The author determined the phasic structure of the systole of the left ventricle by the method of polycardiography and hemodynamics in 20 patients suffering from hypothyrodism. Blood plasma and erythrocyte electrolytes were examined at the same time. Patients with hypothyroidism displayed a phasic syndrome of hypodynamia and a marked correlation between the phase of the synchronous contraction, the period of ejection, the strength of contraction of the left ventricle and the electrolyte content. Sodium and magnesium produced the greatest influence on the phasic structure of the systole; potassium and calcium had a lesser effect. The heart stroke volume diminished; as to the cardiac index, expenditure of the energy of cardiac contractions directed to the maintenance of movement of 1 litre of the minute blood volume; the external work, and the peripheral vascular resistance displayed no significant change.
Overall one hundred patients with stage II hypertensive disease (HD) were studied for central haemodynamics and chronocardiometric estimators of the left ventricle (LV) of the heart. Correlation coefficients were calculated between haemodynamic indexes and values for (LV) phase structure. In stage II HD, the above correlations were found to be dependent upon the type of central haemodynamics. But irrespective of the type of blood circulation, there was an inverse correlation between the index exertion-time and phase of slow filling, which fact is believed to be a compensatory mechanism in cardiac performance under stage II HD.
In the course of theoretical education, one of the most important forms is that of lectures, particularly generalizing and problem-solving ones. Practical education (two thirds of the working time) will include the following: keeping patients under observation; practical studies; clinical and clinicoanatomical conferences; professor's rounds; going on duty by departments; participation in conducting and mastering the instrumental methods of investigation. The role of the audience at the lectures is an active one, that of the teacher is to direct and correct. Special attention is payed to individual work of a physician. All of the aforementioned kinds of work will, we believe, help the physician in making progress, upgrading his clinical mentality, attaining knowledge, skills, deontological principles in work. Use of test questions in cardiology approved by Ministry of Health of Ukraine as a national programme for certification of physicians together with the traditional methods of teaching will improve the quality of physician training.
A comparative evaluation of efficacy of magneto- and laser therapy was carried out in 82 patients with osteoarthrosis deformans. The magnetic field and laser irradiation dispelled the pain syndrome and synovitis manifestations. It is recommendable that the multiple-modality therapy of patients with osteoarthrosis deformans should involve magneto- and laser therapy (15 to 20 procedures per one course) that improve results of the treatment being received and allow the time of hospitalization to be reduced at an average by 5 bed-days. Laser appeared to be a very effective mode of treatment. No unfavourable side effects were recordable.
Measured in 119 patients with chronic heart disease (exercise-induced angina in 62, cardiosclerosis in 57 cases) presenting with arrhythmias (extrasystolic arrhythmia, permanent form cardiac fibrillation, association of cardiac fibrillation with ventricular arrhythmia, with n = 58, 38, 23 respectively) were blood levels of thyroid-stimulating hormone (TSH), triiodothyronine (T3) and thyroxin (T4) using immunoradiometric assay. In extrasystolic arrhythmia, permanent form cardiac fibrillation, there were no deviations from the norm in the basal TSH blood levels, reduction in (T3) as well as augmentation of (T4) in cardiac fibrillation. With permanent form cardiac fibrillation associated with ventricular arrhythmia the TSH blood level got decreased, the (T3) and (T4) content undergoing no changes The posttreatment blood content of (T3) tended to augment in extrasystolic arrhythmia.
