[Steroidogenesis in patients with various adrenal and gonadal dysfunctions].

The paper deals with the results of investigations of steroidogenesis in patients with ACTH-dependent Icenko-Cushing's disease (CD), congenital adrenal hyperplasia (CAH), and stromal hyperthecosis (SH). Along with the traditional methods of evaluating the activity of steroid-producing glands (diurnal variations of steroid hormones and their precursors in the peripheral blood, their mean daily concentrations), the authors used a simultaneous selective catheterization technique for adrenal and ovarian veins. No pronounced circadian rhythms were revealed in most of the steroids measured in patients with CD. A sharp rise in the mean daily serum 17-hydroxypregnenolone concentrations coupled with the steady-state 17-hydroxyprogesterone level is considered to reflect the activation of steroid biosynthesis via the delta 5-pathway. CD patients were demonstrated to exhibit great differences in the daily pattern of aldosterone secretion depending on the development of essential hypertension. CAH patients had high 17-hydroxypregnenolone and 17-hydroxyprogesterone levels. Like 17-hydroxyprogesterone, 17-hydroxypregnenolone may be used as a diagnostic marker of CAH. Hyperandrogenism in this disorder was largely due to enhanced adrenal production of testosterone and delta 5 androgens DHA and DHA-S. Unlike CAH, SH was not associated with excessive secretion of either DHA or DHA-S, but the ovaries of these patients released greater amounts of testosterone, which led to hyperandrogenism.

[Comparative characteristics of steroidogenesis and its biorhythm in the hyperfunctioning adrenal cortex].

The paper summarizes the results od several studies of the daily rhythms of steroid hormones in patients with ACTH-dependent Itsenko-Cushing's disease (CD) and congenital adrenal hyperplasia (late-onset forms) (CAH). Normal daily rhythms of adrenal C21- and C19-steroids and ACTH were observed in 23.5% of CD patients. CAH patients had the marked daily rhythms of adrenal androgens and testosterone which were typical of those of cortisole. The ratios of steroid hormones to its precursors provide evidence for enhanced activities of 17-, 11 beta- and 18-hydroxylases in CD patients and normal enzymatic activities in CAH patients, whereas 21-hydroxylase being an exception.

[Interconnection of interleukin-1 and glucocorticoid hormones in regulating the immune response].

IL-1 and hydrocortisone have an opposite and competitive effect on the lymphocyte proliferation in cell culture. In mice recombinant human IL-1 beta dose-dependently induces high level serum corticosterone. Injection of IL-1 beta or hydrocortisone led to a similar alterations in the thymus: reduction of cellularity and decrease in spontaneous thymocyte proliferation. On the other hand in the same doses IL-1 beta stimulates spleen cell proliferation. IL-2 production and enhances serum antibody titres and the number of antibody producing cells in spleen of mice immunized with T-dependent antigen.

[Sex hormones in dyslipoproteinemias and disorders of arterial blood pressure in boys 11-12 years of age].

A survey of boys with hypercholesterolemia, hypertriglyceridemia, hypo-alpha-cholesterolemia, a combination of hypo-alpha-cholesterolemia and hypertriglyceridemia, and hyperalphacholesterolemia, and control subjects from two Moscow districts demonstrated significantly elevated plasma testosterone and estradiol values in boys with hypo-alphacholesterolemia. Increased estradiol was also found in boys with hypercholesterolemia and hyperalphacholesterolemia. An inverse relationship between plasma testosterone level and high density lipoprotein (HDLP) cholesterol, and a direct relationship between the estradiol/testosterone ratio and HDLP cholesterol were established. Individuals with high testosterone and estradiol levels had elevated systolic arterial BP, and those with high systolic arterial BP showed increased testosterone and estradiol levels. It is suggested that reduced high density lipoprotein cholesterol and elevated systolic arterial BP in boys with increased testosterone levels may be regarded as early signs of developing hypo-alphacholesterolemia in combination with arterial hypertension.

[Functional activity of the hypophysis and gonads in women with hypertension].

An attempt is made to demonstrate a relationship between changes in arterial blood pressure and sex steroids, gonadotrophins and mineralocorticoids in 30- to 55-year-old females with essential hypertension, employed in textile industry in Cheboksary. Climacteric and menopausal patterns of sex steroid and gonadotrophin secretion are discussed. A definite relationship between essential hypertension, elevated estradiol and testosterone and lowered gonadotrophins is demonstrated. It is suggested that climacteric and menopausal blood pressure disorders may be related to changes in gonadal and adrenal activity.

