Ultrasonographic diagnosis of pancreatic carcinoma supplemented with CT or ERCP.

The practicability of ultrasonography as the primary imaging method for pancreatic carcinoma was studied in a series of 64 patients with pancreatic carcinoma examined and operated on more than 2 years previously, and CT and ERCP were compared for their expedience in the further examination of uncertain sonographic findings. Ultrasound revealed 42 carcinomas definitely and 8 carcinomas uncertainly (78%) and further indicated dilatation of bile and/or pancreatic ducts in 10 patients. Three sonographic examinations failed, and one resulted in a false negative finding (error of interpretation). ERCP proved to be a little more informative than CT in the further examinations.

Ultrasonography of gastric tumours.

The visualization of the different parts of the stomach in ultrasonography was ascertained, and the information obtained by upper abdominal ultrasonography of patients with gastric tumour was evaluated in a retrospectively collected series. Normally, the whole stomach is not visualized in ultrasonography. A sonography examination can never exclude the possibility of gastric carcinoma, but the carcinoma is often detectable by ultrasound. Ultrasonography also provides some idea of the extent of infiltration. Those performing epigastric ultrasonographies should attach increasing attention to the stomach.

Echo-free and relatively echo-free "renal sinus lipomatosis".

Eleven echo-free and 37 relatively echo-free peripelvic lesions, primarily interpreted as sinus lipomatosis by experienced sonologists, were examined using CT. Eight echo-free lesions proved to be cysts and only three "fibrous fat". Hypodense peripelvic fat accumulations were not totally echo-free in any of the cases, but tended to show up weak echos.

Ultrasonography of carcinoma of the gallbladder. The images and differential diagnostic problems.

Ultrasonography in the diagnosis of carcinoma of the gallbladder was analysed in a series of 5100 patients, who had a grey-scale sonography of the gallbladder more than 2 years before analysis. Carcinoma has been diagnosed in 12 of the patients. Sonography revealed 8 of the carcinomas. In addition one tumour was diagnosed, which was not considered to originate from the gallbladder. Two patients had carcinoma in situ. The examination of one carcinoma patient was a technical failure. There were 9 false positive findings. Of these 9 patients, one had a metastasis, 5 had a lump of sludge, 3 had a wall thickening caused by chronic cholecystitis and one had an adenomyomatosis.

Real-time ultrasonography as the primary imaging method in suspect renal pathology of adults.

In the material of 253 successive patients real-time ultrasound was used instead of urography as the primary imaging method of renal diseases. Ultrasound detected a tumour more reliably and more specifically than urography and detected hydronephrosis with sufficient reliability. In cases of pyelonephritis urography was more informative than ultrasound. In 'diffuse' parenchymal diseases both methods were quite unreliable.

Ergotamine abuse: results of ergotamine discontinuation, with special reference to the plasma concentrations.

Twenty-three patients suffering from continuous headache linked with habitual daily use of ergotamine tartrate were studied. Their headaches were classified clinically, and possible side effects of ergotamine medication, plasma levels of ergotamine, and occurrence of withdrawal symptoms after discontinuation of drug abuse were recorded. Seventeen of the patients were clinically diagnosed as suffering from "ergotamine headache", and seven of them complained of coldness in the extremities. Plasma ergotamine levels were measured by using a radioimmunoassay. In almost half of the patients the 1 h plasma levels after the daily dose were below the detection limit of the procedure (0.12 ng/ml). The duration and severity of the withdrawal symptoms did not correlate with the doses and plasma levels of ergotamine. In only 4 of the 21 patients who were followed up for 3 to 6 months did headache symptoms not improve after ergotamine withdrawal. The results indicate that even small (0.5-1.0 mg/day) doses of ergotamine tartrate taken regularly may cause continuous headache symptoms and withdrawal symptoms after discontinuation.

Cerebrospinal fluid protein findings in cervical syndromes classified by myelography, and in multiple sclerosis.

Determinations of cerebrospinal fluid (CSF) albumin, IgG, albumin blood brain barrier (BBB) permeability and local IgG synthesis indexes in CNS were carried out on 85 patients with various neck, shoulder and upper extremity pain syndromes. CSF was obtained by lumbar puncture in 29, and by lateral neck puncture in 56 of the patients. The patients were classified into 3 different groups according to varying severity of degenerative changes, or cavitation verified by myelography. CSF protein patterns in these patients were compared with lumbar CSF findings in 18 patients with multiple sclerosis. CSF protein changes in patients with abnormal myelographic findings were slight. Protein values were clearly more abnormal in lumbar CSF than in cervical CSF, probably due to a retardation of the CSF flow. Only 3 of 62 patients with a narrowing of the cervical spinal canal had pathological values for IgG synthesis or BBB permeability indexes. On the other hand 14 of 18 patients with multiple sclerosis had abnormal, high values for the IgG index. Thus the present results suggest that investigation of the CSF protein pattern has value in differential diagnosis between patients with multiple sclerosis and degenerative diseases of the cervical spine.

Ultrasound and computed tomography of small asymptomatic haemangiomas of the liver.

Gray-scale ultrasound examinations were performed in 13 patients with 18 small asymptomatic haemangiomas of the liver and computed tomography in 10 of these patients. Different types of ultrasound images were found. One of them appeared as a well-demarcated area of increased echoes with an echo-free halo consisting of vessels, is probably characteristic for haemangioma. The other types of images are considered non-specific. The native CT scans demonstrated a well-demarcated spherical area with low attenuation in 8 cases and an area with attenuation similar to the liver in 4. After contrast medium injection, areas with lower attenuation were found in 7 cases, with irregular accumulation of contrast medium in the periphery in 4, and with distinct vessels in the periphery in one case, 2 haemangiomas had the same attenuation as the liver parenchyma.

Monoamine metabolite levels in cerebrospinal fluid and brain atrophy in lobotomized schizophrenic patients.

Baseline concentrations of homovanillic acid (HVA), 5-hydroxyindole acetic acid (5-HIAA) and MHPG in the cerebrospinal fluid (CSF) were determined in 67 lobotomized and 30 non-lobotomized patients with chronic schizophrenia. In addition, in 69 of these patients the degree of brain atrophy was assessed by a pneumoencephalographic (PEG) technique. There were no significant differences in the concentrations of the monoamine metabolites in the CSF between the two patient groups studied despite the fact that the group of lobotomized schizophrenics had significantly more central and cortical brain atrophy than the group of nonlobotomized schizophrenic patients. The amine metabolite levels were also unrelated to the subtype of schizophrenia, duration of illness, or degree of mental incapacitation.
