[Studies on the lymph drainage of the eye. 2. Drainage of lymph-dependent radioactive substances from the anterior chamber. (author's transl)].

In order to study lymph drainage from the eye colloid solutions (198Au-colloid and 99mTc-sulfur-colloid) were injected into the right anterior chamber of the eye in 12 rabbits. The distribution of the radioactivity was measured in vivo with an Anger-type camera and in vitro with a sodium iodine crystal well-counter after dissection. The drainage of radioactivity from the anterior chamber had an average half-life of 36 min. The drained activity could be found mainly in blood, liver, kidney, and bone tissue. Moreover, a concentration of radioactivity significantly surpassing the concentration in blood was measured in the right retrobulbar space in 75% of the cases and in the cervical lymph nodes in 50% of the cases. These findings substantiate a communication between the anterior chamber of the eye and the retrobulbar space in the form of a lymphatic or a prelymphatic-lymphatic flow.

[Isotope angiographic measurement of blood flow velocity in the human aorta and major arteries. I. Methodology and physiological considerations].

A special 8-channel detector arrangement was used to determine atraumatically the mean blood flow velocities in the aorta and the large arteries of the lower extremities in man. Following intravenous injection of 99mTc-pertechnetate time-activity curves were recorded over defined measuring points over the arteries, from these the appearance times of the tracer determined, and the mean blood flow velocities calculated from the differences between appearance times (the minimal transit times) and the distances between measuring points. The simultaneous determination of the minimal transit times between the vena cava inferior and the root of the aorta, which correspond closely to the total cardiac transit times, permits recognition of cases in whom blood flow velocity is reduced as a result of cardiac disease.

[Automated evaluation and analysis of isotopic heart function tests with minimal cardiac transit times].

The clinical evaluation of cardiac function by non-invasive means with the help of radioactive isotopes registering minimal cardiac transit times (MTTs) is increasingly employed in routine diagnosis. In order to economise on work load and time expenditure and for the purpose of an objective and complete evaluation of data, automatic data processing is desirable. This paper describes a program that consists of 4 sections: 1. examination 2. generation of data 3. processing of data 4. evaluation of data. The program permits a nearly total automated data generation, analysis, calculation, classification, final clinical evaluation and the automated production of a medical report. This greatly reduces the time spent per examination to approximately 5 minutes. The automated evaluation of the data is based on clinical experience with approximately 3.500 measurements in patients with the most frequent cardiac diseases. The result is an objective statement of causes of MTT changes and is highly useful for medical routine application. The control and the final inclusion of the findings into the spectrum of other clinical results remains the subject of the physician's judgement. The advantage of the method is the evaluation of all cardiac segments. This pertains to the evaluation of both atria and both ventricles, as well as of the entire central circulation with and without participation of the lung.

[Isotopic study of cardiac hemodynamics after multiple valve prothesis (author's transl)].

The radiocardiography of the minimal transit times (MTTs) produces cardiologically useful hemodynamic data also in such instances where invasive techniques with the cardiac catheter are technically difficult, as is the case in patients with multiple valve prothesis. The present study reports on MTT measurements in 9 patients with tricuspidal prothesis, 8 of which had additional mitralvalve prothesis and 2 additional aortic valve prothesis. The measurements were made with Tc-99m Pertechnetate, and an Anger camera with electronic data processing was used as detector system. The results indicate that following implantation of valve prothesis the hemodynamic state remained essentially disturbed. The pathologically altered transit times are an expression of disturbed volume ratios. The transit times in the atria remained prolonged by an average factor of 4-5. The ejection of the left ventricle stayed diminished more pronounced than on the right. The values of one patient who was examined before and after implantation of the prothesis in mitral and tricuspidal position, also showed a worsening of the data especially in the atria. Because the cardiac function study with radioisotopes is non invasive, simple, and fast, and leads to only a minor radiation burden, the method may be repeated and without hazard used for control follow-ups.

[Pathologic-anatomic aspects of the formation and complications of diverticula (author's transl)].

