The authors describe their personal experience with the management of mammary asymmetries. A review of their database from January 1998 to January 2005 identified 177 patients with idiopathic breast asymmetries. All these cases had been classified previously into six groups. Bilateral asymmetric hypertrophy and unilateral hypertrophy were treated with reduction mammaplasty. Unilateral hypertrophy with amastia or hypoplasia of the contralateral side was managed with reduction and augmentation mammaplasty. Unilateral amastia or hypoplasia (Poland's syndrome) was treated with a single monopedicle transverse rectus abdominis muscle (TRAM) flap, and asymmetric bilateral hypoplasia was managed with augmentation mammaplasty. Unilateral mammary ptosis was treated with mastopexy and augmentation mammaplasty. The proposed classification, derived from the authors' experience in this field, gives an idea of how they usually treat these patients. It is useful for a first evaluation, but after that, every treatment must always be individualized on a patient-by-patient basis.
BACKGROUND: The authors report their experience with autologous graft rhinoplasties. METHODS: Data were collected retrospectively, with selection of only autologous grafts from 2,000 rhinoplasties performed at the Plastic and Reconstructive Department of the University of Rome Tor Vergata. RESULTS: A total of 62 patients from January 1995 to January 2005 were selected. Most of the patients were treated with the "open tip" technique, whereas 9.7% had a classic endonasal approach. Follow-up evaluation was performed with outpatient visits at 2 and 6 weeks, then at 3, 6, and 12 months. Good aesthetic results were obtained for 93.5% of the patients, and 83.7% had complete satisfaction. CONCLUSION: Autologuos cartilage graft rhinoplasty is an affordable technique easy to learn that widens possibilities of interventions for nasal pyramid reconstruction.

