[Methodologic problems and practical aspects of training gastroenterologists at a specialized department of the Institute of Postgraduate Education of Physicians].

The training of gastroenterologists at the respective specialized department of the Institute for Postgraduate Medical Training should first identify individual shortcomings of postgraduate experience and use them as the basis for further studies employing a variety of forms and methods, with independent clinical work, daily analysis of cases and the mastering of new diagnostic and therapeutic procedures being prominent among those. Experience shows that therapeutists, with at least 5 years of field work behind them, make the best gastroenterologic specialists.

[Innervation of the blood vessels of the human lower extremities in occlusive arterial lesions].

In 63 patients suffering from an obliterating thromboangiitis and atherosclerosis the state of the efferent innervation of the shin and foot vessels has been studied. The material has been obtained during reconstructive surgical operations from various segments of the extremities and in the patients suffering from regional ischemic disorders of various degree. Moreover, adrenergic structures of the lumbar sympathetic ganglia have been investigated after sympathectomy performed. In the patients suffering from thromboangiitis, in the adventitial arterial tunic some foci are revealed containing single adrenergic neural fibers. Topography concerning localization of these foci is extremely asystematic. At atherosclerosis the innervational structures have character of a chronic exhaustion. In the sympathetic ganglia neurons, catecholamine-containing structures are revealed; they are seen as accumulations of norepinephrine granules. As demonstrates the comparison of the clinical and neurohistochemical data, in development of the obliterating thromboangiitis a sharp catecholamine deficiency and a pervent effect of the disturbed innervational connections on the vascular tunics play an essential role. At atherosclerosis the mechanism of the chronic exhaustion of the neuromediators belongs evidently, to the phenomenon of the "postmediatory stage" of the neural elements ontogenesis.

[Choice of treatment method in stomach polyps].

107 patients with gastric polyps were examined. A multi-zone aiming biopsy of mucosa was carried out during fibrogastroscopy, after a method developed at the Institute. A morphologic and morphometric examination of bioptates was performed to assess the degree of changes (degree of dysplasia), according to V. Kh. Vasilenko et al., and Oehlert et al. Since stage III dysplasia (i.e. marked precancer) was detected outside polyps in 17.7% of cases, both the condition of polyps and the morphofunctional state of the entire gastric mucosa were used as a basement in selecting the method of treatment. Indications for different procedures of surgery depending on the degree and expansion of dysplasia in polyps and mucosa are discussed.

[Prognostic significance of the determination of carcinoembryonic antigen in the blood of patients with lung cancer].

Correlation analysis of the results of examination of 86 lung cancer patients treated by surgery and methods other than surgery established an inverse relationship between life-span and carcinoembryonic antigen (CEA) level. An analysis using reagents supplied by CEA-IRE-Sorin Company showed that a CEA level in blood in excess of 10 micrograms/l is associated with the most unfavorable prognosis. The degree and rate of CEA excretion are in direct correlation with life-span and provide the most reliable criterion in evaluating the effectiveness of lung cancer treatment.
