Imaging of splenic rupture.

Results from imaging of 30 splenic ruptures were analyzed. Radiography gave abnormal findings in 7/30 thoracic examinations and 8/19 plain abdominal films. Ultrasonography rarely revealed focal splenic lesions at primary examination, though intraperitoneal fluid was commonly found. Subsequently more focal changes were discovered, and none of the 19 examined patients ultimately had a normal ultrasonogram. There were focal splenic lesions in ten patients, subcapsular fluid in four and intraperitoneal fluid in 14. Computed tomography revealed abnormality in all ten studies patients--irregular splenic parenchyma in six, subcapsular fluid accumulation in five and intraperitoneal fluid in six cases.

Sensitivity of ultrasonography for detecting gallstones.

Previous studies have overestimated the accuracy of ultrasonography for the diagnosis of gallstones by neglecting the systematic checking of false negative findings. In the present series only 80% of 331 cases of gallstones seen at autopsy had been detected by ultrasonography. The most frequent reason for a false negative finding was the small size of the gallstones.

Charcot-Marie-Tooth disease in northern Finland.

This study presents the main clinical and electrophysiological features of patients with Charcot-Marie-Tooth (CMT) disease or hereditary motor and sensory neuropathy (HMSN) in Northern Finland diagnosed during the period 1969-1983. The group consists of 40 patients aged between 5 and 74 years. A positive family history suggesting a dominant inheritance was seen in 23 patients--the rest were classified as sporadic. According to median or sural nerve conduction velocities the disease was graded as demyelinating (HMSN type I) and neuronal (HMSN type II). A clear tendency towards an earlier onset was seen in type I cases compared to those in type II. Both clinically and neurophysiologically the lower arms were more severely affected in type I than in type II. Muscular atrophy of the legs occurred more often in type I than in type II. Electromyographic abnormalities were characteristically neurogenic, though in some advanced cases small motor unit potentials could also be seen. In addition 2 cases originally diagnosed as CMT proved to be distal myopathy on electromyography (EMG). According to electrodiagnostic criteria, patients in the same family could belong to different subgroups of CMT suggesting that the present use of conduction velocities as the discriminating factor between the groups may be unsound.

Airborne transmission of tularemia in farmers.

In a tularemia epidemic during 1982 in northern Finland, 53 patients showed no peripheral portal of entry for infection or associated lymphadenopathy. Respiratory symptoms were observed in 72% of the patients. 26/38 cases had abnormal chest films. Hilar adenopathy was the most common finding (36%). Four patients did not receive antibiotics; 43 received tetracyclines, 5 streptomycin and 1 cefuroxime and amoxycillin. All patients recovered. 50 patients acquired the infection during common farming activities, such as making fresh hay with a hay-cutter, handling dry hay, threshing, etc. Thus, airborne transmission may be an important source of infection in normal farming activities in endemic areas of tularemia.

Caval occlusion caused by a metastasis of adrenal carcinoma.

An intracaval metastasis of adrenal carcinoma in a young woman is described. Ultrasonography visualised the metastasis effectively. Cavography showed the lower margin of the metastasis occluding the vena cava, but not its cranial extension.

Impact of new imaging techniques on survival in cancer of the head of the pancreas and the periampullary region.

The impact of new imaging techniques on diagnostic delay and on survival in carcinoma of the head of the pancreas and the periampullary region was investigated. The study comprised 67 patients treated before (1968-1975) and 82 treated after (1979-1981) the introduction of new imaging techniques at Oulu University Central Hospital. The symptomatology, diagnostic delay and survival rates were identical in both groups, but the number of patients per year increased significantly during the latter period. The study indicated that the new imaging techniques, despite their increased diagnostic yield, did not influence diagnostic delay or survival in pancreatic and periampullary carcinoma.

Sensitivity of single and double contrast barium enema in the detection of colorectal carcinoma.

