[The kallikrein-kinin system in pregnant women with gestosis before and during complex therapy].

Blood and urinary levels of kallikrein-kinin system were studied with chromogenic substrates both in healthy pregnant females and those with gestoses before and under the combined treatment. The activation of kallikrein-kinin system in treatment. The activation of kallikrein-kinin system in pregnancy was established as was the interrelation of kinin and plasma body systems. Kallikrein excretion with urine was found dependent on gestosis severity and the correlation between active kallikrein and its preliminary stage in the plasma. Alterations revealed should be considered in the course of combined treatment if preparations possible of affecting the kallikrein-kinin system were used.

[Cytospectrophotometric research on hemoglobin in human erythrocytes. I. The methemoglobin content in intact erythrocytes and its alteration under the influence of chromosmon, ascorbic acid, riboflavin and glutathione].

The blood of healthy men and patients with methemoglobinemia of different genesis was incubated with chromosmon, ascorbic acid, riboflavin and glutathione, the percentage of erythrocytes with thorn-shaped protuberances-echinocytes being subsequently determined in the blood smears. The absorbtion spectra at the range 400-650 nm were investigated both in the smooth erythrocytes and in echinocytes. A correlation was found between the percentage of echinocytes and the methemoglobin content in the blood. The methemoglobin amount in the echinocytes was determined to be higher than in the smooth erythrocytes. It is discovered that effects of chromosmon, glutathione and riboflavin on production of methemoglobin depend on the dose, individual peculiarities of erythrocytes and on the illness that caused methemoglobinemia. The calculation of echinocyte percentage may be used as an express-diagnostics of methemoglobinemia and for purposes of studying the effect of methemoglobin-producing substances and drugs.

[Clinical aspects of indirect myocardial revascularization in patients with ischemic heart disease].

The article analyses the experience in surgical treatment of patients with ischemic heart disease (504 cases) by indirect revascularization of the myocardium by creating aseptic mediastino-exo-pericarditis. Clinical characteristic of the patients is given, 81.1% of them had a history of myocardial infarction; 57.7% were invalids; the diagnosis was confirmed by coronarography in 56.2%; 92.1% of patients who underwent operation were related to III-IV functional classes. Late-5-year results showed marked improvement of the condition in 47.3%, moderate improvement in 31.6% and absence of an effect in 21.1% of patients. Postoperative lethality was 1.1%.

[Ability of a lipopolysaccharide toxin to intensify the functional response of human thrombocytes to thrombin stimulation].

The preincubation of isolated washed human platelets with lipopolysaccharide toxin (LPS) from Salmonella typhimurium (10 min, 37 degrees) speeds up the thrombin-induced aggregation. Besides, LPS-pretreatment was shown to alter the turnover of polyphosphoinositides stimulated by thrombin and significantly elevated concentrations of diacylglycerol and thromboxane B2 in comparison with control platelets. However, LPS does not influence on the lipid fluidizing effect of thrombin action as testifies by ESR spectroscopy with doxyl-stearic acids as spin-probes. An LPS pretreatment of platelets that induces no aggregation by itself is proposed might transform cells into a state of hidden activation" associated with the accumulation of such products as diacylglycerol and precursors of thromboxane B2. Addition of thrombin to such "preactivated" platelets could cause a more effective aggregation.

[Phosphoinositide breakdown and diacyl glycerin formation in human thrombocytes as affected by a lipopolysaccharide toxin].

The interaction of lipopolysaccharide toxin (LPS) with isolated washed human blood platelets has been studied. LPS was found to induce the rapid (1-2 min) and marked (15-20%) breakdown of mono- and polyphosphoinositides and formation of significant amounts of diacylglycerols (ca. 20%). However TxB2 biosynthesis from endogenous 14C-arachidonic acid was stimulated by LPS incubation only by ca. 20%. Phosphatidylcholine and phosphatidylethanolamine were also hydrolysed by ca. 8 and 12%, respectively, presumably via the activation of endogenous phospholipase A2. Besides, LPS caused the decreasing of the lipid fluidity of a platelet plasma membrane as was shown by ESR spectroscopy using doxylstearic acid probes. All these changes by LPS induce no aggregation of platelets. It is concluded that an enhancement of a phosphoinositide cycle is not a possibly necessary and sufficient condition for a platelet aggregation.

[Use of urokinase in acute myocardial infarction].

Clinical course of the disease and the formation of the necrotic focus (as evidenced by precordial mapping from 35 ECG leads) were assessed in 88 patients with acute myocardial infarction, treated with 1,000,000 IU urokinase, in comparison to 41 untreated control patients. Thrombolytic therapy, started within the first 6 hours of myocardial infarction, was associated with a better clinical course of the disease. Urokinase administration after 6 hours from the onset of the symptoms produced no clinical improvement, did not limit the necrotic focus and failed to reduce mortality, as compared to the controls. The assessment of the efficiency of thrombolytic treatment demonstrated an at least 30% increment of venous flow in 45% of patients. Intravenous urokinase administration was accompanied by an activation of blood fibrinolytic system, and there was a tendency to fibrinolysis depression on the day following the administration.

[Serine protease activity in blood plasma during the healing of aseptic and infected wounds under experimental conditions].

