The authors prospectively and retrospectively mutually compared the diagnostic value of urography, ultrasonography (USG), and computer tomography (CT) in four patients, with the diagnosis misstated prior to operation. The most valuable findings were provided by CT. Mere utilization of these methods does not exclude the possibility of diagnostic misstatements. In three patients the ultimate diagnosis was assessed on the basis of operative revision and in one patient, by means of cytologic examination of the renal punctate. Owing to retrospective evaluation of examinations, diagnostically valuable symptoms which could have lead to the correct diagnosis, were distinguished. Hence, examinations must necessarily be evaluated in complex with the general clinical state. (Fig. 6, Ref. 14.).
The authors present an account on a neonate with dextro-lateral renal venous thrombosis. They focus attention on the diagnostic and therapeutic procedure and compare it with available data from the literature. Contrary to data in the literature, they did not observe in the acute stage of renal venous thrombosis signs of disseminated intravascular coagulation in peripheral blood. It did not prove possible to elucidate the action of any of the factors leading to the development of renal venous thrombosis. After evidence of permanent functional loss of the right kidney nephrectomy was performed. Histopathological examination provided evidence of obliterating thrombosis of the renal veins with partial recanalization and calcifications. The authors emphasize the necessity of early diagnosis of renal venous thrombosis and adequate treatment based on the revealed findings.
The effect of neoadjuvant chemotherapy regimen utilizing cisdiaminodichloroplatinum (CDDP) with continuous 5-day infusion of 5-fluorouracil (5-FU) was analyzed during the treatment of 23 patients with histologically established epidermoid carcinoma of the oral cavity. All but four patients were in clinical stage III or IV of the disease and seven of them had an inoperable tumor. No one had been treated previously. Complete response after three courses of the chemotherapy was obtained in 13 patients (56.5%) and partial response in 8 (34.8%). The overall response amounted to 91.3%. Only two tumors showed no response to the therapy after one and two courses respectively. The former patient did not continue the chemotherapy due to toxicity and the latter because of its minimal effect. During therapy the disease did not progress in any of the patients studied. Neoadjuvant chemotherapy with CDDP and continuous 5-FU infusion lasting 120 hours was found to be both tolerable and an effective part of the complex management of oral cavity carcinoma without the risk of delaying subsequent definitive treatment. The authors present an account of results of examinations in 162 patients with cancer of the prostate, benign hyperplasia of the prostate, inflammatory diseases of the prostate and patients with a normal finding on palpation of the prostate. The patients were subjected to clinical, laboratory, histopathological and transrectal ultrasonography of the prostate. In one patient a CT examination of the prostate was made. In the group were seven patients with cystic prostate lesions (4.3%). In the group of patients with inflammatory diseases of the prostate they observed cystic changes in five patients (14.2%). The most important group was formed by two patients with abscesses of the prostate and one patient with an ascended utriculus prostaticus. Transrectal ultrasonography of the prostate was the supreme method for the diagnosis of cystic prostate lesions.
The authors present case-histories of two patients with abscesses of the seminal vesicles. They emphasize the asset of transrectal ultrasonographic examination for the early diagnosis of an abscess of the seminal vesicles. Antibiotic therapy, puncture of the seminal vesicles and evacuation of pus proved effective therapeutic methods.
The authors give an account of the application of USG as the primary examination method of the kidneys and retroperitoneum based on the examination of 2330 patients aged 15-93 years. A positive USG finding was recorded in 44%. A urological case-history was recorded in 81.5% of the patients. Patients in the 6th decade were affected most frequently. Cystic changes were found in 15.1%, chronic inflammatory changes in 11.24%. Tumours of the kidneys were present in 4.3% patients. The examination was unsuccessful in 5.1%, whereby it supported in 66% the negative urological finding. Ultrasonography is recommended as the method of first choice in patients with urological symptomatology after the basic clinical and laboratory examination.
The initiation of anticoagulants and drugs with platelet aggregation inhibiting properties is justified for the treatment of myocardial infarction in a great number of conditions. The well-known complications of this therapy are gastro-intestinal haemorrhage and the concomitant discomfort. In the period from 1983 to 1986 Histodil was given as adjuvant to therapy for infarction patients with ulcer, gastro-intestinal haemorrhage, hyperacidity or other epigastric burn-pain syndrome in history. The patients treated at our department between 1979-82 had not received Histodil for this purpose. According to the retrospective examinations the preventive effect of Histodil prophylaxis was 100%. Therefore the drug may be recommended to myocardial infarction patients in whom the inhibition of haemostasis means an increased risk of gastro-intestinal alteration.

