A comparison of levocetirizine and desloratadine in the histamine-induced wheal and flare response in human skin in vivo.

BACKGROUND: The histamine-induced wheal and flare response was used to compare quantitatively the antihistaminic potency of levocetirizine and desloratadine. METHODS: In this double-blind, placebo-controlled crossover study, 24 healthy male non-atopic volunteers received weekly single doses of 1.25, 2.5 or 5 mg levocetirizine, 2.5, 5 or 10 mg desloratadine, or placebo. Four hours after dosing, histamine (100 mg/ml) skin prick tests were performed on the volar surface of both forearms. The diameters of the wheals and flares were measured 10 minutes later. Sedation was evaluated using a visual analogue scale and a motricity test. The effects of individual drug doses were compared using Student's t-test for paired data and the overall effects of the two drugs by ANOVA. RESULTS: All doses of levocetirizine significantly (P < 0.0001) inhibited both wheals and flares in a dose-related manner. Only the 10 mg dose of desloratadine achieved significant inhibition of response. ANOVA showed levocetirizine to be significantly (P < 0.0001) more active than desloratadine. Neither drug caused significant sedation or loss of motricity. CONCLUSION: Levocetirizine is significantly more effective than desloratadine in inhibiting wheal and flare responses to histamine in human skin in vivo, with 1.25 mg levocetirizine being more effective than 10 mg desloratadine.

The oral ecosystem: implications for education.

We propose a model that is applicable to oral health education. The model describes the oral cavity in a complexity-based ecological context. This concept includes the premise that factors from different organisational levels (biological, individual, community, society) interact in a complex way with the potential to 'stress' the ecosystem and thereby provoke changes. This mode of action complies with the understanding of the oral cavity as a complex adaptive system. An ecological model is actively used in the undergraduate problem-based curriculum at the Faculty of Odontology, Malmö University, Sweden and has recently been applied as a conceptual basis for the new dental curriculum being established at the University of Tromsø in Northern Norway. The purpose is to encourage and promote an ecological, health-oriented view and to stimulate reflections on premises for oral health and diseases in an integrated context.

Argentophilic structures of miracidia and cercariae of Philophthalmus lucipetus (Philophthalmidae: Trematoda) from Israel.

Argentophilic structures of Philophthalmus lucipetus miracidia and cercariae from Israel are described. Eighty-four of 87 miracidia examined displayed an epidermal plate arrangement of 6:8:4:2 = 20, similar to other Philophthalmus species. Twenty papilla-like structures are arranged on the terebratorium in 3 groups, along 1 axis. Sixteen body papillae are located at the bases of epidermal plates of row 1. Eyespots are mediodorsal, between rows 1 and 2. Excretory pores are lateral, between rows 2 and 3. Features common to Israeli and Bulgarian isolates, differentiating them from other species, include the presence of 16 body papillae as opposed to 10 in other species, and a maximum of 20 papillae on the terebratorium as opposed to 19 in the others. About 3% of the miracidia displayed different plate arrangements. Among the argentophilic structures of P. lucipetus cercariae, the Israeli and Bulgarian P. lucipetus show a common pattern of 2-4 excretory pores in the tail, but arrangement of cephalic CI3 and CI5 papillae in the 2 isolates is insufficiently unequivocal for species determination. The data presented show that miracidial characteristics, rather than those of cercariae, aid in determining the species of philophthalmids. They also support former evidence attesting to the identity of the Bulgarian and Israeli species.

Argentophilic structures of the miracidia and cercariae of Philophthalmus distomatosa n. comb. from Israel.

The morphology and patterns of distribution of the argentophilic structures of miracidia and cercariae of Philophthalmus distomatosa n. comb. are described. The epidermal plate arrangement of the vast majority (94.2%) of miracidia studied conformed to the formula 6:8:4:2 = 20. The rest (5.8%) displayed the following patterns of arrangement of epidermal plates: 6:7:4:2 = 19; 6:6:4:2 = 18; and 6:4:4:2 = 16. Twenty papillalike structures were observed on the terebratorium. They were arranged along 1 axis, in 3 groups. Most commonly, 16 papillae were present on the body, located between epidermal plates of the first and second rows. The eye spots were located dorsally, near interepidermal space S1. The numbers, arrangements, and locations of the excretory pores varied. Patterns of distribution of the tegumentary papillae of P. distomatosa n. comb. cercariae are given, including those of the cephalic regions, periacetabular region, and tail. Argentophilic structures of the mentioned larval stages and of other parameters clearly distinguishing between the presently described species and those of P. lucipetus from Israel are summarized. On the basis of comparison of the presently described larval stages and those of other Melanoides tuberculata-associated larval stages of Philophthalmus in Israel and Jordan, the possibility exists that a third species occurs in Israel.

Influence of Electro-Acoustic Effects on the Electro-Optic Response of Charged Colloids.

Low-frequencyanomalous electro-optic behavior of colloidal systems (sign reversal and deviations from Kerr low) is considered in the light of electrically induced acoustic modes. The latter were recently detected and investigated in samples of isotropic spherical particles. Their linear dependence on field intensity explains the low-field "permanent dipole" behavior of charged colloids. The coupling of anisotropy and density fluctuations results in the complicated frequency curves of the electro-optic responses of anisometric particles. Copyright 1999 Academic Press.

Argentophilic structures of the miracidium of Echinochasmus perfoliatus (Trematoda: Echinostomatidae).

