[Gamma-camera scintigraphy and radioimmunological study of the hormones of the hypophyseal-gonadal axis to evaluate testicular function in patients with hypogonadism and infertility].

Gamma-camera and radioimmunological study of hormones of hypophyseal-gonadal axis were carried out on 34 patients with hypogonadism and infertility after successfully performed orchidopexy. The results from this study showed that gamma-camera scintigraphy was exact and noninvasive method for determining the structure and function of testes after operative intervention. The results of the established hormonal deviations in the secretion of hormones of hypophyseal-gonadal axis are sure and early criterium for diagnosis of infertility in these patients. They could indicate the therapeutic management in these patients.

[The concentration of carcinoembryonic antigen and carbohydrate antigen Ca-19-9 in the blood serum in cancerous diseases of the gastrointestinal tract].

The results of comparative investigation of 266 cancer patients, 44 patients with benign G. I. tract diseases and 66 healthy subjects using 2 tumor markers (CEA and Ca-19-9) showed the diagnostic value of both methods, not exceeding 38-42%. The most informative were the data on patients with pancreatic and colorectal cancers as well as in patients with detected metastases. Significant differences in the sensitivity, specificity and diagnostic informative value between the two markers were undetectable.

[A patient with psoriatic arthritis and secondary amyloidosis].

A case of a man suffering from psoriasis vulgaris is presented. The onset of the disease was at the age of 5. From the age of 14 all joints were gradually affected by the type of severe, symmetrical psoriatic arthritis which led to aseptic necrosis of the hip joints. In the course of the disease, from 1988 on, proteinuria and nephrotic syndrome were found. The renal biopsy proved amyloidosis which is considered secondary, developed on the basis of psoriatic arthritis. The severe psoriatic arthritis with especially emphasized clinical activity is in this case the pathologic antigenic stimulus for the amyloid formation. The clinical course and the histologic findings from the renal biopsy point to serious structural disorders of the renal parenchyma which determine the progressing tendency toward chronic renal failure.

[Changes of laboratory hematologic indices in patients with chronic glomerulopathies following treatment with superhigh doses of cyclophosphamide].

41 patients with active glomerulopathies treated with super high doses of Cyclophosphamide were studied. The following indices were followed up dynamically: hemoglobin concentration, number and morphology of erythrocytes, number and morphology of leucocytes and differential count. Hemoglobin concentration and erythrocyte count remain in the reference ranges in most of the patients. They fall more considerably compared to the initial values between the 90-120 day after the treatment. This fall improves spontaneously and does not require discontinuance of the treatment. The single application of super high dose of Cyclophosphamide does not influence significantly the leucopoiesis. Because of the parallel application of glucocorticosteroids no fall of the leucocyte count is observed and they remain in their reference ranges.

[Hormonal aspects in the diagnosis of hirsutism].

Hormones of hypophyseal-adrenal axes were determined in 135 girls with hirsutism. On the basis of changes in the hormonal levels it was established that the syndrome of Stein-Leventhal in 62 girls (54% of the patients) was dependent on luteinizing hormone (LH) and was characterized by an increased index of LH/FSH over 3.0 as well as by an increased level of testosterone; hirsutism in 73 girls (67% of the patients) was dependent on adrenocorticotropic hormone (ACTH) and was characterized by an increased level of ACTH and cortisol secretion; in the remaining patients hirsutism was a combination of both forms of dependence on LH and ACTH and an increase of hormones of both axes was present or was idiopathic with normal hormonal plasma levels, but with increased skin conversion and metabolic testosterone clearance.

[Comparative study of parathyroid function by serum parathyroid hormone, total adenosine cyclic monophosphate in the urine and its nephrogenic fraction].

The immune heterogeneity of the parathyroid hormone and the possibility of its peripheral activation by 1.25 (OH)2 vitamin D3 hinders the precise evaluation of the functional state of parathyroid glands by direct determination of the hormone in the serum and also the creation of a suppressive test similar to the suppressive tests applied in the study of other hormones. A study was carried out which proves that the indirect examination of the parathyroid function by determining the cyclic adenosine monophosphate in urine and its nephrogenic fraction (i.e. by the effects of the active parathyroid hormone) is of greater importance for clinical practice than the direct determination of the parathyroid hormone in peripheral blood. This is the method of choice in the diagnosis of the subclinical variant of primary normocalcemic hyperparathyroidism.

[Serum osteocalcin level as a marker of the functional state of osteoblasts after oral calcium tolerance test].

