Light and electron microscopy of rabbit testes infected with Treponema pallidum (Nichols strain): nature of deposited mucopolysaccharides and localisation of treponemes.

The mucopolysaccharide nature of the material deposited in rabbit testes infected with Treponema pallidum was confirmed by histochemical staining with alcian blue. Differential staining of mucopolysaccharides showed the presence of sulphated mucopolysaccharides as an almost constant feature, whereas in little more than half of the orchitic testes studied variable deposits of hyaluronic acid were seen. The treponemes were almost exclusively present in the areas rich in mucopolysaccharide. A combination staining with the Warthin-Starry method and alcian blue showed treponemes in close association with pre-existing fibrils and cells contained in these fibrils. The latter findings were confirmed by electron microscopy, and the fibroblasts to which treponemes adhered displayed the characteristics of activated cells. The close parallel between the histopathological changes observed here and their descriptions in published reports shows that our specific strain still behaves the same as the original Nichols pathogenic strain of T pallidum.

The detection of basal cell determinants in human basal cell carcinomas using two different monoclonal antibodies.

This report deals with the reaction pattern(s) of two monoclonal antibodies (MoAbs) with normal skin and basal cell carcinomas (BCC). Using indirect immunoperoxidase (IIP) and indirect immunofluorescence (IIF) techniques, MoAb 12 G7 was observed to react with a determinant related to the cell membrane of the epidermal basal cells. In the IIP technique MoAb 12 G7 showed a positive reaction with 32 out of 34 BCC (94%), while in IIF all the 14 BCC that were studied were positive. In most cases only the cells at the periphery of the tumour nests were stained. MoAb 253 B7 reacted with cytoplasmic determinant(s) of the epidermal basal cells both in the IIF as well as in the IIP techniques. Using the IIP technique only 5 out of 34 BCC (15%) showed a positive reaction with this MoAb. Four of the 5 positively staining tumours showed aggressive histological features. Using IIF technique only 2 out of 14 BCC were positive. The results presented in this communication are discussed with regard to the possible expression of selective differentiation and tumor-associated determinant(s) in BCC.

Intracerebral malignant schwannoma.

A case of highly malignant primary intracerebral schwannoma is presented in a boy aged 15 years. The histological, ultrastructural and immunocytochemical properties were consistent with a partly epithelioid schwannoma. All reports so far published of 18 intracerebral schwannomas were of benign tumors, one case was semi-malignant. As far as we know, this is the first report of a highly malignant intracerebral schwannoma.

Subepidermal bullous autoimmune disease associated with immune nephritis. Immunomorphologic studies.

There are only a few reports of the association of immune-complex glomerulonephritis with bullous pemphigoid. The occurrence of immune-complex nephritis with linear IgA bullous disease has not been previously reported. We report two cases, one a case of bullous pemphigoid and the other a case of linear IgA bullous disease, both of which were associated with immune-complex glomerulonephritis. The clinical and immunomorphologic data are briefly discussed.

Infantile seborrheic dermatitis. Morphological and immunopathological study.

The pathomechanisms of infantile seborrheic dermatitis (ISD) are unknown. An allergic reaction to candidal antigens is one of the hypotheses. The skin of 6 patients suffering from ISD was studied by immunostaining techniques. The lymphocytic infiltrate was composed of Leu4-reactive T cells in which Leu3a-reactive (helper/inducer) cells predominated. Direct immunofluorescence studies revealed no significant deposits of immunoglobulins or complement factors. No candidal antigens could be demonstrated, either in epidermis or in dermis, although candida species were cultured from different sites in 4 of the 6 patients.

Oncocytic variant of choroid plexus papilloma.

Two cases of oncocytic plexus papilloma are presented which were in the fourth ventricle and cerebello-pontine angle in women aged 70 and 63 years, respectively. The cytoplasm of the transformed tumor cells was enlarged, eosinophilic, and granular; and it was characterized ultrastructurally by innumerable mitochondria. Most of them had lamellar cristae, a clear matrix, and contained dense granules. These 'oncocytes' are identical with oncocytes of the human thyroid, parathyroid, bronchi, kidneys, adrenals, salivary gland and myocardium, and non-neoplastic choroid plexus epithelium. As far as we know, this is the first report of an oncocytic variant of the choroid plexus papilloma. Two unusual cases are described.