Argentophilic structures of the miracidium of Echinochasmus perfoliatus were described from material collected in the vicinity of Vladivostok, Far East of Russia. Impregnated with 0.5% solution of AgNO3 miracidium showed 21 epidermal plates arranged in four rows: 6 + 9 + 4 + 2. Up to 23 papilla-like structures on the terebratorium were arranged along three axes and in four groups. A single papilla was located at the base of each of ventral and dorsal epidermal plates of the first row. Two papillae were located at the base of each of lateral epidermal plates of the first row. The eyespots were located posterior to the first row of plates. Two excretory pores were located anterior to the last row of plates. The results obtained were compared with the argentophilic structures of closely related species of the genus Echinochasmus.

Electrically Induced Shear Waves in Colloidal Crystals

The static light scattering method is used for the detection of acoustic waves induced in colloidal crystals and liquids by low-frequency electric pulses. The method is sensitive to both density and shear modes and can be applied for the study of the liquid-crystalline phase transitions. At low fields the variations of light scattering intensity follow both the amplitude and the phase variations of the wave motion and enable the determination of resonance parameters, hence of viscoelastic parameters of the colloidal system.

Argentophilic structures of the miracidium of Echinostoma trivolvis (Cort, 1914) (Trematoda: Echinostomatidae).

Argentophilic structures of the miracidium of Echinostoma trivolvis were described from 80 specimens reared from material originally collected in eastern Pennsylvania, U.S.A. Miracidia were impregnated with 0.5% aqueous silver nitrate solution. The miracidium has 18 epidermal plates arranged in 4 rows of 6 + 6 + 4 + 2 = 18. Up to 20 papillalike structures on the terebratorium were arranged along 3 axes and in 5 groups. A single papilla was located at the base of each of the 6 epidermal plates of the first row. The eyespots were located posterior to the first row of plates and 2 excretory pores were located anterior to the last row of plates.

[A method for placing and using an indwelling epidural catheter for analgesia in incurable cancer patients].

Operative technique for implantation of indwelling epidural catheter produce of the firm "Braun" (Germany) is described. During the implantation the analgesics zone is checked with Lidocaine. The catheter pouch is fixed over a bone pad in a place well visible by the patient. For permanent pain relief Dipidolor is applied twice daily one quarter of the ampoule. This catheter is implanted for pains originating from the lower extremities, the pelvis and the abdomen. The method was applied on 2 patients with very good results obtained with minimal amount of opiates.

[Sexual disorders in men following radical operations on the rectum for carcinoma].

This is the first study in Bulgaria of postoperative sexual disorders following radical surgical operations for rectal cancer. The study group comprised 64 patients. Six months after the operation only 15 (24.4 per cent) were able to have erection. Fourty nine patients who complained of inability to have erection received long-term treatment with vitamins, tranquilizers, cholinomimetic drugs, beta-blockers and medicines increasing bladder tonus. Normal erection was re-established in 31 (63.3 per cent) of these patients. The 18 (36.7 per cent) patients unaffected by drug treatment reported of possible ejaculation. This is an indication that impairment of sympathetic innervation of the genitals after operations of the rectum is of transient nature, whereas parasympathetic damage may be definitive.

[A combined method for treating metastases of colorectal carcinoma to the liver].

The method is a combination of operative treatment of metastases in the liver and regional intra-arterial chemotherapy. The indications for operative treatment of the underlying tumor, the metastases in the liver and the regional chemotherapy are pointed out. Two groups of patients with colorectal cancer and remote metastases in the liver are presented: one received surgical treatment alone, the other combined therapy. It is assumed that the combined treatment of the basic tumor, the metastases in the liver plus regional intra-arterial chemotherapy is a new trend in the treatment of advanced colorectal cancer metastasized in the liver.

[The extracorporeal lithotripsy of gallstones in Bulgaria].

Extracorporal lithotripsy is a new, modern method of treatment of gallstone disease. The effect of the treatment was evaluated as follows: full--fragments up to 2 mm in size (in 60.5% of the patients), partial--fragments between 2 and 5 mm in size (in 30.1% of the patients) and no effect--fragments larger than 5 mm or lack of fragmentation (in 9.4% of the patients). Complications during the manipulation and after it were very rare--allergic in 1.7% and obstructive in 6.7% of the patients. In only 3 patients on operation had to be done. 84 patients were followed up for 270 or more days. In 10.4% of the patients a full clearance of the fragments was found, and in 28.2% of the patients there was only a reduction of their number. Extracorporal lithotripsy followed by litholytic therapy is a perspective method for gallstone disease treatment. It has wide indications, few contraindications, is well tolerated and is with a low complication rate.

Strigea falconispalumbi (Trematoda: Strigeidae): argentophilic structures of the miracidium.

A study of the argentophilic structures of the miracidium of the strigeid digenean Strigea falconispalumbi VIBORG, 1795 was carried out. Miracidia were hatched under laboratory conditions from eggs obtained from adult S. falconispalumbi from the intestine of a naturally infected Buzzard (Buteo buteo). Miracidia were stained with 0.5%. Silver Nitrate solution using standard techniques. The epidermal plates of the miracidium are arranged in four tiers according to the formula (6 + 8 + 4 + 3) = 21. The terabratorium bears 20 papillare. Papillae are also present at the bases of the epidermal plates of the first tier, and also at the margins of the terabratorium between the epidermal plates. The two pores of the excretory ducts are positioned laterally between the third and fourth tiers of epidermal plates.
