[Surgical approach for destructive cholecystitis in elderly and old patients].

The individual surgical policy in the treatment of patients over 60 years of age with destructive cholecystitis was developed. Urgent radical surgical procedures using total intravenous anesthesia with endotracheal intubation and ALV were performed in patients with a low surgical and anesthetic risk and without concomitant acute pancreatitis and obstructive jaundice. Cholecystostomy and delayed cholecystectomy were performed in patients with these concomitant pathologies. Palliative operations were performed in patients with high surgical and anesthetic risk. Patients with disseminated peritonitis underwent cholecystectomy through laparotomy using total intravenous anesthesia with epidural blockade. Choice of method of cholecystectomy and anesthetic management depended on nature of concomitant diseases and complications. Proposed individual surgical policy permitted to decrease postoperative lethality to 0.8%.

[The effects of enkephalins on the water-salt metabolism of rats in ontogeny].

The experiments were carried out in white Wistar rats of three ages: 25-30 days (Group 1, n = 102), 45-50 days (group 2, n = 62), over 180 days (group 3, n = 115). The water loading was given through a gastric tube in an amount of 5% of the body weight. A synthetic analogue of leu-enkephalin (SAE) was injected intraperitoneally in a dose of 100 mg/kg. The control animals received physiological saline. In the baby rats of group 1, administration of SAE slowed down the polyuric and ionouretic reactions in response to the water loading. In older rats of group 2 administration of SAE enhanced water elimination but with this the osmolarity of the blood plasma increased together with the content of potassium and sodium ions in it. In adult rats (group 3) injections of SAE produced a smooth diuretic reaction which retained constant ion levels in the blood plasma.

[Plasma cortisol levels in the newborn infants with hypoxic and traumatic injuries of the spinal cord].

Adrenal glucocorticoid function was studied in 56 newborns with natal injuries of the cervical section of the spine and the contribution of the hypoxic factor to such injuries defined. The babies were examined by the cliniconeurologic and electron-neuromyographic methods in order to specify the level of the injury. Blood serum and umbilical blood hydrocortisone levels were radioimmunoassayed immediately at birth and on days 5-7 of life. Spinal injury at the C1-C4 level was associated with low hydrocortisone levels, that may be regarded as an additional criterion for the differentiation of the level of injury in traumas of the cervical portion of the spine in the newborns.

[Effect of opiate receptor agonists on the course of hemorrhagic shock in rats].

The experiments have been performed on 93 male rats, weighing 200-250 g. In acute blood loss various arterial pressure (AP) changes have been demonstrated--the marked hypertension is being changed by gradual AP increase. The injection of m-receptors' agonist DAGO prevents systolic and diastolic AP increase, agonist DADL prevents diastolic AP increase in acute momentary blood loss. In gradual blood loss DAGO (more than DADL) slows down both the decrease and the subsequent AP increase in rats. DAGO is determined to decrease, and DADL--to increase the minute blood volume. The mechanisms of opioids' action and their significance in pathogenesis of hemodynamic disturbances in shock are being discussed.

[Ultrastructure of the peri-infarction zone during treatment of experimental myocardial infarct with cardiolin].

Cardiolin therapy influence on the periinfarction zone cardiomyocyte ultrastructure, sarcolemma permeability for colloidal lantan, myocardium energy level were studied in 25 white noninbred rats with experimental myocardial infarction. 6 hours after coronary occlusion ultrastructural damage in treated rats was less pronounced and mitochondria energy efficiency ratio was higher than those in the control group. 24 hours after the occlusion the tendency to normalization of the cardiomyocyte damage in treated rat myocardium was demonstrated, while in non-treated animals ischemic damage becomes irreversible, this being confirmed by the test with colloidal lantan. Cardiolin therapy prevents the infarction zone expansion by decreasing ischemic damage of periinfarction cardiomyocytes and by stimulating regeneration processes in these cells.

[Use of synthetic analogs of enkephalins as antiatherogenic means].

The experiments have been performed on 70 male rats weighing 200-250 gr., 53 patients with obliterating atherosclerosis of lower extremities have been examined. Leu-enkephalin analogues (dalargin and DEFN) administration in a dose of 0.1 mg/kg has been determined to decrease cholesterol levels in the blood in acute and chronic experimental hypercholesterolemia. The use of pharmacological preparation -- enkephalin analogue of dalargin in complex treatment of the obliterating atherosclerosis of lower extremities leads to cholesterol concentration decrease, parathormone concentration decrease in the blood of patients and hyperlactatemia decrease in venous blood of the damaged extremity. Leu-enkephalin analogues, action mechanisms and the possibility of their use for atherosclerosis treatment is discussed.

Parathyroid gland function and the possibility of its correction in patients with obliterative vascular disease of the lower limbs.

Parathyroid gland function and the possibility of its correction were studied in 43 patients with obliterative vascular disease of the lower limbs, and in 24 healthy, mostly male, controls. Parathormone (PTH) concentrations in blood plasma samples were determined by radioimmunoassay. Patients with obliterative vascular diseases of the lower limbs were found to have increased activity of parathyroid gland. The role played by hyperparathyroidism in the pathogenesis of obliterative vascular diseases of the lower limbs as well as the possibility of its correction by betablocker and encephalin analogue administration is discussed.

[A simple and informative method of diagnosing myocardial infarct, based on a determination of blood density].

An increased density of blood revealed in myocardial infarction patients served the basis for a simple and highly informative method developed by the authors to early diagnose myocardial infarction when other methods are not sensitive. The high density of blood is registered upon placing a drop of blood into flakes with sodium chloride solution of different concentration.