The results are presented of an oral calcium tolerance test with 1,000 mg calcium in 20 patients with recurrent renal calcium calculosis, a woman with primary hyperparathyroidism and incipient renal failure (serum creatinine 1.8 mg%), creatinine clearance 55 ml/min) and 9 healthy persons as controls. The serum osteocalcin level was determined before and after the oral test. The results show that the serum osteocalcin level alone is of no differential diagnostic value for differentiation of the various types of hypercalciuria in patients with recurrent renal calcium calculosis. As a marker of osteoblasts functional state however the determination of serum osteocalcin level is of great importance for the early diagnosis of osteoporosis. In 3 patients with renal hypercalciuria, often leading to general osteoporosis, an acute rise of serum osteocalcin level was found after the oral calcium tolerance test. High osteocalcin level was also found in the patient with primary hyperparathyroidism and incipient renal failure.

[Parathyroid function in recurrent calcium nephrolithiasis and chronic kidney failure before and after oral calcium loading].

A case is presented of a female patient with recurrent calcium renal calculosis, chronic calculous pyelonephritis and chronic renal failure II degree to whom an oral calcium tolerance test was performed. The data of the test are characteristic for parathyroid hyperfunction. The possible cause of these changed is discussed. The conclusion is reached that the oral calcium tolerance test is of differential diagnostic value in patients with initial renal failure, too.

[Clinical, laboratory and morphological characteristics of mesangioproliferative glomerulonephritis].

The mesangioproliferative glomerulonephritis (MPGN) is the most frequent morphological type of primary glomerulonephritis and it was found in 42.7% of the patients studied. The MPGN is not a single nosologic entity which is proved by the immunofluorescent findings, clinical and laboratory characteristics. Several immunofluorescent types with characteristic clinico-laboratory constellation and different etiopathogenesis could be defined. The most clearly defined types of MPGN are those with leading IgA and IgM precipitates. The similar immunofluorescent findings, clinico-laboratory characteristics, course and susceptibility to symptomatic and pathogenetic treatment in MPGN with leading IgG and C3 precipitates lead to the suggestion that there may exist different "phase" states in the course of the different types of MPGN. In spite of some characteristic differences in the symptomatology and course of the different types of MPGN their differentiation is possible only by kidney biopsy.

Subunits of luteinizing hormone-human chorionic gonadotropin receptor from bovine corpora lutea.

A batch of 24 mg of luteinizing hormone-human chorionic gonadotropin (LH-hCG) receptor was isolated from bovine corpora lutea. The LH-hCG receptor showed specific binding with hCG. The receptor-hCG complex activated the regulatory Ns protein isolated from rabbit liver, which in turn stimulated adenylate cyclase to convert ATP into cAMP in vitro, attesting to the biological activity of the purified LH-hCG receptor. The LH-hCG receptor was treated with 2% sodium dodecyl sulfate (SDS) to prepare the molecular weight (Mr) 280K dimer and with 50 mM dithiothreitol (DTT) to prepare the Mr 120K monomer and subunits of Mr 85K and 38K. Oligomers of various molecular weights were recovered from gel filtration columns due to the reassociation of disulfide bonds between monomers and subunits. Hence, the receptor monomer was also dissociated into subunits of Mr 85K and 38K by reduction of -S-S-bonds with 50 mM DTT in 2% SDS and alkylation of sulfhydryl groups in the presence of 100 mM N-ethyl-maleimide. The subunits were separated by gel filtration through columns of Ultrogel AcA-44 and Sephadex G-75. The yields of the purified alkylated subunits of Mr 85K and 38K were 1.8 and 1.5 mg, respectively. Each subunit migrated as a single entity in SDS-polyacrylamide gel electrophoresis. The monomer of the receptor of Mr 120K showed specific binding with 125I-hCG, suggesting it to be the minimum molecular weight functional unit of the receptor. The Mr 85K and 38K subunits bound 125I-hCG, which could not be displaced with unlabeled hCG.(ABSTRACT TRUNCATED AT 250 WORDS)

[Potentials for studying aldosterone by using the Adrenoprobe (a preliminary report)].

A preliminary communication is presented about the results, obtained from aldosterone determination via the Adrenoprobe. The method is stressed to deserve attention since it is rapid, easy to perform and the results are obtained immediately after the investigation. On the other hand, the discrepancies between the results in some patients, obtained radioimmunologically and via the Adrenoprobe, revealed that further parallel investigations would be necessary (in order to obtain more data on the accuracy of aldosterone determination via the Adrenoprobe.

[Parathyroid hormone values in patients on chronic dialysis treatment].

It has been stressed that the values of radioimmunologically determined PTH in patients on chronic dialysis treatment are considerably high and the duration of dialysis treatment has no marked effect on them. Relatively good correlation dependence between the serum levels of calcium and PTH is established only in the presence of hypocalcemia. In all other cases the correlation dependence between PTH and calcium as well as that with phosphorus and alkaline phosphatase is negligible so that their serum values cannot be used to judge the level of PTH. The dynamic follow up of the PTH values is emphasized to be performed with one and the same kit, hence avoiding the methodical errors.
