bSTH and IGF-I levels in blood and milk of black and white dairy cows before and after application of recombinantly derived bovine somatotropin.

In an 8 week trial period six black and white dairy cows were injected with Optiflex, a slow release formulation of recombinantly derived bovine somatotropin (BST). Three cows each were injected at two-weeks (BST-14) or four weeks (BST-28) intervals. The dosage was 640 mg BST per injection. Average milk yield increased by 18.3% and 13.8% in groups BST-14 and BST-28 respectively. Blood plasma levels of bSTH and IGF-I increased till day 10 post-injection; thereafter they declined, except IGF-I levels in group BST-14 which persisted continuously elevated with minor variations. No bSTH above detection level could be assayed in the milk of treated animals. The IGF-I level in milk was slightly elevated after BST-application but stayed within the normal range.

[Craniotabes, craniomalacia (Wieland) and active ricketts in infants].

The symptom "craniotabes" means rickets only in few cases. Determinations of calcium, alkaline phosphatase, phosphorus and if possible 25-OH-vitamin D are necessary for confirmation of the rickets-diagnosis. As denotation for the not rickets-caused forms of craniotabes the definition positioning caused craniotabes is suggested.

[Vitamin D-induced tissue calcinosis and arteriosclerosis changes. I: A contribution to the 60 year history of vitamin D research with special reference to childhood].

Already in 1927, a short time after the detection of the vitamin D, the danger of a hypervitaminosis was put into the field of view. Overdoses of vitamin D may effect calciferous infiltrations in the soft tissue and arteriosclerotic changes. These findings are important especially under consideration of the up-to-now large-dosed vitamin-D-application in the field of the rachitis-prophylaxis. The appearance of nephrocalcinoses could have been demonstrated in predisposed infants and children suffering from disorders in calcium metabolism. In order to optimize the rachitis-prophylaxis in the GDR we comment on the problems and dangers by vitamin-D-overdosages on the basis of animal-experimental and clinical examinations.

Arteriosclerosis↗

[Vitamin D-induced tissue calcinosis and arteriosclerosis changes. II. Current knowledge and conclusions for preventive vitamin D administration in infancy and early childhood].

By means of the actual literature we clear up that vitamin-D is considered to be a risk factor for arteriosclerosis. In vitamin-D-susceptible children elevated vitamin doses may--to begin with--lead to a nephrocalcinosis and--subsequently--to a coronary sclerosis. The combination of a nourishment, rich in calcium, an over-supply of vitamin-D, and high cholesterol-contents, is considered to be especially unfavourable. In order to avoid a potential danger of arteriosclerosis the rachitis-prophylaxis in infants and babies should be taken up with physiological vitamin-D-dosages.

A 5 year interdisciplinary control of iodine deficiency in the GDR.

The interdisciplinary iodine prophylaxis in the GDR is a successful measure for control of ID. The iodization of nearly all the salt is planed perhaps with reduction of the iodine concentration. However, next further steps may be done in cooperation of the Interdisciplinary Iodine Commission with the "Arbeitskreis: Jodmangel" in the FRG.

Thyroid hormone metabolism under extreme body exercises.

In two runs over a distance of 75 km and 45 km as well as in a marathon (42.2 km), the thyroid hormones (T4, T3 and TSH in all runs and rT3 in two of them) were determined before and after the runs. The performance of the runners was analyzed, with their age considered in the analysis. We found that the changes of thyroid hormones are characterized by individual differences. Younger runners and those with better performances had significantly higher T4 serum concentrations and unaltered T3 and TSH concentrations, whereas older sportsmen and those with worse performances reacted to the stressful stimulus with a significant decrease of T4, T3 and TSH. This response was interpreted as an exhaustion reaction, i.e. reduced adaptation responses of thyroid hormone metabolism. Therefore, these investigations provide an assessment of the state of fitness, the optimization of training and the avoidance of damage caused by overly stressful physical exercise

Effect of endurance exercise on somatomedin-C/insulin-like growth factor I concentration in male and female runners.