Cigarette smoke has many detrimental effects on health, with consequences such as cardiovascular diseases, respiratory diseases, and tumors. In plastic surgery, these effects appear during the wound healing process. This retrospective study showed wound healing in 57 patients who had undergone abdominal dermolipectomy surgery. The patients were divided into two groups: smokers and nonsmokers. According to the results, smokers face a great complication risk for surgical wounds, which cause aesthetically more undesirable scars than observed in nonsmokers. The authors hypothesize that abstinence from smoking for 4 to 12 weeks before surgery would improve the quality of the scars.
The authors report their experience in five years of treating breast implant capsular contractures with an external ultrasonic device that facilitates the closed capsulotomy technique. A set of 52 patients have been treated with a 82.6% of improvement at a year follow up. Methods of application and results are discussed.
Many of the unsatisfactory results in cosmetic surgery are due not to the technique employed itself but to the virtual changes that our work has imposed in other areas. These changes are especially evident in the face, due to the small dimensions of its organs. The surgeon should do a thorough evaluation of the area to be treated prior to the surgery and make her/him notice not only the usual existing asymmetries, but also the modifications which probably will be noticed in the neighboring parts after surgery, to avoid later problems. The surgeon will discover then the many irregularities a patient may have, which have not been noticed by the patient before. These, if not detected in time, are weapons that may act as a boomerang against the surgeon later.
Traditional therapies in malignant tumors are often inadequate, either because the tumors aren not responsive or because the tolerance dosage has little effect. With this in mind, our study examined advanced tumors of the lower bowel and liver. We evaluated the results of 10 cases of anorectal epidermoid cancer. We treated these patients with radiofrequency hyperthermia, since previous radiotherapy was unsuccessful. The patients had significant tumor remission, along with a 'free-of-disease' follow-up of 10 months. In this paper, we also report the positive response in 10 cases of multifocal liver tumors. All these tumors had metastasized from gastrointestinal sites and were not responsive to traditional therapies. These patients had remission with a follow-up of 6 months. In the first cases, we associated 'low-dose' radiotherapy with hyperthermia; in the latter cases, we induced chemoembolization and also associated it with hyperthermia. We emphasize these associations and suggest the very important role of intraoperative hyperthermia in absolutely inoperable cases or in those cases previously diagnosed as unresectable.
AIMS AND BACKGROUND: In the last ten years validation of the sentinel lymph node (SLN) concept has led to modification of the surgical approach for patients with intermediate-risk cutaneous melanoma. METHODS AND STUDY DESIGN: Forty-eight patients affected by cutaneous melanoma with a Breslow thickness between 0.65 and 4 mm were enrolled in the study. Approximately 2 mCi of radiotracer and 1 mL of vital blue dye were injected in each patient around the site of the primary lesion. Lymphoscintigraphy was performed until the lymphatic basin and the respective SLN were localized. The whole surgical procedure consisted of enlargement of the surgical margins followed by localization and excision of the SLN(s) by using both radiotracer and vital dye. Whenever the SLN proved to be histologically positive for metastasis, complete regional lymphadenectomy was performed. RESULTS: Within 15 minutes of radiotracer administration the lymphatic basin was localized in all 4 patients by lymphoscintigraphy. Vital dye and radiotracer successfully allowed SLN localization and excision in 46 of 48 patients (97%); in one case the SLN was detected by radiotracer alone. The SLN proved to be metastatic in six (13%) of 46 evaluable patients; interestingly, in three of them the presence of metastatic cells was revealed only by immunohistochemistry. All patients with tumor-positive SLNs had primary lesions with a Breslow thickness = 2 mm. CONCLUSIONS: Sentinel lymphadenectomy is able to identify lymph node involvement in patients with cutaneous melanoma with a Breslow thickness > 1 mm, thus avoiding the risks associated with radical regional lymphadenectomy. Lymphoscintigraphy proved to be an important tool to obtain correct preoperative localization of the drainage basin, especially for melanomas located on the face and trunk.
The authors report their experience in the treatment of middle skull base fractures with TMJ involvement of ten patients affected by fractures of the glenoid cavity and of the petrous part of the temporal bone, with TMJ disfunction and facial nerve palsy. The treatment has been based on functional rest, a liquid diet for 5 days and than a functional physiotherapy aimed at rehabilitation of the TMJ. The efficacy of treatment has been showed after 2 months by restoring the dental occlusion and the TMJ's function in nine out of ten cases. Three out of four patients (75%) at the same control showed a full resolution of the facial nerve palsy. At an eighth month's follow-up all the cases had a complete resolution of the symptoms. Finally no significant studies have been reported in literature to set standardized protocols for the treatment of glenoid cavity fractures. According to the experience of authors the fractures of the medium cranial base involving the glenoid cavity should be treated as the current trends of intracapsular TMJ fractures. The orthopedic-functional treatment turns out particularly efficacious when there isn't a reduction of mandibular vertical heights.

Cyclosporine-A (Cy-A) selectively inhibits T cell function with minimal effect on humoral immunity. It is used specially in transplantation patients and in some cases, like other drugs, it causes gingival hyperplasia. In this article, after a review of the most recent pathogenetic hypothesis, the authors present their experience based on 10 cases of gingival hyperplasia in kidney transplantation patients in treatment with Cy-A.
The Darier-Ferrand dermatofibrosarcoma is a cutaneous lesion with a clinic and recidivant progress that appears in both sexes in the same measure. The metastasis are rare. The authors describe a kind of treatment effected in 13 cases arrived to their observation.

We review the tumors localized in the orbito-palpebral region. Their prevalence varies between 2.5 and 16.8%. Skin epitheliomas are more frequently seen. We discuss the surgical techniques for the repair and reconstruction of the orbital region.
The aim of this article is to present the personal experience on the treatment of 2 groups of patients presenting the same problem: the recovery of teeth 13 and 23 in the dental arch. The 1(st) group consists of adult patients aged approximately 20-28 years with a retained position of the permanent canines and permanence of the respective deciduous teeth in the dental arch. In the 2(nd) group, the loss of the deciduous teeth had not been followed by the physiological eruption of the permanent teeth. The authors' aim is to demonstrate that similar orthodontic treatments can represent valid solutions for the treatment of pathologies with different etiopathogenesis.