Diverticulosis is favored by general (erect posture), diatetic (low-roughage diet) and anatomic factors (discontinuity and particularly spiral arrangement of the musculature). A pouch of mucous membrane passes through the bowel wall in some cases, which makes comparison to the false diverticula of the so-called trabecular urinary bladder understandable. An important feature of the pathogenesis is the displaceable layer between mucosa and musculature. Diverticulosis is the basis of a disease, i.e. diverticulitis: this begins with stercoraceous pressure ulcers, and causes granulation tissue local peridiverticulitis. In peridiverticulitis, confluated inflammation sometimes involves all the sigmoid ensheathing it, with pernicious fomation of scars, i.e. perisigmoiditis. In this way, a disease of the intestinal wall becomes a disease of the intestine. The symptoms of perisigmoiditis are similar to those of carcinoma of the sigma.

[Obliterative angiopathy of placental stem villi (author's transl)].

Out of 136 placentas with vascular obliterations, 25 cases were placentas of children born alive, in 92 cases the placentas belonged to children born dead. In 19 cases we had no data on the baby. In placentas of babies born alive, the same vascular changes (subtotal and total obliterations, septal partitions of vascular lumina) were found as in those of dead-born children, although considerably less severe. Vascular obliterations should not be considered as post-mortal alterations of the placenta blood vessels, since only quantitative differences could be proved. Septum-like partitions are hardly ever found in placentas of babies born alive, in dead-born babies they are more frequent. They seem to present recanalizations, and are understood as a compensation mechanism for a placental insufficiency caused by vascular obliterations. The accentuated collagenization of the placental periphery, noticed in placentas of babies born alive, is being interpreted as the consequence of an impaired blood circulation, caused by partial and total vascular obliterations. The high collagen rate in the placental periphery in placentas of the dead-born is probably a reaction to the diminished fetal circulation. Endangitis obliterations in 73 placentas out of 4600 pregnancies of cases with late abortions, premature deliveries, perinatal death, underweigh and small for gestational age babies, impaired adaptation in newborns of mothers with proteinuria and hypertension speak strongly for assuming that endangitis obliterans presents a form of placental insufficiency. Endangitis obliterans of the placental blood-vessels has, however, been discovered frequently after Rubella infection in early pregnancy. The etiological factors of the endovascular process can be multiple, the morphological and the pathophysiological reactions are the same.

[Persistent diarrhea in rheumatic disease].

The case history of a 52-year-old male with weight loss, steatorrhea and arthritis is presented. During clinical pathological conference, Whipple's disease was strongly suggested. The diagnosis could be proved morphologically. Antibiotic therapy with tetracycline 1 g daily for three months caused a prompt improvement. Small bowel biopsy showed disappearance of PAS-containing macrophages.

[Fibrin clots in placental vessels in intrauterine shock (author's transl)].

In 56 placentas there were fibrin clots in the vessels of the chorionic plate and the stem villi. Fifty infants survived. The fibrin clots in the relatively large placental vessels are considered to be the result of intrauterine shock. A similar pathogenetic concept is postulated as in the well-known diseminated fibrin thromboembolism which is evident in the visceral organs of children who died in the perinatal period. This identification of these fibrin clots in the vessels of the stem villi, especially in cases of so-called risk deliveries, allows the risk for newborn to be determined during their first days of life (hyaline-membrane disease, hemorrhagic diathesis, etc.). One can testify the intrauterine shock by these findings, even in newborn who survive the respiratory distress syndrome.

[Isotope studies of cerebral circulation using the Fucks-Knipping gamma camera: use of a multi-parameter system with an intravenous tracer injection technic].

