[Beta 2-microglobulin test in the diagnosis of chronic kidney diseases].

A study was made of the content of low molecular protein B2-microglobulin in the blood and urine of 126 patients with chronic renal diseases and 95 healthy persons. As a result of the study it was shown that B2-microglobulin concentration in the blood grows with age. The maximum level of B2-microglobulin was marked in patients with chronic glomerulonephritis. A high level of the urinary excretion of B2-microglobulin with a moderate rise of its concentration in the blood is typical of patients with chronic pyelonephritis during exacerbation. Indices of the B2-microglobulin test are closely related to renal function. The B2-microglobulin test is of great diagnostic significance as it provides an opportunity to establish the nature of proteinuria, site and expression of renal pathologic processes.

[Endocrine regulators of calcium metabolism (calcitonin and parathormone) in patients with myocardial infarction during various periods of observation].

The purpose of the study was a comprehensive analysis of the level of calcitonin and parathormone in patients with acute myocardial infarction (MI) and postinfarction cardiosclerosis as compared to similar data on healthy persons and coronary patients without a previous history of myocardial infarction. A total of 122 persons were examined using a radioimmunoassay and biochemical methods. The data obtained demonstrated an increase in the level of circulating calcitonin in patients with chronic CHD including postinfarction cardiosclerosis. The phasing of changes was revealed in the patients with acute MI: hypercalcitoninemia and hyperparathyrinemia on the 1st and 27-th-30th day of disease. Hypocalcemia was noted on the 14th-16th day of disease in the presence of normalization of the level of both hormones.

[Systemic evaluation of thyroid function in women and men from radioimmunoassay data].

The author presents a new variant of the integral quantitative systemic evaluation of thyroid function. Its essence is as follows: the integral index of evaluation is based on the simultaneous registration of 3 thyroid indices TTH, T3 and T4. The proposed method makes it possible to unravel changes in thyroid function concealed by homeostasis in women depending on the menstruation phase and reflects in a summary form by means of one figure the entire activities of the hypophyseal-thyroid system.

[Correction of protein metabolism in myocardial infarction according to the results of radionuclide studies with 131-albumin during during treatment with a number of drugs (retabolyl, methyluracil, panangin, strophanthin) and combinations of them].

The effect of a series of drugs (retabolil, strophanthin, retabolil with strophanthin, retabolil with panangin , methyluracil with panangin ) on protein metabolism was investigated in 246 patients with acute myocardial infarction. Retabolil both alone and combined with panangin , and a methyluracil-- panangin combination were found to have anticatabolic and protein synthesizing effects. Strophanthin, when used in the acute period of myocardial infarction, contributes to an increase in the protein decay rate and a decrease in protein synthesis. When it is employed in combination with metabolism corrective agents (retabolil), the adverse effects of strophanthin mentioned above are eliminated.

[Therapeutic use of an enriched insulin-glucose-heparin mixture in acute myocardial infarct].

Data on clinical efficiency of concentrated insulin-glucose-heparin mixture in 52 patients with acute myocardial infarction are presented. In the control group (19 patients) routine treatment was supplemented by heparin administration. The efficiency of the insulin-glucose-heparin mixture was assessed on the basis of general clinical data as well as radioimmuno-assays of plasma myoglobin patterns. The treated patients demonstrated a more favourable course of infarction, and manifest positive changes in their ECG parameters and blood transaminase activity. Myoglobinemia augmentation was inhibited within the first hours of myocardial infarction. It is suggested that the therapeutic mixture examined is conducive to limiting the area of myocardial lesion.

[Radionuclide methods of determining digoxin levels].

The digoxin level in the blood was determined in 92 patients with blood circulatory insufficiency using radioimmunoassays (Sea Ire-Sorin kits) and radionuclide (86 Rb) methods. Pharmacokinetics of digoxin was evaluated on the basis of a two-compartmental mathematical model (by J. Y. Wagner). Direct correlation between the value of a maintenance dose and digoxin concentration in the blood was established. Digoxin values in the blood determined by the radionuclide method, were somewhat higher than the indices of the radioimmunological method. According to this method digoxin therapeutic concentrations in the blood were: 0.4-2.8 ng/ml; according to the radionuclide method 0.6-4.8 ng/ml. Symptoms of digoxin intoxication (overdosage) in rising its concentration in the blood up to 1.1-3.3 ng/ml by the radioimmunoassay and up to 2.2-6.1 ng/ml by the radionuclide method were recorded. It can be accounted for by the fact that bigger values of digoxin active metabolites are determined by the radionuclide method, whereas the radioimmunoassay is more specific to digoxin. Pharmacokinetic parameters (distribution volume, digoxin clearance, the period of semi-elimination) calculated by two methods, brought about the same results. Regularities of the changes of digoxin pharmacokinetics in patients with disturbed renal function are analysed.
