[Treatment of acute thromboses and embolisms of the arteries of the extremities in cardiosclerosis patients].

An experience with the surgical and conservative treatment of 196 patients with acute thromboses and embolism of the extremity arteries in patients with arteriosclerotic and postinfarction cardiosclerosis has shown the conservative treatment of arterial embolism to have little effect. Results of the surgical treatment were regularly better in patients admitted to the hospital at earlier terms after acute occlusion of the vessel, with mild ischemia and the absence of a considerable atherosclerotic lesion of the extremity interrupting the major blood flow. For prophylactics of a repeated embolism of considerable significance were found to be correcting operations on the heart and the continuous anticoagulating therapy.

[Revascularization syndrome in the surgery of acute embolic arterial occlusions of the limbs].

In 96 patients with acute arterial embolic occlusion of the iliofemoral segment before and after reparative operations the hemodynamics, renal activity and other parameters were studied. The revascularization syndrome is believed by the author to always develop in reestablishing the patency of the extremity vessels. It requires correction. The development of the revascularization syndrome includes several periods: latency, revascularization, toxemia and reconvalescence.

[Postischemic disorders in the surgery of acute intestinal obstruction].

Acute intestinal obstruction accompanied by disorders in the mesenterial blood circulation is one of the variations of the ischemic injury of the intestine. After recovery of blood circulation in the portion of the intestine subjected to ischemia, the products of autolysis of the intestinal wall come into the liver and general blood flow and bring about the development of intoxication and cardiopulmonary insufficiency. The therapeutic measures should be directed to stabilization of indices of hemodynamic microcirculation, to liquidation of metabolic acidosis and stimulation of urination.

[Problems in the surgical treatment of the thromboembolic complications of rheumatism].

After having studied the results of the surgical and conservative treatment in thromboses and emboli of the aorta and main arteries of the limbs in 32 patients with rheumatism the authors note a high percentage of mortality and postoperative complications. A valuable intense antirheumatic therapy, complex of measures combating inclusion syndrome allow to improve the results of the treatment of these severe cases.

[Recurrence of varicosity of the lower extremities].

The authors analyse the causes of the recurrence of varicosis of the lower extremities superficial veins in 40 patients. They believe that successful treatment of such cases in warranted by a complex clinico-functional examination including a compulsory angiography aimed at revealing of the causes of the recurrence. Surgical intervention should be reduced to the elimination of errors admitted in primary operations.

[Lamblia-induced cholecystitis].

Case reports of 32 patients with lambliasis induced cholecystitis, treated in the surgical clinic for a 28-year period, are analysed. The clinical data allow establishing the differential diagnosis between cholecystitis attack induced by the presence of Lamblia in bile ducts and an attack induced by the presence of concretions in the gallbladder. It is the authors' opinion that the complex exploration of patients should be carried out. A carefully selected anamnesis, data of multiple duodenal sounding and coprologic studies serve as a valuable adjunct in establishing the correct diagnosis and therapeutic tactics.

[Sodium hypochlorite in the treatment of bacteremic circulatory shock].

17 patients with massive urinary infection and bacteriemic circulatory shock received a combined intensive treatment including indirect electrochemical blood oxidation (IEBO) with 0.06% solution of sodium hypochlorite (SHC). SHC intravenous infusions resulted in stabilization of arterial pressure within 24 hours. IEBO in the treatment of incipient bacteriemic shock improves prognosis.
