II-III dumping syndrome in 5 patients. The outcomes of treatment were studied in follow-up periods of up to 3 years. The results were good in 10 patients, satisfactory in 2, and poor in 1 patient. Recurrent peptic ulcers were not encountered. The clinical picture of biliary reflux gastritis was completely relieved in all of the 8 patients. The dumping syndrome phenomena were arrested in 2 patients and relieved significantly in another 2.

[The effect of physical training on the indices of the general hemodynamics and kinin-kallikrein system activity in hypertension patients].

Forty men aged 30-60 years suffering from stage-II essential hypertension were examined for the general hemodynamics, the level of prekallikrein (PK) and kallikrein-like activity (KLA) of blood plasma, and kallikrein excretion with urine. The examinations were made before and after 30 weeks of exercise. Before such a course of exercise the response of the cardiovascular system and of the kinin-kallikrein system (KKS) to exercise was characterized by a significant rise of systolic, diastolic medium-hemodynamic arterial pressure, heart rate and kallikrein excretion with urine together with the lack changes in PK and KLA of blood plasma. No correlations were established between the KKS, which is likely to be determined by the disease. After the course of exercise there was a significant reduction of all the initial characteristics of the general hemodynamics, KLA, a close to significant increase of the PK level and a well-defined tendency towards lowering of kallikrein excretion with urine, and less remarkable response of all the parameters of both systems to standard exercise. A reverse correlation (r = -0.57) was established between the PK level and KLA at rest which may attest to a positive influence of exercise on the recovery of the correlations between the parameters of the KKS.

[A photometric method of determining the activity of antithrombin III using a peptide coumarylamide substrate].

Toz-Gly-Pro-Arg-4-methylcoumaryl-7-amide is recommended for antithrombin-III (AT-III) photometry as a substrate. This substrate is cleaved by thrombin at 366 nm. AT-III photometry with this substrate is in accord with the requirements to methods used in clinical laboratory diagnosis, and the results of such measurement are in good correlation with the data of AT-III analysis with Chromozym-TH substrate. Use of photometry with methylcoumaryl substrates for measuring protease activities is more available than the traditional fluorometry.

[Possible relationship between polyunsaturated fatty acids and signs of left ventricle hypertrophy].

In aborigines from the costal and continental areas of Chukotka who differ in dietary polyunsaturated fatty acid (PFA) n-3 intake, specific features of the composition of fatty acids in cell membranes were revealed in a model of erythrocytes: the costal inhabitants showed a higher proportion of PFA n-3 mainly through eicosapentaenic acid and lower proportion of major PFA n-6. With this, plasma lipoprotein lipids in whose composition the acids transported to the cell membranes displayed the similar regularities in changes in fatty acid composition. Only a far larger portion of eicosapentaenic acid in the cell membranes from the costal inhabitants than from the continental inhabitants was associated with greater mass of the left ventricular myocardium. It was concluded that transport of lipids containing high PFA n-3 in the composition of lipoproteins to the cell membranes caused an increase of the acids in them and, evidently, a change in physicochemical and, therefore, functional properties of cells.

Computer analysis of receptor-mediated endocytosis and exocytosis mechanisms: two pathways for the processing of TCR-CD3 receptor complexes of T lymphocytes.

A new mathematical model, based on a hypothesis in which two types of molecular complex which differ in their processing rate are involved in ligand binding at the cell surface, is proposed for the processing of ligand-receptor complexes. The model describes the kinetics of anti-human-CD3 monoclonal antibody endocytosis, exocytosis, and degradation by both normal and malignant T lymphocytes. The rates for the individual stages of processing of these ligand-receptor complexes are evaluated.

[The treatment of patients with the hemorrhagic form of erysipelas by using amben and parmidine].

Amben (fibrinolysis inhibitor) and parmidin (bradykinin inhibitor) were used as pathogenetic treatment of 38 and 36 patients with hemorrhagic erysipelas respectively. Early amben treatment proved clinically rather effective and was accompanied by normalization of fibrinolysis and kallikrein-kinin system. Parmidin treatment resulted only in pain control, other subjective signs, normalization of the kallikrein-kinin system while fibrinolysis activity was maintained at former levels.

[Gastric motility and post-gastrectomy dumping syndrome].

A direct relationship was revealed between disorders of gastric motility before the operation and the frequency and severity of the post-gastrectomy dumping syndrome in patients with peptic ulcer. The level of increase in the frequency and amplitude of the contractions and the basal tonus of the gastric stump may serve as an objective criterion for establishing the severity of the dumping syndrome. Patients with an initial hyperkinetic type of motility are predisposed to the dumping syndrome, which must be borne in mind in choosing a variant of the operation and the method for creating the gastrointestinal anastomosis.
