[Clinical and morphological characteristics of infective endocarditis in HIV-infected drug addicts].

Infective endocarditis (IE) in drug addicts is characterized by the involvement of the right heart with evolving septic pulmonary thromboembolism. Staphylococcus aureus is the causative agent of IE in 71.8% of cases. The specific feature of the course of IE in drug addicts is a relatively low hospital mortality rate (35.5%). The main cause of death was generalized infection with a developing multiple organ dysfunction. Pyonecrotic foci in the organs and tissues and septic vasculitis were histologically detected along with pronounced dystrophic changes. The presence of morphological signs of HIV infection at early stages, as well as viral hepatitis C do not affect mortality rates in drug addicts with IE.

[Placentitis induced by hepatitis viruses].

The results of morphological studies of 66 placentas from women suffering from various forms of viral hepatitis and HBs antigen carriers as well as results of a pathomorphological study of a late abortion, 3 stillborns and 2 newborns are presented. 33 placentas showed lymphocytic infiltration of varying degree and vasculitis. Of most importance were cellular changes, particularly in the decidua, with the appearance in the nuclei of inclusions similar to those found in the liver affected with viral hepatitis. A correlation is observed in some cases between liver changes in the mother, placental alterations and changes in the child.

[The immunomodulating and metabolic actions of thymalin in an experimental herpetic infection].

In experiments on the intracerebral inoculation of herpes virus, type I, into 38 CBA mice the phagocytic activity of peritoneal exudate, cells, spontaneous migration of leukocytes and their response to phytohemagglutinin (PHA), the activity of key enzymes of gluconeogenesis, lipid peroxidation in liver tissue, as well as the level of glucose and glycosylated hemoglobin in the blood, were determined simultaneously with the histological study of the brain and the main internal organs of the animals, receiving thymalin treatment and not receiving it. As demonstrated in these experiments, the development of experimental infection was accompanied by the inhibition of the phagocytic activity of peritoneal macrophages and response of leukocytes to PHA, as well as metabolic shifts in liver tissue. Treatment with thymalin produced a combined immunomodulating and metabolic effect.

[Rotavirus infection in adults].

Out of 8 patients who died at the age of 36-80 years rotavirus infection was diagnosed virologically as a main disease in 3 cases, usually in combination with other diseases. Characteristic features were as follows: acute onset, involvement of serous and mucous membranes of mainly small bowel, frequent serous peritonitis, possibility of generalization with the involvement of the gall bladder, liver, heart, lungs, kidneys. Typical histological changes were nuclear damage in various cells which increased in size and became bizarre in shape.

[Chronic course of influenza with extrapulmonary involvement].

Results of the investigation of 265 placentas and 387 necropsies with the use of a wide spectrum of morphological, immunological and virological methods are presented. The existence of extrapulmonary damages, those of placenta (50 observations) and brain (17 observations), is shown. The variants of the influenza infection (persisting, chronic and manifesting) are discussed. The possibilities of a long-term survival of the infectious agent in the long-living brain and placental structures are considered. The proofs are presented showing the secretion by these organs of local factors of non-specific defense such as interferon.

[The morphological characteristics of diphtheria in adults under current conditions].

Clinico-anatomical analysis of 112 lethal cases of diphtheria in adults showed a grave course of the disease in chronic alcoholics at the age of 40-49 years. The morphological changes were characterized by a complex of typical changes at the primary focus, intracanalicular and hematogenous dissemination with high bacteremia. The morphology of pneumonia caused by Corynebacterium diphtheria is described for the first time. At the early stages of the disease infectious-toxic degeneration of the myocardium was documented with predominating alteration of the atrioventricular node or contractile myocardium. The above morphological features may be due to peculiarities of patients and decreased subjects as well as possible changes in the pathogen characteristics.

[Morphologic characterization of the contemporary dysentery caused by Shigella flexneri 2A].

There is a tendency in St. Petersburg since 1990s to an increase of dysentery morbidity and lethality mainly caused by Flexner Shigella 2a. Morphology of the disease in adults is given on the material of 286 necropsies in 1993-1994 in the S. P. Botkin Infectious Hospital. Males (mean age 50 years) and among them chronic alcoholics prevailed; females (mean age 70 years) were frequently with a grave cardiovascular pathology. Advanced inflammation (fibrinous, ulcerative and hemorrhagic) of all colon and distal part of the small bowel was a common finding. Hemorrhagic infarctions of the ileum which were not described so far were observed in 10% of the cases.

[The role of mass morphological study of placenta for prognosis of infectious processes in newborns].

The results of the long-term morphological study of a great number of placentas in the cities of St. Petersburg and Novgorod have been summarized. These results show frequent occurrence of infection of various etiology. The efficiency of revealing the risk groups among newborns is documented by a considerable decrease of infant mortality in Novgorod (from 20.6 in 1986 to 11.9 in 1996). The same phenomenon is demonstrated in St. Petersburg where a decrease in the incidence of intrauterine infections was noted at the autopsy material. A detailed study of catamnesis of children infected in utero showed an increase of minor neurological symptoms and higher incidence of acute respiratory viral infections.
