[Features of angioplasty and stenting in atherosclerotic stenting of coronary artery occlusions].

The authors analyze results of coronary angioplasty in 257 patients with ischemic heart disease who had occlusions of the coronary arteries. Clinical, angiographic and other findings showing the duration of having the occlusions of the coronary artery are described in detail. The details of the steps of the technique of the endovascular intervention are considered including the most difficult step--passing through the occluded portion of the artery. Great attention is given by the authors to the criteria of using the correct instruments (conducting catheter, conductors, balloon-catheters) during the operation. Good results were obtained in 80.2% of the patients after coronary angioplasty. Stenting of the coronary arteries was performed in 65 patients (25.3%).

[Diagnosis and surgical treatment of pathologic tortuosity of carotid and vertebral arteries].

The article summarizes an experience with 179 surgical interventions on patients with pathological tortuosity of brachiocephalic arteries. The optimal described diagnostic algorithm for this pathology included ultrasonic dopplerography, transcranial dopplerography, angiography or magnetic resonance tomography in the angiography regimen. Clinical and angiographic indications to surgical treatment of this pathology were determined. The operative treatment was carried on in patients with neurological symptoms having kinks of brachiocephalic arteries. The intraoperative strategy depending on the type and localization of the disease are analyzed. Stabile regression of manifestations of vascular cerebral insufficiency after surgical correction of the pathological tortuosity of the brachiocephalic arteries was achieved in 76% of the patients.

[Characteristics of lymph flow in patients with post-mastectomy syndrome, based on lymphoscintigraphy data].

Lymphoscintigraphy allows to get intravital contrast of functionally valuable lymph collectors of the extremity. A comparative analysis was made of the state of lymph flow in healthy subjects, in patients with breast cancer and in patients with the completed cycle of treatment of breast cancer having postmastectomy edema and without it. The investigation was performed in 280 extremities. Principal variants of the normal lymph flow (collector, diffuse, nodal) were marked out. Possible compensatory mechanisms ameliorating the draining function of the extremity were followed in patients with blocked lymph outflow. Programs of prognosis and treatment of patients with preclinical stage of postmastectomy edema are outlined.

[Significance of invasive monitoring of central hemodynamics for the timely correction of their disturbances in operations on the abdominal aorta].

Central hemodynamics was investigated with the thermistor Swan-Hanz catheter in 50 patients with the Leriche syndrome in whom aorto-femoral bifurcation shunting was performed. It was found that hemodynamic profiles of arterial hypotonia appearing at different stages of the operation can be expressed as hypovolemia, lower peripheral resistance of the vessels or the syndrome of low cardiac ejection. The therapeutic strategy (inotropic maintenance, vasodilators, infusion therapy) should be differentiated depending on the hemodynamic profile of hypotonia.

[Application of different textile vascular grafts in the aorto-femoral location and ways for their improvement].

A comparative analysis of 220 reconstructions of the aortofemoral segment with the application of vascular prostheses of firms "Sever" and "Vascutek" was made. It was established that in patients with obvious anemia and hemodilution (hematocrit less than 25%, hemoglobin less that 80 g/l) an important condition for less total blood loss was to impart the zero surgical porosity to the graft wall at the preoperative stage. However, the biological impregnation used for these purposes reinforces the inflammatory component in the implantation of the vascular graft. The authors propose to use a more inert polytetrafluoroethylene film to impart the zero surgical porosity to the graft wall.

[Modified laparoscopic fundoplication in the treatment of gastroesophageal reflux].

The results of laparoscopic fundoplication in patients with the gastroesophageal reflux disease are described. Operations were performed by the classical Dor method (19 patients) and by a modified method (35 patients). An analysis of results of the operation has shown effectiveness and safety of using the modified method of laparoscopic fundoplication.

[Complications after angioplasty and stenting of coronary arteries in ischemic heart disease patients].