[Effect of plasmapheresis on plasma hormone levels in patients with hereditary hypercholesterolemia].

It is shown that plasmapheresis in the volume of 30-46 ml/kg using 2997 cellular separator (IBM) does not significantly reduce testosterone, estradiol, aldosterone, parathyroid hormone and calcitonin concentrations. The initially high level of insulin and C-peptide was reduced, angiotensin-I concentration lowered and somatotropic hormone concentration doubled. It is suggested that plasmapheresis stimulates lipolytic and anabolic processes in the organism.

[Effect of ACTH on the corticosteroid and progesterone levels in female hamadryas baboons in relation to the menstrual cycle phase].

The effect of exogenous ACTH on the endogenous function of the steroid-producing glands has been investigated in hamadryas baboon females depending on the level of sex hormones in the body. The content of corticosteroids and progesterone in peripheral blood plasma was measured by radioimmunoassay. It has been established that the time course of changes in the adrenal gland response to administration of a standard ACTH dose was of the same type in the different menstrual cycle phases. The inhibitory effect of ACTH on progesterone secretion in ovarian corpus luteum being active has been revealed.

[Neurophysiological analysis of the development of endocrine and hypertensive reactions in long-term emotional stress].

A study was made of the effect of chronic emotional stress on the formation of hypertension in animals. This was shown to be related to dynamic changes in the function of the CNS, particularly in the hypothalamic apparatus of the neuroendocrine control. The above changes played a role in the formation of hypertensive vascular reactions accompanied by a high hormonal secretion of the adrenal cortex and thyroid. During stabilization of high arterial blood pressure at the late stages of the "after-effect", the hormonal secretion returns to normal.

[Diurnal and seasonal rhythms of androgen content of the plasma in Macaca mulatta].

Circadian and seasonal rhythms of testosterone-, 5 alpha-dihydrotestosterone and androstendione content were studied in the blood plasma of adult male macaca rhesus by radioimmunoassay. The circadian changes in androgen concentration depended on the seasonal and experimental conditions. Circadian variations of androgen concentration had a definite, significant character in the animals, which were previously adapted to experimental conditions. Minimum testosterone and 5 alpha-dihydrotestosterone content was noted at 18 o'clock and maximum at 21 to 24 o'clock. Circadian rhythm of androstendione content was characterized by a decrease in the evening, by a rise within the night and by the maximum value at 6 to 9 o'clock. Significant seasonal changes were observed in mean daily testosterone- and 5 alpha-dihydrotestosterone-, but hot in androstendione content. High levels of both steroids were recorded in autumn and winter, being lower in spring and the lowest in summer.

[Pattern of steroid hormones in patients with acute leukemia].

The cortisol metabolism was studied in patients with different variants of acute leukemia. In total amount of C21-steroid excreted with urine the portion of cortisol was decreased, the increased cortisol conversion into 20-dihydrometabolites and the decreased cortisol portion, conversed through the stage of cortisone into tetrahydrocortisone, were observed. Also, disturbances in steroid conjugation with glucuronic and sulfuric acids and the increased progesterone concentration in peripheral blood plasma were noted.

[Comparative characterization of radioimmunologic and non-isotopic methods of hormonal analysis].

Measurements of 8 hormones (prolactin, FSH, LH, thyrotropic hormone--TTH, estradiol, hydrocortisone, thyroxine, and triiodothyronine--T3) were carried out by the Amerleit system and radioimmunoassay. The hormones were measured in the same blood plasma samples of patients with various endocrine conditions, admitted for examinations to the Endocrinology Center of the Russian Academy of Medical Sciences. Employment of the Amerleit system for hormonal analysis helps obtain the results compatible with the results of RIA for the majority of the measured hormones. The methods for measuring LH, estradiol and T3 are to be improved. The Amerleit system is highly productive and permits getting the results within 3 hrs. A trained laboratory assistant can measure the hormonal levels in 120-160 blood samples within a day. High efficacy of the technology is indispensable in realization of the screening programs. The absence of radioactivity, safety, high productivity, reliability, accuracy and good storage of the kits (up to 6-8 months) are the principal advantages of the Amerleit system over the existent radioimmunologic systems.