The intensity and dynamism of the toxicity of the neoadjuvant chemotherapy regimen utilizing cis-diaminodichloro-platinum (CDDP) with continuous 5-day infusion of 5-fluorouracil (5-FU) was recorded and analyzed during the treatment of 23 patients with histologically established epidermoid carcinoma of the oral cavity. The most frequent sign of toxicity was leukopenia and nausea/vomiting, both occurring in 78% of cases. Leukopenia was more frequent and serious than reported in other studies. On the contrary, nephrotoxicity was mild and less frequent (17%) compared to literary data. Diarrhea was associated with the administration of CDDP in 31%, minimal hair loss was recorded in 21% of cases. Other signs of toxicity were rare and mild. The degree of severity of toxicity was increasing with the number of treatment courses only with respect to hematologic parameters. No life-threatening complications were recorded. In seven cases continuous treatment had to be interrupted after three days due to leukopenia. With regard to toxicity, the applied treatment schedule (three courses) is fully acceptable, safe and suitable as neoadjuvant chemotherapy in the complex management of oral cavity carcinoma without the risk of delaying subsequent definitive treatment.
A postmortem examination was performed on a male child who had lived to twelve months of age, at the Institute of Pathology of the Medical Faculty of Comenius University in Bratislava, Czechoslovakia. The clinical diagnosis had been mucoviscidosis, bronchopneumonia, and dystrophy. Findings characteristic of cystic pancreas fibrosis and bronchopneumonia of the right lower lung lobe were macroscopically recorded and were microscopically established. Massive fibrotic and necrotic lesions were primarily recorded from the left cardiac ventricle and were considered to be unusual findings. Damage to the myocardium is a very rare complication of mucoviscidosis. Less than 50 cases have so far been described, according to Benesová et al. (1983).
Perfringolysin O is a thiol-activated cytolytic exotoxin the primary receptor of which is the membrane cholesterol on the cell surface. The effect of perfringolysin O was tested in various hepatocyte preparations. (i) Smears of fresh liver exposed to a mild H2O2 (1.0 mM) injury for 10 min at 37 degrees C, develop a 'peroxide-induced autofluorescence' (PIAF) on the membrane proteins. PIAF is suitable for measuring the average lateral diffusion constant (D) of the membrane proteins by means of fluorescence recovery after photobleaching technique (FRAP). Incubation for 5 min with 600 or 2000 units/ml of the perfringolysin O resulted in a significant increase (32 and 46%, respectively) of D as compared to the controls of the same age group (13-14 months). Various tests like heat denaturation of cholesterol saturation of perfringolysin O before its application as well as thiol-activation of the smears with dithiothreitol revealed that the increase of D is a specific toxin effect due mot probably to the reaction of perfringolysin O with cholesterol. (ii) Isolated hepatocytes were exposed to perfringolysin O and their viability as well as the release of two cytosolic enzymes (lactate dehydrogenase and glutamic-pyruvic transaminase) were measured; 40-60 units/ml of perfringolysin O in 30 min reduced the viability of the hepatocytes to zero and caused a release of about 70% of both cytosolic enzymes. The significance of the results is discussed from the points of view of both the toxin-effect and the FRAP method.
A consistent chondrogenesis takes place in micro high-density cultures of chick limb bud mesenchyme cells stage 22-24. The effect of an increased generation of OH(.) free radicals by Fenton reaction was tested in these cultures. Components of Fenton reaction (i.e., ferrous iron in form of ADP-Fe2+ complex in 0.1 mM concentration, or 0.2 mM H2O2, or the combination of these components with each other, as well as with 0.2 mM ascorbate) were supplemented to the culture medium after the first 24 h. ADP-Fe2+ complex resulted in a drastic decrease of the frequency and confluence of the cartilage nodules seen in light microscope, accompanied electron microscopically by a strikingly increased frequency of occurrence of lipofuscin-like, residual bodies in the cells. Biochemical methods revealed a significant decrease of both the DNA and glycosaminoglycan contents (to 48.6 and 20.7% of the controls, respectively), in day 6 cultures. H2O2 alone caused similar alterations of the cultures, whereas the combination of it with ADP-Fe2+ complex proved to be lethal for the cells. Ascorbate when added to the ADP-Fe2+-treated cultures displayed a slight protective effect for the glycosaminoglycan content but not for DNA. The results are interpreted in terms of free radical theory of aging.