The authors have made an analysis of most frequent complications of angioplasty and stenting of coronary arteries in 1900 patients with various forms of ischemic heart disease. The mechanisms, pathomorphology, angiographic and clinical manifestations of such complications as dissection of the coronary artery intima, acute occlusion of the coronary artery, "no-reflow" syndrome, perforation and rupture of the coronary artery, acute thrombosis in the stent area and other complications are considered in detail. Special medical strategy, both therapeutic and surgical, is proposed for each case. The authors propose a number of prophylactic measures allowing to avoid the complications described or to minimize them. Intraoperative lethality in this group of patients was 0.5% (10 patients), during the first week after endovascular intervention 26 patients died (1.4%).

[Dyslipoproteinemia and prognosis of the course of ischemic heart disease after coronary bypass].

The article deals with the observation of 88 patients with ischemic heart disease, 62 of them had coexistent dyslipoproteidemia. More pronounced atherosclerotic lesions of the coronary artery were found to develop in patients with dyslipoproteidemia influencing the degree of ischemic heart disease before coronary bypass. Within the period of 10 years after operation dyslipoproteidemia patients have 3.5 times higher risk of the development of myocardial infarction, more than 2 times higher risk of reappearance of stenocardia and death from cardiovascular diseases. Patients with dyslipoproteidemia have 2 times higher probability of generalization of the atherosclerotic process in patients with is chemic heart disease at later terms after coronary bypass.

[Effects of the dual chamber electrocardiostimulation on the systolic and diastolic function of the myocardium].

An assessment of the indicators of the systolic and diastolic functions of the left ventricle was made in 52 patients against the background of electrostimulation with a dual chamber electro-cardiostimulator with different atrioventricular delays (185, 125 and 79 ms). Different variants of the reactions to electrostimulation with a shorter atrioventricular delay were detected: the positive dynamics of the diastolic indicators was registered in the hypertrophic type of the diastolic dysfunction, aggravation of the diastolic disorders--in the restrictive type.

[Effectiveness and safety of catheter ablation of the heart conduction tracts in tachyarrhythmia].

An analysis of 490 operations of radiofrequency catheter destruction in 442 patients aged from 5 to 83 years has been made. 284 patients (64.3%) had congenital and "idiopathic" character of rhythm disturbances. The greater part of operations--459 (93.7%)--were performed on patients with supraventricular tachycardias, 31 operations (6.3%)--on patients with ventricular arrhythmias. Efficacy of operations on patients with congenital diseases of the conducting system of the heart was 94.8-100%, frequency of recurrences being 3.3-8%. Successful rhythm correction in 78% of patients with atrial flutter was due to the risk of appearance of recurrences and development of atrial fibrillation at late periods. Creation of atrio-ventricular blockade abolishes tachysystole in patients with ciliary arrhythmia, but life quality depends on the adequate system of stimulation. The frequency of complications after the catheter destruction is associated with the complex character of the intervention and increases when the manipulations are to be performed in the left chambers of the heart.

[Angioplasty for atherosclerotic lesions of the left main coronary artery].

The authors analyze results of coronary angioplasty and stenting in patients with ischemic heart disease with a lesion of the left main coronary artery. Injuries of the left main coronary artery took place in 14 (0.74%) out of 1900 patients after coronary angioplasty. A detailed analysis of the operation technique, indications and contraindications for endovascular intervention is given. The results of balloon angioplasty with and without the following stenting are compared.

[Surgical treatment of recurrent goiter].

The authors have analyzed and generalized their 20-years experience with surgical treatment of 247 patients with recurrent goiter. Among them 48 patients had recurrent toxic goiter and 199 patients had nontoxic goiter. Postoperative complications developed in 8.3% of the patients with recurrent toxic goiter and in 6% of the patients with nontoxic goiter. The overall postoperative lethality was 1.2%. Operative treatment is recommended by the authors for recurrent toxic and nontoxic goiter, conservative treatment with radioactive iodine is thought to be expedient for a repeated recurrence. For preventing recurrences of toxic and nontoxic goiter after the first operation the administration of small does of the thyroid hormones is considered to be expedient during 1.5-2 months.
