Two cases are described with paraneoplastic amyloidosis, advancing in patients with a basic etiological moment--myeloma and hypernephroma. The clinical picture, in both patients was dominated by the nephrotic syndrome and rapidly progressing renal insufficiency, leading to the fatal end within a very short term. That term in the patient with hypernephroma was only several months after making the diagnosis, and in the patient with myeloma--up to two years. Renal punch biopsy was performed to both patients during their life time.

Experiments are carried out to study the effect of isotheoline (IST) on the secretion of prolactin (PRL), somatotrophic hormone (STH) and testosterone, using cats and Wistar albino rats of both sexes. PRL, STH and testosterone using cats and wistar albino rats of both sexes. PRL, STH and testosterone were determined radioimmunologically at the 10th, 60th and 120th min after the administration of IST in doses of 0.5 to 5 mg/kg. Comparative experiments were carried out with bromocryptine and glaucine--1 mg/kg i.p., as well as experiments involving the administration of IST on the background of the dopaminergic antagonists haloperidol and pymoside--1 mg/kg i.p. The results obtained show that in doses not exceeding 1 mg/ks IST increases the plasma level of testosterone and, similar to bromocryptine, reduces the plasma level of PRL and increases that of STH. These effects are less pronounced for the 5 mg/kg dose and they are antagonized by the dopaminergic antagonists used. It is assumed that IST, similar to bromocryptine, achieves the described effects through selective stimulation of the D2 dopaminergic receptors involved in neuro-endocrine mechanisms responsible for the control and release of the hormones studied.
The determination of the endocrine state of the patients with chronic renal insufficiency is of significance because of the endocrine sexual disorders often observed in those patients. According to literature data, hyperprolactinemia in combination with gonad dysfunction is often observed in the sera of those patients. Hyperprolactinemia was established, with the present study, in the examined 47 patients (18 females and 29 males) with chronic renal insufficiency, the normal physiological difference in prolactin level depending on sex, being eliminated in those patients. The hyperprolactinemia in the patients examined, as compared with the prolactin level in a control group of healthy subjects (10 females and 12 males) is statistically significant (p less than 0.01). Very likely that hyperprolactinemia in the patients with chronic renal insufficiency is complexly conditioned by the disturbed regulation and disturbed degradation and elimination.
The values of PGf2 alpha were studied in 20 renal patients with renal hypertension, with and without chronic renal insufficiency via a radioimmunologic method. A control group of 10 healthy volunteers wer used without data from arterial hypertension. Values (672.0 +/- 99.5 pg/ml), being, with statistically significant difference, increased as compared with the healthy volunteers (347.13 +/- 49.9 pg/ml) were found in renal patients with chronic renal insufficiency. With the advancement of CRI in patients with renal hypertension, PG concentration was also increased (505.5 +/- 77.6 pg/ml) but it was not significant as in the patients without CRI. The elevated values of PGF2+ alpha suggest their participation in the pathogenesis of renal hypertension.