Segmental neurofibromatosis.

A child with segmental neurofibromatosis is reported. The clinical, histopathological and ultrastructural characteristics of this rare condition are described. This entity is considered to be a localized form of von Recklinghausen's disease. The aetiology is not clear, but somatic mutation in early embryonic development may be important.

Oral lupus erythematosus: markers of immunologic injury.

Tissue speciments were taken from three regions in a patient with oral progressive lupus erythematosus (LE): the tongue, the buccal mucosa and clinically uninvolved cutaneous tissue. They were subjected to immunocytochemical studies by use of markers. HLA-DR antigen expression on mucosal epithelial cells was found in association with a predominance of activated helper T-Cells in the lesional tissue. Langerhans cells (OKT-6) and B-cells (Leu-IO, B1 and B2) were nearly absent in the mucosal lesions. The presence of populations of macrophages (MY-4) in the lesional epithelium might suggest an important role for these cells, rather than for Langerhans cells, in the afferent limb of cellular immunity in this case of LE.

Erythromelalgia caused by platelet-mediated arteriolar inflammation and thrombosis in thrombocythemia.

Erythromelalgia was the presenting symptom in 26 of 40 patients with thrombocythemia in its primary form or when associated with polycythemia vera. The localized painful burning, redness, and warm congestion in the extremities could be accurately documented with thermography. Skin punch biopsy samples taken from the affected areas showed typical arteriolar inflammation, fibromuscular intima proliferation, and thrombotic occlusions. Erythromelalgia often progressed to ischemic acrocyanosis or necrosis in toes or fingers. Complete relief of pain and restoration of microvascular circulation disturbances was obtained with the cyclo-oxygenase inhibitors aspirin and indomethacin, but not with sodium-salicylate or the platelet inhibitors dipyridamole, sulfinpyrazone, ticlopidine, and dazoxiben. The erythromelalgia was alleviated during busulfan-induced remissions of thrombocythemia and its recurrence coincided with relapsing thrombocythemia. These observations suggest a causal relationship between erythromelalgia and thrombocythemia, in which platelet-mediated inflammatory and occlusive arteriolar changes play a part in the etiology of erythromelalgia.

Linear IgA disease and chronic active hepatitis--a coincidence or not?

Linear IgA disease (granular type) associated with chronic active hepatitis in a sixteen-year-old male patient is described. The diagnosis was established by clinical and histopathological findings supported by immunofluorescence studies. The immunofluorescence pattern (granular IgA deposits along the basement membrane, more pronounced at tips of papillae) showed much transition in dermatitis herpetiformis and also to a lesser extent in pemphigoid (linear IgG and IgM deposits along the basement membrane). Chronic active hepatitis as linear IgA disease are significantly correlated with HLA B8. This is the first report of the association of linear IgA disease and chronic active hepatitis.

Transfusion syphilis, survival of Treponema pallidum in stored donor blood. II. Dose dependence of experimentally determined survival times.

The hypothesis that experimentally determined survival times of Treponema pallidum in stored donor blood could be related to the number of treponemes initially present in the treponeme-blood mixtures was investigated by inoculating rabbits with three graded doses of treponemes suspended in donor blood and stored at 4 degrees C for various periods of time. The storage periods up to which living treponemes could be detected in the testes of the inoculated rabbits as well as those storage periods up to which seroconversion occurred are related to the number of treponemes present in the treponeme-blood mixtures. Increasing numbers of treponemes present in the donor blood resulted in longer periods up to which positive results in both parameters were found. All available evidence suggests that the upper limit of seroconversion coincides with the upper limit of treponemal survival. Inoculation with 5 X 10(4) treponemes per ml of donor blood resulted in a treponemal survival time of 48 h, inoculation with 1.25 X 10(6) treponemes per ml in a treponemal survival time of 72 h and inoculation with 2.5 X 10(7) treponemes per ml donor blood in a survival time of 120 h.