In order to analyze cerebral blood flow for diagnostic purposes, Indium 113m-DTPA was i.v. injected into seated patients, and time-activity curves were registered by a multifacet gamma camera (Fucks-Knipping) from 8 regions of interest, from the median area and from the right and left side of the head: 3 from the hemisphere, 3 from the brain stem, 2 from the large arteries of the neck. The results from 14 patients with chronic impairment of cerebral blood flow were compared with normal data from 20 healthy individuals. The time activity curves were analyzed for peak-height, mean transit time, and ratio peak to plateau height. The individual parameters were investigated for the degree of their correlation in a multiparameter system. The results from the patients indicated for the different single parameters a non-uniform response. There was also no correlation between the deviation of different parameters, between single pathological parameters and the degree and the site of blood flow impairment. On the other hand, a positive correlation was found between the number of pathological parameters and the degree of blood flow impairment irrespective of the site of its localization. Reason for this result probably is the variable location and degree of blood flow compensation by collaterals, which predominantly are expected at the cerebral base. The data indicate the potential usefulness of the application of the multiparameter analysis to quantitatively detect the degree of impairment of cerebral blood flow without regard to its topography. In this respect, the method, here described, promises to be of clinical value for non-invasive and non-hazardous diagnostic screening of cerebral blood flow.

[Minimal cardiac transit times in the diagnosis of heart disease: measurements with the gamma retina V and indium-113m. The influence on central volume relationships of long-term digitalis therapy in patients with latent and manifest coronary insufficiency].

Using Indium-113m and the Gamma Retina V (Fucks-Knipping Camera), the minimal cardiac transit times (MTTs) were measured radiocardiographically from the right auricle to the aortic root. This analysis served to determine the relation between stroke volume and the segment volume of the part of circulation between the right auricle and the aortic root. In 39 patients with myocardial insufficiency of different clinical degree the effectiveness of digitalization was, up to a period of 5 years, measured by means of the volume relation mentioned above. The following conclusions can be drawn from the results: digitalization of patients with myocardial insufficiency leads to an improvement of the impaired relation of central volumes. In patients with diminished cardiac reserve the improvement is drastic and often results in a nearly complete normalization. The data remain constant during therapy even for an observation period of 5 years. Digitalization of patients with congestive heart failure only leads to a partial improvement. In contrast to patients with diminished cardiac reserve this effect is temporary. The different behaviour of the relation between stroke volume and segment volume in patients with diminished cardiac reserve and congestive heart failure under prolonged administration of digitalis points to the necessity of treatment with digitalis in the early stage of myocardial disease.

[Isotope-angiographic measurement of aortic flow velocity for differential diagnosis of aortic sclerosis].

The speed of flow between aortic arch and abdominal aorta was determined from the corresponding minimal transit time (MTT) measured with 113m-In-DTPA and the Fucks-Knipping gamma camera. A total of 144 individuals was examined: 62 normal persons, 34 patients with cardiac insufficiency; 31 patients with hyperkinetic syndrome (effort syndrome and hyperthyroidism) and 17 patients with isolated aortic sclerosis without impairment of cardiac function. In all individual groups there was a highly significant correlation between speed of flow and heart rate. For a given heart rate speed of flow was diminished in cases of cardiac insufficiency and aortic sclerosis; it was accelerated in patients with hyperkinetic syndrome. By plotting the speed of flow through the aorta, normalized to an arbitary heart rate, against the corresponding MTT, also normalized to the same heart rate, the presence of aortic sclerosis could be differentially recognized in 14 of the 17 patients with this disease.

[Minimal cardiac transit time in thyroid disorders and in the effort syndrome].

Minimal transit times (MTTs) were determined with the Fucks-Knipping gamma camera from differences between arrival times in consecutive cardiac compartments of blood radioactively labelled with 113m-In-DTPA Measurements were made on 50 patients with effort syndrome, 59 patients with hyperthyroidism before and after thyroid suppression therapy partly suffering from associated heart disease, and 25 patients with hypothyroidism before and after therapy with thyroid hormone. In patients with effort syndrome and hyperthyroidism, MTTs were often, but not always shortened below the control values; consequently, in case of associated heart disease, MTT-prolongation may be masked in such instances where hyperthyroidism causes MTT shortening. Hypothyroidism caused MTTs to be prolonged. The prolonged MTTs reverted to normal values upon adequate therapy with thyroid hormone.