Although X-ray microanalysis represents a useful tool for identifying electron dense histochemical end-products, quantitative microanalytic measurements are seriously hampered in the case of the activities of certain membrane-bound enzymes. For example, the electron histochemical methods revealing K+-dependent pNPPase activity result in a very fine, granular reaction product of lead phosphate. Therefore microanalytic, densitometric of similar evaluations of the reaction, even in semiquantitative terms are not practical by the usual procedures. This paper describes a method of X-ray microanalysis of thick sections (0.5 micron) processed for K+-pNPPase, where a sufficient amount of lead is present for X-ray microanalytic determination. The analysis is performed in the line scan mode on transversely cut membrane profiles by means of the line scan ratemeter of an EDAX System F. This yields quantitative data on the relative lead concentrations in the vicinity of the cell membrane. A method is proposed for calculation of relative enzyme activities based on the Pb-signal of the ratemeter curve and the average "noise"-level of the cytoplasm, containing also non-specifically bound lead. This method avoids the necessity of measuring the section thickness; it may be useful for a variety of purposes in the electron microscopic histochemistry of membrane-bound enzymes.
Young, adult and old male CFY rats (2, 12 and 24 mth of age, respectively) were treated with a daily dose of 30 mg RbCl/100 g body weight, in form of aqueous solution injected intraperitoneally for 14 days. A considerable part of the intracellular K+-content of the body was replaced by Rb+ during this treatment. After cessation of the RbCl injections, a relative steady state came into being in each age group, called Rb+-release period. During this period Rb+ and K+ contents of the blood serum and the cisternal CSF were measured by atomic absorption spectrophotometry, and of the intracellular space of brain cortical cells by energy-dispersive X-ray microanalysis. Ultrastructural features of the brain cortex were also checked by transmission electron microscopy. For X-ray microanalysis, the L-line of Rb at 1.694 keV energy was used at 10 kV accelerating voltage in a scanning electron microscope equipped with an EDAX System F. Rb+ and K+ concentrations were obtained for the cellular dry mass and converted into wet concentrations on the basis of intracellular water contents known from former experiments. Rb+-replacement of K+ did not cause any ultrastructural alteration in the brain cortex. However, the Rb+ accumulation displayed a very significant age-dependent increase: at the beginning of release, adult and old rats had 32.6 and 44.7 mM Rb+ in their intracellular water as against the 8.6 mM found in the young group, and similar proportional difference persisted during 20 days of the release. Rb+ discrimination ratios (DR) calculated either for the blood or the CSF displayed very considerable age-dependent increase: the values of the adult and old groups were 191 and 242% of the young one, indicating that the passive Rb+ (and K+) permeability of the nerve cell membrane decreases throughout the life span of rats. These results give further support to the membrane hypothesis of aging.
Synaptosomal and myelin fractions were isolated from the brain cortex of young, adult and old male CFY rats (2, 12 and 24 mth, respectively). The purity of the fractions was tested by transmission electron microscopy and marker enzyme assays. The cholesterol content of the fractions was also determined. Samples of the fractions were labelled with diphenylhexatriene (DPH) and the fluorescence anisotropy (r) of the label was measured at various optical densities. The values of r extrapolated to zero optical density were compared in the age groups and used for calculating the 'microviscosity' of the membranes. The 'microviscosity' of synaptosomal membranes displayed a significant age-dependent increase: from 2.3 +/- 0.02 (SD) in the young group it increased to 2.6 +/- 0.03 poise by the age of 24 mth at 37 degrees C. Most of this increase occurred between the adult and old age. The cholesterol content of the synaptosomes also increased significantly during aging. Centrophenoxine (CPH)-treatment with 100 mg/kg body weight daily dose for 2 mth was able to reverse the age-dependent alterations of both the membrane 'microviscosity' and the cholesterol content in the synaptosomes: the values returned nearly to the adult level. The results obtained are interpreted in terms of the membrane hypothesis of aging attributing to primary role to the free-radical induced membrane damage in cellular aging.
Energy-dispersive X-ray microanalysis was applied on human intraoperative biopsy materials of different thyroid tumors. To ensure suitability of these tissue pieces for quantitative microanalysis in freeze-fractured, freeze-dried bulk specimens, sampling was carried out with strictly defined criteria. Benign adenomas and differentiated and anaplastic carcinomas were selected for the studies on the basis of pathohistological investigations of the same specimen. The results of the tumor cells were compared to those obtained in apparently normal human epithelial cells. The number of normal cells analyzed was 349, whereas in the tumors 408, 423, and 891 cells were measured in the benign, differentiated, and anaplastic groups, respectively. Intracellular monovalent contents were calculated as percentage of cell dry mass; then, Na+:K+ molar ratios were calculated for each cell individually. Due mostly to the increase of Na+ content, the distribution histograms of the Na+:K+ molar ratio show an increase in the number of cells with a higher Na+:K+ ratio with increasing malignancy of the tumors studied. The differences proved to be statistically highly significant by the chi 2 test. Thus, in human thyroid, increasing malignancy is associated with increasing intracellular Na+:K+ ratio. The results give further support to the theory of C. D. Cone (J. Theor. Biol., 30: 151-181, 1971) according to which the sustained depolarization of the cell membrane results in an increased rate of cell division.