Activity of serine proteases--kallikrein-like activity (KLA) and plasmin-like activity (PLA)--were studied in the blood plasma of 220 sexually mature male Wistar rats with simulated aseptic and infected surface wounds in the time course before the operation, daily from the 1st to 10th day, as well as 12 and 15 days after the surgery. During the healing of aseptic and especially infected wound blood plasma KLA and PLA were found to be decreasing, thus indicating the active role of factor-XII-dependent systems in wound healing, reflecting the response of the body to operation-induced injury and wound infection, and dependent on the character and stage of wound healing course.

[Comparative assessment of stress electrocardiographic and echocardiographic tests in the diagnosis of insufficiency of the coronary circulation].

Altogether 29 patients (14 with a characteristic clinical picture of CHD, 6 with probable angina and 9 with nonischemic cardialgia) were investigated by methods of two-dimensional echocardiography (echoCG) at rest and in combination with isometric exercise and left atrial transesophageal pacing test (LATPT) as well as by electrocardiographic exercise tests (bicycle ergometry testing and LATPT). EchCG exercise tests were conducted in most of the examinees (96.6%) whereas bicycle ergometry results were obtained in 75.9% of the patients (p less than 0.05). A significantly higher frequency of positive results of echoCG exercise tests in patients with typical CHD as compared to electrocardiographic exercise tests, and significantly high correlation of the results of the combination of echoCG with isometric exercise and echoCG with a degree of the pain syndrome in the heart area were revealed. The results obtained made it possible to recommend echoCG exercise tests for the diagnosis of coronary insufficiency.

[Accessory left-ventricular chorda and the early ventricular repolarization syndrome].

Electrocardiographic symptoms were evaluated in 53 patients with an additional left-ventricular chorda (ALVC), detected by two-dimensional echocardiography. Signs of early ventricular repolarization (EVR) were identified in 45 (84.9%) patients. A transverse ALVC was more common in EVR-free patients (87.5%), as compared to patients with ALVC + EVR combinations (51.1%; p less than 0.05).

[Disorders of immune homeostasis in patients with tumors of the gastrointestinal tract].

To study immune homeostasis status and to improve the diagnostic value of immunological tests in healthy donors and patients with benign and malignant gastrointestinal tumors, 13 clinico-immunological parameters were identified and measured simultaneously in most of the examinees. A new method of analysis of clinico-immunological information based on stage-by-stage visual evaluation of data shown on dispersion programs and identification of areas corresponding to "normal" immune homeostasis is suggested. Data obtained simultaneously from blastogenic reaction of lymphocytes, cytotoxic test for levels of natural killers in blood and tests for determining subpopulational levels of T-lymphocytes were shown to be diagnostically valuable.

[Secretion of immunoglobulins in malignant melanoma of the skin].

The study was concerned with the dynamics of correlations between IgA, IgM and IgG levels involved in pigmented neoplasm development in 154 cancer patients and 75 healthy subjects. The dynamics was found to represent stages of tumor development rather than changes in absolute indexes of serum immunoglobulin concentrations. The nature of changes in the structure of correlations suggested a possible switch from IgM to IgG and IgM to IgA secretion, i.e. disturbances in B-lymphocyte differentiation. Said changes were observed at a certain stage of melanoma progression, namely, during metastatic spreading.

[Inability of large granular lymphocytes to recirculate through blood-lymph barriers].

The level of large granular lymphocytes (LGL) has been determined in thoracic lymph duct and peripheral blood of cattle. It has been shown that in contrast to the blood, these cells are present in the lymph in minor quantities. Unlike blood LGL, lymph LGL have smaller granules in cytoplasm. It is concluded that LGL do not recirculate from blood to central lymph.

[The left coronary artery in ischemic heart disease patients (data from 2-dimensional echocardiography)].

Two-dimensional echocardiography was used to study the state of the proximal part of the left coronary artery (LCA) in 106 males: 39 patients after anterior myocardial infarction, 28--after posteroinferior myocardial infarction, 11 patients with angina pectoris without infarction, and 28 healthy persons. The comparison of the results of coronary radiography and echocardiography in 21 patients showed that the sensitivity and specificity of echocardiography in the diagnosis of stenosis of the left main coronary artery was 80 and 100%, respectively. Methods of assessment of echographic density of the LCA walls on the basis of comparison of their density with echographic density of the walls of the aorta and aortic valve showed that in the CHD patients LCA wall density was increased as compared to that of the healthy persons, and in the patients after anterio myocardial infarction it was increased as compared to that of the healthy persons and other CHD patients. The method permitted the detection of an increase in LCA wall echographic density which was typical of arterial atherosclerosis, and the assessment of a degree of arterial wall atherosclerotic density.

[Immunologic criteria of the evaluation of the severity of the condition of patients with mechanical jaundice].

Examination of 62 patients with mechanical jaundice has established a pronounced disproportion in the ratio of the amount of "general" T-, "active" T- and "theophylline-sensitive" T-lymphocytes which suggests the critical state of the patient especially if it persists in the early postoperative period. The dynamics of the content of serum immunoglobulin A reflects the degree of resolution of hypertension in bile ducts while the clinical value of this test is not high enough due to its being very time-taking.
