The effect of inhaled verapamil on resting bronchial tone and airway contractions induced by histamine and acetylcholine in normal and asthmatic subjects.

In man, the influence of calcium entry blockers (CEB) on nonspecific bronchial sensitivity and resting bronchial tone is controversial. In 10 asthmatic and 8 normal subjects we recorded specific airway conductance (Gaw/VL) and flow volume loops before, 30, 60, and 90 min after the inhalation of saline, 10 (V10) and 20 mg (V20) of verapamil. The dose of inhaled histamine and acetylcholine producing Gaw/VL of -40% (PD40H and PD40ACH, respectively) with and without pretreatment with saline, V10, and, in 15 subjects, V20 was also determined. We measured plasma verapamil concentrations immediately after the end of nebulization of V10 and V20, and 30 and 60 min later. In normal subjects, V10 and V20 produced a maximal % delta Gaw/VL of 22.30 (+/- 19.50) and 33.00 (+/- 15.82), respectively (p less than 0.05). In asthmatics, V10 and V20 produced comparable % delta Gaw/VL of 22.00 (+/- 22.50) and 38.60 (+/- 38.60), respectively. This bronchodilating effect involved predominantly the large airways, persisted for 60 to 90 min, was reproducible, affected only some subjects (11 of 18), and was independent of the resting Gaw/VL, degree of bronchial sensitivity to H and ACH, and the time course of plasma verapamil concentration. The latter reached a maximum of 24.3 +/- 7.1 ng/ml after V20. In both normal and asthmatic subjects, saline or V10 did not significantly alter PD40H and PD40ACH. In normal subjects, pretreatment with V20 increased PD40H 5.3 times and PD40ACH 3.22 times (p less than 0.05). Except in 2 asthmatics, in whom V20 decreased PD40 and PD40ACH, it increased significantly PD40ACH (dose ratio: 3.15, p less than 0.05) but not PD40H.(ABSTRACT TRUNCATED AT 250 WORDS)

Lorcainide kinetics and protein binding in patients with end-stage renal disease.

Lorcainide is a new antiarrhythmic drug undergoing clinical investigation for management of patients with ventricular arrhythmias. In this study we investigated the kinetic profile of lorcainide in nine patients with end-stage renal disease. A single intravenous bolus of 100 mg of the drug was injected while the patients were undergoing hemodialysis or during the off-dialysis period. Renal disease did not alter the kinetic properties of lorcainide; the elimination t1/2 beta during hemodialysis was 8.61 +/- 6.35 h, not significantly different from 7.04 +/- 4.12 h off-dialysis. Serum protein binding of lorcainide was investigated in vitro, and the percent binding of lorcainide to serum proteins of normal volunteers and renal or cardiac patients was not significantly different. These data suggest that renal disease should not alter either the dose or the dosing interval of lorcainide.

[Economic evaluation of the method of gradual formation of groups of dairy cows from primiparae free of infectious mastitis].

Employing the turnover of a dairy-cow herd with primiparas to control infectious mastitis is beneficial because young dairy cows, as a rule free of inflammations of the mammary gland, are not included among dairy cows already infected or suspected of being infected and so it is possible to form groups of dairy cows free of mastitis. For this intention an agricultural enterprise was chosen in potato production region I. In the given period 316 to 336 dairy cows of Bohemian Spotted breed were followed. To express the impact of the infection on the milk efficiency and to draw economic conclusions on the efficiency of treating infectious mastitis we observed the total milk yield of the different dairy cows in one lactation. In the given enterprise unsuccessful treatment of an infected dairy cow in three lactations resulted in a total loss of 4,648 Kcs. Therefore from the economic aspect it appears to be profitable to replace productive, but ill, older dairy cows by healthy first-calvers because the negative effect of the mastitis on the production of milk increases with every successive lactation.

A new method of haemostasis following transvesical prostatectomy.

Description of an efficient modification of haemostasis following transvesical prostatectomy with the help of semicircular tightening sutures of the bed. 25 patients have been operated as described here and none of them experienced postoperative complications.

[Determination of the effective concentration of Jodonal a for the disinfection of the skin and teats after milking].

The bactericidal effectivity of Jodonal A in 1:10, 1:5, and 1:3 solutions was tested on human skin and on the teats of cow mammary glands. The 1:3 dilution ratio proved best for three-minute exposure. Jodonal A used in this concentration in 522 cows for the post-milking disinfection of teats for 10 months exerted no harmful effect on the skin of the mammary glands.

[Devitalizing effect of Jodonal A in vitro on bacteria subject to a short-term exposure].

In a three-minute exposure in vitro Jodonal A devitalized a culture of serological group B streptococci in a 2% concentration, Staphylococcus aureus in a 16% concentration, Pneumococcus in a 4.5% concentration, Corynebacterium pyogenes in a 2.5% concentration, Pseudomonas aeruginosa in a 3% concentration, and Klebsiella pneumoniae in a 2% concentration. Hence Jodonal A concentrations higher than 16% should be tested for udder teat disinfection after the removal of teat cups.