By means of 3 endurance exercises, the effect of a several-hour intensive somatic stress on the changes of the Sm-C/IGF-I concentration was tested during, immediately after and on the day following the exercise. Exp. 1: Marathon with 17 male sportsmen in 2 groups with different glucose supply. Exp. 2: 45-km crosscountry run with 41 males. Exp. 3: Three 20-km runs with 8 young females at intervals of 3 months. In the marathon, no significant changes of the Sm-C/IGF-I concentration were found between the start, half distance and final values. The exogenous glucose supply (continuous or discontinuous) had also no effect. The tendency of a slight decrease of the Sm-C/IGF-I concentration by 0.14 U/ml (p greater than 0.05) was observed between start and finish in the 45 km crosscountry run lasting one hour longer. In the three 20-km runs, reproducible, slightly increased levels were measured at the end, whereas a decrease to the initial value or even below was detected on the following day (p greater than 0.05). The insignificant alterations of the Sm-C/IGF-I concentration measured in the 3 variants of races show that neither the hormonal changes stimulating the Sm-C/IGF-I synthesis (e.g. increase of GH and prolactin) nor inhibiting factors (energy deficiency) clearly dominate during strenuous exercises. The binding of carrier protein prevents great variations of the Sm-C/IGF-I level even under the condition of 3- to 4-hour extreme endurance exercises.

[Multiple endocrine neoplasia of the II B type].

Multiple endocrine neoplasia, type II B (MEN II B) includes thyroid carcinoma, pheochromocytoma, ganglioneuromatosis and marfanoid habitus. Its short-term prognosis is determined by the tumour of the adrenal gland, whereas long-term prognosis is determined by the thyroid carcinoma. Often the well-defined but rare syndrome is diagnosed late. The MEN II B's therapy consists of thyroidectomy and adrenalectomy uni- or bilateral. The 5 year-rate of survival is 75%. Own observations are reported.

Interdisciplinary aspects of iodine prophylaxis in German Democratic Republic.

Endemic goiter still occurs in several European countries. Alimentary iodine deficiency could be proved as the main cause of endemic prevalence. Extensive epidemiologic surveys showed the great medical and economical impact of this matter for the GDR. Over 200 million Marks might be spent for diagnosis and treatment of thyroid patients per year. Also in the field of veterinary medicine and animal production iodine deficiency plays an important role. Under the conditions of iodine deficiency the fodder containing nitrate and glucosinolates (e.g. bruised rape extraction) results in additional goitrogenic effects (so-called indirect or relative iodine deficiency). Due to iodine deficiency disorders considerable losses of yields in cattle, pigs and sheep occurred. The content of iodine in animal products used for human nutrition was subsequently low. Considering the ecological implication of these problems an Interdisciplinary Iodine Commission within the Society for Endocrinology and Diseases of Metabolism of the GDR was established. Together with the governement authorities it worked out corresponding recommendations for the prophylaxis and control of its efficiency. In 1986 the iodination of most of the packed salt (32 mg KIO3/kg = = 19 mg I/kg) and the administration of iodinated mineral mixtures for the animal production were started for pigs in the whole territory of GDR and for cattle in the southern counties of the GDR only. These activities have been accompanied by information of TV as well as in the newspapers and medical journals. Thus, an improved supply of iodine to the population through iodinated table salt and animal products can be taken into account. The efficiency of these prophylactic measures is controlled interdisciplinarily.(ABSTRACT TRUNCATED AT 250 WORDS)

Three injections of human chorionic gonadotropin are as effective as ten injections in the treatment of cryptorchidism.

Two schedules of human chorionic gonadotropin (hCG) administration were compared in a study of 332 boys (473 testes) aged 1-13 years with cryptorchidism. One group of patients received 10 hCG injections according to the International Health Foundation (IHF) schedule while the other group received 3 hCG injections at intervals of 7-10 days (1-3 years = 3 x 1,000 IU; 3-6 years = 3 x 1,500 IU; 6-10 years = 3 x 3,000 IU; 10-13 years = 3 x 5,000 IU hCG). Results from both schedules were comparable during the first and also during a second treatment period at the age of 1-3 and 3-6 years; between 6 and 13 years of age the IHF schedule was more successful.
