Laser irradiation and balloon dilatation in experimental transluminal angioplasty.

Percutaneous transluminal balloon angioplasty (PTA) is being currently used in patients with coronary artery disease. Laser irradiation (LI) has been effective in vaporizing atherosclerotic plaques. The purpose of this work was twofold: to compare PTA and LI techniques in the recanalization of experimental obstructive arterial lesions, and to evaluate the possibility of reducing the failures and local complications and increasing the success rate of PTA by the combined use of LI. Atherosclerotic iliac stenoses were induced in 27 rabbits; lesions were evaluated angiographically before and after intervention and were studied histologically and by electron microscopy. Argon-ion LI delivered through microlens-tip optic fibers reduced the stenotic area from 64.2 +/- 21.8% to 40.3 +/- 10.7% (n = 10, P less than .01) and PTA from 60.7 +/- 15.9% to 30.4 +/- 7.7% (n = 9, P less than .01). However, percentage reduction was higher in PTA-treated stenoses (48.4 +/- 10.1% vs 34.5 +/- 13.5%, P less than .0125). In eight more rabbits, low power LI (4.55 +/- 1.25 J) was delivered after PTA in dilated segments. Post-PTA LI further decreased stenoses (from 31.2 +/- 7.8% to 29.1 +/- 8.1%, P less than .0125); laser-irradiated segments showed diffuse carbonization of the disrupted intimal layer. The normalized transtenotic pressure gradient decreased significantly in all groups: LI reduced the gradient from .40 +/- .25 to .17 +/- .07 (P = .005); PTA from .37 +/- .14 to .11 +/- .04 (P = .001); LI after PTA from .40 +/- .16 to .12 +/- .06 (P = .001). Thus, LI is effective (less than PTA) in relieving experimental atherosclerotic stenoses and seems useful when combined with PTA.

Detection of insulin autoantibodies using an ELISA technique in first-degree relatives of IDDM patients and in autoimmune patients.

We have improved an ELISA technique for the detection of IgG and IgM class IAA demonstrating the sensitivity and the specificity of antigen-antibody reaction. In sera of 135 first degree relatives of IDDM patients and of 128 autoimmune patients "at risk" for IDDM either positive or negative for islet cell antibodies (ICA), the prevalence of IAA IgG/IgM in all groups studied was significantly (p less than 0.001) higher than in controls, but the highest frequency of IAA was observed in the ICA-positive subjects. A similar association was also observed in complement-fixing ICA (CF-ICA) positive subjects. Future prospective studies will be necessary to establish the role of IAA as marker of B-cell damage.

Effect of prolonged gliclazide therapy in non-insulin dependent diabetic subjects.

The effect of prolonged gliclazide treatment on diabetic metabolic control was studied in 10 subjects with non-insulin dependent diabetes mellitus. Patients were examined before, after 15 days of treatment with diet alone and, again, after 60 days of treatment with diet plus gliclazide. Gliclazide did not restore the abnormality of blood glucose, free insulin and C-peptide response to an intensive stimulus of glucose load, although fasting and after-load blood glucose, fasting glycosylated haemoglobin, alanine and lactate significantly decreased after prolonged treatment with diet plus gliclazide, but not with diet alone. These findings support the assumption that the efficacy of prolonged treatment with gliclazide might be related to its extrapancreatic effects on glucose homeostasis.

Calcium hormones in diabetic pregnancy.

To evaluate calcium hormones in diabetic pregnancy, eleven diabetic pregnant women were studied by measuring serum total calcium, phosphorus by conventional methods, and parathyroid hormone, calcitonin and 25-hydroxycholecalciferol, by radioimmunologic methods, serially every four weeks, beginning at 16th week throughout 36th week of gestation. Serum total calcium, phosphorus and parathyroid hormone levels dit not differ between diabetic and non diabetic pregnant women, nor between all pregnant women and age-matched non pregnant healthy women. Serum calcitonin levels, although significantly higher than those of age-matched non pregnant women, did not differ between diabetic and non diabetic pregnant women, maintaining constant with the progression of pregnancy. Serum 25-hydroxycholecalciferol levels, although contained within the normal range, increased significantly in the third trimester of gestation both in non diabetic pregnant women and, even though later, in diabetic pregnant women.

Blood oxygen affinity in large white pig.

Large white pig and human blood oxygen affinities are different due to different primary structures of hemoglobin. Empirical equations are reported to predict the oxygen partial pressure at half-saturation of hemoglobin (p50) from known values for pH, pCO2 and 2,3-diphosphoglycerate, with an accuracy of +/- 0.82 torr.

[Clinico-immunological studies of 32 cases of bone tumors].

We have studied 32 patients with different bone neoplasias. 18 had osteogenic sarcoma. Clinical and immunological observations led us to consider our results as a general approach to the problem. No specific conclusions can be made. We describe 3 patients treated with levamisole therapy and two with transfer factor. In single patients some useful observations are connected about relationship between treated and non-treated patients.

Artificial excretion of weakly bound middle molecules by hemoanadiafiltration.

The concept of a subtractive therapeutic intervention in several diseases (shock, acute liver, failure, poisoning) to remove toxic middle molecules (MM) has led to the devising of a particular ultrafiltration system (anadiafiltration) to clear weakly bound MM. Clinical hemoanadiafiltration (HADF) with a polyacrylonitrile filter and in vitro experiments with a series of marker substances, have shown HADF efficacy in clearing MM, irrespective of their molecular weight, with protein binding K between 50 and 150.

[Thermal tachypnea in pigs].

In anaesthetised and tracheotomised pig, the reflex of panting has been evaluated. The thermal tachypnea showed two phases: initially respiratory frequency increases likely to body temperature but at 41,4 degrees C is observed apnea. When temperature rose above 42 degrees C the respiratory frequency further increased the changes in blood CO2, O2 and pH were not significant. During apnea is present a slight alkalosis.

[Respiratory work in panting in swine].

The dynamic work of breathing (Wdyn) has been measured during thermal tachypnea in pig anaesthetized and tracheotomized. During euthermia Wdin(cm H2O/R1/min-1) increases with ventilation (L.P.S.) according to Wdyn = 1,09 V2 + 1,33 V3; when body temperature rose above 39 degrees C for a given ventilation Wdyn decreases. In all animals has been observed a change of airways size and probably of respiratory tract of Weibel.