The preoperative barium enema of the 188 colorectal carcinoma patients operated at the Oulu University Central Hospital (Finland) during 1977-1982 were examined retrospectively. Altogether 112 single contrast studies and 87 double contrast studies had been made on these patients. The single contrast barium enemas had resulted in a correct diagnosis of colorectal carcinoma in 93 cases (sensitivity 83%). The correct diagnosis in the double contrast studies numbered 71 (sensitivity 82%). Most of the overlooked carcinomas were located in the caecum, in the sigmoid or the rectum. Most of the errors made in the single contrast studies were due to detection errors and poor evacuation. The most common failures in double contrast enemas were detection errors and nonvisualisation of the sigmoid. The authors recommend use of the double contrast technique and suggest that the two methods of barium enema be used to complement each other. A false negative diagnosis delayed the operation of the colorectal carcinoma patients by 2.2 months (median).

Ultrasonography in the diagnosis of abdominal aortic aneurysms.

An abdominal aortic aneurysm was detected in 77 patients among 16 488 abdominal ultrasonographies (US) performed in 1978-1983. In 62 cases the US finding was confirmed by operation, autopsy or other imaging method, and the US finding proved true in 60 cases. The length and diameter were accurately estimated and accompanying thrombosis reliably visualised. An error rate of 24% was observed in assessing the relation of the aneurysm to the renal arteries. US is recommended as the primary imaging mode in suspected abdominal aortic aneurysm. Screening of the abdominal aorta is recommended in elderly patients with abdominal pain and/or referred for abdominal US.

Sensitivity of ultrasonography in the demonstration of common bile duct stones and its ranking in comparison with intravenous cholangiography and endoscopic retrograde cholangiopancreatography.

A series of 75 patients with surgically verified common bile duct (CBD) stones were re-examined in retrospect, in order to ascertain the sensitivity of ultrasonography (US) in the detection of CBD. The sensitivity of US turned out to be 19%. US detected bile duct dilatation in 58% and all the CBD stones detected or suspected were seen in cases with bile duct dilatation. The sensitivity of intravenous cholangiography (IVC) proved to be 24%. Endoscopic retrograde cholangiopancreatography (ERCP), however, was found to be by far the most sensitive method, since it demonstrated 84% of the CBD stones.

Cranio-caudal movements of the liver, pancreas and kidneys in respiration.

The range of movement of the liver, pancreas and kidneys in maximum and normal respiration, and the ability to suspend respiration repeatedly leaving the organs in exactly the same position with the aid of given instructions or a combination of instructions and a simple tool were investigated. The range of movement of the most mobile organ (liver) was 5.5 cm during maximum, 2.5 cm during normal, 0.9 cm during suspended respiration, and 0.2 cm during suspended respiration with a bar placed above the abdomen.

Sonographic and cholecystographic diagnosis of cholesterolosis of the gallbladder.

Cholesterolosis is difficult to detect preoperatively. Ultrasonography can reveal it as polypoid densities usually without shadows. Cholecystography reveals cholesterolosis as contrast medium defects attached to the gallbladder wall. In our material of 27 patients with cholesterolosis (operatively verified cases) ultrasonography was superior to cholecystography in detecting cholesterolosis.

The significance of preoperative shoulder arthrography in painful arc patients.

Preoperative shoulder arthrography was performed on 34 patients with painful arc syndromes. Rotator cuff rupture was diagnosed in 20 cases. Out of these the arthrography correctly showed rupture in 17 cases, while in one case rupture was not seen and two examinations were unsuccessful. Rotator cuff rupture was not found in 14 cases at operation. Of these, arthrography had been negative in eight cases and in six cases the rupture had merely been suspected. Sensitivity of shoulder arthrography in showing rotator cuff rupture was thus 85%, specificity 57% and accuracy 74%. The biceps tendon was intact in all cases with rotator cuff rupture. So-called "bicipital leakage" was seen in four cases, but its significance remains unclear.

Angiographic diagnosis at gastrointestinal hemorrhage.

The position of angiography in the diagnosis of gastrointestinal hemorrhage is discussed on the basis of a series of 37 patients. In this series, there were angiographic signs of bleeding in 13 cases (35%). The largest group consisted of angiodysplasias. In 24 cases, angiography revealed no signs of bleeding. The cause of symptoms was discovered later in 17 of these cases. Hemorrhagic gastritis and ventricular ulcer were the main causes of bleeding in the latter group.
